Extramammary Paget’s disease: Updates in the workup and management

Objective:Extramammary Paget’s disease(EmPD)is a rare cutaneous malignant *** to its rarity,there is a paucity of data regarding best treatment *** primarily affects apocrine gland-bearing skin areas such as the vulva,scrotum,and *** objective was to provide a present-day rationale for diagnosis,pathogenesis,and treatment of EmPD with a focus on recent progress in workup and management of the ***:Literature on EmPD until February 2022 was assessed through Pubmed,mEDLINE databases,and Google scholar.A narrative review of the most relevant articles was ***:EmPD usually presents with indolent growth while usually being diagnosed primarily as carcinoma in *** foundation of EmPD treatment centers around prompt and accurate diagnosis,wide local or mohs micrographic surgical excision with proper management towards the margin status,and careful consideration for lymphadenectomy in patients with regionally positive *** chemotherapies are alternative treatments modality for patients with distant metastases;however,they sometimes have suboptimal *** present,there is no agreement regarding adjuvant or systemic therapies,although recent studies have shown several insights into the molecular pathogenesis,tumor biology,and genomics of the development and advancement of EmPD,which may lead to novel and targeted treatment approaches for metastatic EmPD in the ***:Patients with EmPD should seek care from physicians with expertise in disease management and patient *** patients should be surveilled with close follow-up to evaluate them for disease recurrence or *** collaborations with groups such as the Global Society for Rare Genitourinary Tumors,and especially patient support groups are crucial in designing clinical trials to help elucidate more robust data in this orphan disease.

Influences of Freshwater from major Rivers on Global Ocean Circulation and Temperatures in the mIT Ocean General Circulation model

Responses of global ocean circulation and temperature to freshwater runoff from major rivers were studied by blocking regional runoff in the global ocean general circulation model （OGCm） developed at the massachusetts Institute of Technology. Runoff into the tropical Atlantic, the western North Pacific, and the Bay of Bengal and northern Arabian Sea were selectively blocked. The blocking of river runoff first resulted in a salinity increase near the river mouths （2 practical salinity units）. The saltier and, therefore, denser water was then transported to higher latitudes in the North Atlantic, North Pacific, and southern Indian Ocean by the mean currents. The subsequent density contrasts between northern and southern hemispheric oceans resulted in changes in major ocean currents. These anomalous ocean currents lead to significant temperature changes （I^C -2~C） by the resulting anomalous heat transports. The current and temperature anomalies created by the blocked river runoff propagated from one ocean basin to others via coastal and equatorial Kelvin waves. This study suggests that river runoff may be playing an important role in oceanic salinity, temperature, and circulations; and that partially or fully blocking major rivers to divert freshwater for societal purposes might significantly change ocean salinity, circulations, temperature, and atmospheric climate. Further studies are necessary to assess the role of river runoff in the coupled atmosphere-ocean system.

Spatial Variability of mass movements in the Satluj Valley,Himachal Pradesh during 1990～2006

Satluj Valley is known to have a history of landslides and related mass movement activities since the geological times. Geological and geomorphological settings combined with anthropogenic activities constitute a propensity towards slope failure. During the last two decades,the area witnessed substantial increase in athropogenic pressure,mainly due to the exploitation of hydropower potential,changing landuse pattern and population growth. In addition,a shift of the climatic patterns in the form of larger area falling under the influence of rains was observed. These natural as well as anthropogenic changes in the area have resulted in increased spatial coverage of landslide in the area. This paper documents these changes during 1990～2006.

How sensory neurons encode axonal injury signals has been a longstanding area of neurological *** into the cellular and transcriptional changes in the injured neurons have driven many new therapeutic strategies to improve *** contrast,less focus has been centered on the systemic or transneuronal changes that may arise from these injuries and how they may impact or factor into the alterations in gene expression,physiology and neuropathology,such as mirror image pain,arising in regions either directly contralateral or those remote to the injury *** in this area is clinically *** patients with chronic unilateral pain due to multiple pathologies,many of which involve nerve trauma,there is a very high incidence of bilateral sensory abnormalities reported with 33–50%of patients with mechanical abnormalities reporting contralateral sensory abnormalities(Konopka et al.,2012).This has implications when evaluating sensory abnormalities or pain states in patients,as the contralateral side is often used as the non-affected ***,the bilateral nature of these sensory abnormalities,albeit usually less severe on the contralateral side,support that a systemic or central nervous system component is involved in this aspect of the *** highlights the need to better understand the mechanisms underlying these bilateral responses as this will likely impact therapeutic treatments for these patients.

Gastrointestinal tuberculosis accounts for 3%of the extrapulmonary tuberculosis with ileocaecal region being the common site of involvement up to 75%.Primary involvement of appendix is very rare and accounts for only 0.6%to 2.9%of gastrointestinal tuberculosis in the absence tubercular focus *** pre-operative investigations usually give non-specific results. The diagnosis in most instances made only after *** we report a case of primary appendicular tuberculosis in a patient presented with caecal perforation.

Centrifuge modeling of PGD response of buried pipe

A new centrifuge based method for determining the response of continuous buried pipe to PGD is presented. The physical characteristics of the RPI's 100 g-ton geotechnical centrifuge and the current lifeline experiment split-box are described: The split-box contains the model pipeline and surrounding soil and is manufactured such that half can be offset, in flight, simulating PGD. In addition, governing similitude relations which allow one to determine the physical characteristics, (diameter, wall thickness and material modulus of elasticity) of the model pipeline are presented. Finally, recorded strains induced in two buried pipes with prototype diameters of 0.63 m and 0.95 m (24 and 36 inch) subject to 0.6 and 2.0 meters (2 and 6 feet) of full scale fault offsets and presented and compared to corresponding FE results.

The Potential Contribution of Wildlife Sanctuary to Forest Conservation: A Case Study from Binsar Wildlife Sanctuary

Forest vegetation of a protected area （Binsar Wildlife Sanctuary） in Kumaun region （west Himalaya） was analysed for structure, composition and representativeness across three different altitudinal belts, lower （1,6oo-1,8oo m a.s.1.）, middle （1,900-2,100 m a.s.1.） and upper （2,200-2,400 m a.s.1.） during 2oo9-2011 using standard phytosociological methods. Four aspects （east, west, north and south） in each altitudinal belt were chosen for sampling to depict maximum representation of vegetation in the sanctuary. Population structure and regeneration behaviour was analysed seasonally for two years to show the establishment and growth of tree species. A total of 147 plant species were recorded from the entire region of which 27 tree species were selected for detailed study. Highest number was recorded at upper （18 species）, and lowest at lower altitudinal belt （15 species）. The relative proportion of species richness showed higher contribution of tree layer at each altitudinal belt. The population structure, based on the number of individuals, revealed a greater proportion of seedling layer at each altitudinal belt. The relative proportion of seedlings increases significantly along altitudinal belts （p〈0.05） while opposite trends were observed in sapling and tree layers. The density of sapling and seedling species varied non-significantly across seasons （p〉0.05）. The density values decreased in summer and increased during rainy season. As far as the regeneration status is concerned, middle and upper altitudinal belts showed maximum number of species with fair regeneration as compared to lower altitudinal belt. Overall density diameter distribution of tree species showed highest species density and richness in the smallest girth class and decreased in the succeeding girth classes. This study suggests that patterns of regeneration behaviour would determine future structural and compositional changes in the forest communities. It is suggested that the compositional changes v

Spin–spin interactions in defects in solids from mixed all-electron and pseudopotential first-principles calculations

Understanding the quantum dynamics of spin defects and their coherence properties requires an accurate modeling of spinspin interaction in solids and molecules,for example by using spin Hamiltonians with parameters obtained from first principles *** present a real-space approach based on density functional theory for the calculation of spin-Hamiltonian parameters,where only selected atoms are treated at the all-electron level,while the rest of the system is described with the pseudopotential *** approach permits calculations for systems containing more than 1000 atoms,as demonstrated for defects in diamond and silicon *** show that only a small number of atoms surrounding the defect needs to be treated at the all-electron level,in order to obtain an overall all-electron accuracy for hyperfine and zero-field splitting *** also present results for coherence times,computed with the cluster correlation expansion method,highlighting the importance of accurate spin-Hamiltonian parameters for quantitative predictions of spin dynamics.
