板坯的表面横裂多发现于连铸的弯曲或矫直处,并与γ-α相变温度区域的热延性有关。许多研究表明,裂纹是由于晶界处片层状铁素体的应变集中所导致的。在传统流程中的弯曲矫直期间,板坯的表面温度主要考虑来避开脆化温度区的。在该研究中,提出了一个新的概念:利用铸坯表层微观组织控制来预防横向裂纹。通过板坯表层组织控制（SSC）能够抑制晶界处片层状铁素体的形成,方法有二冷区初期的强冷却和随后的潜热引起的再加热。现场的凝固高温拉伸试验证明,通过微观结构控制可以消除脆性极限。SSC对裂纹敏感性的影响已经通过半工业化连铸机在试验上得到了验证。在浇铸之后进行的是弯曲。在钢坯表面上的慢冷使横裂纹在实验上再现,即传统冷却方式可以产生具有片层状铁素体的正常微观结构。通过表层组织控制（SSC）裂纹是可以预防的。传统连铸也可采用这些操作方法。

本项试验进一步查明青椒保护地栽培每1000平方米保留4600个主枝,采取每株保留2个主枝比每株保留4个主枝的增产幅度下限为13.6%.采取每枝保留2主枝,叶面积系数为4左右,保持了比较合理的群体结构,同时可使单位面积的根系持有量增加3.5%以上.并阐明根系数量和小区产量之间有密切的正相关关系.故此研究作物的群体结构不能局限于地上部,根系的数量和分布状况也是不可忽视的问题.

Functional Confirmation Using a Medical X-Ray System of a Semiconductor Survey Meter

In recent years, semiconductor survey meters have been developed and are in increasing demand worldwide. This study determined if it is possible to use the X-ray system installed in each medical facility to calculate the time constant of a semiconductor survey meter and confirm the meter’s function. An additional filter was attached to the medical X-ray system to satisfy the standards of N-60 to N-120, more copper plates were added as needed, and the first and second half-value layers were calculated to enable comparisons of the facility’s X-ray system quality with the N-60 to N-120 quality values. Next, we used a medical X-ray system to measure the leakage dose and calculate the time constant of the survey meter. The functionality of the meter was then checked and compared with the energy characteristics of the meter. The experimental results showed that it was possible to use a medical X-ray system to reproduce the N-60 to N-120 radiation quality values and to calculate the time constant from the measured results, assuming actual leakage dosimetry for that radiation quality. We also found that the calibration factor was equivalent to that of the energy characteristics of the survey meter.

Pulmonary <i>Mycobacterium avium </i>Complex Disease Requiring Differentiation from Recurrence of Lung Cancer during the Follow-Up Period for Lung Cancer

Case 1 was a 49-year-old woman who visited with a dry cough. She had an underlying disease of lung adenocarcinoma and received cancer immunotherapy because of an ALK-positive response and several cancer chemotherapies. The clinical effect was a complete response. Chest CT was performed because of continuous dry cough, and a new tumor shadow was recognized in the lingula portion of the left upper lobe. We performed CT-guided lung biopsy and could aspirate pus-fluid. The culture test for acid-fast bacilli was positive and the causative microorganism was identified as Mycobacterium avium by the DDH method. The final diagnosis was pulmonary abscess due to M. avium. Treatment using combined chemotherapy including CAM was performed and a good clinical response was obtained. Case 2 was a 67-year-old man who had a past history of surgical resection of lung adenocarcinoma eight and two years ago and received several cancer chemotherapies and radiation therapy. Because a new nodular shadow appeared in the right middle lobe one year ago and showed strong positivity on PET/CT, surgical resection was performed with the suspected recurrence of lung cancer. Subsequently, the histological diagnosis was epithelioid granuloma and a culture test of acid-fast bacilli was positive, with the identification of Mycobacterium intracellulare by the DDH method. Combined chemotherapy was not performed because the lesion was completely resected. Afterwards, a new nodular shadow appeared in the left lower lobe again and bronchoscopy was performed. Because M. intracellulare was isolated from the local specimen, we diagnosed the patient with recurrence of pulmonary MAC disease and combined chemotherapy including CAM was performed for one year. Finally, the nodular lesion disappeared. It is difficult to differentiate pulmonary MAC disease from lung cancer. Therefore, careful follow-up of patients with lung cancer while keeping in mind the possible complication of pulmonary MAC disease is necessary.

Single-incision laparoscopic surgery is a rapidly evolving field as a bridge between traditional laparoscopic surgery and natural orifice transluminal endoscopic *** report one of the initial clinical experiences in Japan with this new *** cases of gallbladder diseases were selected for this new technique.A single curved intra-umbilical 25-mm incision was made by pulling out the umbilicus.A 12-mm trocar was placed through an open approach,and the abdominal cavity was explored with a 10-mm semiflexible *** 5-mm ports were inserted laterally from the laparoscope port.A 2-mm mini-loop retractor was inserted to retract the fundus of the *** was performed using an electric cautery hook and an Endograsper *** were two women and two men with a mean age of 50.5 years(range:40-61 years).All procedures were completed successfully without any perioperative *** all cases,there was no need to extend the skin *** operative time was 88.8 *** follow-up didnot reveal any umbili-cal wound ***-incision laparoscopic cholecystectomy is feasible and a promising alternative method as scarless abdominal surgery for the treatment of some patients with gallbladder disease.

Survival from cardiac arrest due to sushi suffocation

BACKGROUND: Sushi suffocation is relatively uncommon, and it is an unignorable cause of sudden death; however, no reports on sushi suffocation have been ***: A 60-year-old man was referred to our hospital for post resuscitative intensive care. He had choked on sushi and collapsed in the dining room of a mental hospital. A nursing assistant summoned a physician who attempted to extract the sushi. External cardiac massage was initiated after 7 minutes had elapsed and followed by endotracheal intubation. Return of spontaneous circulation was achieved after 7 minutes of resuscitation. A bronchoscopy demonstrated a large amount of shari in the trachea and right bronchus, which was removed with alligator forceps and a wire ***: Neurological recovery was evident on day 2 of admission. He was transferred back to the mental hospital with no neurological ***: Emergency physicians should consider sushi suffocation, including its clinical features and management.

Evaluation of Radiation Exposure Literacy among Mammography Examinees Using Radiation Dose Distribution in Mammography Examinations

Introduction: Radiation is indispensable for diagnosis and treatment and is widely used in medicine. This study aimed to determine patients’ knowledge and ability to understand radiation exposure to assess the usefulness of providing information on radiation doses to examinees. Material and Methods: The radiation exposure literacy of mammography examinees was assessed using a new scale consisting of the following five items: “collection of information from various sources”, “selection of information necessary for oneself”, “understanding and communication of information”, “judgment of the reliability of information”, and “ability to plan and act based on information”. We analyzed the relationship of these items with examinees’ attributes and clarified the level of understanding of radiation exposure dose associated with mammography examinations by providing examinees with visual information in the form of color maps. Results: The relationship between “information collection” and several attributes of radiation exposure literacy was strong. In addition, providing visual information on radiation doses using dose distribution maps in mammography examinations could deepen the understanding of radiation doses among examinees. Conclusion: By understanding the radiation exposure literacy of examinees and comparing it with their attributes, it is possible to provide suggestions for developing methods of providing radiation dose information tailored to individual examinees. In addition, improving radiation exposure literacy will enable the development of skills necessary to provide safe medical care to medical examinees.

Changes of shear-wave velocity by interferon-based therapy in chronic hepatitis C

AIM: To evaluate the changes of shear-wave velocity(Vs) by acoustic radiation force impulse after treatment in chronic hepatitis ***: Eighty-seven patients with chronic hepatitis C were consecutively treated with combinations of interferon(IFN) plus ribavirin(RBV). Vs value(m/s) was measured with acoustic radiation force impulse before treatment, at end of treatment(EOT), 1 year after EOT, and 2 years after ***: In patients with a sustained virological response(SVR)(n = 41), Vs significantly decreased at EOT [1.19(1.07-1.37), P = 0.0004], 1 year after EOT [1.10(1.00-1.22), P = 0.0001], and 2 years after EOT [1.05(0.95-1.16), P < 0.0001] compared with baseline [1.27(1.11-1.49)]. In patients with a relapse(n = 26), Vs did not significantly decrease at EOT [1.23(1.12-1.55)], 1 year after EOT [1.20(1.12-1.80)], and 2 years after EOT [1.41(1.08-2.01)] compared with baseline [1.39(1.15-1.57)]. In patients with a nonvirological response(n = 20), Vs did not significantly decrease at EOT [1.64(1.43-2.06)], 1 year after EOT [1.66(1.30-1.95)], and 2 years after EOT [1.61(1.36-2.37)] compared with baseline [1.80(1.54-2.01)]. Among genotype 1 patients, baseline Vs was significantly lower in SVR patients [1.28(1.04-1.40)] than in non-SVR patients [1.56(1.20-1.83)](P = 0.0142).CONCLUSION: Reduction of Vs values was shown in SVR patients after IFN-plus-RBV therapy by acoustic radiation force impulse.

A minimally invasive multiple percutaneous drainage technique for acute necrotizing pancreatitis

BACKGROUND: In approximately 20% of patients, necrotizing pancreatitis is complicated with severe acute pancreatitis, with high morbidity and mortality rates. Minimally invasive step-up approach is both safe and effective, but sometimes requires multiple access ***: A 62-year-old woman was admitted with diabetic ketoacidosis, and initial computed tomography(CT) revealed no evidence of acute pancreatitis. She was clinically improved with insulin therapy, fl uid administration, and electrolyte replacement. However, on the 14 th day of admission, she developed a high-grade fever, and CT demonstrated evidence of acute necrotizing pancreatitis with a large collection of peripancreatic fl uid. Percutaneous transgastric drainage was performed and a 14 French gauge(Fr) pigtail catheter was placed 1 week later, which drained copious pus. Because of persistent high-grade fever and poor clinical improvement, multiple 8 and 10 Fr pigtail catheters were placed via the initial drainage route, allowing the safe and effective drainage of the extensive necrotic tissue that was occupying the bilateral anterior pararenal ***: After drainage, the patient recovered well and the last catheter was removed on day 123 of ***: Multiple percutaneous drainage requires both careful judgment and specialist skills. The perforation of the colon and small bowel as well as the injury of the kidney and major vessels can occur. The current technique appears to be safe and minimally invasive compared with other drainage methods in patients with extended, infected necrotic pancreatic pseudocysts.
