Two topics were focused. The first one was about the gamma-ray scintillator, Pr^3＋：Lu3Al5O12 （LuAG）. The second one was about neutron scintillator, Ce^3＋：^6LiCaAlF6 and Eu^2＋：^6LiCaAlF6 （^6LiCAF）. Those scintillators have been developed very recently for modem imaging applications in the medical and homeland security fields. In both cases, the rare earth ions are playing the crucial role as emission centers. Pr^3＋ in LuAG provided fast 5d→4f transition providing noticeably shorter decay time than that of Ce^3＋. Among several candidate hosts, LuAG showed the best performance. Bulk crystal growth, basic scintillation properties, two-dimensional gamma-ray imaging and positron emission mammography （PEM） application were demonstrated. Due to the international situation, the homeland security was compromized by illicit traffic of explosives, drugs, nuclear materials, etc. and the ways to its improvement became an important R＆D topic. For this purpose the Ce and Eu doped LiCAF appeared competitive candidates. Especially, when substitution of 3He neutron detectors was considered, the discrimination ability of gamma-ray from alpha-ray was important. Bulk crystal growth, basic scintillation properties and two-dimensional neutron imaging were demonstrated.

Background: Cardiovascular (CV) diseases are the most frequent cause of death in hemodialysis (HD) patients. The aim of this study was to identify risk factors for CV events in HD patients with preserved left ventricular ejection fraction (PEF). Objectives: A total of 213 HD patients (69 years, 64.1% males) were enrolled. Demographic, laboratory test, and echocardiographic data were recorded, and CV events during the 32-month follow-up period were documented. Results: During the follow-up period, 31 patients (14.6%) died and 50 patients (23.5%) suffered a fatal or nonfatal CV event. Age and serum albumin, C-reactive protein, total cholesterol, non-HDL cholesterol, and NT-proBNP levels were associated with CV events according to a univariate analyses. A multivariate analysis identified a low serum albumin value

Evaluation of the Bending Loss of the Hollow Optical Fiber for Application of the Carbon Dioxide Laser to Endoscopic Therapy

Since carbon dioxide laser is excellent for incision, hemostasis, coagulation, and vaporization of soft tissues, it has been widely applied in clinical treatments as the laser knife. In these days, flexible thin hollow optical fibers transmitting mid-infrared light have been developed, and the application of carbon dioxide laser to endoscopic therapy has become possible. However, it is expected that the irradiation effect is influenced by the change in the laser power at the tip of the hollow optical fiber due to the change in the transmittance by the bending loss. The purpose of this research is to quantitatively evaluate the change in the output power and therapeutic effect by bending the hollow optical fiber in a gastrointestinal endoscope. The change in the transmittance of the hollow optical fiber due to the insertion of the fiber into the endoscope and bending of the head of the endoscope was measured. Then, the relationship between the irradiated laser power and the incision depth for a porcine stomach was investigated. As the results, the most significant decrease in the transmittance of the hollow optical fiber was caused by the insertion of the fiber into the instrument channel of the endoscope, and bending of the head of the endoscope with the angle of 90° decreased the output laser power and incision depth by 10% and 25%, respectively. Therefore, it was confirmed that the bending loss of the hollow optical fiber due to the bending of the head of the endoscope had no significant influence on the endoscopic therapy using the carbon dioxide laser.

Immunohistochemical Analysis of p16 Expression in Uterine Smooth Muscle Tumors

Objective: Immunohistochemistry with p16 antibody is used as a diagnostic marker in several gynecologic pathologies. The purpose of the present study was to evaluate the diagnostic and prognostic significance of p16 immunohistochemistry in various uterine smooth muscle tumors. Methods: Tissue specimens, including 29 leiomyomas, 13 smooth muscle tumors of uncertain malignant potential (STUMP), and 17 leiomyosarcomas, were obtained from 59 patients with uterine smooth muscle tumors. The prevalence of p16 expression in various uterine smooth muscle tumors was examined using immunohistochemistry methods and we investigated the association between p16 expression and various clinicopathologic parameters, including prognosis of leiomyosarcomas. Results: The prevalence of p16 expression of leiomyomas, STUMP, and leiomyosarcomas tumors was 10.3%, 38.4%, and 76.4%, respectively. The prevalence and immunohistochemistry scores of p16 were significantly higher in the leiomyosarcomas group than in the leiomyomas or STUMP tumor groups. In the leiomyosarcomas groups, p16 immunohistochemistry scores were significantly higher in the groups with more advanced stage, higher mitotic activity, and recurrence. The relationship among p16 immunohistochemistry scores and age, tumor size, extent of necrosis, and degree of myometrial invasion was not significant. High expression of p16 in leiomyosarcomas was linked to shorter disease-free and overall survival. Conclusions: The use of p16 immunohisitochemistry is a useful marker in differential diagnosis in various uterine smooth muscle tumors. Moreover, p16 overexpression possibly plays an important role in tumor progression and we demonstrate that p16 may be a predictor of overall survival in patients with leiomyosarcoma tumors.

Despite recent advances in surgical techniques and perioperative management, the prognosis of pancreatic cancer(PCa) remains extremely poor. To provide optimal treatment for each patient with Pca, superior biomarkers are urgently needed in all phases of management from early detection to staging, treatment monitoring, and prognosis. In the blood of patients with cancer, circulating tumor cells(CTCs) and cell-free nucleic acids(cf NAs), such as DNA, m RNA, and noncoding RNA have been recognized. In the recent years, their presence in the blood has encouraged researchers to investigate their potential use as novel blood biomarkers, and numerous studies have demonstrated their potential clinical utility as a biomarker for certain types of cancer. This concept, called "liquid biopsy" has been focused on as a less invasive, alternative approach to cancer tissue biopsy for obtaining genetic and epigenetic aberrations that contribute to oncogenesis and cancer progression. In this article, we review the available literature on CTCs and cfN As in patients with cancer, particularly focusing on PCa, and discuss future perspectives in this field.

Segmental gastrectomy with radical lymph node dissection for early gastric cancer

AIM: To describe a new surgical technique and evaluate the early results of segmental gastrectomy (SG) with modified D2 lymph node (LN) dissection for early gastric cancer (EGC). METHODS: Fourteen patients with EGC underwent SG with modified D2 dissection from 2006 to 2008. Their operative results and postoperative courses were compared with those of 17 patients who had distal gastrectomy (DG) for EGC during the same period. RESULTS: Operating time, blood loss, and hospital stay were similar between the 2 groups. Postoperative complications developed significantly more frequently in the DG group than in the SG group. Mean number of dissected LNs per each station in the SG group was comparable with that in the DG group. Postoperative recovery of body weight was significantly better in the SG group than in the DG group. The incidence of reflux esophagitis and gastritis after surgery was less frequent in the SG group than in the DG ***: SG with modified D2 LN dissection may be a new function-preserving gastrectomy that is feasible for treatment of EGC with possible LN involvement.

Inflammatory pseudotumor of the liver and spleen diagnosed by percutaneous needle biopsy

An inflammatory pseudotumor (IPT) is a relatively rare lesion characterized by chronic inf iltration of inflammatory cells and areas of f ibrosis. IPTs are diff icult to diagnose because of the absence of specif ic symptoms or of characteristic hematological or radiological f indings. In this study, a case of a woman aged over 70 years was reported, who presented with a general malaise lasting more than two months. A computed tomography scan demonstrated a diffusely spread lesion of the liver with a portal vein occlusion and a splenic lesion surrounded by a soft density layer. Since the percutaneous liver biopsy showed f indings that suggested an IPT, although the radiological f indings did not exclude the possibility of a malignancy, we performed a percutaneous spleen biopsy to enable a more defi nitive diagnosis. The microscopic f indings from the spleen specimen lead us to a diagnosis of IPT involving the liver and spleen. Sub-sequent steroid pulse therapy was effective, and rapid resolution of the disease was observed.

Medical students’ willingness to work in post-conflict areas: A qualitative study in Sri Lanka

Background: The north-east (NE) region of Sri Lanka observed a critical health workers’ shortage after the long-lasting armed conflict. This study aimed to explore medical students’ attitudes towards working in the NE and to identify factors determining such attitudes. Methods: A semi-structured, self-administered questionnaire survey was conducted in two medical schools, one in the NE and the other near the capital, in October 2004. Data were qualitatively analysed using the framework approach. Results: Three main themes were identified: 1) Professional motives and career plans;2) Students’ perceptions of the healthcare situation in the NE;and 3) Students’ choice of the NE as a future practice location. It was found that familiarity with the difficulties faced by the NE people was a major motivation for medical students to work in the NE in the future. For NE students, familiarity was linked to their sense of belonging. For non-NE students, their personal experience of the NE familiarized them with the difficult situation there, which positively influenced their willingness to work there. Demotivations to work in the NE were poor working and living conditions, fewer opportunities for postgraduate education, language differences, insecurity, and fear of an unpleasant social response from the NE communities. Conclusions: NE local medical students had a sense of belonging to the NE and compassion for the Tamil people as members of the ethnic group. They were willing to work in the NE if their concerns about difficult working and living conditions and postgraduate education could be solved. Non-NE students who were familiar with the NE situation through their personal experience also showed a willingness to work there;thus, early exposure programmes in medical education might help to increase the health workforce in the NE. It is also expected that non-NE physicians working for the NE people would facilitate reconciliation and the rebuilding of trust between two ethnic
