Autophagy serves an innate immune function in defending the host against invading bacteria,including group A Streptococcus(GAS).Autophagy is regulated by numerous host proteins,including the endogenous negative regulator calpain,a cytosolic protease.Globally disseminated serotypeM1T1 GAS strains associated with high invasive disease potential express numerous virulence factors and resist autophagic clearance.Upon in vitro infection of human epithelial cell lines with representative wild-type GAS M1T1 strain 5448(M1.5448),we observed increased calpain activation linked to a specific GAS virulence factor,the interleukin-8 protease SpyCEP.Calpain activation inhibited autophagy and decreased capture of cytosolic GAS in autophagosomes.In contrast,the serotype M6 GAS strain JRS4(M6.JRS4),which is highly susceptible to host autophagy-mediated killing,expresses low levels of SpyCEP and does not activate calpain.Overexpression of SpyCEP in M6.JRS4 stimulated calpain activation,inhibited autophagy,and significantly decreased bacterial capture in autophagosomes.These paired loss-and gain-of-function studies reveal a novel role for the bacterial protease SpyCEP in enabling GAS M1 evasion of autophagy and host innate immune clearance.

Invasive infection caused by Streptococcus pyogenes emm89 strains has been increasing in several countries linked to a recently emergent clade of emm89 strains,designated clade 3.In Japan,the features of emm89 S.pyogenes strains,such as clade classification,remains unknown.In this study,we collected emm89 strains isolated from both streptococcal toxic shock syndrome(STSS)(89 STSS isolates)and noninvasive infections(72 non-STSS isolates)in Japan from 2011 to 2019,and conducted wholegenome sequencing and comparative analysis,which resulted in classification of a large majority into clade 3 regardless of disease severity.In addition,invasive disease-associated factors were found among emm89 strains,including mutations of control of virulence sensor,and absence of the hylP1 gene encoding hyaluronidase.These findings provide new insights into genetic features of emm89 strains.

In glaucoma,a leading cause of blindness,retinal ganglion cells are progressively damaged.Intraocular pressure reduction is the only established treatment for glaucoma(Collaborative Normal-Tension Glaucoma Study Group,1998;Vass et al.,2007).However,in some cases,visual field loss progresses despite sufficiently reduced intraocular pressure(Collaborative Normal-Tension Glaucoma Study Group,1998;Killer and Pircher,2018).While intraocular pressure and age are known risk factors for glaucoma(Ernest et al.,2013),the underlying mechanisms of glaucoma progression are not fully understood.

Aortic stenosis(AS)is the most common valvular disease in the older adults'population,and its prevalence will likely continue to increase with the aging society.[1]Surgical aortic valve replacement(SAVR)for elderly patients is known as a high-risk procedure.[2-4]Transcatheter aortic valve implantation(TAVI)is a reasonable alternative to SAVR for older and inoperable patients with AS,and TAVI significantly improves outcomes in elderly patients with severe AS and serious comorbidities,[5,6]TAVI achieves a 20%relative reduction in mortality compared with SAVR.

无法及时翻译发言内容是汉日同声传译中,译员经常遇到的问题之一。针对此课题,本文围绕译者的翻译语速是否影响译出率,以及翻译语速的不同对翻译效果所产生何种影响展开分析。首先,通过视频网站下载“日本记者俱乐部”举行的“中日同声传译招待会”有关视频,对视频内容进行转写,并记录每次发言与其被译出所需的时间。之后,计算每位发言者和译员的语速,及译语对原语信息的还原度(译出率)。在尝试对译员语速与译出率进行相关性分析后,对比持不同语速译员的译语文本,描述和总结其中呈现出的翻译特征。从分析结果来看,译员语速与译出率之间存在明显负相关,证实了随着译员语速不断提升,译出率会随之下降这一假设。同时,也发现译员翻译速度与发言者的语速之间存在强烈的正相关,这直接影响到译员语速与译出率之间的关系。此外,还发现翻译速度较快的译员,与翻译速度较慢的译员相比,总体来说水准的发挥更加稳定,每个句子的译出率变化也较小。

Perceptions of nurses in Japan toward their patients' expectations of care: A qualitative study

Objectives: This study aimed to investigate ideal nurse involvement based on the expectations of patients.Data on conflicts between nurses and patients were obtained.The patient situation involved standard nursing treatment,rather than acute phase or palliative care.Methods: Questionnaires were distributed among senior nurses attending a series of trainings in 2012 and 2013.The nurses were requested to return their completed questionnaires within two weeks.We ensured the effectiveness of the interview process to obtain accurate answers.The sample comprised 240 head nurses and assistant head nurses who were asked to respond anonymously to 57 questions about non-acute (stable) psychiatric or physical nurse-patient scenarios.Qualitative data analysis was conducted using these responses.Results: We received 41 completed responses (response rate =17.1%).The expectations of patients and their families were reflected in five categories,namely,inference,empathic understanding,listening attitude,individual treatment,and reliable skills and explanations.Inference was independently categorized as a particularly strong characteristic of Japanese patients' expectations.Conclusions: Nursing care in situations where conflicts or misunderstandings may arise can be improved by encouraging nurses to be attentive to the moods,feelings,and expectations of patients and their families.The findings from this study can improve the quality of Japanese nursing care with regard to sensing (inferring) and reacting to the expectations of patients.

Transcatheter arterial infusion for advanced hepatocellular carcinoma: Who are candidates?

AIM: To elucidate anticancer effects of transcatheter arterial infusion chemotherapy(TAI) in patients with hepatocellular carcinoma(HCC). METHODS: Data from a total of 95 patients with HCC who received TAI were analyzed retrospectively. The efficacy of TAI was evaluated according to the Response Evaluation Criteria in Cancer of the Liver. Overall survival was calculated from the date of initial treatment to the date of death or last follow-up. Survival curves were calculated by the Kaplan-Meier method, and differences in survival were evaluated by the log rank test. Clinical variables that were identified as statistically different by a univariate analysis were included into the Cox proportional hazard regression model for multivariate analysis. A prognostic index based on the regression coefficients derived from variables identified by the multivariate analysis was constructed. Stratification of the patients was conducted using this prognostic index. RESULTS: The patient group was comprised of 76 men and 19 women with an average age of 68 years(range: 37-82 years). Six patients(6.3%) showedcomplete response and 18 patients(18.9%) showed partial response, for an overall response rate of 25.2%. The median overall survival was 27.6 mo, and the proportions of survivors at 1, 2, and 5 years were 67.4%, 54.0%, and 17.4%, respectively. Multivariate analysis demonstrated that no prior transcatheter arterial chemoembolization, lactate dehydrogenase < 230 IU/L, and performance status of 0 were the independent favorable prognostic factors. The development of a 0-3-point prognostic score index was based on the sum of these three prognostic factors. Subsequently, the patients were categorized into three groups: those with a good(prognostic index = 0-1; n = 54), intermediate(prognostic index = 2; n = 26), or poor(prognostic index = 3; n = 15) prognosis. The median survival times in these three groups were 41.0, 21.2, and 6.8 mo, respectively(P < 0.01). CONCLUSION: Our simple prognostic index may be helpful for management of p
