Epidemiological characteristics of childhood migraine syndrome in Serbia

This study investigated childhood migraine, examining the clinical characteristics of different childhood migraine types, predisposing factors, and possible medical treatments in a sample of children in Serbia. This was the first epidemiological study in Serbia using the operational diagnostic criteria of the International Headache Society. The present study examined 30 636 children between 1988 and 2006, including 15 434 （50.38%） males and 15 202 （49.62%） females, in nine towns in Vojvodina, a northern province in Serbia. Migraine was reported in 2 644 （8.63%） children aged 3 to 7 years, consisting of 1 189 （8.0%） males and 1 455 （9.6%） females. Migraine with aura accounted for 25.55%, migraine without aura for 67.21% and other migraine syndromes for 7.23% of reported migraine sufferers. The mean age of first migraine attack across groups was 5 years and 1.8 months. Migraine with aura was associated with an earlier onset than migraine without aura Migraine was more frequent among younger groups of children who were breastfed for a shorter period of time. Child healthcare in Vojvodina has a high level of specialization, with more than half of the children reporting migraine diagnosed and treated by neuropediatricians. Socioeconomic status, duration of breastfeeding, birth order, and age of entering full-day nursery care were found to be important factors for childhood migraine. These findings suggest that healthcare providers should educate parents about these predisposing factors, so they can be identified and avoided early.

The process of designing roundabouts is an iterative process through which, in several checks, the design elements of a roundabout get optimized. Existing regulations for roundabouts involve swept path analyses, sight distance analyses and speed analyses of vehicles passing through the roundabout. Speed analyses are done mostly based on two models, Dutch and American. Each of these two models, in their own way takes into account design elements of the roundabouts, and the US model also envisions the construction of vehicle paths through the roundabout. Main assumption of both models is that vehicle paths through roundabouts consist of few connected radii. US models for path definition takes into account safety distances from marked lines and geometric elements（curbs） at the entrance and exit and through roundabout. Experimentally determined elements of the vehicle path through the roundabout, do not correspond to those recommendations. Comparison of the measured speed at the roundabouts and speed calculated according to aforementioned models at several roundabouts in Croatia, showed a significant difference. An experimental research was conducted as a first step in developing a new model for operating speed through roundabouts. The research aimed to define the basic path elements of vehicle movement in the roundabout at which the maximum speed is achieved. Results of the study are presented in this paper.

AIM： To evaluate glutathione transferase theta 1 and mu 1（GSTT1 and GSTM1） polymorphisms as determinants of primary open angle glaucoma（POAG） risk, independently or in combination with cigarette smoking, hypertension and diabetes ***： A case-control study with 102 POAG patients and 202 age and gender-matched controls was carried out. Multiplex-polymerase chain reaction method was used for the analysis of GSTM1 and GSTT1 polymorphisms. The differences between two groups were tested by the t-test or χ~2 test. Logistic regression analysis was used for assessing the risk for disease ***： The presence of GSTM1-null genotype did not contribute independently towards the risk of POAG. However, individuals with GSTT1-active genotype were at almost two-fold increased risk to develop glaucoma（P=0.044） which increased up to 4.36 when combined with GSTM1-null carriers（P=0.024）. When glutathione transferase（GST） genotypes were analyzed in association with cigarette smoking, hypertension and diabetes, only carriers of GSTT1-active genotype had significantly increased risk of POAG development in comparison with GSTT1-null genotype individuals with no history of smoking, hypertension and diabetes, respectively （OR=3.52, P=0.003; OR=10.02, P〈0.001; OR=4.53, P=0.002）. CONCLUSION： The results obtained indicate that both GSTM1-null and GSTT1-active genotypes are associated with increased POAG risk among smokers, suggesting potential gene-environment interaction in glaucoma development.

AIM: To study the efficacy of tenofovir disoproxil fumarate (TDF) at low dose in a small open trial of chronic hepatitis B patients with advanced stage disease. METHODS: Eleven patients were treated with TDF 75 mg for a median period of 80 (range, 24-576) wk and then 7 cases were shifted to an adefovir 10 mg treatment group. All patients had been pre-treated with lamivudine: 5 had YMDD resistant mutants and 6 wild- type virus. When TDF was started, 4 patients had low- level viremia and 6 were PCR-negative. RESULTS: During TDF treatment, PCR remained negative in 10 patients, transaminase levels were normal and no significant viral breakthrough was observed. The drug was well tolerated in all cases. When TDF 75 mg was substituted with adefovir 10 mg, 3 out of 7 patients had a persistent viral rebound (2700-130 000 copies/mL), in whom lamivudine had to be reintroduced. CONCLUSION: Low-dose TDF monotherapy can control HBV viremia for an extended period of time without the emergence of resistance and is more potent than adefovir at the standard dosage. The use of a reduced dose of TDF could diminish the cost of therapy in low-income countries, but further studies in a larger population and in HBeAg-positive subjects are needed.

Detection of gelatinase B activity in serum of gastric cancer patients

AIM： To determine the proteolytic activity and expression of gelatinase B in serum of gastric cancer patients and their correlation with the stage of the tumor. METHODS： Sera from 23 patients who underwent surgery for primary gastric cancer as the experimental group and from 11 as the control group were used to determine the proteolytic activity and its inhibition by EDTA and 1,10-phenanthroline. Gelatinase B activity was detected by SDS polyacrylamide gel electrophoresis （SDS-PAGE） and SDS-PAGE zymography. RESULTS： Proteolytic enzyme activity was increased in gastric cancer patients when compared to the control group （,0〈0.05）. The proteinases were determined to be metalloproteinases upon inhibition test with specific metalloproteinase inhibitors 1,10-phenanthroline （P〈0.05） and EDTA （P〈0.01）. SDS-PAGE and SDS-PAGE zymography revealed gelatinase B （proMMP-9） activity and its molecular mass of 92 ku. CONCLUSION： Proteinase activity is overexpressed in serum of gastric cancer patients. AIM：To translate the Glaucoma Quality of Life-15（GQL-15）to Serbian language and asses its validity and reliability in the population of Serbian ***：The study included 177 glaucoma *** parameters（visual acuity,mean defect and square root of loss variance of visual field）and socio-demographic data were *** were stratified according to the Nelson’s glaucoma staging system as mild,moderate and advanced *** patients filled out the GQL-15 and National Eye Institute Visual Function Questionnaire（NEI-VFQ 25）.The GQL-15 was translated following the internationally-accepted methodology,and its psychometric properties were assessed by using classical test theory and Rasch ***：The mean total score for the GQL-15 was 20.68±*** Cronbach’s alpha coefficient for the whole scale was 0.89（central and near vision,α=0.24;peripheral vision,α=0.85;glare and dark adaptation,α=0.83）.Factor analysis established 4 factors（70.3% of variance）：two corresponding to the original factors and two new factors specific for the Serbian *** GQL-15 score correlated positively with almost all clinical parameters and NEI-VFQ 25 proving good criterion *** of the GQL-15 total score on test-retest confirmed appropriate scale reproducibility（ρ=0.96,P〈0.001）.The GQL-15 discriminated well advanced from mild and moderate *** Rasch analysis we obtained adequate item（0.95）reliability *** all items had infit and outfit mean squares in the accepted ***：Serbian version of the GQL-15 demonstrates adequate reliability and *** version of the GQL-15is a valid instrument for evaluation of quality of life among Serbian speaking patients with glaucoma and can be applied in daily clinical work.
