BACKGROUNd Septic shock is a severe form of sepsis characterised by deterioration in circulatory and cellular-metabolic *** standard therapy,the outcomes are *** adjuvant therapy,such as CytoSorb®extracorporeal haemoadsorption device,has been investigated and shown promising ***,there is a lack of some guidance to make clinical decisions on the use of CytoSorb®haemoadsorption as an adjuvant therapy in septic shock in Indian ***,this expert consensus was *** To formulate/establish specific consensus statements on the use of CytoSorb®haemoadsorption treatment based on the best available evidence and contextualised to the Indian *** We performed a comprehensive literature on CytoSorb®haemoadsorption in sepsis,septic shock in PubMed selecting papers published between January 2011 and March 20232021 in English *** statements for a consensus document were developed based on the summarised literature analysis and identification of knowledge *** a modified delphi approach combining evidence appraisal and expert opinion,the following topics related to CytoSorb®in septic shock were addressed:need for adjuvant therapy,initiation timeline,need for Interleukin-6 levels,duration of therapy,change of adsorbers,safety,prerequisite condition,efficacy endpoints and management *** expert members from critical care,emergency medicine,and the intensive care participated and voted on nine statements and one open-ended *** Eleven expert members from critical care,emergency medicine,and the intensive care participated and voted on nine statements and one open-ended *** 11 experts in the consensus group(100%)participated in the first,second and third round of *** three iterative voting rounds and adapting two statements,consensus was achieved on nine statements out of nine *** consensus expert panel also recognised the necessity to form an associatio

Fatigue crack growth simulations of 3-d linear elastic cracks under thermal load by XFEM

This paper deals with the fatigue crack growth simulations of three-dimensional linear elastic cracks by XFEM under cyclic thermal load. Both temperature and displacement approximations are extrinsically enriched by Heaviside and crack front enrichment functions. Crack growth is modelled by successive linear extensions, and the end points of these linear extensions are joined by cubic spline segments to obtain a modified crack front. different crack geometries such as planer, non-planer and arbitrary spline shape cracks are simulated under thermal shock, adiabatic and isothermal loads to reveal the sturdiness and versatility of the XFEM approach.

In this perspective,we have highlighted the current literature and explained the synthesis,structure,morphology,modification strategies,and photocatalytic applications of emerging BiPO_(4)-based *** BiPO_(4)is a large bandgap photocatalyst,it uses UV light for the excitation of electrons,and also,the recombination of charge carriers is an issue in BiPO_(4).various novel modification strategies of BiPO_(4)photocatalysts *** modifications,heterojunction formation,phase-junctions,surface plasmon resonance,Schottky junction have been successfully proposed and *** modifications enhance the light absorption and inhibit the recombination of charge carriers BiPO_(4)***,future aspects for further research on BiPO_(4)-based photocatalysts are also *** expects that BiPO_(4)-based photocatalysts represent a promising strategy for developing practical photocatalysts for energy and environmental remediation applications.

Transfusion-transmitted virus in association with hepatitis A-E viral infections in various forms of liver diseases in India

AIM： To describe the prevalence of transfusiontransmitted virus （TTV） infection in association with hepatitis A-E viral infections in different forms of liver diseases in North India. METHOdS： Sera from a total number of 137 patients, including 37 patients with acute viral hepatitis （AVH）, 37 patients with chronic viral hepatitis （CVH）, 31 patients with cirrhosis of liver and 32 patients with fulminant hepatic failure （FHF）, were analyzed both for TTV-dNA and hepatitis A-E viral markers. Presence of hepatitis B virus （HBV）, hepatitis C virus （HCV） and hepatitis E virus （HEV） infections was detected in different proportions in different groups. Moreover, rrV-dNA was simultaneously tested in 100 healthy blood donors also. RESULTS： None of the patients had hepatitis A virus （HAV） and hepatitis d virus （HdV） infections. Overall prevalence of TTV-dNA was detected in 27.1% cases with AVH, 18.9% cases with CVH, 48.4% cases with cirrhosis and 9.4% cases with FHF. TTV-dNA simultaneously tested in 100 healthy blood donors showed 27% positivity. On establishing a relation between TTV infection with other hepatitis viral infections, TTV demonstrated co-infection with HBV, HCV and HEV in these disease groups. Correlation of TTV with ALT level in sera did not demonstrate high ALT level in TTV-infected patients, suggesting that TI-V does not cause severe liver damage. CONCLUSION： TTV infection is prevalent both in patients and healthy individuals in India. However, it does not have any significant correlation with other hepatitis viral infections, nor does it produce an evidence of severe liver damage in patients with liver diseases.

Effects of mother tree ages, different rooting mediums, light conditions and auxin treatments on rooting behaviour of dalbergia sissoo branch cuttings

dalbergia sissoo Roxb. is one of the promising multipurpose tree species of South Asia. Most of the plantations of d. sissoo from seeds are facing severe threats due to the die-back disease, which ultimately causes death of this potential tree-species within a few months. Vegetative propagation could avoid the die-back disease. Thirty mother trees of different age-groups of d. sissoo were selected for evaluating the rooting behaviour of branch cuttings from d. sissoo as influenced by auxins （IAA or IBA at 100, 200, 500 mg·L^-1）, ages of mother trees （10, 4 and 2 years old） and different environment conditions, i.e., different mediums （soil and sand） or light conditions （in shade and open condition）. The results show that application of IAA and IBA induced more numbers of cuttings （collected from 10-year-old mother trees） to root compared to control. Branch cuttings of d. sissoo collected from 10-year-old mother trees and planted in soil bed in open conditions had 100.0% of cuttings to root in IAA （100 mg·L^-1） and IBA （200 mg·L^-1） treatments. Both rooting medium （Soil and sand） influenced significantly （p〈0.05） on rooting response of branch cuttings. Soil medium was found to achieve maximum no. of branch cuttings to root, compared to sand medium.

Objective:To explore and identify the most potent antihyperglycemic fraction from the ethanol extract of Rhododendron arboreum(***)***:Normal and streptozotocin induced diabetic rats were treated with all four fractions of *** flowers for short term and with fraction 3 for long term *** completion of the treatment,a range of indicators were tested including fasting blood glucose,plasma protein,haemoglobin A1C,insulin secretion,body weight,blood lipid profile and carbohydrate metabolism regulating enzymes of ***:In short term study,the fraction 3(Active fraction)produced a significant(Pduction(73.6%)in blood glucose level at a dose of 200 mg/kg after the treatment in the diabetic *** of active fraction(200 and 400 mg/kg)once daily for 30 d in streptozotocin diabetic rats resulted in a significant(Pd glucose level,hemoglobin A1C,serum urea and creatinine with significant but a increase in insulin level similar to standard drug ***,the active fraction showed antihyperlipidemic activity as evidenced by significant(Pdecreases in serum serum total cholesterol,triglycerides,low density lipoprotein cholesterol and very low density cholesterol levels coupled together with elevation of high density lipoprotein cholesterol in the diabetic ***:The active fraction of *** flowers decreases streptozotocin induced hyperglycemia by promoting insulin secretion and glycolysis and by decreasing gluconeogenesis.

Optimization and Shelf-Life Evaluation of Pearl Millet Based Halwa dry Mix

Indian traditional sweet dish i.e. halwa was developed from pearl millet grains. Pearl millet grains （Pennisetum typhoides var. PHB-2168） were steamed for 20 min at 1.05 kg cm^2 in order to minimize lipase activity, total phenols, phytic acid and tannins content. Steamed grains were dried （12 ± 0.5% moisture content）, pearled, milled, sieved through sieve opening size of 0.234 mm and were used for study. Central composite rotatable design （CCRd） with three independent variables i.e. vanaspati, sugar and water for rehydration were used to design the experiments. Sensory responses and rehydration ratio were used to study the individual and interactive effects of variables. Sensory score for colour varied from taste 6.3-7.9, mouth feel 6.4-7.8, overall acceptability （OAA） 6.5-7.9 and rehydration ratio （RR） from 1.4 to 2.3. All the responses fitted into quadratic equation with R2 〉 0.80. Halwa dry mix was also evaluated for stability in polyethylene pouches at ambient conditions and the peroxide and free fatty acids value were 21.35 meqO2 kg^-1 fat, 0.55% （oleic acid）, respectively after 6 months of storage. Based on compromise optimization, optimum levels of the ingredients recommended with 86.4% desirability were： vanaspati 38.6 g 100 g^-1 pearl millet semolina （PMS）, sugar 88.7 g 100 g^-1 PMS and water for rehydration 151 mL 100 g^-1 dry mix.

Purpose: To conduct a randomized prospective clinical trial to compare primary vitrectomy without scleral buckling versus conventional scleral buckling surger y in pseudophakic primary retinal detachment (PPRd) in terms of anatomic attachm ent rate, functional outcome and complications. Methods: Fifty consecutive eyes of 50 patients with PPRd were randomized into two groups, with 25 patients in ea ch of group 1 (scleral buckling group) and group 2 (pars plana vitrectomy withou t buckling group) in a hospital setting and followed up at 1 week, 2 weeks, 6 we eks and 6 months. Results: A primary reattachment rate of 76%(19 retinas) was o btained in group 1, while a reattachment rate of 84%(21 retinas) was achieved i n group 2. The final anatomic reattachment rate was 100%in both groups. The cau ses of failure in group 1were proliferative vitreoretinopathy in five eyes and o pen break/ missed break in one eye. The causes of failure in group 2 were missed break/op en break in three eyes and proliferative vitreoretinopathy in one eye. Best corr ected visual acuity (BCVA) at 2 weeks was better in group 1, while the final BCV A at end of 6 months was two lines better in group 2. The mean change in refract ive error was-1.38 d in group 1 and-0.85 d in group 2. Conclusions: Pars plana vitrectomywithout buckling provides an effective treatment for PPRd and results in better longterm visual and anatomic outcomes than conventional scleral buckl ing.

在全球金融危机期间,澳大利亚经济被广泛地认为较之全球其他经济体表现良好,尽管如此,该国金融市场仍然经历了极端的波动。为了理解澳大利亚新兴经济实体较之成熟经济实体受到此类极端事件影响的程度,本文运用4个风险衡量指标,将新兴企业指数(EMCOX)涵盖范围内的新兴实体和标准普尔/澳大利亚证券交易所200指数涵盖范围内的成熟实体进行了比较。四个风险指标中的前两者是市场及信用风险的传统衡量指标——风险价值(var)及违约距离(dd);后两者则关注分布尾部的极端风险,包括条件风险价值(Cvar)和条件违约距离(Cdd),其中条件违约距离(Cdd)是本文作者独创的,可将条件风险价值技术应用到违约风险的度量中。将这些指标应用于全球金融危机发生之前及危机阶段,便可以发现:所采用的一切指标对于新兴市场股份都显示出更高的风险,在危机阶段新兴企业和成熟企业投资组合之间的价差缩小,违约风险价差在分布尾部处达到最大。这一信息对于投资者和贷款人在极端经济环境下决定其股票或贷款投资组合都具有重要意义。

BACKGROUNd despite various therapies to treat sepsis,it is one of the leading causes of mortality in the intensive care unit patients *** about the pathophysiology of sepsis has sparked interest in extracorporeal therapies(ECT)which are intended to balance the dysregulation of the immune system by removing excessive levels of inflammatory *** To review recent data on the use of ECT in sepsis and to assess their effects on various inflammatory and clinical *** In this review,an extensive English literature search was conducted from the last two decades to identify the use of ECT in sepsis.A total of 68 articles from peer-reviewed and indexed journals were selected excluding publications with only *** Results showed that ECT techniques such as high-volume hemofiltration,coupled plasma adsorption/filtration,resin or polymer adsorbers,and CytoSorb®are emerging as adjunct therapies to improve hemodynamic stability in ***®has the most published data in regard to the use in the field of septic shock with reports on improved survival rates and lowered sequential organ failure assessment scores,lactate levels,total leucocyte count,platelet count,interleukin-IL-6,IL-10,and TNF *** Clinical acceptance of ECT in sepsis and septic shock is currently still limited due to a lack of large random clinical *** addition to patient-tailored therapies,future research developments with therapies targeting the cellular level of the immune response are expected.
