Revisiting Acid-Fast Bacilli Microscopy of Concentrated Sputum Smears as an Efficient Tool for the Diagnosis of Tuberculosis: A Study from a Tertiary Care Centre in Southern India

Background and objectives: With 2.2 million new cases every year, Tuberculosis (TB) continues to be an epidemic of large proportions in India. Conventional direct sputum smear microscopy, though limited in its sensitivity, is still the most common method of testing for TB. Newer techniques such as concentrated sputum microscopy, have shown some promise in improving this limited sensitivity. We have compared the efficacy of concentrated sputum versus the direct smear technique in 1000 sputum samples of patients suspected to be suffering from TB. Methods: A total of 1000 sputum specimens were collected for direct acid-fast bacilli (AFB) smear, concentrated AFB smear and culture from St. John’s Medical College and Hospital. 39 contaminated samples were (3.9%) omitted during the final analysis. Mycobacterial culture was used as the reference standard method for the detection of TB. Results: 184 and 198 of the 961 samples were found to AFB positive by direct smear microscopy and concentrated smear technique respectively. The measured sensitivity and specificity of direct smear microscopy were 69.86% and 95.82%, while that of concentrated smear microscopy was 76.71% and 95.96 % respectively. 33 samples found to be negative by the direct smear method turned out to be positive by the concentrated smear technique. Conclusions: Though our study suggests no significant statistical difference between the two techniques of detecting pulmonary tuberculosis, we recommend the use of the concentrated technique in centres such as ours, where facilities are already in place. In this way, the number of cases of TB that remain untreated may significantly come down.

A Comparative Study of Drug Susceptibility Testing Techniques for Identification of Drug Resistant TB in a Tertiary Care Centre, South India

India tops the global list for Drug resistant Tuberculosis, but inadequate and expensive laboratory culture techniques have led to delay in the diagnosis and treatment. We studied the potential of an alternative method which could be cost-effective by combining the drugs in the same tube for identification of drug resistance. Drug Susceptibility Test (DST) results of 1000 sputum samples are got from suspected TB patients against INH (isoniazid) and Rifampicin by two techniques: a) a modified technique with both drugs in the same MGIT tube and b) a standard technique with the antibiotics in separate MGIT tubes for the diagnosis of MDR-TB (Multidrug Resistant). 39 samples were contaminated and were excluded from final analysis. 198 were smear positives by the concentrated Ziehl-Neelsen’s staining method. 219 were found to be culture positive out of which 195 were identified as M. tuberculosis complex. 40 (20.5%) strains were identified as MDR-TB by the conventional method and 39 were picked up by the modified DST. INH and Rifampicin mono-resistance accounted for 32 (16.4%) and 4 (2%) respectively. 99% concordance was observed between the two tests in categorizing MDR-TB. Similarly modified technique with combination of the second line Antibiotics-Ofloxacin, Kanamycin and Capreomycin was applied on the identified MDR strains in a stepwise manner. 6 (15%) were identified as Pre-XDR strains and 2 (5%) were found to be XDR-TB strains. This study implies that combining drugs in the same tube may be an equivalent and possibly a cost-effective alternative which needs to be explored further.

Management of Strawberry Grey Mold Disease Using Biocontrol Agents and Plant Extracts

Strawberry (Fragaria × ananassa Duch.) is a significant global soft fruit crop, prized for its nutrient content and pleasant flavor. However, diseases, particularly grey mold caused by Botrytis cinerea Pers. Fr. poses major constraints to strawberry production and productivity. Grey mold severely impacts fruit quality and quantity, diminishing market value. This study evaluated five B. cinerea isolates from various locations in the Ri-Bhoi district of Meghalaya. All isolates were pathogenic, with isolate SGM 2 identified as highly virulent. Host range studies showed the pathogen-producing symptoms in the fava bean pods, marigold, gerbera, and chrysanthemum flowers and in the fava bean, gerbera, and lettuce leaves. In vitro tests revealed that neem extract (15% w/v) achieved the highest mycelial growth inhibition at 76.66%, while black turmeric extract (5% w/v) had the lowest inhibition at 9.62%. Dual culture methods with bio-control agents indicated that Bacillus subtilis recorded the highest mean inhibition at 77.03%, while Pseudomonas fluorescens had the lowest at 20.36% against the two virulent isolates. Pot evaluations demonstrated that B. subtilis resulted in the lowest percent disease index at 20.59%, followed by neem extract at 23.31%, with the highest disease index in the control group at 42.51%. Additionally, B. subtilis significantly improved plant growth, yielding an average of 0.32 kg compared to 0.14 kg in the control. The promising results of B. subtilis and neem leaf extract from this study suggest their potential for eco-friendly managing grey mold in strawberries under field conditions.

Effect of FFD-410 (HiOwna-Jr.), a Balanced Herbal Nutritional Supplement on Biochemical, Hematological, Immunological and Cognitive Parameters of Wistar Rats

Present study was aimed to evaluate the effect HiOwna-Jr. (FFD-410) on vital biochemical, hematological, immunological and cognitive functions in rats;also in vitro antioxidant studies were carried out to evaluate the antioxidant capacity of FFD-410. Animals of respective groups were treatment with FFD-410 for 90 days and blood samples were collected for the estimation of biochemical, hematological parameters and serum immunoglobulin levels;haemoagglutination assay was carried out using Sheep Red blood cells (SRBC’s). In addition, effect of FFD-410 on cognition and memory was evaluated by modified elevated plus maze test. Apart from in vivo studies, in vitro studies such as DPPH radical scavenging assay, reducing power assay and ORAC assays were carried out to evaluate the free radical scavenging and antioxidant activity of FFD-410. Pretreatment with FFD-410 for 90 days did not bring about any change in serum biochemical and hematological parameters and relative organ weights etc., which could account for its wide safety margin at tested dose levels (2.5 and 5.0 g/kg, p.o.). However, FFD-410 showed potent immunostimulant activity by elevating the serum immunoglobulins and haemoagglutination titer values, also the pretreatment with FFD-410 showed dose dependent improvement in short-term cognition and memory in elevated plus maze test. Furthermore, in vitro antioxidant studies FFD-410 exhibited significant and dose dependent free radical scavenging and antioxidant activity as assessed by DPPH (IC50 value of 1162.6 μg/ml), reducing power and ORAC assays (Trolox equivalence/g of 76.1). These findings suggest that, FFD-410 possess very good antioxidant, cognition improving and potent immunostimulant properties. Also, there was no significant change in the serum biochemical and hematological parameters and relative organ weights were observed after 90 days treatment with FFD-410, which could account for its wide safety of margin at tested dose levels (2.5 and5.0 g/kg. p.o

Synthesis and evaluation of novel analogues of mangiferin as potent antipyretic

Objective:To screen different analogues of mangiferin pharmacologically for antipyretic activity. Methods:The naturally occurring xanthone glycoside mangiferin was isolated by column chromatography from the elhanolic extract of stem bark of Mangifera *** was further converted to 5-(N-phenylamino methyleno) mangiferin.5-(N-p-chlorophenylamino methyleno) mangiferin,5-(N-2-methyl phenylamino methyleno) mangiferin.5-(N-p-methoxy phenylamino methyleno) mangiferin.5-(N,N-diphenylamino methyleno) mangiferin,5-(N-α-napthylamino methyleno) mangiferin and 5-(N-4-methyl phenylamino methyleno) mangiferin *** synthesized compounds were further screened for antipyretic activity along with mangiferin at a dose level of 100 and 200 tng/*** and its analogues were characterized by melting point and R_f value determination and through spectral technique like UV,IR,and NMR spectral ***:The antipyretic activity of mangiferin as well as all analogues was found to be more significant in at higher dose ie.200 mg/kg which was depicted thmugh a decrease in rectal temperature up to 3 ***:The antipyretic activity of mangiferin and its analogues may be attributed to inhibition in synthesis of TNF-αand anti-oxidant activity associated with amelioration of inflammatory actions of cytokines.
