Clinical outcomes in patients with native valve infective endocarditis and diabetes mellitus

BACKGROUND There is limited data on the difference in the clinical characteristics and outcomes of patients with severe coronavirus disease 2019(COVID-19)infection in the summer compared to the fall *** To compare the sociodemographic,clinical characteristics,and outcomes among mechanically ventilated patients with severe COVID-19 infection admitted to the intensive care unit(ICU)during the summer and fall surges in the year *** We included patients admitted to the ICU and treated with invasive mechanical ventilation for COVID-19 associated respiratory failure between April 1 and December 31,*** were categorized into summer surge for ICU admissions between June 15,2020,and August 15,2020,and fall surge between October 15,2020,and December 31,*** compared patients'characteristics and outcomes using descriptive and inferential *** A total of 220 patients were admitted to the Grady Memorial Hospital ICU and mechanically ventilated for COVID-19 associated hypoxemic respiratory failure during the period considered(125 during the summer surge and 95 during the fall surge).More women were admitted in the fall compared to summer(41.1%vs 36.8%,difference,4.3%;95%CI:1.2,7.5).Patients admitted in the fall had fewer comorbidities(chronic obstructive pulmonary disease,stroke,diabetes mellitus,obstructive sleep apnea and body mass index≥35 kg/m2).Overall,patients in the fall had a lower ICU mortality rate(27.4%vs 38.4%,difference,-11.0;95%CI:-6.4,-18.2),shorter length of stay on the mechanical ventilator(7 d vs 11 d,difference,4 d;95% CI:2.1,6.6)and shorter ICU length of stay(9 d vs 14 d,difference,5 d;95% CI:2.7,9.4).CONCLUSION Patients admitted with severe COVID-19 infection requiring mechanical ventilation had better outcomes in the fall than *** difference observed is likely attributable to a better understanding of the condition and advances in treatment strategies.

Activated Protein C Resistance in Patients with Pre-Eclampsia in Lagos, Nigeria

Background: Preeclampsia is reported to complicate 2% - 8% of pregnancies globally and is an important cause of maternal and perinatal morbidity and mortality. The aetiology and pathogenesis are still poorly understood and substantial improvement has not been made in the prediction, prevention and treatment of the disease. Objective: To compare the frequency of activated protein C resistance (APC-R) in patients with pre-eclampsia to that of normotensive pregnant women and to determine the correlation between activated protein ratio (APC-ratio) and the severity of pre-eclampsia. Methodology: A cross-sectional study was carried out in 100 pre-eclamptic patients and 100 normotensive pregnant controls. The APC-ratio was determined using the modified activated partial thromboplastin time. Study participants with APC-ratio of less than 2.0 were defined as having APC-R. Data was analyzed using SPSS version 22.0. Results: Mean APC-ratio was significantly lower in pre-eclamptics (2.89 ± 1.70) compared to normotensive pregnant women (3.57 ± 1.06) (p = 0.0008) and the levels were also higher in mild (2.95 ± 1.15) compared to severe pre-eclamptics (2.62 ± 1.14). The frequency of APC-R was 26% among women with pre-eclampsia compared to 4% among normotensive controls (p = 0.000). Among 100 pre-eclamptic women 7 (21.2%) out of 33 with mild pre–eclampsia had APC-R, while 19 (28.4%) out of 67 with severe pre-eclampsia had APC-R. APC-ratio had a significant negative correlation with mean arterial blood pressure (r = −0.324;p = 0.000) and proteinuria (r = −0.379;p = 0.000) among study participants. Conclusion: The frequency of activated protein c resistance is significantly higher in pre-eclamptics compared to normotensive pregnant women and this is more pronounced in those with severe pre-eclampsia compared with those with mild disease. APC-R may therefore be used as a marker of severity in the disease.

Nutritional assessment,glycemic indices and anti-diabetic potentials of dough meal generated from optimized blends of matured plantain,soya cake and wheat bran flours

Management of blood glucose levels associated with type 2 diabetes using nutrients derived from food without having a harmful effect on one’s health is gradually gaining research *** dough meal blends were developed using optimal mixture design of response surface methodology(RSM).The optimum blends based on the protein and crude fibre contents were as follows:PSW1:mature plantain 70.0%+soya cake 25.0%+wheat bran 5.0%;PSW2:mature plantain 68.0%+soya cake 23.0%+wheat bran 9.00%;PSW3:mature plantain 69.0%+soya cake 24.0%+wheat bran 7.0%,while 100% mature plantain flour(PLNT)and cerolina were used as *** nutritional value,amino acid profile,pasting properties,α-amylase,α-glucosidase inhibition potential,in-vitro glycemic index and consumer acceptability were determined using standard *** showed that the inclusion of soya cake and wheat bran significantly increased(P<0.05)protein 14.08 g/100 g and fibre 3.02 g/100 g as *** higher(P<0.05)branched chain and aromatic amino acids were observed in the composite dough meal compared to *** peak viscosity(Pv),trough viscosity(Tv)and final viscosity(Fv)are lower in dough meal blends with increase in incorporation of soya cake and wheat ***3 showed the highest essential amino acid index,predicted biological value,α-glucosidase,α-amylase inhibition and lowest estimated glycaemic index at(P≤0.05).Hence,the study established that PSW3 showed the best potentials to modulate blood sugar level.

Opening of liver mitochondrial permeability transition pore in streptozotocin-induced diabetic rats and its inhibition by methanol fraction of Ficus mucoso(Welw) root bark

Objective: Several pathologies arise from the inappropriate opening of the mitochondrial permeability transition(mPT) pore. In this regard, inhibition of mPT pore represents a cytoprotective approach to preserve mitochondrial function for treatment of diseases characterized by excessive tissue wastage such as diabetes mellitus. The aim of this study, therefore, was to study the effects of fractions of Ficus mucoso, a medicinal plant used in the traditional treatment of diabetes, on mPT pore in normal and streptozotocin(STZ)-induced diabetic rat ***: Different solvent fractions of the crude methanol extract of F. mucoso were obtained by vacuum liquid chromatography and were tested on the mPT pore. Of all the fractions tested, methanol fraction of F. mucoso(MFFM) was the most potent and was used for in vivo studies. Diabetes mellitus was induced by a single intraperitoneal injection of 60 mg/kg STZ, while treatment lasted for 14 d. In vivo, 30 male Wistar rats were divided into five groups: A, normo-glycemic control(distilled water);B, STZ(65 mg/kg;diabetic control);C, STZ + MFFM(25 mg/kg);D, STZ + MFFM(50 mg/kg);E, STZ + glibenclamide(5 mg/kg). The mPT, mitochondrial ATPase activity, lipid peroxidation and cytochrome c release were assessed spectrophotometrically while blood glucose levels were monitored using ***: In vitro, the solvent fractions of F. mucoso, at all concentrations tested, had no effect on the mPT pore, in the absence of calcium, with no significant release of cytochrome c. Interestingly, calciumdependent pore opening was inhibited by all solvent fractions of F. mucoso, with the MFFM having the highest inhibitory effect of 83% at 3 mg/mL. Hypoglycemic efficacy of Ocimum gratissimum and Vernonia amygdalina compared with insulin and glibenclamide in type Ⅰ and type Ⅱ diabetic rat models

Objective: To compare the efficacy of Ocimum gratissimum (O. gratissimum) and Vernonia amygdalina (V. amygdalina) with those of insulin and ***: Type Ⅰ and Ⅱ diabetes mellitus (DM) were induced by a single intraperitoneal injection of 65 mg/kg of streptozotocin and intraperitoneal administration of nicotinamide (100 mg/kg) along with streptozotocin, respectively. The state of diabetes was confirmed weekly by testing blood glucose level using a ***: The weekly blood glucose levels were higher in type I DM than in type Ⅱ DM. Type Ⅰ DM plus O. gratissimum showed a weekly progressive significant reduction in blood glucose compared to type Ⅰ DM control. Type Ⅰ DM control showed a duration dependent significant higher blood glucose concentration compared to normal control. Type I DM plus V. amygdalina also showed a time dependent significant lower glucose level compared to normal control and type Ⅰ DM control. Combination treatment of type Ⅰ DM (O. gratissimum plus V. amygdalina) showed a significantly elevated glucose concentration compared to normal control which was similar to type I DM control. Insulin treatment in type I DM showed a weekly progressive significant reduction of glucose concentration compared to normal control and type I DM control. Type Ⅱ DM control showed a fairly constant blood glucose concentration throughout the duration of treatment that was significantly higher than that of the normal control. Type Ⅱ DM plus O. gratissimum showed a fairly steady significant reduction of glucose concentration compared to type Ⅱ DM control and normal control. Type Ⅱ DM plus V. amygdalina also showed a fairly constant significant reduction of glucose concentration compared to type Ⅱ DM control and normal control.
