Proposed criteria to differentiate heterogeneous eosinophilic gastrointestinal disorders of the esophagus, including eosinophilic esophageal myositis

AIM To define clinical criteria to differentiate eosinophilic gastrointestinal disorder(Eo GD) in the esophagus. METHODS Our criteria were defined based on the analyses of the clinical presentation of eosinophilic esophagitis(Eo E), subepithelial eosinophilic esophagitis(s Eo E) and eosinophilic esophageal myositis(Eo EM), identified by endoscopy, manometry and serum immunoglobulin E levels(s-Ig E), in combination with histological and polymerase chain reaction analyses on esophageal tissue *** In five patients with Eo E, endoscopy revealed longitudinal furrows and white plaques in all, and fixed rings in two. In one patient with s Eo E and four with Eo EM, endoscopy showed luminal compression only. Using manometry, failed peristalsis was observed in patients with Eo E and s Eo E with some variation, while Eo EM was associated with hypercontractile or hypertensive peristalsis, with elevated s-Ig E. Histology revealed the following eosinophils per high-power field values. Eo E = 41.4 ± 7.9 in the epithelium and 2.3 ± 1.5 in the subepithelium; s Eo E = 3 in the epithelium and 35 in the subepithelium(conventional biopsy); Eo EM = none in the epithelium, 10.7 ± 11.7 in the subepithelium(conventional biopsy or endoscopic mucosal resection) and 46.8 ± 16.5 in the muscularis propria(peroral esophageal muscle biopsy). Presence of dilated epithelial intercellular space and downward papillae elongation were specific to Eo E. Eotaxin-3, IL-5 and IL-13 were overexpressed in Eo *** Based on clinical and histological data, we identified criteria, which differentiated between Eo E, s Eo E and Eo EM, and reflected a different pathogenesis between these esophageal Eo GDs.

Seatbelt paths of the pregnant women sitting in the rear seat of a motor vehicle

Purpose： Seatbelt use during pregnancy is important to improve maternal and fetal survival after motor vehicle collisions. However, because the rear seatbelt of a motor vehicle tends to make contact with the neck, even if it is adequately used, some pregnant women sitting in the rear seat opt not to fasten the belt. The purpose of this study is to explore seatbelt-neck contact for pregnant women sitting in the rear seat of a motor vehicle. Methods： We carried out an anthropometric study. Japanese women who were _〉30 weeks pregnant （n = 12） sat in the left side of the rear seat of a typical mid-size passenger sedan and fastened the seatbelt. Seating posture was investigated by measuring the coordinates of the anthropometric data points of the pregnant women （head, shoulder, hip joint, and knee joint）. The belt path was analyzed by measuring the clearance between the belt and the sternum or navel. Results： Among the 12 pregnant women at 33.9 week _＋ 3.3 week gestation, the shoulder belt deviated to the right side and subsequently contacted to the neck in four pregnant women （Contact group）. The height of the Contact group was significantly shorter than that of Non-contact group （152.3 cm ~ 3.0 cm vs. 159.0 cm ＋ 3.3 cm, p = 0.008）. Regarding the relative position of the seatbelt to the subject＇s body, the distances from the top of the sternum to the center of the shoulder belt were significantly shorter in Contact group （3.9 cm ~ 3.5 cm） than that in the Non-contact group （8.0 cm ~ 1.6 cm, p = 0.03）. However, no significant difference was found for the distance from the umbilicus to the center of the lap belt. Conclusion： Our findings show that because of short height and late term of pregnancy with protrusion of the abdomen, the shoulder belt deviates to the right or left, avoiding the protruded uterus, and subsequently makes contact with the neck. Seatbelt systems for rear seats need to be developed to improve passenger safety, especially for pregnant women.

We make use of partitioning of the clock domains into smaller sub-domains in combination with bandwidth conversion, multiple shift frequencies and gated-clocks to achieve greater flexibility when determining an optimal test schedule under tight power constraints.

Role of p53 in lung tissue remodeling

The tumor suppressor gene p53 regulates a wide range of cellular processes including cell cycle progression,proliferation, apoptosis and tissue development and remodeling. Lung cell apoptosis and tissue remodeling have critical roles in many lung diseases. Abnormal proliferation or resistance to apoptosis of lung cells will lead to structural changes of many lung tissues,including the pulmonary vascular wall, small airways and lung parenchyma. Among the many lung diseases caused by vascular cell apoptosis and tissue remodeling are chronic obstructive pulmonary disease, bronchial asthma and pulmonary arterial hypertension. Recent advances in biology and medicine have provided new insights and have resulted in new therapeutic strategies for tissue remodeling in human and animal models. This review is focused on lung disease susceptibility associated with the p53 pathway and describes molecular mechanisms upstream and downstream of p53 in lung tissue remodeling. Improved understanding of structural changes associated with pulmonary vascular remodeling and lung cell apoptosis induced by the p53 pathway may new provide therapeutic targets.

Wireless Power Feeding with Strongly Coupled Magnetic Resonance for a Flying Object

Wireless power feeding was examined with strongly coupled magnetic resonance for an object moving in 3-D space. Electric power was transmitted from the ground to an electrically powered toy helicopter in the air. A lightweight receiver resonator was developed using copper foil. High Q of greater than 200 was obtained. One-side impedance matching the transmitter side was proposed to cope with high transmission efficiency and the receiver’s weight reduction. Results show that the efficiency drop near the ground was drastically improved. Moreover, the measured efficiency showed good agreement with theoretical predictions. A fully equipped helicopter of 6.56 g weight was lifted up with source power of about 5 W to an altitude of approximately 10 cm.

Tumor markers comprise a wide spectrum of biomacromolecules excessively synthesized by a variety of neoplastic cells. These markers can be endogenous products of highly active metabolites from malignant neoplastic cells or the products of newly activated genes. Ideally, tumor markers should be highly sensitive, specific, and reliable with a high prognostic value and organ specificity. In addition, they should reflect the tumor stage. However, no tumor markers identified thus far have all of these characteristics. Nevertheless, most tumor markers show excellent clinical relevance for monitoring the efficacy of a variety of therapies. We herein review how to use the recommended tumor markers to diagnose malignancies, such as gastrointestinal carcinoma, liver cancer, bile duct/pancreatic cancer, lung cancer, breast cancer, gynecologic cancer, and urologic cancer.

BACKGROUND Colorectal cancer(CRC) is an important disease worldwide, accounting for the second highest number of cancer-related deaths and the third highest number of new cancer cases. The blood test is a simple and minimally invasive diagnostic test. However, there is currently no blood test that can accurately diagnose *** To develop a comprehensive, spontaneous, minimally invasive, label-free, bloodbased CRC screening technique based on Raman *** We used Raman spectra recorded using 184 serum samples obtained from patients undergoing colonoscopies. Patients with malignant tumor histories as well as those with cancers in organs other than the large intestine were excluded. Consequently, the specific diseases of 184 patients were CRC(12), rectal neuroendocrine tumor(2), colorectal adenoma(68), colorectal hyperplastic polyp(18), and others(84). We used the 1064-nm wavelength laser for excitation. The power of the laser was set to 200 *** Use of the recorded Raman spectra as training data allowed the construction of a boosted tree CRC prediction model based on machine learning. Therefore, the generalized R^2 values for CRC, adenomas, hyperplastic polyps, and neuroendocrine tumors were 0.9982, 0.9630, 0.9962, and 0.9986, *** For machine learning using Raman spectral data, a highly accurate CRC prediction model with a high R^2 value was constructed. We are currently planning studies to demonstrate the accuracy of this model with a large amount of additional data.

TNF-<i>α</i>and RANKL facilitates the development of orthodontically-induced inflammatory root resorption

Background: The objective of this study was to determine the levels of tumor necrosis factor-alpha (TNF-α) and receptor activator of NF-kB ligand (RANKL) in the gingival crevicular fluid (GCF) in patients with severe root resorption after orthodontic treatment. Materials and Methods: Ten patients who had been receiving orthodontic treatment (5-control subjects and 5-severe root resorption subjects) participated in this study. GCF was collected from all patients. Subjects with severe root resorption (>1/3 of the original root length) were identified. Control group subjects with no loss of the root structure undergoing orthodontic treatment were also identified. The GCF was collected non-invasively from the mesial and distal sides of each of the upper central and lateral incisors using filter paper strips. The eluted GCF was used for a Western blot analysis with Antibodies against TNF-α and soluble RANKL (sRANKL). Ten male 6-week-old Wistar rats were subjected to orthodontic force of 50 g to induce a mesially tipping movement of the upper first molars for 7 days. The expression levels of TNF-α and RANKL proteins were determined in periodontal ligament (PDL) by immunohistochemical analysis. Results: The Western blot analysis showed that the TNF-α and sRANKL expressions were significantly higher in the severe root resorption group than in the control group. In the experimental tooth movement in vivo, resorption lacunae with multinucleated cells were observed in 50 g group. The immunoreactivity for TNF-α and RANKL was detected in PDL tissue subjected to the orthodontic force on day 7. Conclusion: These results suggest that TNF-α and RANKL play important roles in inducing or facilitating the development of orthodontically-induced inflammatory root resorption (OIIRR).

Measuring the Cognitive Impact of Laughter on Elderly People with Mild Cognitive Impairment in Japan

Objective: To evaluate the effect of laughter on cognition in elderly with mild cognitive impairment (MCI) through an appropriately designed intervention. Methods: The intervention involved watching a Japanese comedy routine (Manzai) for approximately twenty minutes, once a week for ten weeks. Participants were asked to paint, as a simple exercise, in addition to watching the show. Twenty-seven patients with MCI from the convalescent ward of a general hospital in the Kansai region of Japan. We measured cognition by evaluating five cognitive function domains before (baseline) and after the intervention. We used the Wilcoxon signed rank test, a distribution-free method, to compare baseline and post-intervention data. Ethical Consideration: Participants were given a document explaining the study. Only those who officially agreed to participate were enrolled. Results: Mean age of patients was 85.0 ± 2.8 years;average education was 8.6 ± 2.8 years. Three cognitive function domains had significantly different average scores after the intervention: 1) Exercise: 44.4 ± 8.9 points at baseline, 36.3 ± 10.2 post-intervention (p = 0.014);2) Word memory: 40.6 ± 7.2 at baseline, 43.1 ± 8.8 post-intervention (p = 0.002);and 3) Animal name recollection: 35.3 ± 8.4 at baseline, 38.1 ± 9.0 post-intervention (p = 0.003). Discussion: The intervention led to significantly higher cognitive scores in exercise, word memory, and animal name recollection domains, suggesting that interventions focused on laughter and simple exercise may improve cognition in elderly patients with MCI.
