Trends of Seroprevalence, Epidemiology and Clinical Presentation of HIV in North India: A Tertiary Care Hospital Based Study

Introduction: HIV infection represents a major public health problem for both developing and developed countries as it has grown to global pandemic. Spectrum of clinical presentation of HIV can greatly vary with geopolitical, socioeconomic and cultural environment. Aims: The aim of this study was to estimate the prevalence, socio-demographic conditions, clinical presentations, opportunistic infections and the possible associated risk factors for acquiring HIV infection. Materials and Methods: An observational prospective study was conducted from January 2010 to December 2014 at our centre situated in north India. As per the strategy and policy prescribed by NACO, tests were performed on the serum samples. Results: Out of the total 35369 clients tested for HIV infection, 292 were found to be HIV-1 seropositive. Two HIV-2 cases were found among the studied population. The seroprevalence of HIV was found to be 0.83% (294/35369). Mean age of the study group was 30 ± 5.65 (range: 02 - 80) years. Overall positivity rates among attendees were found to be 0.97% (138/14098), 0.96% (35/3610), 0.82% (7/850), 0.78% (22/2810) and 0.65% (92/14001) in the years 2010, 2011, 2012, 2013 and 2014 respectively. Heterosexual route of transmission was the major route of infection in 78.2 % patients. At the time of presentation, it was observed that Tuberculosis (14.9%) was the most common opportunistic infection. Conclusion: The study shows that there is a decreasing trend in the HIV prevalence in North India. However, the number of HIV cases is still significant and suggests the need for focused prevention efforts in high-risk groups.

BACKGROUND Coronavirus disease 2019(COVID-19)patients with malignancy are published worldwide but are lacking in data from *** To characterize COVID-19 related mortality outcomes within 30 d of diagnosis with HRCT score and RT-PCR Ct value-based viral load in various solid *** Patients included in this study were with an active or previous malignancy and with confirmed severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection from the institute *** collected data on demographic details,baseline clinical conditions,medications,cancer diagnosis,treatment and the COVID-19 disease *** primary endpoint was the association between the mortality outcome and the potential prognostic variables,specially,HRCT score,RT-PCR Ct value-based viral load,*** logistic regression analyses treatment received in 30 *** Out of 131 patients,123 met inclusion criteria for our *** median age was 57 years(interquartile range=19-82)while 7(5.7%)were aged 75 years or *** most prevalent malignancies were of GUT origin 49(39.8%),hepatopancreatobiliary(HPB)40(32.5%).109(88.6%)patients were on active anticancer treatment,115(93.5%)had active(measurable)*** analysis on May 20,2021,26(21.1%)patients had *** logistic regression analysis,independent factors associated with an increased 30-d mortality were in patients with the symptomatic *** in the last 4 wk,number of comorbidities(≥2 vs none:3.43,1.08-8.56).The univariate analysis showed that the risk of death was significantly associated with the HRCT score:for moderate(8-15)[odds ratio(OR):3.44;95%confidence interval(CI):1.3-9.12;P=0.0132],severe(>15)(OR:7.44;95%CI:1.58-35.1;P=0.0112).CONCLUSION To the best of our knowledge,this is the first study from India reporting the association of HRCT score and RT-PCR Ct value-based 30-d mortality outcomes in SARS-CoV-2 infected cancer patients.

I Do Biotech Dental Implants: Prospective Multicentric Study after 5 Years of Functional Loading

Introduction: I Do Biotech’s implants were developed starting in 2014. Since then, they obtained GMP and KFDA licenses for distribution in 2015. The main objective of this paper is to determine the survival rate of I Do Biotech implants five years after the first surgery. Material and Methods: 1000 implants were used on 480 prostheses across 10 clinics on 320 healthy, non-smoker and non-diabetic patients, chosen at random, of which 160 are male and 160 female, all in the age range of 30 to 50 years old. The failure rate was studied related to the patient’s gender, the length and diameter of the implant, anatomical location, the percentage of peri-implantitis, prosthodontic failures and the patient’s quality of life. Discussion: The results obtained are similar to those of Van Steenberghe D. Dieter-Busenlechner, E. Serrano Catauria and far superior to those of Sáenz Guzmán. Failure rates vary greatly from study to study due to the heterogeneity of the samples in the other research papers. Conclusion: The overall implant failure rate at 5 years is 1.7%. The factors affecting significantly the survival rate are: the implant diameter, its length and the anatomic area. Failure ratios increase significantly when the diameter or the length of the implant decreases, and when they are placed in the posterior maxilla (up to 4.3%).

Solar rooftop in India: Policies,challenges and outlook

Solar photovoltaic rooftop has emerged as a potential green technology to address climate change issues by reducing reliance on conventional fossil fuel based energy. With a strong commitment to increase the renewable sources based energy capacity to 175 GW by 2022, India has a target to install 100 GW of solar energy capacity. Of this 40 GW would be the share of grid connected solar PV rooftop. This paper examines global growth in solar energy, world's major rooftop installed capacity countries' policies and solar rooftop policy instruments in India. The current Indian goals, issues & challenges in achieving them and trends in further development are discussed.

Experimental investigation on mechanical properties of binary and ternary blended pervious concrete

The purpose of the investigation was to study the effect of binary and ternary blends of cement on the mechanical properties of pervious concrete(PC)specimen through destructive(DT)and non-destructive testing(NDT).Various combinations of fly ash(FA),limestone powder(LP),metakaolin(MK),and silica fume(SF)as mineral admixtures have been investigated to partially replace the cement up to 30%by weight in *** cube specimens of size 150 mm×150 mm×150 mm of binary and ternary blends of mineral admixture of pervious concrete were prepared to conduct standard compressive strength test and split tensile test at 7 and 28 days of *** ultrasonic pulse velocity(UPV)test and Rebound Hammer test were used as a non-destructive testing tool to substantiate the robustness of PC and to determine the approximate mechanical properties where other destructive testing tools are not feasible in case of inplace pervious *** the pervious concrete made with LP based ternary blends(PLM and PLS)were found to perform better than FA based ternary blends(PFM and PFS)and control mix(PC)in destructive and non-destructive testing.

The hemispheric asymmetry of solar activity during the last century and the solar dynamo

We believe the Babcock-Leighton process of poloidal field generation to be the main source of irregularity in the solar cycle. The random nature of this process may make the poloidal field in one hemisphere stronger than that in the other hemisphere at the end of a cycle. We expect this to induce an asymmetry in the next sunspot cycle. We look for evidence of this in the observational data and then model it theoretically with our dynamo code. Since actual polar field measurements exist only from the 1970s, we use the polar faculae number data recorded by Sheeley （1991, 2008） as a proxy of the polar field and estimate the hemispheric asymmetry of the polar field in different solar minima during the major part of the twentieth century. This asymmetry is found to have a reasonable correlation with the asymmetry of the next cycle. We then run our dynamo code by feeding information about this asymmetry at the successive minima and compare the results with observational data. We find that the theoretically computed asymmetries of different cycles compare favorably with the observational data, with the correlation coefficient being 0.73. Due to the coupling between the two hemispheres, any hemispheric asymmetry tends to get attenuated with time. The hemispheric asymmetry of a cycle either from observational data or from theoretical calculations statistically tends to be less than the asymmetry in the polar field （as inferred from the faculae data） in the preceding minimum. This reduction factor turns out to be 0.43 and 0.51 respectively in observational data and theoretical simulations.

Optimal timing for plastic surgical procedures for common congenital anomalies:A review

Apart from listening to the cry of a healthy newborn,it is the declaration by the attending paediatrician in the labour room that the child is normal which brings utmost joy to *** global incidence of children born with congenital anomalies has been reported to be 3%-6%with more than 90%of these occurring in low-and middle-income group *** exact percentages/total numbers of children requiring surgical treatment cannot be estimated for several *** children are operated under several surgical disciplines,viz,paediatric-,plastic reconstructive,neuro-,cardiothoracic-,orthopaedic surgery *** conditions may be life-threatening,e.g.,trachea-oesophageal fistula,critical pulmonary stenosis,*** require immediate surgical ***,e.g.,hydrocephalus,may need intervention as soon as the patient is fit for ***,e.g.,patent ductus arteriosus need‘wait and watch’policy up to a certain age in the hope of spontaneous *** extremely important category is that of patients where the operative intervention is done based on their *** all the congenital anomalies coming under care of a plastic surgeon are operated as elective surgery(many as multiple stages of correction)at appropriate *** are advantages and disadvantages of intervention at different *** this article,we present a review of optimal timings,along with reasoning,for surgery of many of the common congenital anomalies which are treated by plastic ***,paediatricians and general practitioners/family physicians,who most often are the first ones to come across such children,must know to guide the parents appropriately and convincingly impress upon the them as to why their child should not be operated immediately and also the consequences of too soon or too late.

Tunnelling through weak and fragile rocks of Himalayas

A considerable amount of tunnelling has been going on in India for various projects such as hydroelectric power, irrigation, roads and railways. Most of these projects are located in Himalayas, far away from the urban areas. Tunnelling through weak and jointed rock masses such as the one in the Himalayas is a challenging task for the planners, designers, engineers and geologists because of high overburden, thickly vegetated surface, weak, poor and fragile rocks and highly varying geology with the presence of numerous small and big shear zones, faults, etc. Due to these reasons, various tunnelling problems have been faced in the past and are still being encountered. Failures and the problems may be regarded as challenges and opportunities for generating new knowledge base and thereby increasing self-reliance in tunnelling. The experiences of Himalayan tunnelling through weak and fragile rocks covering varying and mixed geology, understanding on tunnelling in squeezing ground conditions and applicability of TBM in Himalayas are presented. It has also been highlighted that the probe holes planning, drilling and monitoring shall be followed seriously to reduce the geological surprises.
