to evaluate the functional and anatomical results of macular hole surgery and to explore its effect on patients’Health-Related Quality Of Life (HR-QOL) and to investigate the associations between self-reported HR-QOL and conventional measures of visual function. Case series. the National Eye Institute 25-Item v isual Function Questionnaire (vFQ-25) and the 36-Item Short-Form Health Surve y (SF-36) were self-administered by 30 patients before and 4 months after macu lar hole surgery. Preoperative, intraoperative, and postoperative clinical data were collected including visual acuity, contrast sensitivity, and metamorphopsia . multi-item scales rating different aspects of HR-QOL were compared before an d after surgery, and their correlation with traditional methods of outcome evalu ation was analyzed. macular hole closure was achieved in 26 patients (87%). mea n LogmAR visual acuity improved by 6±10 letters for distance and 7 ±12 letters for near. metamorphopsia was reduced by a mean of 35±70 squares on Amsler char t, and Pelli-Robson contrast sensitivity decreased by a mean of -0.09 ±0.3 lo g units postoperatively. the vFQ-25 composite score as well as scale scores ass ociated with general vision, near vision, vision-related mental health, and rol e difficulties were significantly improved (P ter successful closure o f macular hole. Conversely out of the eight SF-36 health concepts, limitation i n usual role activities because of emotional problems was the only one that sign ificantly improved postoperatively. Both baseline and postoperative best-correc ted visual acuity significantly correlated with most of the vFQ subscale scores before and after surgery, respectively. In this case-se-ries, macular hole sur gery appears to have a beneficial effect on patients’subjective perception of v isual function. the use of vision-targeted health status questionnaires in conj unction with detailed clinical examination provides a more comprehensive overvie w of indivi

Reports implicating specific transmissible agents in multiple sclerosis (mS) susceptibility continue to appear. We therefore re-evaluated mS risk in 687 stepsiblings of 19 746 mS index cases. We found the risk of mS to be indistinguishable from that of the general population after diagnostic verification. these results are coherent with studies of adopted children, half siblings and conjugals, showing no risk attributable to the familial microenvironment. this family based genetic epidemiological approach found no trace of transmissibility other than genetic from one affected individual to another in the high prevalence area of Canada. this adds to existing data showing that the action of environment in influencing mS risk is operative at a population level.

目的研究多发性硬化(mS)者尸检mR成像铁测量与铁染色的关系。材料与方法本研究经机构审查委员会批准,并获得受试者和(或)其家属的知情同意。应用4.7tmR设备对死亡时间不超过28h的3例mS病人以及1例死亡前1年患mS的病人行基于横向弛豫(t2加权,R2图和R2*图)及相位成像4种方法的mRI检查。脑质提取后,用Perls铁反应进行铁染色。应用mR成像对6块皮质下灰质结构的感兴趣区进行铁测量,而后运用分散线性最小二乘回归法分析其与铁染色病理切片相片中对应位置的相关性。白质病灶中的铁以染色确定,并与mRI相对照。结果对于皮质下灰质内的铁质,R2*图的受试者间相关性(R2=0.857,0.628和0.685;P均t2加权成像受试者间相关性(R2=0.463,0.582,0.650和0.551;PmR成像方法都与铁质有显著的线性相关性,有望用于确定mS病人皮质下灰质结构的铁质情况,其中R2*图应作为首选。确定mS病灶内部铁质状况的可靠检查方法尚未建立。

IGRt技术在肺癌SBRt治疗靶区位置误差测定中的应用

目的:探讨图像引导放射治疗(image guided radiation therapy,IGRt)技术在测定并校正肺部恶性肿瘤体部立体定向放射治疗(stereotactic body radiation therapy,SBRt)过程中靶区位置误差的意义。方法:选择复旦大学附属肿瘤医院收治的14... 详细信息

目的:探讨图像引导放射治疗(image guided radiation therapy,IGRt)技术在测定并校正肺部恶性肿瘤体部立体定向放射治疗(stereotactic body radiation therapy,SBRt)过程中靶区位置误差的意义。方法:选择复旦大学附属肿瘤医院收治的14例肺部恶性肿瘤并接受SBRt的患者。每次治疗前后均进行千伏级锥形束Ct(kv-CBCt)扫描,与计划Ct图像进行在线配准,校正误差后进行治疗。治疗后再次进行CBCt扫描并配准。分别记录治疗前后靶区位置误差。根据临床靶区(Ctv)-计划靶区(Ptv)外放公式mPtv(外放)=2.5Σ+0.7σ计算Ptv外扩大小。结果:治疗前靶区位置左右、头脚、腹背方向误差最大值分别为10.0mm、16.9mm、8.7mm,误差平均值分别为(2.4±2.8)mm、(3.4±4.6)mm、(2.9±3.4)mm。在线校正并治疗后,靶区位置左右、头脚、腹背方向的误差最大值分别为7.6mm、4.6mm、7.0mm,误差平均值分别为(2.1±2.0)mm、(1.5±1.6)mm、(1.7±2.0)mm。无图像引导Ptv外扩大小为左右7.96mm、头脚11.72mm、腹背9.63mm,有图像引导并在线校正Ptv外扩大小为左右6.65mm、头脚4.87mm、腹背5.65mm。结论:IGRt技术可即时校正SBRt治疗前由摆位引起的靶区位置误差,并测定治疗过程中的靶区位置移动,能提高治疗精度,并有助于Ptv外扩的制定。

一项关于缺血性卒中患者-148C/t纤维蛋白原基因多态性和纤维蛋白原水平的病例对照研究

Background: to determine whether -148 C/t fibrinogen gene promoter polymorp hism increases stroke risk by modifying the fibrinogen level. Design: A case-c ontrol study of patients with first ever ischaemic stroke, confirmed by computed tomography. methods: venous blood samples were collected for fibrinogen and rou tine coagulation tests one week after the stroke, and after three months in abou t half the patients. Population controls were age and sex matched. -148 G/t fi brinogen polymorphism was determinedly polymerase chain reaction followed by dig estion with restriction enzymes HindIII/Alul. Results: there were 124 patients a nd 125 controls, mean age 56 years (range 18 to 75); 34 patients (27% ) and 41 controls (33% ) were heterozygous for -148 C/t fibrinogen polymorphism, six p atients (5% ) and five controls (4% ) had the t/t genotype. the odds ratio of ischaemic stroke associated with CC homozygotes v t carriers was 0.8 (95% conf idence interval, 0.5 to 1.4). Relative risk for ischaemic stroke associated with fibrinogen levels in the highest quartile was 3.9 (1.9 to 8;4) at one week, dec reasing to 1.4 (0.6 to 3.3) at three months. Conclusions: -148 C/t fibrinogen gene polymorphism was not a strong risk factor for ischaemic stroke. High fibrin ogen levels early after acute stroke probably represent an acute phase response.

JUNO is a multi-purpose neutrino observatory under construction in the south of *** publication presents new sensitivity estimates for the measurement of the △m_(31)^(2),△m_(21)^(2),sin^(2)θ_(12),and sin^(2)θ_(13) oscillation parameters using reactor antineutrinos,which is one of the primary physics goals of the *** sensitivities are obtained using the best knowledge available to date on the location and overburden of the experimental site,the nuclear reactors in the surrounding area and beyond,the detector response uncertainties,and the reactor antineutrino spectral shape constraints expected from the tAO satellite *** is found that the △m_(21)^(2) and sin^(2)θ_(12) oscillation parameters will be determined to 0.5%precision or better in six years of data *** the same period,the △m_(31)^(2) parameter will be determined to about 0.2%precision for each mass ordering *** new precision represents approximately an order of magnitude improvement over existing constraints for these three parameters.

the Jiangmen Underground Neutrino Observatory(JUNO)is a large liquid scintillator detector designed to explore many topics in fundamental *** this study,the potential of searching for proton decay in the p→νk^(+)mode with JUNO is *** kaon and its decay particles feature a clear three-fold coincidence signature that results in a high efficiency for ***,the excellent energy resolution of JUNO permits suppression of the sizable background caused by other delayed *** on these advantages,the detection efficiency for the proton decay via p→νk^(+)is 36.9%±4.9%with a background level of 0.2±0.05(syst)±0.2(stat)events after 10 years of data *** estimated sensitivity based on 200 kton-years of exposure is 9.6×1033 years,which is competitive with the current best limits on the proton lifetime in this channel and complements the use of different detection technologies.
