Self-care actions for the maintenance of the arteriovenous fistula:An integrative review

Objective:To identify self-care actions for the maintenance of arteriovenous fistula of renal ***:An integrative review study was conducted and literature were searched in Medline/PubMed,Scopus,CINAHL LILACS,BdeNF and SciELO Library databases using the descriptors chronic renal insufficiency,arteriovenous fistula,self-care,and *** inclusion criteria were that the documents be written in Portuguese,English,and Spanish,full text available,published in the last five years,and that they address the research *** articles,theses,dissertations,editorials of nonscientific journals,and research studies that did not follow the necessary methodological rigor were *** were analyzed with the IRAMUJTEQ ***:Fifteen articles were selected and comprised the final *** classes of self-care actions emerged from the text segments analysis and grouped into three categories:(1)Self-care actions that maintain the arteriovenous fistula;(2)Self-care actions for the prevention and the monitoring of complications with arteriovenous fistula;(3)Self-care actions directed at the perioperative period of arteriovenous fistula ***:The results allowed us to identify important care for the maintenance of arteriovenous fistula *** self-care actions identified in this study can guide a nursing care policy for implementation with protocols that help identify problems related to self-care actions and,thus,subsidize the development of actions aimed at the renal ***,more studies with high levels of evidence that identify self-care actions with arteriovenous fistula and the factors involved in its implementation are needed.

Background: Suicide among physicians is a serious public health issue, with an extremely complex and multifactorial behavior. Aim: The aim of this study was to use the theme “Suicide among Physicians” to exemplify the analysis of methodological similarities between the scientific content available at MEDLINE and BVS databases, as scientific research tools. Methods: This is a systematic review with metanalysis. The following combinations of keywords were used for data search in the referred databases: “suicide” AND “physicians” AND “public heath”. Results: Three hundred and thirteen publications were identified, but only 16 studies were chosen. Great association was found between MEDLINE and BVS databases and the Odds Ratio regarding the theme: “Suicide among physicians”. Conclusions: Considering the similarities found in the utilization of the two analyzed databases, it was possible to identify that suicide among physicians is associated with the exercise of an important professional role in the society and in the workplace. The disease caused by the SARS-CoV-2 virus has spread very rapidly throughout the world, causing various clinical manifestations and affecting several organs and systems. However, the possibility of vertical transmission of COVID-19 in symptomatic pregnant women has been questioned. The aim of this work was to report cases of possible vertical transmission in pregnant women affected by the SARS-CoV-2 virus, as well as its clinical repercussions on the newborn. In this paper, we conducted a cross-sectional retrospective study by analyzing medical records of pregnant women admitted at the military hospital, in Rio de Janeiro, Brazil, diagnosed with COVID-19 and who had positive newborns. To evaluate epidemiological data, clinical manifestations, laboratorial and histopathological changes were related to such vertical transmission. Only six tested newborns were positive for COVID-19. The mothers had no respiratory symptoms, and the gestational age was at term, except for one newborn born prematurely due to detection of pericadial effusion. Three neonates had respiratory symptoms, requiring ventilatory support. None of the mothers or newborns died. During the pandemic, possible vertical transmission cases of infected pregnant women in the third trimester of pregnancy were observed. According to the case reports published in the literature, they illustrate phenotypes of neonatal involvement with varied clinical manifestations and severity;however, further scientific studies are needed to prove the effective vertical transmission of SARS-CoV-2 in positive pregnant women.

Enteroparasitoses in Patients with Rheumatoid Arthritis

Background: Patients with rheumatic diseases have profound alterations in the immune system as a result of underlying diseases and the treatments used, which increases the risk of occurrence and severity of infections, including enteroparasitosis. The current treatment for rheumatoid arthritis involves immunosuppressive therapies powerfully needed for screening infectious processes. The prevalence of parasitic infections in rheumatoid arthritis patients is not currently documented in literature. The objective of the current study was to determine the prevalence of parasitic diseases in a population with RA. Methods: We collected demographic and socioeconomic data from 67 patients at the Hospital Universitário de Brasília from July 2015 to April 2016. All patients underwent a parasitological examination of their stool and multiple variables were analyzed using Poisson regression method. Results: The mean age of patients was 53.9 years. They were predominantly in women (94%) and caucasian (47.8%). The mean disease duration was 9.2 years and most patients had the disease in remission or light activity. The prevalence of parasitic infections in these patients was 11.9%, all cases being that of protozoa of the following species: Endolimax nana, Entamoeba histolytica and Entamoeba coli. The final multivariate analysis indicated that the presence of disease had a significant statistical relationship between the presence of enteroparasitosis and rheumatoid arthritis with no fatigue by VAS (p = 0.0488) and best current health index by VAS (p = 0.0012). Conclusion: This study indicates that the prevalence of enteroparasitoses in a population with Rheumatoid Arthritis was 11.9% and all cases were found of protozoa.

Persistent Xerophthalmia in a Patient with Rheumatological Disease and Priiviary Sjogren’s Syndrome: Case Report from Northern Brazil

This case study aims to contribute to the literature in order to highlight the importance of this collaboration between medical specialties. A female patient R.N.N. F, age 66, from the city of Manaus, with a previous diagnosis of Sjogren’s syndrome in regular follow-up by the Rheumatology team at the Araujo lima outpatient clinic and referred to the Ophthalmology sector for complementary evaluation related to visual discomfort. The fundoscopy performed in the patient was within normal limits, but the symptoms experienced by her proved to be an important clinical finding, which has ratified the need for regular and multidisciplinary follow-up. This report unequivocally demonstrates that even in the face of tests considered within the expected limits for a given population, the clinical presentation can be specific and particular for each analyzed individual. Early screening exams should contemplate the patient in a holistic and individualized way whenever possible.

Cholelithiasis is a common disease in the general population. One of its risk factors is the type 2 diabetes mellitus, related to metabolic abnormalities associated to overweight, obesity, insulin resistance, hypertriglyceridemia and dietary habits. The main risk factors to cholelithiasis are genetic susceptibility, ileum resection, diabetes mellitus, obesity, hemolytic anemia, age, estrogens and cirrhosis. This study aims to make a correlation between diabetes mellitus and obesity versus prevalence of cholelithiasis in these patients.

Objective: As isolates of Cryptococcus are frequently kept collection stocks in institutions, sometimes without proper characterization, we sought to determine the genotype profiles, protease and phospholipase activities “in vitro” and the susceptibility testing for azoles and amphotericin B. Methodology: 84 isolates from several regions of Brazil (40 samples from clinical origin and 44 isolates from environmental origin) were maintained at the microorganism’s bank of the Biomedical Science Institute (ICB-USP) of the São Paulo University, in São Paulo, Brazil. This isolates was submitted fungal strains determination, DNA extraction and purification, determination of genotype by URA5 gene RFLP of CGB-positive isolates, protease and phospholipase activity and susceptibility to antifungals. Results: Of six CGB positive isolates tested by RFLP-PCR, only four presented a genomic profile consistent C. gattii species (VGII), while two other were C. neoformans (VNI and VNIII), indicating the existence of canavanine-resistante C. neoformans isolates in the culture collections. The clinical isolates secreted higher levels of phospholipase and environmental isolates but no differences were observed for the protease levels. Almost all isolates were sensible to azoles and amphotericin B. Conclusion: We point out in this research the existence of C. neoformans strains resistant to canavanine and intrinsic characteristic of C. gatti. These results demonstrate the importance to perform a detailed characterization of isolates kept in culture collections.

GTD (Gestational Trophoblastic Disease) is a pathology that encompasses benign and malignant clinical forms, affects women of childbearing age, has a variable incidence and is more frequent in developing or underdeveloped countries, colliding with the economic barrier. The frequent absence of clear protocols and guidelines for the correct diagnosis and treatment of the aforementioned pathology results in inadequate risk classification, imprecise treatment and failed post-therapeutic observation, increasing the risk of relapses, morbidity and mortality. The present study aims to compare the different national and international guidelines in the management of GTD, through an integrative review. Nine articles were selected and it was observed that the main international reference centers are agreed with the management suggested by the IFGO (International Federation of Gynecology and Obstetrics), being the conduct in the Hydatidiform Mole (HM): evacuation by suction and curettage under ultrasound guidance, followed by hCG monitoring every 1 - 2 weeks until normalized;in low-risk GTN (Gestational Trophoblastic Neoplasm): chemotherapy with methotrexate or actinomycin D, in high-risk: EMA/CO protocol, in ultra-high-risk EMA/PE, methotrexate with radiotherapy for brain metastases. All medical societies recommend the registration of these patients in GTD screening centers, endorse the use of the IFGO scoring system (2000) and recommend the surgical management of placental site trophoblastic or epithelioid tumors, as chemotherapy is less effective in these cases. The controversies are in the proper follow-up after the treatment of HM, use of ultrasound to evacuate the uterus, administration of anti-D immunoglobulin, time of oxytocin infusion and rescue regimens that can be used in cases of resistant or recurrent GTN.
