Health Literacy: Prevalence among Elderly Care Givers and Its Impact on the Frequency of Elderly Hospitalization and Elderly Health Related Quality of Life

Caring for the elderly is an extremely lonely and frustrating vigil. The care giver has the responsibility of caring for an elderly one to the myriad decisions on different topics such as diabetes care or cancer treatment. Care givers with low levels of health literacy have less health knowledge, worse management of chronic disease and lower use of preventive services. This study was conducted to determine the prevalence of health literacy among elderly care givers and its impact on the frequency of elderly hospitalization and elderly health related quality of life (QOL). The study was carried out on 200 elderly patients and their corresponding care givers. Rapid Estimate of Adult Literacy in Medicine (REALM) and Newest Vital Sign (NVS) were used to assess caregivers’ health literacy. Assessment of the elderly health related QOL was done by the use of short form-12 health survey (SF-12). The results were as followed, in elderly patients;92 were males and 108 were females. The mean elderly age was 69.4 ± 8.8. The mean age of caregivers was 42.1 ± 12.9 years. The prevalence of inadequate health literacy among elderly caregivers was 75.0%. There were significant associations between care givers, health literacy and the frequency of elderly hospitalization (p = 0.001), duration of hospital stay (p = 0.009), and the elderly health related QOL (p = 0.001). The study concluded that inadequate health literacy is a problem among elderly care givers in Egypt. Healthcare professionals must be made aware of this problem, which is to provide simplified educational materials to elderly care givers to maximize elderly care.

Most studies investigating genetics of dementia have focused on Alzheimer’s disease, but little is known about the genetics of vascular dementia (VD). The aim of this study was to identify the association between Apolipoprotein E4 (Apo E4) genotype and VD in cerebrally infarcted patients. The study was conducted on 100 patients with cerebral infarction: 50 had VD (cases) and 50 didn’t have dementia (controls). Diagnosis of VD was based on Mini-Mental State Examination, Cambridge Cognitive Examination (CAMCOG), the Diagnostic and Statistical Manual of Mental Disorders 4th edition criteria for the diagnosis of VD (DSM-IV), Hachinski Ischemic Score, and computed tomography of the brain (CT brain). Apo E4 allele was assessed through DNA genotyping. The study showed that hypertension (p = 0.027, OR = 4.71), diabetes mellitus (p = 0.003, OR = 6.05) and Apo E4 allele (p = 0.017, OR = 13.39) were the independent risk factors of VD among studied participants. The study concluded that cerebrally infarcted patients with Apo E4 genotype are at high risk of developing VD.

Chronic infection with hepatitis C virus (HCV) is one of the most important causes of chronic liver disease, which can progress to cirrhosis and hepatocellular carcinoma (HCC). It is well established that advanced forms of the disease are accompanied by overt and global cognitive deficits (hepatic encephalopathy) but now there is a growing evidence that the alterations in cerebral function in patients with chronic HCV infection may appear long before the development of severe liver cirrhosis, it has been hypothesized that it is related to a direct effect of HCV on the brain;or the neurotoxic effect of HCV-related systemic inflammation. The purpose of the study was to assess the possible existence of executive dysfunction in chronic HCV infected patients. One hundred elderly patients aged 60 years and above agreed to participate in this study;50 patients were HCV positive (cases) and 50 patients were HCV negative (controls). All participants were subjected to the following: diagnosis of HCV by detection of HCV antibodies using ELISA technique, non-invasive assessment of liver condition, and evaluation of executive functions using 5 neuropsychological tests. The study showed that there was a significant difference between cases and controls regarding 2 executive function tests related to the phonological loop component of working memory among HCV positive patients. The study concluded that chronic HCV infection is accompanied by dysexecutive performance.

Sarcopenia is highly prevalent in community dwelling older adults in many countries;however, the prevalence of sarcopenia in nursing home older residents is not well characterized. The aim of this study is to assess the prevalence of sarcopenia in nursing home older residents in Cairo. Cross sectional study was performed among 357 nursing home residents in Cairo. The European Working Group on Sarcopenia in Older People (EWGSOP) recommendation was used for diagnosing sarcopenia. The study showed that the prevalence of sarcopenia in nursing home older residents in Cairo was 17.7%, 22.2% in elderly men and 14.4% in elderly women. Age, underweight and lack of physical activity were found to be associated with sarcopenia among studied participants. The study concluded that sarcopenia is an emerging health problem in nursing home older residents in Cairo.

Background: Cognitive impairment is a major health issue particularly with the increasing aging population. There are around 47.5 million dementia cases globally. Traffic air pollution issue is a chief environmental problem principally in the mega cities such as Cairo. Methodology: In a Cross sectional and comparative research study the study subjects recruited involved 200 individuals, categorized into two research groups: 100 from Cairo’s elderly home residents and 100 from EL-Gharbaya’s elderly home residents. Results: Statistical linear regression analysis revealed that fine particulate matter, carbon monoxide, and nitric oxide have a statistically significant impact on cognitive function (p values Conclusions: Traffic related air pollutants were strongly associated with cognitive impairment within elderly population in geriatric home residents in Egypt. Regarding to statistically significant difference in concentration of traffic related air pollutants between urban and rural areas, urban areas were more polluted than rural areas.

Objective: This study aimed to assess the relationship of cognition, depression and anxiety to glycemic control in elders with diabetes. DM is a chronic medical condition. Its control depends on adherence to medical therapy and making decisions related to lifestyle changes. This decision making capacity is affected by many factors including cognition and psychological status. Design: It was a case control study. Setting: It was done in Ain Shams University Hospital inpatients and DM outpatient clinic, Cairo, Egypt. Participants: Of the one hundred diabetic patients aged ≥ 60 years, 50 had Hemoglobin A1c (HbA1c) ≥ 7.5 (cases) and 50 had Hb A1c 7.5 (controls). Measurements: Cognition was assessed using minimental status examination (MMSE) test, Mattis Organic Mental Syndrome Screening Examination (MOMSSE) and Cambridge Cognitive Examination (CAMCOG) test. Geriatric depression scale-15 (GDS-15) was performed for depression assessment, while anxiety was assessed by DSM IV criteria. Laboratory investigations included: fasting blood sugar (FBS), post-prandial blood sugar (PPBS), glycated haemoglobin (Hb A1c), low density lipoprotein (LDL), high-density lipoprotein (HDL), total cholesterol, and triglycerides (TG). Results: Significant difference was found between the two groups regarding scores of cognitive tests: MMSE score (p = 0.004);below average (p = 0.02) and average scores (p = 0.05) of MOMSSE;CAMCOG score (p = 0.015);and CAMCOG divided items score including orientation (p = 0.003), comprehension (p = 0.005), expression (p = 0.020), attention (p = 0.002), and abstraction (p = 0.008) as well as depression screening scores (P = 0.002). Using Receiver Operating Characteristic, CAMCOG had better sensitivity and MOMSSE had better specificity. Conclusion: Cognitive impairment was associated with poor glycemic control, and impairment in attention and abstraction, related to executive function, functions were found to be associated with poor glycemic control. Background: The 8 th edition of the American Joint Committee on Cancer(AJCC) staging system for pancreatic exocrine adenocarcinoma has been released. The current study seeks to assess the 7 th and 8 th editions among patients registered within the surveillance, epidemiology and end results(SEER) ***: SEER database(2010–2013) has been accessed through SEER*Stat program and AJCC 8 th edition stages were reconstructed utilizing the collaborative stage descriptions. Kaplan–Meier analysis of overall survival and pancreatic cancer-specific survival analyses(according to both 7 th and 8 th editions and according to whether pathological or clinical staging were conducted) has been performed. Multivariate analysis of factors affecting pancreatic cancer-specific survival was also conducted through a Cox proportional hazard ***: A total of 18 948 patients with pancreatic adenocarcinoma were identified in the period from 2010–2013. Pancreatic cancer-specific survival among pathologically staged patients and according to the 8 th edition showed significant differences for all pair wise comparisons among different stages(P < 0.0001) except for the comparison between stage IA and stage IB(P = 0.307) and the comparison between stage IB and stage IIA(P = 0.116). Moreover, P value for stage IA vs IIA was 0.014; while pancreatic cancer-specific survival according to the 7 th edition among pathologically staged patients showed significant differences for all pair wise comparisons among different stages(P < 0.0001) except for the comparison between IA and IB(P = 0.072), the comparison between stage IIA and stage IIB(P = 0.065), the comparison between stage IIA and stage Ⅲ(P = 0.059) and the comparison between IIB and Ⅲ(P = 0.595).Among clinically staged patients(i.e. those who did not undergo initial radical surgery), the prognostic performance of both 7 th and 8 th stages for both overall survival and pancreatic cancer-specific survival was ***: Th

Recent seismic events have raised concerns over the safety and vulnerability of reinforced concrete moment resisting frame ＂RC-MRF＂ buildings. The seismic response of such buildings is greatly dependent on the computational tools used and the inherent assumptions in the modelling process. Thus, it is essential to investigate the sensitivity of the response demands to the corresponding modelling assumption. Many parameters and assumptions are justified to generate effective structural finite element（FE） models of buildings to simulate lateral behaviour and evaluate seismic design demands. As such, the present study focuses on the development of reliable FE models with various levels of refinement. The effects of the FE modelling assumptions on the seismic response demands on the design of buildings are investigated. the predictive ability of a FE model is tied to the accuracy of numerical analysis; a numerical analysis is performed for a series of symmetric buildings in active seismic zones. The results of the seismic response demands are presented in a comparative format to confirm drift and strength limits requirements. A proposed model is formulated based on a simplified modeling approach, where the most refined model is used to calibrate the simplified model.
