Intussusception of the bowel in adults:A review

Intussusception of the bowel is defined as the telescop-ing of a proximal segment of the gastrointestinal tract within the lumen of the adjacent segment.This condi-tion is frequent in children and presents with the classic triad of cramping abdominal pain,bloody diarrhea and a palpable tender mass.However,bowel intussusception in adults is considered a rare condition,accounting for 5% of all cases of intussusceptions and almost 1%-5%of bowel obstruction.Eight to twenty percent of cases are idiopathic,without a lead point lesion.Secondary intus-susception is caused by organic lesions,such as inflam-matory bowel disease,postoperative adhesions,Meckel's diverticulum,benign and malignant lesions,metastatic neoplasms or even iatrogenically,due to the presence of intestinal tubes,jejunostomy feeding tubes or after gas-tric surgery.Computed tomography is the most sensitive diagnostic modality and can distinguish between intus-susceptions with and without a lead point.Surgery is the definitive treatment of adult intussusceptions.Formal bowel resection with oncological principles is followed for every case where a malignancy is suspected.Reduction of the intussuscepted bowel is considered safe for benign lesions in order to limit the extent of resection or to avoid the short bowel syndrome in certain circumstances.

In the last few years we have witnessed a vast expansion of our knowledge regarding the molecular and genetic profile of gastric cancer.The molecular subtypes described have shed light on the pathogenesis of the disease,thus prompting the development of new therapeutic strategies and favoring a more individualized approach for treatment.Most of the clinical trials for so called targeted therapies could be considered,at best,partially successful.In addition,checkpoint inhibitors ha-ve recently been added to our armamentarium in later stages of the disease,and combinations with chemotherapy and targeted agents are currently under development.In view of the rapid advances of molecular oncology,a new challenge for the clinical oncologist arises: The appropriate patient selection for each new therapy,which can be made possible only through the implementation of predictive biomarkers in our therapy decision making.

Leptomeningeal metastases originated from esophagogastric junction/gastric cancer:A brief report of two cases

Leptomeningeal carcinomatosis is a very rare manifestation in patients diagnosed with esophagogastric junction and gastric cancer. Its prognosis is ominous and therapy outcomes are disappointing. Herein, we present two patients; one initially diagnosed with gastric cancer and leptomeningeal carcinomatosis but no other evidence of metastatic disease and the other one initially diagnosed with esophagogastric junction cancer, who recurred solitary with leptomeningeal seedings several years after the initial diagnosis and treatment. Furthermore, a thorough and short review of the literature is carried out.

AIM:To test whether clamping during liver surgery predisposes to hepatic vein thrombosis.METHODS:We performed a retrospective analysis of 210 patients who underwent liver resection with simultaneous inflow and outflow occlusion.Intraoperatively,flow in the hepatic veins was assessed by Doppler ultrasonography during the reperfusion phase.Postoperatively,patency of the hepatic veins was assessed by contrast-enhanced CT angiography,when necessary after 3-6 mo follow up.RESULTS:Twelve patients(5.7%) developed intraoperative liver remnant swelling.However,intraoperative ultrasonography did not reveal evidence of hepatic vein thrombosis.In three of these patients a kinking of the common trunk of the middle and left hepatic veins hindering outflow was recognized and was managed successfully bysuturing the liver remnant to the diaphragm.Twenty three patients(10.9%) who developed signs of mild outflow obstruction postoperatively,had no evidence of thrombi in the hepatic veins or flow disturbances on ultrasonography and contrast-enhanced CT angiography,while hospitalized.Long term assessment of the patency of the hepatic veins over a 3-6 mo follow-up period did not reveal thrombi formation or clinical manifestations of out flow obstruction.CONCLUSION:Extrahepatic dissection and clamping of the hepatic veins does not predispose to clinically important thrombosis.

One of the most important tools in film analysis is "segmentation", as defined by the major film theorist Christian Metz, with the help of the "great syntagmatic" (Grande Syntagmatique); an array, which will guarantee the classification of any kind of shot in any kind of film and its consequent decoding. The division into sections allowed a clinical approach of high validity, highlighting the hidden rhymes, the power of repetition, the underlying pulses of the film, "decoding", in a way, their soul; in essence their meaning. However, by the mid-1970s, the semiotics of cinema was considered outdated and unrealistic, mainly because it failed to solve a problem to which it was believed that it held the key to guarantee the "translation" of any abstract cinematic form. In this text, we attempt to explore what semiotics can do today and how far "segmentation" can take us in the face of a particularly "opaque" film, which is made of fragments of impressions, disparate elements and contrasts, such as The Tree of Life (2011) by Terrence Malick.

Non-invasive cardiac stress imaging plays a central role in the assessment of patients with known or suspected coronary artery disease.The current guidelines suggest estimation of the myocardial ischaemic burden as a criterion for revascularisation on prognostic grounds despite the lack of standardised reporting of the magnitude of ischaemia on various non-invasive imaging methods.Future studies should aim to accurately describe the relationship between myocardial ischaemic burden as assessed by cardiovascular magnetic resonance imaging and mortality.

A 53-year-old male patient with a large hydatid cyst of the left hemidiaphragm and smaller secondary cysts located in the left thoracic cavity and upper left abdominal quadrant presented with two progressively enlarging lipoma-like masses in the left hypochondrium and under the left scapulae respectively. Total excision of all the cysts was performed through a bilateral subcostal incision, with the left hemidiaphragm near totally excised and replaced by a synthetic bilayer mesh.

Plexiform schwannoma is a benign tumour arising from the sheath of myelinated nerve fibres that may occur in any part of the body. It is very rare in the head and neck region and poses diagnostic challenges. Here we present two unusual cases. The first is about a fifteen-year-old boy with a large plexiform schwannoma in the right retro maxillary region, extending up to but not invading the orbit. Initial cytology reported it as a pleomorphic adenoma, but histological findings after excision confirmed it to be a benign plexiform schwannoma. A zygomatic osteotomy was required to excise the tumour in its entirety. The second reported case is about solitary lesion on the tongue of a 46-year-old Chinese man, clinically mimicking a mucocoele. This case report highlights the diagnostic challenges posed by plexiform schwannoma, and the role of imaging and cytopathology, as well as the importance of definitive histopathological analysis. It also describes the surgical management of these lesions, and provides a review of the literature, in particular of cases occurring in the head and neck region.
