Positive association between triglyceride glucose index and central systolic blood pressure among hypertensive adults

BACKGROUND While studies have suggested the association between triglyceride-glucose(TyG)index,a reliable surrogate for insulin resistance and hypertension data are limited to the correlation of TyG and central blood pressure.This study aims to test the hypothesis that a higher TyG index is associated with elevated central systolic blood pressure(cSBP).METHODS A total of 9249 Chinese hypertensive adults from the H-type Hypertension and Stroke Prevention and Control Project were analyzed in this study.cSBP was measured noninvasively using an A-Pulse CASPro device.TyG index was calculated as ln[fasting triglycerides(mg/dL)×fasting glucose(mg/dL)/2].Smoothing curve and multivariate linear regression models[beta coefficient(β)with 95%CI]were applied to analyze the association between TyG index and cSBP.Subgroup analyses were conducted to explore potential modifications to such a correlation.RESULTS The overall mean TyG index is 8.8±0.7,and the total mean cSBP is 131.3±12.8 mmHg.TyG index was observed to be independently and positively associated with cSBP among the total population(β=0.92,95%CI:0.53–1.31,P<0.001),and participants who do not use antihypertensive drugs(β=1.03,95%CI:0.46–1.60,P<0.001),which is in accordance with the result of the smoothing curve.The association between TyG index and cSBP appears robust in all tested subgroups.CONCLUSIONS TyG index is positively and independently associated with cSBP among hypertensive adults.Our study result suggests that TyG index might serve as an effective marker for vascular function.

BACKGROUND The cardiovascular hazards of total homocysteine(tHcy)are long known.In addition,despite the acknow-ledgment on the importance of low ankle-brachial index(ABI)(<0.9),borderline ABI(0.91-0.99)was once commonly overlooked.This study aims to explore the independent and joint effect of tHcy level and borderline ABI on all-cause death in hypertensive population.METHODS This study included 10,538 participants from China H-type Hypertension Registry Study.ABI was described into two groups:normal ABI(1.00-1.40)and borderline ABI.tHcy level was also divided into two groups:<15.02 and≥15.02μmo/L.Four groups were analyzed,using COX proportional hazard regression model,separately and pairwise to observe the independ-ent and joint effect on all-cause death.RESULTS A total of 126(1.2%)deaths were observed in the 1.7 years follow-up time.Borderline ABI has a higher predicted risk of death than normal ABI(HR=1.87,95%CI:1.17-3.00)after adjusting for potential covariates.Compare with tHcy level<15.02μmo/L(low tHcy),those with tHcy≥15.02μmo/L(high tHcy)had higher risk to event outcome(HR=1.99,95%CI:1.30-3.05).According to the cumulative hazard curve,group with borderline ABI and high tHcy level has significantly higher altitude and larger increasing rate over follow-up period compare to other groups.Among those with borderline ABI,participants with high tHcy had higher death risk than those with low tHcy,nevertheless,no significant different between borderline and normal ABI among those with low tHcy levels.CONCLUSIONS Borderline ABI and tHcy level both have independent predictive value on all-cause death.The combined group of borderline ABI and high tHcy has highest risk factor of outcomes,which suggested the mutual additive value of border-line ABI and tHcy.More attention should be given to the importance of borderline ABI in hypertensive population,especially with elevated tHcy level.

The solar flare is a sudden eruptive solar phenomenon with significant enhancements in solar X-ray and Extreme Ultraviolet radiations,resulting in large amounts of energy being injected into the planetary atmosphere.Case studies have been extensively presented to analyze the effect of extremely large flares on the Martian upper atmosphere,but the general features of the Martian thermospheric response to flares are still poorly understood.In this work,we select 12 intense solar flares that occurred between 2015 and 2017 and investigate the densities and compositional variations of the dayside Martian thermosphere to these flares with the aid of the measurements made by the Mars Atmosphere and Volatile EvolutioN.The statistical studies indicate that the responses of the Martian thermosphere to flares are complicated that both the class of the flare and the wavelength of the enhanced radiation may have prominent influences on the thermal expansion of the atmosphere and the atmospheric photochemical reactions.

Dynamic Setting Method of Assessment Indicators for Power Curves of Renewable Energy Sources Considering Scarcity of Reserve Resources

With the increasing proportion of renewable energy sources(RESs)in power grid,the reserve resource(RR)scarcity for correcting power deviation of RESs has become a potential issue.Consequently,the power curve of RES needs to be more rigorously assessed.The RR scarcity varies during different time periods,so the values of assessment indicators should be dynamically adjusted.The assessment indicators in this paper include two aspects,i.e.,deviation exemption ratio and penalty price.Firstly,this paper proposes a method for dynamically calculating the supply capacity and RR cost,primarily taking into account the operating status of thermal units,forecast information of RES,and load curve.Secondly,after clarifying the logical relationship between the degree of RR scarcity and the values of assessment indicators,this paper establishes a mapping function between them.Based on this mapping function,a dynamic setting method for assessment indicators is proposed.In the future,RES will generally be equipped with battery energy storage systems(BESSs).Reasonably utilizing BESSs to reduce the power deviation of RESs can increase the expected income of RESs.Therefore,this paper proposes a power curve optimization strategy for RESs considering self-owned BESSs.The case study demonstrates that the dynamic setting method of assessment indicators can increase the revenue of RESs while ensuring that the penalty fees paid by RESs to the grid are sufficient to cover the RR costs.Additionally,the power curve optimization strategy can help RESs further increase income and fully utilize BESSs to reduce power deviation.

BACKGROUND Hepatic arterial infusion chemotherapy(HAIC)has been proven to be an ideal choice for treating unresectable hepatocellular carcinoma(uHCC).HAIC-based treatment showed great potential for treating uHCC.However,large-scale studies on HAIC-based treatments and meta-analyses of first-line treatments for uHCC are lacking.AIM To investigate better first-line treatment options for uHCC and to assess the safety and efficacy of HAIC combined with angiogenesis inhibitors,programmed cell death of protein 1(PD-1)and its ligand(PD-L1)blockers(triple therapy)under real-world conditions.METHODS Several electronic databases were searched to identify eligible randomized controlled trials for this meta-analysis.Study-level pooled analyses of hazard ratios(HRs)and odds ratios(ORs)were performed.This was a retrospective single-center study involving 442 patients with uHCC who received triple therapy or angiogenesis inhibitors plus PD-1/PD-L1 blockades(AIPB)at Sun Yat-sen University Cancer Center from January 2018 to April 2023.Propensity score matching(PSM)was performed to balance the bias between the groups.The Kaplan-Meier method and cox regression were used to analyse the survival data,and the log-rank test was used to compare the suvival time between the groups.RESULTS A total of 13 randomized controlled trials were included.HAIC alone and in combination with sorafenib were found to be effective treatments(P values for ORs:HAIC,0.95;for HRs:HAIC+sorafenib,0.04).After PSM,176 HCC patients were included in the analysis.The triple therapy group(n=88)had a longer median overall survival than the AIPB group(n=88)(31.6 months vs 14.6 months,P<0.001)and a greater incidence of adverse events(94.3%vs 75.4%,P<0.001).CONCLUSION This meta-analysis suggests that HAIC-based treatments are likely to be the best choice for uHCC.Our findings confirm that triple therapy is more effective for uHCC patients than AIPB.

Estimation of Physiologic Ability and Surgical Stress scoring system for predicting complications following abdominal surgery: A metaanalysis spanning 2004 to 2022

BACKGROUND Postoperative complications remain a paramount concern for surgeons and healthcare practitioners.AIM To present a comprehensive analysis of the Estimation of Physiologic Ability and Surgical Stress(E-PASS)scoring system’s efficacy in predicting postoperative complications following abdominal surgery.METHODS A systematic search of published studies was conducted,yielding 17 studies with pertinent data.Parameters such as preoperative risk score(PRS),surgical stress score(SSS),comprehensive risk score(CRS),postoperative complications,post-operative mortality,and other clinical data were collected for meta-analysis.Forest plots were employed for continuous and binary variables,withχ2 tests assessing heterogeneity(P value).RESULTS Patients experiencing complications after abdominal surgery exhibited significantly higher E-PASS scores compared to those without complications[mean difference and 95%confidence interval(CI)of PRS:0.10(0.05-0.15);SSS:0.04(0.001-0.08);CRS:0.19(0.07-0.31)].Following the exclusion of low-quality studies,results remained valid with no discernible heterogeneity.Subgroup analysis indicated that variations in sample size and age may contribute to hetero-geneity in CRS analysis.Binary variable meta-analysis demonstrated a correlation between high CRS and increased postoperative complication rates[odds ratio(OR)(95%CI):3.01(1.83-4.95)],with a significant association observed between high CRS and postoperative mortality[OR(95%CI):15.49(3.75-64.01)].CONCLUSION In summary,postoperative complications in abdominal surgery,as assessed by the E-PASS scoring system,are consistently linked to elevated PRS,SSS,and CRS scores.High CRS scores emerge as risk factors for heightened morbidity and mortality.This study establishes the accuracy of the E-PASS scoring system in predicting postoperative morbidity and mortality in abdominal surgery,underscoring its potential for widespread adoption in effective risk assessment.

Unnatural amino acids(UAAs)have gained significant attention in protein engineering and drug development owing to their ability to introduce new chemical functionalities to proteins.In eukaryotes,genetic code expansion(GCE)enables the incorporation of UAAs and facilitates posttranscriptional modification(PTM),which is not feasible in prokaryotic systems.GCE is also a powerful tool for cell or animal imaging,the monitoring of protein interactions in target cells,drug development,and switch regulation.Therefore,there is keen interest in utilizing GCE in eukaryotic systems.This review provides an overview of the application of GCE in eukaryotic systems and discusses current challenges that need to be addressed.

Mortality from cerebrovascular diseases in China:Exploration of recent and future trends

Background:Cerebrovascular disease(CVD)ranks among the foremost factors responsible for mortality on a global scale.The mortality patterns of CVDs and temporal trends in China need to be well-illustrated and updated.Methods:We collected mortality data on patients with CVD from Chinese Center for Disease Control and Prevention’s Disease Surveillance Points(CDC-DSP)system.The mortality of CVD in 2020 was described by age,sex,residence,and region.The temporal trend from 2013 to 2019 was evaluated using joinpoint regression,and estimated rates of decline were extrapolated until 2030 using time series models.Results:In 2019,the age-standardized mortality in China(ASMRC)per 100,000 individuals was 113.2.The ASMRC for males(137.7/10^(5))and rural areas(123.0/10^(5))were both higher when stratified by gender and urban/rural residence.The central region had the highest mortality(126.5/10^(5)),the western region had a slightly lower mortality(123.5/10^(5)),and the eastern region had the lowest mortality(97.3/10^(5)).The age-specific mortality showed an accelerated upward trend from aged 55-59 years,with maximum mortality observed in individuals over 85 years of age.The age-standardized mortality of CVD decreased by 2.43%(95%confidence interval,1.02-3.81%)annually from 2013 to 2019.Notably,the age-specific mortality of CVD increased from 2013 to 2019 for the age group of over 85 years.In 2020,both the absolute number of CVD cases and the crude mortality of CVD have increased compared to their values in 2019.The estimated total deaths due to CVD were estimated to reach 2.3 million in 2025 and 2.4 million in 2030.Conclusion:The heightened focus on the burden of CVD among males,rural areas,the central and western of China,and individuals aged 75 years and above has emerged as a pivotal determinant in further decreasing mortalities,consequently presenting novel challenges to strategies for disease prevention and control.

First Passage Density of Brownian Motion with Two-sided Piecewise Linear Boundaries

The first passage time has many applications in fields like finance,econometrics,statistics,and biology.However,explicit formulas for the first passage density have only been obtained for a few cases.This paper derives an explicit formula for the first passage density of Brownian motion with twosided piecewise continuous boundaries which may have some points of discontinuity.Approximations are used to obtain a simplified formula for estimating the first passage density.Moreover,the results are also generalized to the case of two-sided general nonlinear boundaries.Simulations can be easily carried out with Monte Carlo method and it is demonstrated for several typical two-sided boundaries that the proposed approximation method offers a highly accurate approximation of first passage density.

Effects of acid mine drainage on photochemical and biological degradation of dissolved organic matter in karst river water

Dissolved organic matter(DOM)can be removed or transformed by photochemical and biological processes,producing the negative effect of transforming organic carbon into inorganic carbon,which plays a vital role in the karst carbon cycle.However,acid mine drainage(AMD)will affect this process,so the degradation of DOM in karst river water(KRW)needs to be studied in this context.In this study,to reveal the evolution processes of DOM under photochemical and biological conditions in AMD-impacted KRW,AMD and KRW were mixed in different ratios under conditions of visible light irradiation(VL),biodegradation(BD),ultraviolet irradiation(UV)and ultraviolet irradiation+biodegradation(UV+BD).The average DOC concentrations in samples after mixing AMD and KRW in different proportions decreased significantly(by 23%)in UV+BD,which was 1.2–1.4 times higher than under the other conditions and would lead to a significant release of inorganic carbon.Further analysis of the fluorescence parameters via parallel factor analysis(PARAFAC)revealed that the DOM fluorescence components in AMD comprised mainly protein-like substances derived from autochthonous components,while the DOM fluorescence components in KRW were mainly humic-like substances with both autochthonous and allochthonous sources.Therefore,AMD could promote both the photochemical and biological degradation of DOM in karst receiving streams,resulting in the conversion of DOC to inorganic carbon.The results showed that the synergistic effects of UV+BD and AMD accelerated the degradation of DOM and the release of inorganic carbon in KRW,thus affecting the stability of the karst carbon cycle.
