Genetic analysis of transcription factors in dopaminergic neuronal development in Parkinson’s disease

Background:Genetic variants of dopaminergic transcription factor-encoding genes are suggested to be Parkinson’s disease(PD)risk factors;however,no comprehensive analyses of these genes in patients with PD have been undertaken.Therefore,we aimed to genetically analyze 16 dopaminergic transcription factor genes in Chinese patients with PD.Methods:Whole-exome sequencing(WES)was performed using a Chinese cohort comprising 1917 unrelated patients with familial or sporadic early-onset PD and 1652 controls.Additionally,whole-genome sequencing(WGS)was performed using another Chinese cohort comprising 1962 unrelated patients with sporadic late-onset PD and 1279 controls.Results:We detected 308 rare and 208 rare protein-altering variants in the WES and WGS cohorts,respectively.Gene-based association analyses of rare variants suggested that MSX1 is enriched in sporadic late-onset PD.However,the significance did not pass the Bonferroni correction.Meanwhile,72 and 1730 common variants were found in the WES and WGS cohorts,respectively.Unfortunately,single-variant logistic association analyses did not identify significant associations between common variants and PD.Conclusions:Variants of 16 typical dopaminergic transcription factors might not be major genetic risk factors for PD in Chinese patients.However,we highlight the complexity of PD and the need for extensive research elucidating its etiology.

益气健脾法联合针刺治疗顽固型面瘫的临床疗效

目的研究益气健脾法联合针刺治疗顽固型面瘫的临床疗效。方法选取2021年1月到2022年2月于鄂尔多斯市中医医院治疗的60例顽固性面瘫患者作为研究对象,随机分为治疗组与对照组,每组30例。对照组采用针刺治疗,治疗组在针刺疗法上基于益气健脾类中药煎服。比较两组临床疗效、面部神经功能变化情况及不良反应发生情况。结果治疗组治疗总有效率为86.67%,高于对照组的63.33%,差异有统计学意义(P<0.05)。治疗后,两组面部功能评分均高于治疗前,且治疗组高于对照组,差异有统计学意义(P<0.05);治疗后,两组口眼歪斜、眼睑缩小、面色淡白或晦暗积分均低于治疗前,且治疗组低于对照组,差异有统计学意义(P<0.05)。两组未出现任何不良反应。结论益气健脾法联合针刺治疗顽固型面瘫效果显著,能有效减轻面部神经炎症,加快恢复面部肌群运动功能,且安全性较高,值得临床推广应用。

甲状腺癌术后疲劳综合征是甲状腺癌术后常见的临床综合征,现代医学对于本病的认识仍处于探索阶段,尚未明确其发病机制,治疗具有一定的局限性,疗效不甚显著。中医学对于本病的治疗有着独特的见解,较现代医学而言更为安全有效,具有一定的优势。中医学认为脏腑以气化为用,气化主要的运动形式为升降出入。气机调畅,气升降出入如常,机体的物质与能量得以转化,气化方能正常运行。反之,气机失畅,气化亦会失司。宋宗良教授基于气机升降理论治疗本病,认为本病的发生与肝脾气机升降失调密切相关,手术创伤,失血失液,阴血亏耗,破血夺气,中焦气机紊乱,加之术后情志郁结,肝之气机不畅,进一步影响中焦气机升降,气血运化乏源,津液输布失畅,易于疲劳。治疗法则以调畅气机为主,以加味逍遥散、柴胡疏肝散、四逆散为基础方,使肝脾气机升降转输如常,协调平衡,脏腑各司其职,诸证自除。现代药理研究亦证实加味逍遥散中的诸多中药成分具有抗肿瘤、免疫调节等诸多药理作用。此方用于临床,疗效显著,值得学习借鉴,希翼能够为甲状腺术后疲劳综合征的治疗提供一种新的治疗方案,为今后的临床研究带来一些新的灵感。笔者有幸侍诊左右,现通过总结宋教授经验对本病进行探析。

Extracellular vesicles derived from human dental mesenchymal stem cells stimulated with low-intensity pulsed ultrasound alleviate inflammation-induced bone loss in a mouse model of periodontitis

Extracellular vesicles(EVs)derived from mesenchymal stem cells(MSCs)have emerged as a new mode of intercellular crosstalk and are responsible for many of the thera-peutic effects of MSCs.To promote the application of MSC-EVs,recent studies have focused on the manipulation of MSCs to improve the production of EVs and EV-mediated activities.The current paper details an optimization method using non-invasive low-intensity pulsed ul-trasound(LIPUS)as the stimulation for improving oral MSC-EV production and effectiveness.Stem cells from apical papilla(SCAP),a type of oral mesenchymal stem cell,displayed inten-sity-dependent pro-osteogenic and anti-inflammatory responses to LIPUS without significant cytotoxicity or apoptosis.The stimuli increased the secretion of EVs by promoting the expres-sion of neutral sphingomyelinases in SCAP.In addition,EVs from LIPUS-induced SCAP exhibited stronger efficacy in promoting the osteogenic differentiation and anti-inflammation of peri-odontal ligament cells in vitro and alleviating oral inflammatory bone loss in vivo.In addition,LIPUS stimulation affected the physical characteristics and miRNA cargo of SCAP-EVs.Further investigations indicated that miR-935 is an important mediator of the pro-osteogenic and anti-inflammatory capabilities of LIPUS-induced SCAP-EVs.Taken together,these findings demonstrate that LIPUS is a simple and effective physical method to optimize SCAP-EV produc-tion and efficacy.

Systemic antibiotics increase microbiota pathogenicity and oral bone loss

Periodontitis is the most widespread oral disease and is closely related to the oral microbiota.The oral microbiota is adversely affected by some pharmacologic treatments.Systemic antibiotics are widely used for infectious diseases but can lead to gut dysbiosis,causing negative effects on the human body.Whether systemic antibiotic-induced gut dysbiosis can affect the oral microbiota or even periodontitis has not yet been addressed.In this research,mice were exposed to drinking water containing a cocktail of four antibiotics to explore how systemic antibiotics affect microbiota pathogenicity and oral bone loss.The results demonstrated,for the first time,that gut dysbiosis caused by long-term use of antibiotics can disturb the oral microbiota and aggravate periodontitis.Moreover,the expression of cytokines related to Th17 was increased while transcription factors and cytokines related to Treg were decreased in the periodontal tissue.Fecal microbiota transplantation with normal mice feces restored the gut microbiota and barrier,decreased the pathogenicity of the oral microbiota,reversed the Th17/Treg imbalance in periodontal tissue,and alleviated alveolar bone loss.This study highlights the potential adverse effects of long-term systemic antibiotics-induced gut dysbiosis on the oral microbiota and periodontitis.A Th17/Treg imbalance might be related to this relationship.Importantly,these results reveal that the periodontal condition of patients should be assessed regularly when using systemic antibiotics in clinical practice.

【目的】检索并评价国内外关于临终癌症患者安宁疗护中营养管理的相关证据,为临床工作者提供循证依据。【方法】根据证据资源“6S”模型进行文献检索,检索时间为建库至2022年12月,由2名研究员对纳入文献进行质量评价及证据提取。【结果】共纳入13篇文献,提取28条循证证据,其中A级推荐24条,B级推荐4条。构建营养支持团队是临终癌症患者营养管理的坚实基础,提供药物干预与非药物干预是临终癌症患者营养管理的关键环节,采取肠内、肠外营养是临终癌症患者营养管理的补充手段。【结论】临床工作人员可参考临终癌症患者安宁疗护中营养管理的循证证据,并充分考虑患者及家属的个人意愿,为患者制定个性化的营养支持方案。

Green macroalgae bloom(GMB),with the dominant species of Ulva prolifera,has regularly occurred since 2007 along the China coast.Although disaster prevention and control achieved favorable results in 2020,the satellite-observed GMB annual maximum coverage(AMC)rebounded sharply in 2021 to an unprecedented level.The reasons for this rebound and the significant interannual variability over past 15 years are still open questions.Here,by using long-term time-series(2007-2022)optical and Synthetic Aperture Radar satellite observations(1000+scenes),meteorological data and water quality statistics,the mechanism analysis was performed by exploring effects from natural factors and human activities.Two key determinants for AMC are successfully identified from numerous potential factors which are the macroalgae distribution in a key area(the Subei Shoal)during a critical period(from April to May 20)and the nutrient availability.Furthermore,by using these two parameters,a novel model for AMC prediction(R^(2)=0.87,p<0.01)is proposed and independently validated,which can reasonably explain the significant interannual variability(2014-2021)and agree well with the latest observation in 2022(percentage difference 12%).Finally,suggestions are proposed for future disaster prevention and alleviation.This work may aid future bloom prediction and management measure optimization.

Observation on the Curative Effect of Auricular Acupoint Sticking Combined with Acupoint Sticking in the Treatment of Diabetic Constipation

The study was conducted to observe the clinical effect of acupoint and auricular point application on diabetes patients with constipation.Around 104 patients with diabetes and constipation were selected from the Hospital Shaanxi University of Chinese Medicine from February 2021 to February 2022 as the study subjects.They were divided into groups by drawing lots.The experimental group carried out the combined analysis of acupoint and auricular point application based on the treatment methods of the control group.Meanwhile,the control group carried out the traditional treatment,and the evaluation index,including the improvement of constipation and the clinical effective rate data were included in the analysis.This study showed that,the total effective number of patients,times of defecation per week,time of defecation,and interval of defecation in the experimental group were greater,compared to control group(p<0.05).In summary,diabetes patients with constipation,the auricular plaster and acupoint application can effectively improve the constipation symptoms,shorten the defecation time,improve the cure rate of the disease,and ensure the normal defecation cycle in the patients.

Clinical Effect of Laser Treatment Combined with Compound Xueshuantong Capsule in the Treatment of Diabetic Retinopathy

Objective:To determine the clinical value of compound Xueshuantong capsule in the treatment of diabetic retinopathy on the basis of laser treatment.Methods:Data were collected from 98 patients with diabetic retinopathy from February 2021 to February 2022.They were divided into two groups by drawing lots.The experimental group was treated with compound Xueshuantong capsule and laser treatment,whereas the control group was treated with laser treatment alone.Results:The absorption of exudation,resolution of retinal edema,and absorption of bleeding took longer in the control group;the incidence of visual acuity recovery to more than 0.5 and the patients’blood glucose levels were better in the experimental group,p<0.05.Conclusion:For patients with diabetic retinopathy,laser treatment combined with compound Xueshuantong capsule can effectively improve the visual function and rehabilitation efficiency of patients with diabetic retinopathy as well as stabilize their blood sugar levels.It has high clinical application value.
