BACKGrOUND Although surgical resection is associated with the best long-term outcomes for neuroendocrine liver metastases(NELM),the current indications for and outcomes of surgery for NELM from a population perspective are not well *** To determine the current indications for and outcomes of liver resection(Lr)for NELM using a population-based *** A retrospective review of the 2014-2017 American College of Surgeons National Surgical Quality Improvement Program and targeted hepatectomy databases was performed to identify patients who underwent Lr for *** characteristics and 30-d morbidity and mortality were *** Among 669 patients who underwent Lr for NELM,the median age was 60(interquartile range:51-67)and 51%were *** the number of metastases resected ranged from 1 to 9,the most common(45%)number of tumors resected was *** majority(68%)of patients had a largest tumor size ofrwent partial hepatectomy(71%)while fewer underwent a right or left hepatectomy or *** majority of operations were open(82%)versus laparoscopic(17%)or robotic(1%).In addition,30%of patients underwent intraoperative ablation while 45%had another concomitant operation including cholecystectomy(28.8%),bowel resection(20.2%),or partial pancreatectomy(3.4%).Overall 30-d morbidity and mortality was 29%and 1.3%,*** multivariate analysis,American Society of Anesthesiologists class≥3[odds ratios(Or),Or=2.089,95%confidence intervals(CI):1.197-3.645],open approach(Or=1.867,95%CI:1.148-3.036),right hepatectomy(Or=1.618,95%CI:1.014-2.582),and prolonged operative time of>230 min(Or=1.731,95%CI:1.168-2.565)were associated with higher 30-d morbidity while intraoperative ablation and concomitant procedures were *** Lr for NELM was performed with relatively low postoperative morbidity and *** procedures performed at the time of Lr did not increase morbidity.

Well-differentiated neuroendocrine tumors(NETs)are globally increasing in prevalence and the liver is the most common site of *** liver metastases(NELM)are heterogeneous in clinical presentation and ***,recent advances in diagnostic techniques and therapeutic strategies have improved the multidisciplinary management of this challenging *** feasible,surgical resection of NELM offers the best long-term *** indications for hepatic resection include performance status acceptable for major liver surgery,grade 1 or 2 tumors,absence of extrahepatic disease,adequate size and function of future liver remnant,and feasibility of resecting>90%of *** therapies including concomitant liver ablation are generally safe when used appropriately and may expand the number of patients eligible for *** patients with synchronous resectable NELM,resection of the primary either in a staged or combined fashion is *** patients who are not surgical candidates,liver-directed therapies such as transarterial embolization,chemoembolization,and radioembolization can provide locoregional control and improve symptoms of carcinoid *** systemic therapy options also exist for patients with advanced or progressive *** research efforts are needed to identify novel biomarkers that will define the optimal indications for and sequencing of treatments to be delivered in a personalized fashion.

The nanostructure composite coating is obtained via plasma spraying of Al2O3-13 wt pct TiO2 powder. Brittle and hard lamella results from melted nanostructured powder. Ductile nanostructured matrix forms from unmelted nanostructured particles. Through the adjustment of constituent and nanostructure, hardness/strength and toughness/ductility are balanced and overall properties of the structure composite are achieved.

Fiber-reinforced polymer(FrP)bars are widely used as internal reinforcement replacing the conventional steel bars to prevent from *** the different types of FrP bars,basalt FrP(BFrP)bars have been used in different structural applications and,herein,three already tested concrete beams reinforced with BFrP bars are analyzed using three-dimensional(3-D)finite element analysis(FEA).The beams were tested in four-point *** the FEA the behavior of concrete is simulated using the^Concrete-Damaged Plasticity^^model offered in ABAQUS *** research presented here presents a calibrated model for nonlinear FEA of BFrP concrete beams to predict their response considering both the accuracy and the computational *** calibration process showed that the concrete model should be regularized using a mesh-dependent characteristic length and material-dependent post-yield fracture and crushing energies to provide accurate mesh-size independent *** results were compared to the test results with regard to failure load and crack *** the test results and the numerical results were compared to the design predictions of ACI 440.1r-15 and CSA S806-12,where CSA S806-12 seems to overestimate the shear strength for two beams.

The network switches in the data plane of Software Defined Networking (SDN) are empowered by an elementary process, in which enormous number of packets which resemble big volumes of data are classified into specific flows by matching them against a set of dynamic rules. This basic process accelerates the processing of data, so that instead of processing singular packets repeatedly, corresponding actions are performed on corresponding flows of packets. In this paper, first, we address limitations on a typical packet classification algorithm like Tuple Space Search (TSS). Then, we present a set of different scenarios to parallelize it on different parallel processing platforms, including Graphics Processing Units (GPUs), clusters of Central Processing Units (CPUs), and hybrid clusters. Experimental results show that the hybrid cluster provides the best platform for parallelizing packet classification algorithms, which promises the average throughput rate of 4.2 Million packets per second (Mpps). That is, the hybrid cluster produced by the integration of Compute Unified Device Architecture (CUDA), Message Passing Interface (MPI), and OpenMP programming model could classify 0.24 million packets per second more than the GPU cluster scheme. Such a packet classifier satisfies the required processing speed in the programmable network systems that would be used to communicate big medical data.
