Assessment of quality control system by sigma metrics and quality goal index ratio: A roadmap towards preparation for NABL

AIM To study sigma metrics and quality goal index ratio(QGI). METHODS The retrospective study was conducted at the Clinical Biochemistry Laboratory, PGIMS, Rohtak, which recently became a National Accreditation Board for Testing and Calibration of Laboratories accredited lab as per the International Organization for Standardization 15189:2012 and provides service to a > 1700-bed tertiary care hospital. Data of 16 analytes was extracted over a period of one year from January 2017 to December 2017 for calculation of precision, accuracy, sigma metrics, total error, and QGI. RESULTS The average coefficient of variation ranged from 2.12%(albumin) to 5.42%(creatinine) for level 2 internal quality control and 2%(albumin) to 3.62%(high density lipoprotein-cholesterol) for level 3 internal quality control. Average coefficient of variation of all the parameters was below 5%, reflecting very good precision. The sigma metrics for level 2 indicated that 11(68.5%) of the 16 parameters fall short of meeting Six Sigma quality performance. Of these, five failed to meet minimum sigma quality performance with metrics less than 3, and another six just met minimal acceptable performance with sigma metrics between 3 and 6. For level 3, the data collected indicated eight(50%) of the parameters did not achieve Six Sigma quality performance, out of which three had metrics less than 3, and five had metrics between 3 and 6. QGI ratio indicated that the main problem was inaccuracy in the case of total cholesterol, aspartate transaminase, and alanine transaminase(QGI > 1.2), imprecision in the case of urea(QGI < 0.8), and both imprecision and inaccuracy for *** On the basis of sigma metrics and QGI, it may be concluded that the Clinical Biochemistry Laboratory, PGIMS, Rohtak was able to achieve satisfactory results with world class performance for many analytes one year preceding the accreditation by the National Accreditation Board for Testing and Calibration of Laboratories. Is nitric oxide level affected in postmenopausal women with hypothyroidism?

AIMTo analyze serum levels of nitric oxide （NO）, an indicator of cardiovascular health, in post-menopausal females with and without hypothyroidism. METHODSNO was analyzed colorimetrically in 30 newly diag-nosed hypothyroid postmenopausal females and 30 postmenopausal females with normal thyroid profle. Results were compared and subjected to appropriate statistical analysis. RESULTSThe levels of serum NO were found to be significantlydecreased in postmenopausal females with hypothyroidismas compared to the levels in those with normal thyroidprofile （P value 〈 0.05）. A negative correlation of NOwas observed with thyroid stimulating hormone whereasa positive correlation of NO was observed with free T3 （FT3）, free T4 （FT4）, T3 and T4 though it was statistically signifcant only for FT4 among postmenopausal females with *** hypothyroid females may be at a risk of compromised cardiovascular health as indicated by low NO levels. Regular monitoring and risk assessment is essential for timely intervention.

Sixty out of 75(80%) married males continued to have sex even after knowing their HIV positive *** male and six female spouses did not get infected even after multiple insertive unprotected *** average life span after the diagnosis of HIV infection in Indian males without specific treatment was 4.5±3. 5 yr while in women it was 3.5±2.0 yr,indicating fast progression of AIDS in females,most probably due to gender bias in access to treatment,nutrition,care and also due to hormonal *** tuberculosis was most common and first clinical presentation of HIV associated opportunistic *** study also showed that due to ignorance majority of husbands do not share their HIV positive status with their wives and continue to have unsafe sex.

Comparative and quantitative analysis of antioxidant and scavenging potential of Indigofera tinctoria Linn. extracts

Levels of neutrophil gelatinase-assosciated lipocalin in patients with head and neck squamous cell carcinoma in Indian population from Haryana state

AIM To study the levels of neutrophil gelatinase associated lipocalin(NGAL) in head and neck squamous cell carcinoma(HNSCC).METHODS This was a non randomized case control study conducted at Department of Biochemistry,in collaboration with Regional Cancer Center over a period of one *** study population included 50 adult newly diagnosed HNSCC patients reporting in outpatient department at Regional Cancer Center and compared with 50 healthy *** was estimated by ELISA *** t test and χ~2 test were applied for comparison of means of study *** between groups were analyzed using Pearson correlation coefficient(r) *** Patients with HNSCC exhibited significantly increased levels of NGAL(P < 0.05) as compared to healthy controls(978.88 ± 261.39 ng/mL vs 34.83 ± 7.59 ng/mL).Out of 50,26 patients(52%) were in stage Ⅳ,21(42%) in stage Ⅲ,1(2%) patient in stage Ⅱ and 2(4%) patients were in stage Ⅰ.Metastasis was absent in 98% patients and mean NGAL levels were highest in these patients but P value was not *** NGAL levels were highest in stage Ⅳ [1041.54 ± 222.15 ng/mL(stage Ⅳ) vs 1040 ± 0.00 ng/mL(stage Ⅰ);900 ± 0.00 ng/mL(stage Ⅱ) and 1031.90 ± 202.55 ng/mL(stage Ⅲ)] and χ~2 test was highly significant(P < 0.001).Thirty-six patients(72%) were having moderately differentiated HNSCC and mean NGAL levels were maximum in patients with well differentiated HNSCC(1164 ± 315.64 ng/mL vs 1013.33 ± 161.19 ng/mL in moderately differentiated and 890 ± 11.55 ng/mL in poorly differentiated) and the results were also highly significant(P < 0.001,χ~2 test).CONCLUSION The present work demonstrates a potential role of NGAL as cancer biomarker and its use in monitoring the HNSCC progression.

Study of temporal and spectral characteristics of the X-ray emission from solar flares

Temporal and spectral characteristics of X-ray emission from 60 flares of intensity ≥C class observed by the Solar X-ray Spectrometer （SOXS） during 2003-2011 are presented. We analyze the X-ray emission observed in four and three energy bands by the Si and Cadmium-Zinc-Telluride （CZT） detectors, respectively. The number of peaks in the intensity profile of the flares varies between 1 and 3. We find moderate correlation （R ~=0.2） between the rise time and the peak flux of the first peak of the flare irrespective of energy band, which is indicative of its energy-independent nature. Moreover, the magnetic field complexity of the flaring region is found to be highly anti-correlated （R = 0.61） with the rise time of the flares while positively correlated （R = 0.28） with the peak flux of the flare. The time delay between the peak of the X-ray emission in a given energy band and that in 25-30keV decreases with increasing energy, suggesting conduction cooling is dominant in the lower energies. Analysis of 340 spectra from 14 flares reveals that the peak of differential emission measure （DEM） evolution is delayed by 60-360 s relative to that of the temperature, and this time delay is inversely proportional to the peak flux of the flare. We conclude that temporal and intensity characteristics of flares are dependent on energy as well as the magnetic field configuration of the active region.

Corrosion of Snails (Gastropods) in Acidic Environment and Their Protection

Snails’ protection is essential because this species is to maintain a balanced ecology of water sources. They occur in rivers as well as ponds and balance the pH level of water. But these sources of water are contaminated by effluents, pollutants, acid rain, particulates, biological wastes etc. They can change the pH of water. Water is absorber of carbon dioxide and it converts carbon dioxide into carbonic. Other above mentioned wastes also increase the concentration of H+ ions in water. They produce hostile environment for snails. The outer part of snails is made of CaCO3. It produces chemical reaction in acidic medium and corrosion reaction is accelerated, and thus deterioration starts on the surface of snails. This medium makes their survival become miserable. For this work, corrosion of the snails’ study in the pH values of water is 6.5 in H2CO3 environment. The corrosion rates of snails were calculated by gravimetric methods and potentiostat technique. Aloe Vera was used for corrosion protection in acidic medium. The surface adsorption phenomenon was studied by Langmuir isotherm. Aloe Vera formed thin surface film on the interface of snails which adhered with chemical bonding. It was confirmed by activation energy, heat of adsorption, free energy, enthalpy and entropy. The results of surface coverage area and inhibitors efficiency indicated that Aloe Vera developed a strong protective barrier in the acidic medium.

Primary biliary cirrhosis in India

BACKGROUND: Primary biliary cirrhosis (PBC) is a rare cause of chronic liver disease in India. We analyzed the clinical, biochemical, serological and histological features of patients with PBC for over a 10-year period. METHODS: PBC was diagnosed by the presence of raised level of serum alkaline phosphatase (ALP), anti-mitochondrial antibody (AMA) positivity (1:40 dilution), and/or diagnostic liver histology. RESULTS:Fifteen female patients with mean age of 46.5±11 years were studied. Pruritis (80%) followed by jaundice (67%), skin changes (pigmentation, coarsening, xanthelesma and vitiligo) (67%) and fatigue (60%) were common symptoms. The mean duration of the symptoms was 3.5± 5.4 years (3 months to 20 years). Dryness of eyes was observed in only 2 patients. Hepatomegaly was noted in 87% of the patients and ascites at presentation in 40%. Mean levels of bilirubin and albumin at the time of diagnosis were 3.4±3.3 mg/dl and 3.5±0.8 g/dl, respectively. The level of serum ALP ranged from 54 to 2400 IU/L, with a median being 552 IU/L (2×ULN). In all the 15 patients with AMA positive, 8(53%) were also positive for either anti-nuclear or anti-smooth muscle antibodies. Two patients presented with persistently elevated SAP after an acute hepatitic illness. Liver biopsy was available in 13 patients, diagnostic of PBC Ⅱ & Ⅲ(8) and with evidence of cirrhosis (5). Associated autoimmune disorders were observed in 5 patients (33%). The mean time for follow-up was 26±21 months (1 to 87 months). In 4 deaths, 3 were due to liver related causes. CONCLUSION: PBC is a rare cause of chronic liver disease in India. PBC in India, unlike in the West, presents late, often with features of cirrhosis and decompensation.
