Percutaneous transhepatic cholangioscopy and lithotripsy in treating difficult biliary ductal stones: Two case reports

BACKGROUND Endoscopic retrograde cholangiopancreatography(ERCP) is preferred for managing biliary obstruction in patients with bilio-enteric anastomotic strictures(BEAS) and calculi. In patients whose duodenal anatomy is altered following upper gastrointestinal(UGI) tract surgery, ERCP is technically challenging because the biliary tree becomes difficult to access by per-oral *** endoscopic therapies like balloon-enteroscopy or rendevous-ERCP may be considered but are not always feasible. Biliary sepsis and comorbidities may also make these patients poor candidates for surgical management of their biliary *** SUMMARY We present two 70-year-old caucasian patients admitted as emergencies with obstructive cholangitis. Both patients had BEAS associated with calculi that were predominantly extrahepatic in Patient 1 and intrahepatic in Patient 2. Both patients were unsuitable for conventional ERCP due to surgically-altered UGl anatomy. Emergency biliary drainage was by percutaneous transhepatic cholangiography(PTC) in both cases and after 6-weeks' maturation, PTC tracts were dilated to perform percutaneous transhepatic cholangioscopy and lithotripsy(PTCSL) for duct clearance. BEAS were firstly dilated fluoroscopically,and then biliary stones were flushed into the small bowel or basket-retrieved under visualization provided by the percutaneously-inserted video cholangioscope. Lithotripsy was used to fragment impacted calculi, also under visualization by video cholangioscopy. Satisfactory duct clearance was achieved in Patient 1 after one PTCSL procedure, but Patient 2 required a further procedure to clear persisting intrahepatic calculi. Ultimately both patients had successful stone clearance confirmed by check *** PTCSL offers a pragmatic, feasible and safe method for biliary tract clearance when neither ERCP nor surgical exploration is suitable.

Finding the optimum solution for dispatching in concrete delivery is computationally intractable because it is a NP-hard （non-deterministic polynomial-time hard） problem. Heuristic methods are required to obtain satisfactory solutions. Inefficiencies in mathematical modeling still make concrete dispatching difficult to solve. In reality, complex dispatching systems are mostly handled by human experts, who are able to manage the assigned tasks well. However, the high dependency on human expertise is a considerable challenge for RMC （ready mixed concrete） companies. In this paper, a logical reconstruction of an expert＇s decision making is achieved by two machine learning techniques： decision tree and rule induction. This paper focuses on the expert dispatcher＇s prioritization of customer orders. The proposed method has been tested on a simulation model consisting of a batch plant and three customers per day. The scenarios generated by the simulation model were given to a dispatch manager who was asked to prioritize the customers in each day. The scenarios and the decisions were then input to the machine learning programs, which created generalizations of the expert＇s decisions. Both decision trees and rules approach 80% accuracy in reproducing the human performance.

The usefulness of the apparent redox potential discontinuity(aRPD) in assessments of marine benthic habitat quality was explored at two intertidal mudflats along the north Pacific coast of Canada. Two transects were established at each intertidal site, with three sediment biogeochemistry cores collected from each transect four times over the summer of 2016. Measurements of the sediment pore water dissolved oxygen(DO) content and redox(Eh) conditions were taken at the surface of the core(measured vertically), as well as at increasing depths(1 cm between readings) into the sediment(measured horizontally through predrilled holes in the biogeochemistry corer). While oxic, anoxic, oxidized, and reduced sediment pore water was observed above and below the aRPD, in general, sediment above the aRPD had higher DO content, and higher Eh values than sediment below the aRPD. Therefore, the aRPD depth can be used as a relative indicator of sediment pore water DO and Eh conditions: sediment with a deeper aRPD depth has more available DO, and the pore water has higher Eh values(more oxidized or less reduced) than sediment with a shallower aRPD depth. As such, the aRPD depth is a useful parameter to include in models that assess the quality of marine benthic habitats.

This paper introduces computational fluid used in aerospace engineering, to deal with surface physical and mathematical foundations of CFD, this traffic problems such as queue/platoon distribution, dynamics （CFD）, a numerical traffic flow related problems. approach widely and successfully After a brief introduction of the paper develops CFD implementation methodology for modeling shockwave propagation, and prediction of system performance. Some theoretical and practical applications are discussed in this paper to illustrate the implementation methodology. It is found that CFD approach can facilitate a superior insight into the formation and propagation of congestion, thereby supporting more effective methods to alleviate congestion. In addition, CFD approach is found capable of assessing freeway system performance using less ITS detectors, and enhancing the coverage and reliability of a traffic detection system.

With high energy density and improved safety,rechargeable battery chemistries with a zinc(Zn)metal anode offer promising and sustainable alternatives to those based on lithium metal or lithium-ion intercalation/alloying anode materials;however,the poor electrochemical reversibility of Zn plating/stripping,induced by parasitic reactions with both aqueous and non-aqueous electrolytes,presently limits the practical appeal of these *** recent efforts in rechargeable Zn metal batteries(RZMBs)have achieved certain advancements in Zn metal reversibility,as quantified by the Coulombic efficiency(CE),a standard protocol for CE has not been established,and results across chemistries and systems are often *** importantly,there is still an insufficient understanding regarding the critical factors dictating Zn *** this work,a rigorous,established protocol for determining CE of lithium metal anodes is transplanted to the Zn chemistry and is used for systematically examining how a series of factors including current collector chemistry,current density,temperature,and the upper voltage limit during stripping affect the measured reversibility of different Zn *** support from density functional theory calculations,this standardized Zn CE protocol is then leveraged to identify an important correlation between electrolyte solvation strength toward Zn2+and the measured Zn CE in the corresponding electrolyte,providing new guidance for future development and evaluation of Zn electrolytes.

Accurate pier scour predictions are essential to the safe and efficient design of bridge crossings. Current practice uses empirical formulas largely derived from laboratory experiments to predict local scour depth around single-bridge piers. The resulting formulas are hindered by insufficient consideration of scaling effects and hydrodynamic forces. When applied to full-scale designs, these formula deficiencies lead to excessive over prediction of scour depths and increased construction costs. In an effort to improve the predictive capabilities of the HEC-18 scour model, this work uses field-scale data and nonlinear regression to develop a family of equations optimized for various non-cohesive soil conditions. Improving the predictive capabilities of well-accepted equations saves scarce project dollars without sacrificing safety. To help improve acceptance of modified equations, this work strives to maintain the familiar form of the HEC-18 equation. When compared to the HEC-18 local pier scour equation, this process reduced the mean square error of a validation data set while maintaining over prediction.
