Propensity score matching analysis for clinical impact of braided-type versus laser-cut-type covered self-expandable metal stents for endoscopic ultrasound-guided hepaticogastrostomy

Background:To prevent stent migration during endoscopic ultrasound-guided hepaticogastrostomy(EUS-HGS),intra-scope channel release technique is important,but is unfamiliar to non-expert hands.The self-expandable metal stent(SEMS)is an additional factor to prevent stent migration.However,no compara-tive studies of laser-cut-type and braided-type during EUS-HGS have been reported.The aim of this study was to compare the distance between the intrahepatic bile duct and stomach wall after EUS-HGS among laser-cut-type and braided-type SEMS.Methods:To evaluate stent anchoring function,we measured the distance between the hepatic parenchyma and stomach wall before EUS-HGS,one day after EUS-HGS,and 7 days after EUS-HGS.Also,propensity score matching was performed to create a propensity score for using laser-cut-type group and braided-type group.Results:A total of 142 patients were enrolled in this study.Among them,24 patients underwent EUS-HGS using a laser-cut-type SEMS,and 118 patients underwent EUS-HGS using a braided-type SEMS.EUS-HGS using the laser-cut-type SEMS was mainly performed by non-expert endoscopists(n=21);EUS-HGS using braided-type SEMS was mainly performed by expert endoscopists(n=98).The distance after 1 day was significantly shorter in the laser-cut-type group than that in the braided-type group[2.00(1.70-3.75)vs.6.90(3.72-11.70)mm,P<0.001].In addition,this distance remained significantly shorter in the laser-cut-type group after 7 days.Although these results were similar after propensity score matching analysis,the distance between hepatic parenchyma and stomach after 7 days was increased by 4 mm compared with the distance after 1 day in the braided-type group.On the other hand,in the laser-cut-type group,the distance after 1 day and 7 days was almost the same.Conclusions:EUS-HGS using a laser-cut-type SEMS may be safe to prevent stent migration,even in non-expert hands.

BACKGROUND Oral liposarcoma is an extremely rare lesion that is often clinically misdiagnosed as a benign tumor due to its asymptomatic and indolent clinical course.Here,we report a case of massive low-grade myxoid liposarcoma(MLS)of the floor of the mouth.CASE SUMMARY A 71-year-old man presented with a huge mass in the left floor of the mouth.A biopsy was performed,and a diagnosis of a myxoid tumor suspicious for lowgrade MLS or myxoma was made.Gadolinium-enhanced T1-weighted magnetic resonance imaging showed an intensely enhanced tumor lesion that occupies the left sublingual space and extends to the submandibular space.Submandibular dissection,tumor resection,and reconstruction with a radial forearm flap were performed.The surgical specimen exhibited histologically low-grade MLS.Fused in sarcoma(FUS,also known as TLS)and DNA damage-inducible transcript 3(DDIT3,also known as CHOP)break-apart was not detected in the fluorescence in situ hybridization analysis.The tumor was completely encapsulated and did not require additional treatment.Furthermore,no recurrence was reported 40 mo after surgery.CONCLUSION We experienced an extremely rare,massive,low-grade MLS emerging from the floor of the mouth.Oftentimes,an MLS of the floor of the mouth lacks significant clinical findings and is often misdiagnosed.Although no FUS-DDIT3 fusion gene was detected,a low-grade MLS was ultimately diagnosed based on the histological findings.

AIM:To identify factors contributing to visual improvement after treatment of macular edema(ME)secondary to branch retinal vein occlusion(BRVO),and to assess the interaction between laser therapy and intravitreal ranibizumab(IVR).METHODS:We retrospectively reviewed the medical records of patients who had been treated for BRVO-related ME at our hospital.Records were traceable for at least 12 mo,and evaluated factors included age,sex,medical history,smoking history,treatment methods,foveal hemorrhage,and change in visual acuity.Treatments included laser therapy,IVR,sub-Tenon’s capsule injection of triamcinolone(STTA),a combination,or no intervention.Multivariate logistic regression analysis and interaction terms were used to assess the clinical efficacy of the treatments,and odds ratios(OR)and 95%confidence intervals(CI)were calculated.RESULTS:Seventy-three patients(34 men,39 women;73 eyes)with a mean age of 69.4±12.1 y were included.Patients who underwent IVR monotherapy,laser monotherapy,and STTA+laser had significantly higher best corrected visual acuity at 12 mo compared to baseline(P<0.001,<0.001,and 0.019,respectively).Logistic regression analysis without interaction terms found that IVR was a significant visual acuity recovery factor(adjusted OR:3.89,95%CI:1.25-12.1,P=0.019).Adjusted OR using an interaction model by logistic regression was 16.6(95%CI:2.54-108.47,P=0.003)with IVR treatment,and 8.25(95%CI:1.34-50.57,P=0.023)with laser treatment.No interaction was observed(adjusted OR:0.07,95%CI:0.01-0.75,P=0.029).CONCLUSION:IVR contributes to improvements in visual acuity at 12 mo in ME secondary to BRVO.No interaction is observed between laser therapy and IVR treatments.

A three-dimensional numerical model was developed to predict the behavior of tidal flow by using the σ-coordinate transformation. Conservation equations were solved by hybrid finite analytical techniques. The hydrodynamic model was verified with the analytical solutions for the tidal forcing in an open channel. The simulation results are in good agreement with the analytical solutions.

To the Editor：Trauma, inflammation, iatrogenic procedures such as liverbiopsy and percutaneous transhepatic biliary drainage （PTBD）, vascularanomalies, coagulopathy status, and malignancy can causehemobilia [1]. Malignant causes of hemobilia include cholangiocarcinoma,pancreatic cancer, gallbladder tumor, and liver metastasis,but rarely hepatocellular carcinoma （HCC）. Here, we describeachieving endoscopic hemostasis for hemobilia caused by HCC,which was visualized using the single-operator, SPY-DS cholangioscope（Boston Scientific, Tokyo, Japan） under endoscopic retrogradecholangiopancreatography （ERCP） guidance. This scope is a dramaticimprovement over the SpyGlass system in that it is easier to insert into the biliary tract due to a tapered tip, a 120° digitalfield-of-view provides favorable visualization, and new injection and suction functions have been added via a two-port adaptor.

This article deals with reciprocity requirement for recognition of foreign judgments in Japan. Following German law, Japanese law requires reciprocity to the rendering state in addition to the jurisdiction of the state, the service of process, and the compatibility with Japanese public policy. Although Japanese courts have rarely refused the recognition of foreign judgments for lack of reciprocity for a long time, some Chinese judgments recently have not been recognized for this reason. The author clarifies first with historical review what was the purpose of the Japanese legislator, when the original law of 1890 required the reciprocity by international treaties, and when later the reform law of 1926 required the simple reciprocity that is similarly provided in the current law. The author surveys then the Japanese case law concerning the reciprocity requirement after the reform of 1926. The author focuses further on the reciprocity between Japan and China and compares the Japanese practice with the German one that led to a different result. Last, it is concluded that the reciprocity requirement is contrary to the protection of human rights under Japanese constitution.

The effects of turbulent intensity and vortex scale of simulated natural wind on performance of a horizontal axis wind turbine(HAWT) are mainly investigated in this paper. In particular, the unsteadiness and turbulence of wind in Japan are stronger than ones in Europe and North America in general. Hence, Japanese engineers should take account of the velocity unsteadiness of natural wind at installed open-air location to design a higher performance wind turbine. Using the originally designed five wind turbines on the basis of NACA and MEL blades, the dependencies of the wind frequency and vortex scale of the simulated natural wind are presented. As the results, the power coefficient of the newly designed MEL3-type rotor in the simulated natural wind is 130% larger than one in steady wind.

Budd-Chiari syndrome was formerly described as he- patic vein thrombosis within the liver, but it now in- cludes inferior vena cava (IVC) thrombosis and oth- er conditions that cause hepatic vein outflow obstruc- tion. The author consider that primary hepatic vein thrombosis and primary IVC thrombosis repre- sent two different clinical disorders.
