Surgical management of facial dog bite trauma:A contemporary perspective and review

Objective:To perform a review evaluating management of and complications stemming from dog bite trauma sustained to the head and neck over the past decade.Data Sources:PubMed and Cochrane Library.Methods:The authors searched the PubMed and Cochrane Library databases for relevant published literature.A total of 12 peer-reviewed canine-exclusive series inclusive of 1384 patient cases describing facial dog bite trauma met inclusion criteria.Wounds including fractures,lacerations,contusions,and other soft-tissue injuries were evaluated.Demographics related to clinical course and management,operating room requirements,and antibiotic usage were compiled and analyzed.Initial trauma and surgical management complications were also assessed.Results:75.5%of patients sustaining dog bites required surgical intervention.Of these patients,7.8%suffered from postsurgical complications,including hypertrophic scarring(4.3%),postoperative infection(0.8%),or nerve deficits and persistent paresthesias(0.8%).Prophylactic antibiotics were administered to 44.3%of patients treated for facial dog bites and the overall infection rate was 5.6%.Concomitant fracture was present in 1.0%of patients.Conclusion:Primary closure,often in the OR may be necessary,with few cases requiring grafts or flaps.Surgeons should be aware that the most common complication is hypertrophic scarring.Further research is needed to elucidate the role of prophylactic antibiotics.

Objectives: The objective is to determine whether there are differences in the position of the hyoid bone at rest in natural head position in subjects with mandibular hyperdivergence and to evaluate whether there are differences in hyoid position and antegonial notch depth in mandibular hyperdivergent males and females. Methods and Materials: This is a retrospective cohort study involving a review of lateral cephalometric radiographs of 45 adult men and women with mandibular hyperdivergency. Hyperdivergency was determined by cephalometric ranges of: SN-GoGn as least +2 SD from normal, Y-axis, PP-GoGN, and gonial angle greater than +1SD from normal. Current large scale biomass energy production systems including cellulosic ethanol,gasification,and pyrolysis facilities face significant technical and economic hurdles.Compared with these large scale systems,small distributed biomass energy production systems(DBEPS)are believed to offer advantages including lower capital costs,lower feedstock costs,simplified transportation and logistics and higher returns for biomass producers.DBEPS compliant technologies are expected to make utilization of regional biomass supplies practical and economically viable in the near-term.This paper presents arguments on the need and importance of DBEPS,available DBEPS options,and an economic scenario of DBEPS implementation on an average size farm in the US.

Fluorescence molecular imaging enables the visualization of basic molecular processes such as gene expression,enzyme activity,and disease-specific molecular interactions in vivo using targeted contrast agents,and therefore,is being developed for early detection and in situ characterization of breast cancers.Recent advances in developing near-infrared fluorescent imaging contrast agents have enabled the specific labeling of human breast cancer cells in mouse model systems.In synergy with contrast agent development,this paper describes a needle-based fluorescence molecular imaging device that has the strong potential to be translated into clinical breast biopsy procedures.This microendoscopy probe is based on a gradient-index(GRIN)lens interfaced with a laser scanning microscope.Specifications of the imaging performance,including the field-of-view,transverse resolution,and focus tracking characteristics were calibrated.Orthotopic MDA-MB-231 breast cancer xenografts stably expressing the tdTomato red fluorescent protein(RFP)were used to detect the tumor cells in this tumor model as a proof of principle study.With further development,this technology,in conjunction with the development of clinically applicable,injectable fluorescent molecular imaging agents,promises to perform fluorescence molecular imaging of breast cancers in vivo for breast biopsy guidance.

Objectives The predisposition of intracranial atherosclerotic disease(ICAD)to East Asians over Caucasians infers a genetic basis which,however,remains largely unknown.Higher prevalence of vascular risk factors(VRFs)in Chinese over Caucasian patients who had a stroke,and shared risk factors of ICAD with other stroke subtypes indicate genes related to VRFs and/or other stroke subtypes may also contribute to ICAD.Methods Unrelated symptomatic patients with ICAD were recruited for genome sequencing(GS,60-fold).Rare and potentially deleterious single-nucleotide variants(SNVs)and small insertions/deletions(InDels)were detected in genome-wide and correlated to genes related to VRFs and/or other stroke subtypes.Rare aneuploidies,copy number variants(CNVs)and chromosomal structural rearrangements were also investigated.Lastly,candidate genes were used for pathway and gene ontology enrichment analysis.Results Among 92 patients(mean age at stroke onset 61.0±9.3 years),GS identified likely ICAD-associated rare genomic variants in 54.3%(50/92)of patients.Forty-eight patients(52.2%,48/92)had 59 rare SNVs/InDels reported or predicted to be deleterious in genes related to VRFs and/or other stroke subtypes.None of the 59 rare variants were identified in local subjects without ICAD(n=126).31 SNVs/InDels were related to conventional VRFs,and 28 were discovered in genes related to other stroke subtypes.Our study also showed that rare CNVs(n=7)and structural rearrangement(a balanced translocation)were potentially related to ICAD in 8.7%(8/92)of patients.Lastly,candidate genes were significantly enriched in pathways related to lipoprotein metabolism and cellular lipid catabolic process.Conclusions Our GS study suggests a role of rare genomic variants with various variant types contributing to the development of ICAD in Chinese patients.

Mass-selected anionic cobalt clusters of two different sizes （Co2 and Co13） were deposited onto ultrathin NaC1 films grown on an Au（111） substrate. Using scanning tunneling microscopy experiments and density functional theory simulations, we show that the deposited Co2 cluster dissociates and that the resulting Co atoms dope the NaCI surface by substituting Na ions. In contrast, the larger Co13 cluster does not dissociate and remains stable on top of the NaC1 film. The size- dependent fragmentation of clusters is an important aspect in the understanding of the chemical interaction between size-selected small aggregates of atoms and supporting surfaces.

Dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)enables tumor vascular physiology to be assessed.Within the tumor tissue,contrast agents(gadolinium chelates)extravasate from intravascular into the extravascular extracellular space(EES),which results in a signal increase on T1-weighted MRI.The rate of contrast agents extravasation to EES in the tumor tissue is determined by vessel leakiness and blood flow.Thus,the signal measured on DCE-MRI represents a combination of permeability and perfusion.The semi-quantitative analysis is based on the calculation of heuristic parameters that can be extracted from signal intensity-time curves.These enhancing curves can also be deconvoluted by mathematical modeling to extract quantitative parameters that may reflect tumor perfusion,vascular volume,vessel permeability and angiogenesis.Because hepatocellular carcinoma(HCC)is a hypervascular tumor,many emerging therapies focused on the inhibition of angiogenesis.DCE-MRI combined with a pharmacokinetic model allows us to produce highly reproducible and reliable parametric maps of quantitative parameters in HCC.Successful therapies change quantitative parameters of DCE-MRI,which may be used as early indicators of tumor response to anti-angiogenesis agents that modulate tumor vasculature.In the setting of clinical trials,DCE-MRI may provide relevant clinical information on the pharmacodynamic and biologic effects of novel drugs,monitor treatment response and predict survival outcome in HCC patients.

Molecular Evolution of VEF-Domain-Containing PcG Genes in Plants

Arabidopsis VERNALIZATION2 （VRN2）, EMBRYONIC FLOWER2 （EMF2）, and FERTILIZATION-INDEPENDENT SEED2 （FIS2） are involved in vernalization-mediated flowering, vegetative development, and seed development, respectively. Together with Arabidopsis VEF-L36, they share a VEF domain that is conserved in plants and animals. To investigate the evolution of VEF-domain-containing genes （VEF genes）, we analyzed sequences related to VEF genes across land plants. To date, 24 full-length sequences from 11 angiosperm families and 54 partial sequences from another nine families were identified. The majority of the full-length sequences identified share greatest sequence similarity with and possess the same major domain structure as Arabidopsis EMF2. EMF2-1ike sequences are not only widespread among angiosperms, but are also found in genomic sequences of gymnosperms, lycophyte, and moss. No FIS2- or VEF-L36-1ike sequences were recovered from plants other than Arabidopsis, including from rice and poplar for which whole genomes have been sequenced. Phylogenetic analysis of the full-length sequences showed a high degree of amino acid sequence conservation in EMF2 homologs of closely related taxa. VRN2 homologs are recovered as a clade nested within the larger EMF2 clade. FIS2 and VEF-L36 are recovered in the VRN2 clade. VRN2 clade may have evolved from an EMF2 duplication event that occurred in the rosids prior to the divergence of the eurosid I and eurosid II lineages. We propose that dynamic changes in genome evolution contribute to the generation of the family of VEF-domain-containing genes, Phylogenetic analysis of the VEF domain alone showed that VEF sequences continue to evolve following EM F2NRN2 divergence in accordance with species relationship. Existence of EMF2-1ike sequences in animals and across land plants suggests that a prototype form of EMF2 was present prior to the divergence of the plant and animal lineages.
