The pathogenesis of melasma is believed to be related to heredity, sun exposure, sex hormones, and other factors. The mechanisms underlying the development of melasma include increased melanin synthesis, vascular proliferation within the skin lesions, inflammation, and skin barrier dysfunction. The diagnosis of melasma is mainly based on clinical manifestations and noninvasive techniques. Coupling with recent research progress in melasma, the present consensus comprehensively elaborates the etiology, pathogenesis, clinical manifestations, staging, classification, diagnosis, and treatment of melasma to improve the diagnostic and therapeutic skills of Chinese dermatologists.

奇正~消痛贴膏系藏医传统验方,国家保密处方,具有活血化瘀、消肿止痛作用。自1993年上市以来,被骨科、风湿科、疼痛科、康复科等多个临床科室广泛应用于各类急慢性骨骼肌肉疼痛及各类脊柱、关节、软组织病变的治疗。但因其说明书上对于各类疾病具体使用方法缺乏详细说明,且已发布的指南、专著、临床报告中对奇正~消痛贴膏的临床应用优势病种、用法用量、疗程、安全性等的介绍不详尽或不规范,未形成统一的标准,临床应用无统一标准可依。该共识针对消痛贴膏在临床中的优势病种,在临床医生和患者调研分析、循证医学研究和评价的基础上,结合临床专家应用经验达成共识,规范了奇正~消痛贴膏在急慢性扭挫伤(软组织损伤)、骨关节炎、下腰痛、肩周炎、颈椎病、骨科术后恢复期止痛以及其他骨骼肌肉系统疾病治疗时的用法用量、疗程、联合用药、安全性等内容,以期为临床合理、安全使用奇正~消痛贴膏治疗各类急慢性骨骼肌肉疼痛上提供依据和参考,继而为消痛贴膏的规范应用带来帮助。

病毒性脑(膜)炎是常见的中枢神经系统感染性疾病,是病毒侵袭脑实质与脑膜导致的炎症性疾病。病毒性脑(膜)炎呈世界性分布,临床以急症和重症常见,严重威胁人类健康。多种病毒可导致中枢神经系统感染,包括疱疹病毒、肠道病毒和虫媒病毒等,病原体鉴定仍是病毒性脑(膜)炎在临床上的主要难题。为规范、合理地应用病原体诊断技术开展脑脊液的病毒核酸检测与抗体检测,由国内病毒学、传染病学、临床医学及医学检验等领域的专家组成的专家组,经反复讨论达成本共识,主要从国内外病毒性脑(膜)炎病原种类、检测技术及检测策略等方面进行阐述,并提出针对我国病毒性脑(膜)炎病原体的检测策略和方案,以期为病毒性脑(膜)炎的临床诊断以及疾病预防控制提供参考。

内镜黏膜下剥离术(endoscopic submucosal dissection,ESD)/内镜黏膜切除术(endoscopic mucosal resection,EMR)作为内镜下治疗消化道早期癌的微创手术,其技术日趋发展成熟,多项国际指南和共识均推荐ESD/EMR作为消化道早期癌及其前驱病变的首选治疗方式,ESD/EMR标本的诊断结果影响后续治疗方式的选择,因此ESD/EMR的病理学评估相对于传统外科手术标本应更为严格及精准。在取材制片的过程中存在诸多因素影响ESD/EMR标本病理诊断的准确性。本共识规范了ESD/EMR标本处理以及常规制片的方法,旨在提高病理诊断的准确性,为临床进一步诊疗提供可靠、合理的病理依据。

Intrahepatic cholangiocarcinoma(iCCA)can originate from the large bile duct group(segment bile ducts and area bile ducts),small bile duct group(septal bile ducts and interlobular bile ducts),and terminal bile duct group(bile ductules and canals of Hering)of the intrahepatic biliary tree,which can be histopathological corresponding to large duct type iCCA,small duct type iCCA and iCCA with ductal plate malformation pattern,and cholangiolocarcinoma,*** challenge in pathological diagnosis of above subtypes of iCCA falls in the distinction of cellular morphologies,tissue structures,growth patterns,invasive behaviors,immunophenotypes,molecular mutations,and surgical *** these reasons,this expert consensus provides nine recommendations as a reference for standardizing and refining the diagnosis of pathological subtypes of iCCA,mainly based on the 5th edition of the World Health Organization Classification of Tumours of the Digestive System.

Emerged evidence has indicated that immunosuppression is involved in the occurrence and development of *** provide clinical practice recommendations on the immune function in sepsis,an expert consensus focusing on the monitoring and treatment of sepsis-induced immunosuppression was *** related to the immune monitoring and treatment of sepsis were retrieved from PubMed,Web of Science,and Chinese National Knowledge Infrastructure to design items and expert opinions were collected through an online ***,the Delphi method was used to form consensus opinions,and RAND appropriateness method was developed to provide consistency evaluation and recommendation levels for consensus *** consensus achieved satisfactory results through two rounds of questionnaire survey,with 2 statements rated as perfect consistency,13 as very good consistency,and 9 as good *** summarizing the results,a total of 14 strong recommended opinions,8 weak recommended opinions and 2 non-recommended opinions were ***,a face-to-face discussion of the consensus opinions was performed through an online meeting,and all judges unanimously agreed on the content of this *** summary,this expert consensus provides a preliminary guidance for the monitoring and treatment of immunosuppression in patients with sepsis.

经自然腔道取标本手术(NOSES)通过无切口理念与常规手术器械的结合,表现出极佳的微创效果,并带给患者全新的就医感受.目前,尽管NOSES已广泛用于结直肠肿瘤治疗,大量研究也表明相比常规腹腔镜手术,NOSES具有恢复快、创伤小、疼痛轻等优势,但绝大多数研究均为小样本、单中心回顾性研究.本研究旨在通过分析我国5055例结直肠肿瘤NOSES患者数据,进一步探讨NOSES的短期疗效与预后.研究对象中,经直肠NOSES占整体比例的90.3%,经阴道NOSES占9.7%.NOSES术后短期并发症发生率为14.1%,吻合口瘘发生率为4.9%,腹盆腔感染率为1.9%.患者3年总生存率为93.2%,无病生存率为82.2%,局部复发率为3.6%.研究结果提示NOSES具有较好的安全性与可行性,为其应用于结直肠肿瘤治疗提供了证据.

呼吸道合胞病毒(RSV)是造成儿童急性下呼吸道感染发病和住院的主要病原体之一,也是目前导致5岁以下儿童罹患下呼吸道感染(LRTI)的首要病毒性病原,严重危害儿童健康。为进一步推进RSV防控工作,全面探讨RSV疾病负担、流行规律变迁、防治手段研发与监管及防控相关策略的制定等,中华医学会中华医学杂志编委会于2022年12月召开了“中华RSV论坛”。与会专家围绕RSV研究现状及发展需求,交流探讨后形成了我国儿童RSV感染防控专家倡议:(1)加强对RSV流行特征及疾病负担研究;(2)联合多部门多领域,开展多中心长期持续性监测;(3)重视疫苗、单克隆抗体及检测方法的创新、研发与引入工作;(4)推动RSV免疫预防策略的制定与实施;(5)统筹协调合理分配儿科医疗卫生资源,防止院内感染。
