Heterozygous nucleotide-binding oligomerization domain-2 mutations affect monocyte maturation in Crohn's disease

AIM： To investigate the function of monocytes in Crohn＇s disease （CD） patients and to correlate this with disease- associated nucleotide-binding oligomerization domain-2 （NOD2） gene variants. METHODS： Monocytes from 47 consecutively referred CD patients and 9 healthy blood donors were cultured with interleukin （IL）-4 and granulocyte-macrophage colony-stimulating factor （GM-CSF）, and stimulated with lipopolysaccharide （LPS） or muramyldipeptide （MDP）, the putative ligand of NOD2. RESULTS： We found that monocytes from CD patients differentiated in vitro to mature dendritic cells （DCs）, as determined by immunophenotype and morphology. IVOD2 genotype was assessed in all subjects, and we observed high CD86 expression on immature and LPS-stimulated DCs in IVOD2 mutated CD patients, as compared with wtlVOD2 CD patients and controls. By contrast, CD86 expression levels of DCs induced to maturity with MDP derived from IVOD2-mutated subjects were comparable to those of normal subjects. The amount of IL-12p70 in patient-cell cultures was larger than in controls aEer LPS treatment, but not aEer treatment with MDR CONCLUSION： Our results suggest that DCs obtained from patients with mutations in the IVOD2 gene display an activated phenotype characterized by high CD86 expression, but have a diminished response to MDP when compared to the terminal differentiation phase. We speculate that the altered differentiation of monocytes might lead to an imbalance between inflammation and the killing ability of monocytes, and may be relevant to the pathogenesis of CD.

BACKGROUND:Common bile duct(CBD)stones are known to pass spontaneously in a significant number of patients. This study investigated the rate of spontaneous CBD stones passage in a series of patients presenting with jaundice due to *** patients were managed surgically,allowing CBD intervention to be avoided in the event of spontaneous passage of CBD stones. METHOD:Retrospective analysis of patients presenting with jaundice due to CBD stones,and managed surgically with laparoscopic cholecystectomy and intra-operative cholangiogram with or without CBD exploration. RESULTS:The jaundice settled pre-operatively in 76/108 patients, and in 60/108 the CBD stones had passed spontaneously by the time of *** 60 patients avoided any intervention to their CBD. CONCLUSIONS:CBD stones pass spontaneously in more than half of jaundiced *** management(laparoscopic cholecystectomy and intra-operative cholangiogram,with willingness to perform CBD exploration if positive)allows the avoidance of CBD intervention in these patients.

Targeted precision neuroendocrine neoplasms (NEN) surgery may represent an innovative, yet promising, development of surgical procedures to achieve a radical resection of the primary tumour (by intra-operative confirmation of tumour-free margins) and to improve the identification of small sized lesions (that might otherwise be missed by either pre-operative functional imaging, ultrasound or manual palpation during surgery). From a practical point of view, radioguided surgery (RGS) requires a close collaboration between the nuclear medicine and the surgical teams, so that a small amount of radiopharmaceutical is intravenously injected in the patient right before surgery. The tumour lesions’ emitted radiation is then detected by a hand-held probe by the surgeon intra-operatively.

It is not well known how low temperatures, like a subarctic steppe–tundra climate, influence reproductive traits of ectothermic vertebrates. To begin answering this question, we studied male and female reproductive systems of Salamandrella keyserlingii inhabiting a Tomsk population（southeast of Western Siberia）, Russia, in ecological and physiological terms. In males, before spermiation, the testicular size and weight in late April–early May were greatest of all. Spermiation occurred during breeding immigration in spring when mean air temperature was above 10°С, and at the same time rain fell. After spermiation, the testicular size and weight decreased sharply, and the diameter of the vasa deferentia increased. ＂Spawning＂（i.e., simultaneous extrusion of sperm and oviposition） occurred from late April to late May, and this duration fluctuated in temperature and humidity. The testicular size and weight increased in summer. Sperm mass was detected in the testes by the smear method in April–September, except in June when single fragmented unrealized sperm was detected and in July when spermatids were detected. In females, ovarian weight was greatest in spring before ovulation. From late June, vitellogenesis began in ovarian follicles, in which mint green yolks accumulated. Melanin deposited in the surface of the ovary from July when oviducts were hypertrophying. In contrast, some large-sized females did not show any sexual maturity shortly before hibernation（although these females may be subadults）. These results suggest that low temperatures in Siberia induce early timing of gamete maturation in females, but the females＇ reproductive cycle might also be biennial. A reproductive cycle in males was annual with the completion of the gamete maturation process in August.

Despite the good prognosis of the vast majority of breast cancer cases,about 10%is firstly diagnosed with stage IV,while 20-30%of localized disease treated with curative intent will eventually relapse(1).Hepatic metastases are the third site of relapse,after lymph-nodes and bone,but,due to the early systemic nature of breast cancer,rarely the recurrence is liver-limited.
