Pouchitis is not a rare complication that develops after an ileal-pouch anastomosis, performed after colectomy in patients refractory to treatment or with complicated ulcerative colitis. This condition may become chronic and unresponsive to medical therapies, including corticosteroids, antibiotics and probiotics. The advent of biological therapies(tumor necrosis factor-α inhibitors) has changed the course of these complications. In particular, in these cases, infliximab(IFX) may represent a safe and effective therapy in order to avoid the subsequent operation for a permanent ileostomy. This article reviews the therapeutic effects of one of the most widely used anti-tumor necrosis factor-α molecules, IFX, for the treatment of complicated pouchitis(refractory to conventional treatment and/or fistulizing).

AIM: To evaluate the prevalence of lactose intolerance (LI) following a load of 12.5 g in patients diagnosed as high-grade malabsorbers using the hydrogen breath test (HBT)-***: Ninety patients showing high-grade malabsorption at HBT-25 were submitted to a second HBT with a lactose load of 12.5 *** hydrogen production,area under the curve of hydrogen excretion and occurrence of symptoms were ***: Only 16 patients (17.77%) with positive HBT-25 proved positive at *** production was lower as compared to HBT-25 (peak value 21.55 parts per million (ppm) ± 29.54 SD vs 99.43 ppm ± 40.01 SD; P < 0.001).Symptoms were present in only 13 *** absence of symptoms during the high-dose test has a high negative predictive value (0.84) for a negative low-dose *** presence of symptoms during the first test was not useful for predicting a positive low-dose test (positive predictive value 0.06-0.31).CONCLUSION: Most patients with a positive HBT-25 normally absorb a lower dose of lactose and a strict lactose restriction on the basis of a "standard" HBT is,in most instances,***,the 25 g lactose tolerance test should probably be substituted by the 12.5 g test in the diagnosis of LI,and in providing dietary guidelines to patients with suspected lactose malabsorption/intolerance.

AIM:To investigate prevalence,type and time of onset of extraintestinal manifestations(EIMs)in a series of Italian inflammatory bowel disease(IBD)***:Data of 811 IBD consecutive patients,first referred to our Centre from 2000 to 2011,were retrospectively ***:Eight hundred and eleven IBD patients(437M,374 F)were studied:595 ulcerative colitis(UC)(73.4%)and 216 Crohn’s disease(CD)(26.6%).Among these,329(40.6%)showed EIMs:210 UC(35.3%)and119 CD(55.1%)(P<0.0001).Considering the time of the diagnosis of IBD,37 EIMs(11.2%)were developed before,229(69.6%)after and 63(19.2%)were *** type of EIM were as follows:240 musculoskeletal(29.6%),in 72 CD patients and in 168 UC(P<0.0001);47 mucocutaneous(5.8%),in 26 CD and in21 UC(P=0.0049);26 ocular(3.2%),in 16 CD and in10 UC(CD 7.4%vs UC 1.7%,P=0.0093);6 hepatobiliary(0.8%);10 endocrinological(1.2%).In particular,with regards to the involvement of the musculoskeletal system,arthritis Type 1 was found in 41 CD(19%)and in 61 UC(10.2%)(P=0.0012)and Type 2 in 25 CD(11.6%)and in 100 UC(16.8%)(P=0.0012).CONCLUSION:Mucocutaneous manifestations,arthritis Type 1 and uveitis were significantly more frequent in CD than *** complications of the musculoskeletal system were the mostly observed ones,often with symptoms more severe than intestinal ones,confirming the need for close cooperation with rheumatologists.

Objective To investigate the role and molecular mechanism of exosomal miR-224-5p in colorectal cancer(CRC).Methods The miR-224-5p expression in CRC patient tissues and cell-derived exosomes was measured by laser capture microdissection and qRT-PCR,***-luciferase reporter gene assay was used to determine the target gene of *** protein expressions of p53 and unc-51 like kinase 2(ULK2)in CRC cells were detected by western *** cytometry was used to detect cell cycle and *** proliferation was measured by CCK8 and EdU *** The miR-224-5p expression was upregulated in CRC tissues and increased progressively with the rise of CRC *** cells secreted extracellular miR-224-5p mainly in an exosome-dependent manner,and then miR-224-5p could be transferred to surrounding tumor cells to regulate cell proliferation in the form of autocrine or ***,ULK2 was characterized as a direct target of miR-224-5p and was downregulated in CRC ***,ULK2 inhibited CRC cell proliferation in a p53-dependent ***,exosome-derived miR-224-5p partially reversed the proliferation regulation of ULK2 on CRC *** Our findings demonstrate that exosome-transmitted miR-224-5p promotes p53-dependent cell proliferation by targeting ULK2 in CRC,which may offer promising targets for CRC prevention and therapy.

Harmful algal blooms are important threats to reservoir *** a 15-year period,we sampled five Argentinian reservoirs to identify the responsible species for harmful algal blooms and determine the water quality factors driving their *** reservoirs exhibit diverse morphological and hydrological characteristics and are distributed across varying climatic *** blooms included an array of species,including toxin producers such as Raphidiopsis raciborskii,Aphanizomenon gracile,and Microcystis *** contrast,dinoflagellate blooms were predominantly characterized by *** cyanobacterial blooms primarily occurred in the shallower reservoirs located in warmer regions,whereas dinoflagellate blooms occurred where temperatures were *** most intense blooms occurred during the summer,and although cyanobacterial and chlorophyte blooms co-occurred,they never coexisted with dinoflagellate *** identified cyanotoxins in the reservoirs over the past three years,a phenomenon previously unreported in the *** that these reservoirs are drinking water sources for human populations,ongoing and systematic monitoring is needed to protect public *** the proliferation of algae and cyanobacteria in reservoirs requires resource management at the watershed level,and in the case of the Itiyuro and El Limon,requires binational management with Bolivia.

In the last years,axonal conductance of action potential trains became a novel subject of study,changing the view of axons,from a static cable-like compartment to a more complex and dynamic system(Debanne et al.,2011).Axonal computation,indeed,is canonically constituted by the action of voltage-gated ion channels,such as the classic Na+and K+channels,but recent studies demonstrated that it can be modulated by the action of other ion channel pumps,and metabolic factors(Byczkowicz et al.,2019;Zang and Marder.

Dogs engage in play behavior at every age and the play bow is their most iconic playful ***,the function of this posture is still under ***,we selected the Czechoslovakian Wolfdog(CWD)as a model breed to clarify the function of the play *** analyzed frame-by-frame 118 sessions of 24 subjects and recorded 76 play bow *** found that all the play bows were performed in the visual field of the playmate suggesting that the sender takes into account the attentional state of the receiver when releasing the *** drawing survival curves and using log-rank test we found that play bow was mainly performed during a short pause in an ongoing session and that its performance triggered the playmate’s reaction *** findings show that play bow functions in restoring the partner motivation to ***,by using a sequential analysis and a generalized mixed model,we found no evidence supporting the metacommunicative function of the play *** signal did not necessarily precede a contact offensive behavior(e.g.,play biting and play pushing)and it was not affected by the level of asymmetry of the play *** conclusion,in CWDs play bow can be considered a visual signal useful to maintain the motivation to play in the ***,we suggest that the mismatched number of play bows emitted by the 2 players in a given session can be predictive of their different motivations to play.

Let L be a pseudo-D-lattice. We prove that the exhaustive lattice uniformities on L which makes the operations of L uniformly continuous form a Boolean algebra isomorphic to the centre of a suitable complete pseudo-D-lattice associated to L. As a consequence, we obtain decomposition theorems such as Lebesgue and Hewitt-Yosida decompositions--and control theorems such as Bartle-Dunford Schwartz and Rybakov theorems--for modular measures on L.
