AIM To investigate the imp.ct of inflammatory bowel disease (IBD) on α2-Heremans-Schmid Glycop.otein(AHSG/fetuin A) and p.tential associations with disease and p.tient *** AHSG serum levels were determined in treatment-na?ve newly-diagnosed p.tients, 96 with ulcerative colitis(UC), 84 with Crohn's disease (CD), 62 with diarrheap.edominant or mixed irritable bowel syndrome (IBS, D- and M- typ.s) and 180 healthy controls (HC), by an enzyme linked immunosorbent assay (ELISA).All p.tients were followed for a minimum p.riod of 3 years at the Gastroenterology Dep.rtment of the University Hosp.tal of Larissa, Greece. C-reactive p.otein(CRp., anti-glycan antibodies, anti-Saccharomyces cerevisiae mannan antibodies Ig G, anti-mannobioside carbohydrate antibodies Ig G, anti-laminariobioside carbohydrate antibodies Ig G and anti-chitobioside carbohydrate antibodies Ig A were also determined via immunonep.elometry and ELISA, *** The mean ± SE of serum AHSG, following adjustment for confounders, was 0.32 ± 0.02 g/L in IBD, 0.32 ± 0.03 g/L in CD and 0.34 ± 0.03 g/L in UC p.tients, significantly lower than in IBS p.tients(0.7 ± 0.018 g/L) and HC (0.71 ± 0.02 g/L) (p. p.rable between the CD and UC group.. Based on AHSG levels IBD p.tients could be distinguished from HC with about 90% sensitivity and sp.cificity. Further adjusted analysis verified the inverse association between AHSG and p.netrating, as well as stricturing CD (p.rtial correlation coefficient: -0.45 and -0.33, resp.ctively) (p. p.and the need for anti-TNFα therap. or surgery, were found (p.rtial correlation coefficients:-0.31,-0.33,-0.41, resp.ctively, p.p.sitive, for at least one marker, had AHSG levels falling within the two lower quartiles (OR = 2.86, 95%CI: 1.5-5.44, p. < 0.001) while

Wegener granulomatosis (WG) is a typ. of vasculitis characterized by the p.esence of anti-neutrop.il cy-top.asmic antibodies (ANCA) and inflammation of small and medium sized vessels with granulomas formation. Most commonly affected organs include up.er and lower resp.ratory tract, kidneys, eyes, nervous system and skin. Kidneys’ involvement has a central p.sition in the classification, diagnosis, treatment and p.ognosis of p.tients with WG, and is characterized by the p.esence of necrotic glomerulonep.ritis and clinical manifestations that vary from microscop.c hematuria to acute renal insufficiency. We describe a case rep.rt of a ten year old boy p.esenting with microscop.c hematuria of glomerular origin and a medical history of orbital p.eudotumor two years before his hosp.talization due to renal symp.oms. Renal biop.y revealed lesions of p.uci-immune glome-rulonep.ritis and findings of granulomatous inflam-mation and necrotizing vasculitis. Serum was p.sitive for p.ANCA antibodies (p.rinuclear staining p.ttern ANCA antibodies). These findings led to the diagnosis of WG of generalized form (according to EULAR/ p.INTO/p.ES criteria). The p.tient has been treated with aggressive immunotherap. with the use of ster-oids, cyclop.osp.amide and mycop.enolate mofetil. Disease remission has been established and retained one year after initial diagnosis. Orbital p.eudotumor, which is a diagnosis of exclusion, has been the initial disease’s clinical manifestation, even though at that time neither the ocular biop.y nor the immunologic workup.had been indicative in terms of WG. Although WG is very rare in children, this disease should always been included in the differential diagnosis in p.tients with similar clinical manifestations and clinicians should emp.asize on the recognition of granulomatous vasculitis in biop.ies as well as on rep.ated tests for ANCA antibodies’ detection in serum. AIM: To evaluate Quality of life(QoL) in chronic heart failure(CHF) in relation to Neuroticism p.rsonality trait and CHF ***: Thirty six consecutive, outp.tients with Chronic Heart Failure(6 females and 30 males, mean age: 54 ± 12 years), with a left ventricular ejection fraction ≤ 45% at op.imal medical treatment at the time of inclusion, were asked to answer the Kansas City Cardiomyop.thy Questionnaire(KCCQ) for Quality ofLife assessment and the NEO Five-Factor p.rsonality Inventory for p.rsonality assessment. All p.tients un-derwent a symp.om limited cardiop.lmonary exercise testing on a cycle-ergometer, in order to access CHF severity. A multivariate linear regression analysis us-ing simultaneous entry of p.edictors was p.rformed to examine which of the CHF variables and of the p.rson-ality variables were correlated indep.ndently to QoL scores in the two summary scales of the KCCQ, namely the Overall Summary Scale and the Clinical Summary ***: The Neuroticism p.rsonality trait score had a significant inverse correlation with the Clinical Sum-mary Score and Overall Summary Score of the KCCQ(r =-0.621, p.p.p.c-tively). KCCQ summary scales did not show significant correlations with the p.rsonality traits of Extraversion, Op.nness, Conscientiousness and Agreeableness. Mul-tivariate linear regression analysis using simultaneous entry of p.edictors was also conducted to determine the best linear combination of statistically significant univari-ate p.edictors such as Neuroticism, VE/VCO2 slop. and VO2 p.ak, for p.edicting KCCQ Clinical Summary Score. The results show Neuroticism(β =-0.37, p.p.(β =-0.31, p.p.ak(β = 0.37, p.p.ndent p.edictors of QoL. In multivariate regression analysis Neuroticism(b =-0.37, p.p. of ventilatory equivalent for carbon dioxide outp.t during exercise,(VE/VCO2 slop.)(b =-0.31, p.p.ak oxygen up.ake(VO2 p.ak)
