Effect of NCPB and VSPL on pain and quality of life in chronic pancreatitis patients

AIM: To compare the effects of neurolytic celiac plexus block (NCPB) and videothoracoscopic splanchnicectomy (VSPL) on pain and quality of life of chronic pancreatitis (CP) ***: Forty-eight small duct CP patients were treated invasively with NCPB (n = 30) or VSPL (n = 18) in two non-randomized, prospective, case-controlled protocols due to chronic pain syndrome, and compared to a control group who were treated conservatively (n = 32). Visual analog scales were used to assess pain and opioid consumption rate was evaluated. In addition, the quality of life was measured using QLQ C-30 for NCPB and FACIT for *** both questionnaires covered similar problems,they could not be compared directly one with ***, the studies were compared by meta-analysis ***: Both procedures resulted in a significant positive effect on pain of CP patients. Opioids were withdrawn totally in 47.0% of NCPB and 36.4% of VSPL patients,and reduced in 53.0% and 45.4% of the respective patient groups. No reduction in opioid usage was observed in the control group. In addition, fatigue and emotional well-being showed improvements. Finally, NCPB demonstrated stronger positive effects on social support, which might possibly be attributed to earlier presentation of patients treated with ***: Both invasive pain treatment methods are effective in CP patients with chronic pain.

Effective stewardship of data is a critical precursor to making data *** goal of this paper is to bring an overview of current state of the art of data management and data stewardship planning solutions(DMP).We begin by arguing why data management is an important vehicle supporting adoption and implementation of the FAIR principles,we describe the background,context and historical development,as well as major driving forces,being research initiatives and *** we provide an overview of the current leading DMP tools in the form of a table presenting the key ***,we elaborate on emerging common standards for DMPs,especially the topic of machine-actionable *** sound DMP is not only a precursor of FAIR data stewardship,but also an integral part of it,we discuss its positioning in the emerging FAIR tools *** building and training activities are an important ingredient in the whole *** not being the primary goal of this paper,we touch also the topic of research workforce support,as tools can be just as much effective as their users are competent to use them *** conclude by discussing the relations of DMP to FAIR principles,as there are other important connections than just being a precursor.

The Influence of Leukocyte and Platelet Concentrate Enrich in Stem Cells on Bone Regeneration Processes:A Clinical and Flow Cytometry Study

This article reports the efficacy of percutaneous autologous leukocyte- and platelet-rich plasma (L-PRP) injection into delayed union site as a minimally invasive method alternative to bone marrow aspirate and open grafting techniques. Each of 15 participants was followed on a regular basis with clinical examinations, roentgenograms. The average time to union was 8.4 weeks after L-PRP injection and the union was achieved in all cases. CD34+/45+ cells counts were increased by 410% and CD34+/45- cells counts were increased by 488% on average in L-PRP. Our investigation showed that L-PRP enrich in stem cells can produce the desired stimulatory response despite a substantial amount of vital bone cells from mesenchymal line. We believe that the using of L-PRP enriched in stem cells, growth factors and antimicrobial proteins might be a promising treatment method in regenerative medicine.

IL28B polymorphism as a predictor of antiviral response in chronic hepatitis C

AIM： To evaluate the effect of single nucleotide poly- morphisms of interleukin （IL）-28B, rs12979860 on progression and treatment response in chronic hepatitis C. METHODS： Patients （n = 64; 37 men, 27 women; mean age, 44 ＋ 12 years） with chronic hepatitis C, genotype 1, received treatment with peg-interferon plus ribavirin. Genotyping of rs12979860 was per- formed on peripheral blood DNA. Histopathological assessment of necroinflammatory grade and fibrosis stage were scored using the METAVIR system on a liver biopsy sample before treatment. Serum viral load, ami- notransferase activity, and insulin level were measured. Insulin resistance index, body mass index, waist/hip ratio, percentage of body fat and fibrosis progression rate were calculated. Applied dose of interferon and ribavirin, platelet and neutrophil count and hemoglobin level were measured. RESULTS： A sustained virological response （SVR） was significantly associated with IL28B polymorphism （CC vs -l-r allele： odds ratio （OR）, 25; CC vs CT allele： OR, 5.4）, inflammation activity （G 〈 1 vs G 〉 1： OR, 3.9）, fibrosis （F 〈 1 vs F 〉 1： OR, 5.9）, platelet count （〉 200 × 109/L vs 〈 200 ×109/L： OR, 4.7; OR in patients with genotype CT： 12.8）, fatty liver （absence vs presence of steatosis： OR, 4.8）, insulin resistance index （〈 2.5 vs 〉 2.5： OR, 3.9）, and baseline HCV viral load （〈 106 IU/mL vs 〉 106 IU/mL： OR, 3.0）. There was no association with age, sex, aminotransferases activity, body mass index, waist/hip ratio, or percentage body fat. There was borderline significance （P = 0.064） of increased fibrosis in patients with the I-I allele, and no differences in the insulin resistance index between groups of patients with CC, CT and -IF alleles （P = 0.12）. Spearman＇s rank correlation coefficient between insulin resistance and stage of fibrosis and body mass index was r = 0.618 and r = 0.605, respectively （P 〈 0.001）. Introduction to the Turbulent Flows Theory An Axially-Symmetric Peaceful Flows

This paper introduces to fluid state physics （fluid mechanics） a new interpretation of physical phenomena taking place in a fluid in motion. It introduces the base of a new theory claiming that every flow has its own internal structure of motion, which is definite organization of motion, rather than a ＂molecular chaos＂, known from the fluid statics. The paper introduces the new notion of structures vector fields of power and momentum and shows every Newtonian fluid flows are dual in character. It shows that the flow of Newtonian fluid has a dual character. It demonstrates on models and further in mathematical interpretation of physical phenomena. It introduces, on the one hand, the cycloidal motion model into the fluid mechanics, ad on the other hand an addition to the known, the classical model of Poiseuille laminar motion. The theory of dualism （double nature of physical phenomena） allows the description of selected characteristics of the flow, either by using the theory ofcycloidal motion （semicycloidal）, or by using the supplemented theory of laminar motion. The dualism theory is useful to describe each type of flows both, laminar and turbulent. This paper is only an introduction to the theory. It has been assigned number 1. It has been granted a high priority, since it contains basic concepts that will be used in others, following papers of long cycle.

The Gutian Invasion in the Middle East as a Possible Anthropogenic Trigger Factor in the Development of the 4.2 ky Event

It is commonly accepted that the current global warming is caused by humans, especially by anthropogenic emission of carbon dioxide. It is justifiably considered to be one of the biggest threats for life on the planet and human civilization. However, since millennia humans have been changing the climate locally and globally. Especially after the development of agriculture and animal husbandry people have been progressively increasing the albedo of the planet. The cooling of the climate that took place since the Neolithic Revolution and lasted until the onset of the Industrial Revolution corresponds better with the growth of human population than with any other factor. The current global warming threatens with drought, hunger, migrations and inundation of coastal areas. However, throughout the human history, it was the cooling that was related to drought and hunger. The cooling and drought that occurred about 4.2 thousand years ago (the 4.2 ky event) were the most severe. It resulted in the collapse of all Neolithic civilizations. Its reason is still not explained. This paper presents a hypothesis how people could unwillingly cause change in monsoons’ strength and direction as well as global climate catastrophe that occurred 4.2 thousand years ago.

Spirituality in Vocational Training of Information and Communication Technology Professionals in the Light of David Hay’s Conception

The key objective of this article is to use the David Hay’s conception of spirituality in the vocational training of Information and Communication Technology （ICT） professionals. The purpose of this research is to investigate the connection between spirituality and vocational training of ICT professionals. The spirituality model founded by David Hay has been used in this research article which contains awareness sensing, mystery sensing, and value sensing. The paper has used qualitative methodology and instruments for data collection are biographical interview. Qualitative data have been analyzed by using Nvivo 11. Initial results show the preliminary picture of the role of spirituality in vocational training of ICT professionals.

Low Dimensionality as a Factor Stimulating Formation of the Cooper-Like Pairs Characteristic for Superconductors

The electron gas examined in a very thin potential tube exhibits some special kind of the excited pairs making them similar to the Cooper pairs. The coupling energy of the pair can be calculated as an amount of energy required to transform the excitation energy of a coupled pair into the one-electron excitation energy. For an extremely thin potential tube the coupling energy of the pair tends to infinity. The gas energy is unstable with respect to the pair excitation which provides a kind of gap near the Fermi level. A decisive part of the gap energy is due to the electron-electron interaction. The gap is attained on condition the length of a thin potential box exceeds some critical value. In the next step, a coherence length in the gas is obtained. This length, combined with a critical magnetic field representing a transition from a superconducting to a normal state, allows us to calculate the penetration depth of the magnetic field for the singlet and triplet excitations. The penetration depth together with the critical magnetic field and energy gap can provide us with a critical current, as well as critical temperature for the superconducting state.
