Celiac disease(CeD)is a chronic gluten-induced enteropathy with plethoric *** typical manifestations of CeD such as chronic diarrhea and malabsorption are widely recognized,however,many patients have atypical manifestations like iron deficiency anemia,idiopathic short stature,hypertransaminesemia or infertility,*** patients often present to the primary care physicians and/or non-gastrointestinal ***,due to a lack of awareness among the healthcare professionals about the various atypical manifestations,many patients are not screened for *** this review,we have summarized the available literature about the prevalence of CeD in various gastrointestinal(chronic diarrhea)and non-gastrointestinal conditions(iron deficiency anemia,short stature,cryptogenic hypertransaminesemia,cryptogenic cirrhosis or idiopathic ataxia etc.)where the diagnosis of CeD should be *** addition,we also discuss special scenarios where screening for CeD should be considered even in absence of symptoms such as patients with type 1 diabetes,Down’s syndrome,and first-degree relatives of patients with ***,we discuss the diagnostic performance and limitations of various screening tests for CeD such as IgA anti-tissue transglutaminase antibodies,antiendomysial antibodies and anti-deamidated gliadin *** on the current recommendations,we propose a diagnostic algorithm for patients with suspected CeD.

Improved Fracture Toughness of Cryorolled and Room Temperature Rolled 6082 Al Alloys

In the present work, 6082 Al alloy has been rolled to 40% and 70% thickness reductions at the cryogenic and room temperatures for the improvement in mechanical and fracture toughness properties. All cryorolled samples are subjected to aging at different temperatures, i.e., 140, 160, and 190 ℃ to improve the strength, ductility, and fracture toughness. The microstructures of the cryorolled （CR） and room temperature rolled （RTR） alloy after 40% and 70% thickness reductions are characterized by FE-SEM to reveal the modes of failure. The results show that the starting bulk Al alloy specimen is fractured in total ductile manner, consisting of well-developed dimples over the entire surface. The mechanical properties and fracture toughness of the 70% CR alloy are found better than 70% RTR alloy due to higher dislocations density and formation of sub-grain structures in the CR alloy.

In this research paper,a solar air heater with triangular fins has been experimentally analysed and ***,an experimental set-up of a solar air heater having triangular fins has been developed at the location of 28.10°N,78.23°*** heat transfer rate through fins and fins efficiency has been determined by the Finite Difference Method model *** experimental data and modeled data of response parameters have been optimized in MINITAB-17 software by the Response Surface Methodology *** creating the response surface design,three input parameters have been selected namely solar intensity,Reynolds number,and fin base-to-height *** range of solar intensity,Reynolds number,and fin base-to-height ratio is 600 to 1000W/m^(2),4000 to 6000,and 0.4 to 0.8 *** response surface design has been analyzed by calculating the outlet temperature,friction factor,Nusselt number,fin efficiency,thermal performance factor,and exergy *** optimum settings of input parameters:solar intensity is 1000 W/m^(2);Reynolds number is 4969.7,and the fin base to height ratio is 0.6060,on which these response:namely outlet temperature of 92.531℃,friction factor of 0.2350,Nusselt number of 127.761,thermal efficiency of 50.836%,thermal performance factor of 1.4947,and exergy efficiency of 8.762%.

The study presented in this paper is in continuation with the paper published by the authors on parallel fuzzy proportional plus fuzzy integral plus fuzzy derivative （FP ＋ FI ＋ FD） controller. It addresses the stability analysis of parallel FP ＋ FI ＋ FD controller. The famous＂small gain theorem＂ is used to study the bounded-input and bounded-output （BIBO） stability of the fuzzy controller. Sufficient BIBO-stability conditions are developed for parallel FP ＋ FI ＋ FD controller. FP ＋ FI ＋ FD controller is derived from the conventional parallel proportional plus integral plus derivative （PID） controller. The parallel FP ＋ FI ＋ FD controller is actually a nonlinear controller with variable gains. It shows much better set-point tracking, disturbance rejection and noise suppression for nonlinear processes as compared to conventional PID controller.

Outcome of repair of chronic tear of the pectoralis major using corkscrew suture anchors by box suture sliding technique

AIM To assess the functional and clinical results of repair of chronic tears of pectoralis major using corkscrew and sliding suture technique. METHODS In this retrospective study, we reviewed the results of pectoralis major repair in 11 chronic cases(> 6 wk) done between September 2011 and December 2014 at our institute. In all cases repair was done by same surgeon using corkscrew suture anchors and box suture sliding technique. At 6 mo, after surgery magnetic resonance imaging was done to see the integrity of the repair. Functional evaluation was done using Penn and ASES scores. Pre and postoperative Isokinetic strength was *** Average follow-up was 48.27 ± 21.0 mo. The Wilcoxon signed rank test was used to evaluate the outcome scores. The average ASES score increased from an average of 54.63 ± 13.0 preoperatively to 95.09 ± 2.60 after surgery at their last follow-up. The average Penn score also increased from 5.72 ± 0.78, 2.81 ± 1.32 and 45.81 ± 1.72 to 9.36 ± 0.80, 8.27 ± 0.90 and 59 ± 1.34 for pain, satisfaction and function respectively. Follow up magnetic resonance imaging(MRI)(at 6 mo) showed continuity and the bulk of pectoralis major muscle in all cases. Average isokinetic strength deficiency in horizontal adduction at 60° was 13.63% ± 6.93% and at 120° was 10.18% ± 4.93% and in flexion at 60° was 10.72% ± 5.08% and at 120° was 6.63% + 3.74%. Results showed that both ASES and Penn score improved significantly(2 tailed P value = 0.0036).CONCLUSION We could conclude from this series that pectoralis major repair even in chronic cases using 5.5 mm corkscrew anchors give excellent functional and cosmetic results. In chronic cases the repairable length of the tendon is not available and sliding suture technique allows for fixation of worn out tendomuscular junction to bone without letting cutting through the muscle.

Experiences with Rice Grown on Permanent Raised Beds: Effect of Crop Establishment Techniques on Water Use, Productivity, Profitability and Soil Physical Properties

In recent years, conventional rice production technologies have been leading to deterioration of soil health and declining farm profitability due to high inputs of water and labor. Conservation agriculture （CA） based resource-conserving technologies i.e. zero-tillage （ZT）, raised-bed planting and direct-seeded rice （DSR） have shown promise as alternatives to conventional production technologies to overcome these problems. Present study was undertaken during 2009-2012 to establish an understanding of how permanent raised bed cropping system could be practiced to save water at the field application level to improve water productivity and also have the capability to enhance productivity, profitability and soil physical quality. The results showed that among different crop establishment techniques, conventional-tilled puddle transplanted rice （CT-TPR） required 14%-25% more water than other techniques. Compared with the CT-TPR system, zero till direct-seeded rice （ZT-DSR） consumed 6%-10%less water with almost equal system productivity and demonstrated higher water productivity. Wide raised beds saved about 15%-24% water and grain yield decrease of about 8%. Direct-seeded rice after ZT or reduced tillage or on unpuddled soil provided more net income than CT-TPR. The CT-TPR system had higher bulk density and penetration resistance due to compaction caused by the repeated wet tillage in rice. The steady-state infiltration rate and soil aggregation （〉 0.25 mm） were higher under permanent beds and ZT and lower in the CT-TPR system. Under CT-TPR, soil aggregation was static across seasons, whereas it improved under no-till and permanent beds. Similarly, mean weight diameter of aggregates was higher under ZT and permanent beds and increased over time. The study reveals that to sustain the rice productivity, CA-based planting techniques can be more viable options. Background:The incidence of prostate cancer is increasing worldwide.A significant proportion of patients developmetastatic disease and are initially prescribed androgen deprivation therapy(ADT).However,subsequent sequences of treatments in real-world settings that may improve overall survival remain an area of active *** and methods:Data were collected from 384 patients presenting with de novo metastatic prostate cancer from 2011 to 2015 at a tertiary cancer *** were categorized into surviving(n=232)and deceased(n=152)groups at the end of 3 *** sequence pattern mining techniques(Generalized Sequential Pattern Mining and Sequential Pattern Discovery using Equivalence Classes)were applied to determine the exact order of the most frequent sets of treatments in each ***:Degarelix,as the initial form of ADT,was uniquely in the surviving *** sequence of ADT followed by abiraterone and docetaxel was uniquely associated with a higher 3-year overall *** followed by fosfestrol was found to have a unique niche among surviving patients with a long duration of response to the initial *** who received chemotherapy followed by radiotherapy and those who received radiotherapy followed by chemotherapy were found more frequently in the deceased ***:We identified unique treatment sequences among surviving and deceased patients at the end of 3 *** should be the preferred formof *** who received ADT followed by abiraterone and chemotherapy showed better *** requiring palliative radiation and chemotherapy in any sequence were significantlymore frequent in the deceased group,identifying the need to offer such patients the most efficacious agents and to target them in clinical trial design.
