Frequent Applications of Organic Matter to Agricultural Soil Increase Fungistasis

Soil-borne plant pathogens are among the most important limiting factors for the productivity of agro-ecosystems. Fungistasis is the natural capability of soils to inhibit the germination and growth of soil-borne fungi in the presence of optimal abiotic conditions. The objective of this study was to assess the effects of different soil managements, in terms of soil amendment types and frequency of application, on fungistasis. For this purpose, a microcosm experiment was performed by conditioning a soil with frequent applications of organic matter with contrasting biochemical quality （i. e., glucose, alfalfa straw and wheat straw）. Thereafter, the fungistasis response was assessed on four fungi （Aspergillus niger, Botrytis cinerea, Pyrenoehaeta lycopersici and Trichoderma harzianum）. Conditioned soils were characterized by measuring microbial activity （soil respiration） and functional diversity using the BIOLOG EcoPlatesTM method. Results showed that irrespective of the fungal species and amendment types, frequent applications of organic matter reduced fungistasis relief and shortened the time required for fungistasis restoration. The frequent addition of easily decomposable organic compounds enhanced soil respiration and its specific catabolic capabilities. This study demonstrated that frequent applications of organic matter affected soil fungistasis likely as a result of higher microbial activity and functional diversity.

Hepatocellular carcinoma and synchronous liver metastases from colorectal cancer in cirrhosis:A case report

A 68-year-old Caucasian man with hepatitis C virus- related cirrhosis was admitted to our Unit in February 2010 for a diagnostic evaluation of three centimetric hypoechoic focal liver lesions detected by regular sur- veillance ultrasound. The subsequent computer tomog- raphy(CT) led to a diagnosis of unifocal hepatocellular carcinoma(HCC) in Ⅵ hepatic segment, defined the other two nodules in the Ⅵ and Ⅶ segment as sus- pected metastases, and showed a luminal narrowing with marked segmental circumferential thickening of the hepatic flexure of the colon. Colonoscopy detected an ulcerated, bleeding and stricturing lesion at the hepatic flexure, which was subsequently defined as ad- enocarcinoma with a moderate degree of differentiation at histological examination. Finally, ultrasound-guided liver biopsy of the three focal liver lesions confirmed the diagnosis of HCC for the nodule in the Ⅵ segment, and characterized the other two lesions as metastases from colorectal cancer. The patient underwent laparo- tomic right hemicolectomy with removal of thirty-nine regional lymph nodes(three of them tested positive for metastasis at histological examination), and simulta- neous laparotomic radio-frequency ablation of both nodule of HCC and metastases. The option of adju- vant chemotherapy was excluded because of the post- surgical onset of ascites. Abdomen CT and positron emission tomography/CT scans performed after 1, 6 and 12 mo highlighted a complete response to treat- ments without any radiotracer accumulation. After 18 mo, the patient died due to progressive liver failure. Our experience emphasizes the potential coexistence of two different neoplasms in a cirrhotic liver and the complexity in the proper diagnosis and management of the two tumours.

Retrospective descriptive analysis of the physiological kinetics of prostate-specific antigen in men older than 75 years

Several studies have compared prostate-specific antigen （PSA） kinetics in men with and without cancer, but there has been no adequate analysis of the longitudinal variation in PSA. The aim of this study was to assess the fluctuations in PSA in a cohort of elderly men in an attempt to define a physiological pattern of PSA kinetics. We searched a specific cohort of patients aged 〉 75 years and with PSA value 〈 2.0 ng mL^-1. A history of all PSA values over the past 10 years was compiled for each patient to create a database of patients fitting the following criteria：（1） minimum of five PSA measurements, （2） over at least 5 years. Exclusion criteria were：（1） PSA 〈 0.2 ng mL^-1 at each measurement and （2） having had more than one PSA test per year. In all, 1 327 patients （mean age： 78.52 years） fit the inclusion criteria. The mean variation from the first to the last PSA test was 0.05 ± 0.43, with a mean follow-up of 6.79 ± 1.71 years. Over the same period, the mean fluctuation from the lowest to the highest PSA value was 0.04 ± 0.55 （P = 0.925）. The mean annual PSA velocity （PSAV） was calculated by dividing the mean variation from the first to the last PSA test by the number of years of observation for each patient and was set at 0.0104 ± 0.1050. Concluding, in a large-scale cohort of elderly individuals considered healthy and evaluated for a considerable follow-up, the average annual PSAV as well as the average fluctuation from the lowest to the highest PSA value are insignificant.

Focus on gastroesophageal reflux disease in patients with cystic fibrosis

Gastroesophageal reflux disease(GERD) is a common gastrointestinal disorder in cystic fibrosis(CF), and based on various studies, its prevalence is elevated since childhood. There are several pathogenetic mechanisms on the basis of association between CF and GERD. However, there are no specific guidelines for GERD in CF patients, so diagnosis is based on guidelines performed on patients not affected by CF. The aim of this review is to provide the pathophysiology, diagnostic and therapeutic options, complications, and future directions in the management of GERD patients with CF.

Markers of systemic inflammation and colorectal adenoma risk: Meta-analysis of observational studies

To perform a meta-analysis of observational studies on inflammatory markers levels and occurrence of colorectal *** and EMBASE databases were searched until March 2016 for the articles reporting on the circulating levels of inflammatory markers, including: C-reactive protein (CRP), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-α) and risk of colorectal adenoma. Random-effects models were used to calculate summary odds ratios (ORs) with 95%CIs for the highest vs lowest category of exposure. Heterogeneity was assessed by using the Q test and I2 statistic. Subgroup analyses were also performed to test for potential source of *** total of 14 case-control studies were included. Ten studies on CRP including a total of 3350 cases and 4168 controls showed non-significant summary (OR = 1.23, 95%CI: 0.98-1.54; I2 = 54%, Pheterogeneity = 0.01) in the general analysis, but significant increased odds when considering only advanced adenoma (OR = 1.59, 95%CI: 1.09-2.32; I2 = 44%, Pheterogeneity = 0.15). Subgroup and stratified analyses revealed a potential influence of smoking status and aspirin use on the association between CRP levels and colorectal adenoma. Five studies examined the association between circulating levels of TNF-α and colorectal adenoma risk, including a total of 1,568 cases and 2,832 controls. The summary OR for the highest vs the lowest category of exposure was 1.00 (95%CI: 0.77-1.29). The relationship between circulating IL-6 levels and colorectal adenoma risk was investigated in 7 studies including a total of 1936 cases and 3611 controls. The summary OR for the highest vs the lowest category of exposure was 1.19 (95%CI: 0.92-1.55).CONCLUSIONSummary of current evidence suggests a positive association of CRP levels and advanced colorectal adenoma risk. The role of potential confounding factors should be further evaluated.

Heart rate variability （HRV） refers to the variations between consecutive heartbeats, which depend on the continuous modulation of the sympathetic and parasympathetic branches of the autonomic nervous system. HRV has been shown to be effective as a predictor of risk after myocardial infarction and an early warning sign of diabetic neuropathy, and in the cardiology setting is now recognized to be a useful tool for risk-stratification after hospital admission and after discharge. Recent evidences suggest that HRV analysis might predict complications even in patients undergoing cardiac surgery, and the present review summarizes the importance of HRV analysis in adult cardiac surgery and the perspectives for HRV use in current clinical practice. Although future larger studies are warranted before HRV can be included into daily clinical practice in adult cardiac surgery, HRV is a novel tool which might detect autonomic instability in the early postoperative phase and during hospital stay, thus predicting or prompt-diagnosing many of the post-operative complications.
