AIM： To investigate the distribution and frequency of advanced polyps over eight years.METHODS：6424 colonoscopies were reviewea aunng the study period 1998 to 2005. The study period was subdivided into period I ： 1998 to 2001 and period 11： 2002-2005. RESULTS： 1856 polyps （33% advanced polyps） and 328 CRCs were detected. The mean ages of the patients with advanced polyps and cancer were 69.2 ± 12.0 and 71.6± 13.8 years, respectively. Advanced polyps were mainly lerc sided （59.5%）. Advanced polyps were found in patients ≤60 years from 17.7% in period Ⅰto 26.3% in period Ⅱ（P 〈 0.05）, especially in male subjects ≤60 years （21.6% vs 31.6%, P 〈 0.05）. Advanced tubuloviUous polyps rose from 21.5% in period I to 29.5% in period Ⅱ （P 〈 0.05）. Whereas cancers in male patients ≤ 60 years were similar in both periods： 23.2% vs 16.5% （P 〉 0.05）.CONCLUSION： Advanced polyps increased significantly in the younger male group in the most recent period and there seems to be a shiE towards a proximal location.

AIM: To explore the association of serum insulin, insulin resistance, and β-cell dysfunction with gallstone disease (GSD) in type 2 diabetics. METHODS: We used a community-based study conducted between 1991 and 1993 in Kinmen, Taiwan to identify type 2 diabetics. A screening program for GSD was performed in 2001 by a panel of specialists who employed real-time ultrasound sonography to examine the abdominal region after the patient had fasted for at least 8 h. Screening was conducted in 2001 on 848 patients diagnosed with type 2 diabetes. The HOMA method was used to compare the profile differences for insulin resistance (HOMA IR) and β-cell dysfunction (HOMA β-cell). RESULTS: We studied 440 type 2 diabetics who attended sonography check-ups. After excluding eight insulin-treated diabetics, the prevalence of GSD among the remaining 432 was 13.9% (26/187) among males and 14.7% (36/245) among females. After adjustment for other GSD-associated risk factors in addition to age and obesity, GSD risk increased among females with levels of serum insulin [4th vs 1st quartile odds ratios (OR) = 4.46 (95%CI: 1.71-11.66)] and HOMA IR [4th vs 1st quartile OR = 4.46 (95%CI: 1.71-11.66)]. Better HOMA β-cell function was significantly related to decreased risk of GSD [4th vs 1st quartile OR = 0.16 (95%CI: 0.03-1.70)]. Among males, age and central obesity were the most significant risk factors for GSD. No association of GSD with serum insulin, HOMA IR, and HOMA β-cell was observed among males. CONCLUSION: Serum insulin, insulin resistance, and β-cell dysfunction are risk factors for GSD in females, but not males with type 2 diabetes.

Intra- and Inter-Observer Reproducibility of Ultrasound Measurements of Pelvic Inlet Diameter in Pregnant Women

Artesunate is a semi-synthetic derivative of artemisinin,a sesquiterpene trioxane lactone isolated from the herb Artemisia annua *** medicinal plant has been used as a remedy for fevers and chills for centuries in *** is a clinically effective drug for both uncomplicated and severe *** have recently found that artesunate may be effective for allergy,asthma and *** reported for the first time that artesunate effectively reduced ovalbumin-induced inflammatory cell recruitment into bron-choalveolar fluid,IL-4,IL-5,IL-13 and eotaxin production,pulmonary eosinophilia,mucus hypersecretion and airway hyperrespons-iveness in a mouse asthma model probably via inhibition of the phosphoinositide 3-kinase(PI3K)/Akt signaling pathway[Cheng et al. (2011) PLoS ONE 6:e20932].Artesunate significantly suppressed ovalbumin-induced increases in oxidative damage markers 8-isoprostane, 8-hydroxy-2-deoxy-guanosine and 3-nitrotyrosine levels,and pro-oxidants iNOS and NADPH oxidase(NOX 1,2,3 and 4) ***,artesunate markedly suppressed passive cutaneous anaphylaxis,passive systemic anaphylaxis,ovalbumin-induced mast cell degranulation in the airways and serum histamine level,and IgE-mediated mast cell degranulation,probably mediated through reduction of inositol trisphosphate(IP) and intracellular Ca ***,artesunate was able to ameliorate pulmonary cell infiltration in a cigarette smoke induced COPD mouse *** together,our results clearly demonstrate broad-spectrum anti-inflammatory effects of artesunate in mouse models of asthma,allergy and COPD by inhibiting PI3K/Akt and IP/Ca signaling pathways. Our present findings support a novel therapeutic use of artesunate in the treatment of allergy,asthma and COPD.

Objectives: To determine intra- and inter-observer reproducibility of the ultrasound measurement of pelvic inlet in the first trimester of pregnancy. Methods: Transabdominal pelvic ultrasound was performed to measure the pelvic inlet anteroposterior diameter using a 2D probe in twelve Chinese women having a singleton pregnancy. Scans were performed in the first trimester by three sonographers. The pelvic inlet anteroposterior diameter was defined as the distance between the anterior surfaces of the sacral promontory and the superior medial border of the pubic bone most adjacent to the pubic symphysis. Intra-observer repeatability was determined and analysis of variance was performed to assess inter-observer measurements. The bias between any two sonographers’ measurements was assessed by calculating the 95% confidence interval for the mean difference between sonographers paired measurements. Results: Intra-observer reproducibility was 0.71 cm. Analysis of variance indicated that there was no significant difference between sonographers’ measurements (p = 0.46). The bias between two sonographers’ measurements ranged from 0.05 to 0.32 cm. Conclusion: It is technically feasible to measure the pelvic inlet diameter using ultrasound (USG) at the first trimester. High inter-observer reproducibility can be achieved. Further studies are required to establish the potential role of this technique and the measurement of the pelvic inlet diameter in prediction of labor outcome.

Dear Editor,Recent studies from us and others have deciphered the clinical relevance of a variety of oncohistones1 in different *** H3K27M and H3K36M mutant histones exert dominant-negative effects on methylation levels of histone H3K27 and H3K36 on wildtype histone protein in pediatric brain cancers2–4 and chondroblastoma,5,6 *** contrast to H3K27M and H3K36M that act in trans to cause global reduction of methylation at the respective residues,the H3G34 mutations,including H3G34V/R and H3G34W/L found in pediatric high-grade glioma and giant cell tumor of the bone,affect the H3K36 methylation in cis and alter the H3K36 methylation on the nucleosome containing the H3G34 mutation(s).

Posterior circulation stroke,especially basilar artery occlusion or the top of basilar syndrome,has a high mortality and morbidity.^(1)Predictors of poor outcome in elude impaired con sciousness,tetraparesis and abno rmal pupils-the prese nee of which will in crease the risk of death or major disability by three-to fourfold.(2,3)If a patient is not eligible for or does not respond to revascularization,there is no specific treatment except prevention and management of medical comorbidities,rehabilitation and secondary stroke prevention.
