Connective tissue diseases (CTDs) are Autoimmune diseases (AIDs) characterized by the appearance of autoantibodies, which are diagnostic markers. Investigations of these autoantibodies play a major role in the management of several autoimmune diseases. The objective of this study was to describe the profile of anti-ENA antibodies according to the clinical symptoms of mixed CTDs in Conakry teaching Hospital. We performed a cross-sectional study during six months. A total of 20 patients was recruited and we measured antibodies using the ELISA technique. The mean age of our patients was 36.5 years, with a predominance of females. Cutaneous and rheumatological signs were the main clinical manifestations. SLP was the most frequent CTDs;the threshold of ENA antibodies positivity was higher in scleroderma with and SLP. Anti-ENA identification reveals the frequency of anti-SSA (83.33%), anti-u1RNP (66.66%) and anti-histone (50%) antibodies. Antinuclear antibodies (ANA) react with various components of the cell nucleus. Their detection is of major interest in the diagnosis of CTDs. Our results highlight the importance of determining the specificity of these antibodies to guide differential diagnosis.

债务问题是撒哈拉以南非洲国家面临的重要经济发展问题。合理利用外部投资、举借部分外债发展本国经济,是大部分非洲国家应对疫情冲击、融入经济全球化、实现国民经济发展的重要手段。然而,过度依赖外部资金无法按时偿还外债,则对经济发展产生负面影响。学界对于外债和经济增长关系的研究一般分为三类,包括外债对经济增长的积极影响、消极影响以及非线性关系影响。马里是非洲中西部的低收入国家,长期依赖外部援助进行经济发展。本文主要研究外债对马里经济增长的影响,并提出适当的政策措施,减少外债对该国经济造成的负面影响,并在一定范围内发挥外债的积极作用,从而达到举债的最佳平衡点。本文采用了较为开放的宏观经济模型,其中主要包括一个外债指标:外债存量(DEX),以及从1990到2020的其它宏观经济变量:马里国内生产总值(GDP)、马里人口(POP)、马里进口价值(IMP)、马里的出口价值(EXP)和马里的公共开支(DEP)。实证分析采用了普通最小二乘法(OLS)、增强的Dickey-Fuller(ADF)单位根检验、Johansen协整检验、格兰杰因果关系检验和误差修正法(ECM)等计量经济学方法。研究结果表明,短期外债对马里的经济增长产生负面影响。协整检验表明,马里政府外债与经济增长之间存在一种非线性的“倒u型”关系。在合理的外债融资范围内,马里政府举借外债将增加社会公共投资,扩大税收基础,刺激马里经济增长。然而,如果马里政府外债超过一定规模的合理范围,将导致公共与私营投资减少、债务本金与利息增加、政府财政紧缩,对马里经济增长的拉动作用逐渐减弱甚至产生负面影响。因此,为了实现最佳的外债平衡点,马里政府需要采取一系列政策措施,包括:加强债务管理和监管、提高外债使用效率、推动经济结构转型升级、加强国际合作。综上所述,马里政府应该采取综合措施,平衡外债规模和使用效率,同时加强债务管理和监管,推动经济结构转型升级,提高国际合作水平,以实现最佳的外债平衡点,促进马里经济的可持续增长。本文的研究表明,尽管马里政府通过举借外债可以在短期内刺激经济增长,但是如果外债规模超过了外汇储备,将对马里经济增长带来负面影响。因此,长期来看,马里政府需要控制债务规模,降低债务风险,并积极参与世界银行和国际货币基金组织的缓债倡议。此外,马里政府还应该加强政府债务的透明度,避免所借贷款被国际资本重组,增加债务压力。同时,马里政府还应该制定相关政策,促进经济作物的多元化种植,扩大农产品的出口,以改善国内的出口结构,降低外债扩张所带来的潜在风险。最终,这些措施将有助于马里政府更加合理有效地利用外债,实现外部融资与经济发展、国内储蓄与本土投资的平衡,提高国内民生水平,增加民众福祉。

Mechanical Complications after Limb Osteosynthesis: Analysis of Etiologic Factors in 42 Cases

The purpose of this study is to identify the causes of mechanical complications of osteosynthesis of members to prevent and improve the results of our surgical treatments. Patients and Method: Retrospective study including all patients diagnosed with mechanical complications after osteosynthesis of the limbs and who received medical follow-up. 42 patients with an average age of 42.2 years, with a male predominance (36 cases) were selected. These complications predominated in the femur with 26 cases. The implants involved were predominantly screwed plates with 25 cases. We investigated and analyzed the different types of dismantling of material as well as the etiological factors that may be at the origin of these disassembly. Results: After a mean follow-up of 15.47 months, four types of complications were identified: material expulsion (20 cases), migration (3 cases), incurvation (7 cases), fracture of material (12 cases). 4 types of factors causing dismantling have been identified: the factors related to the patient, factors related to fracture, the factors related to the implant and factors related to the surgeon. Conclusion: Disassembly of orthopedic implants is most often of multifactorial origin. These factors may act in isolation but are most often entangled in the patient. Proper preoperative planning can help prevent them. However, unexpected stresses of the implant and certain factors related to the ground remain difficult to control.

Reconstruction of Post-Traumatic Bone Loss (BL) Members: Our Experience about 27 Cases

The purpose of this study is to evaluate our results of treatment of post-traumatic bone defect of members through various bone reconstruction techniques. Patients and method: It is a retrospective study continues from January 2008 to December 2012. It involved 27 patients who had bone loss following trauma members. The average age of patients was 36.2 years. We noted a male predominance (22 men). The causes of injuries were dominated by accidents of public roads (12 cases) and ballistic trauma (14 cases). We noted one case of work accident. The BL was localized in 10 cases the tibia, femur in 4 cases, 8 cases in the humerus, 2 cases in the fibula, 1 case was located in the olecranon, 1 case at the distal radius and 1 case to the ball. All lesions were open fractures. After a mean of support for 3 days, we performed emergency around 19 associated with internal fixation (10 cases external fixation, internal fixation 9 cases), 8 single around. Bone reconstruction was performed secondarily in the majority of cases (24 cases). Only 4 reconstructions were performed in emergency. Reconstruction techniques were dominated by conventional grafts (24 cases) associated or not with the technique of induced membrane. Bone reconstruction techniques we used were as follows: 24 cases of traditional autografts and 3 cases of intertibio-fibular graft. After an average decline of 43.6 months, our patients were evaluated clinically (axis and limb length) and radiological (bone healing). Results: bone healing was achieved in 22 cases after a mean of 3.8 surgeries and an average of 14.8 months. Conclusion: the main principles governing the reconstruction of a defect are pre healing associated infection and repair soft tissue whenever it can do. The current trend is to hybridization of the reconstruction process of the great losses of substance by developing strategies based on both the deliberate shortening.

The quality of surface waters is often altered by the presence of bacteria of faecal origin, as a result of untimely discharge of solid and liquid waste from households. The Sonfonia Lake located in the commune of Ratoma is under the influence of various forms of pollution resulting for the most part from anthropic activities. Field visits, interviews with resource persons and bacteriological analysis were carried out to assess the level of this pollution. Two water sampling campaigns were carried out during the low-water period and two others during the flood period of 2021. The comparison of the levels of faecal contamination bacteria in the present study with a previous one carried out in 2018 on the waters of the same lake, indicates an increase in the level of faecal coliforms and faecal streptococci that exceed the indicator values recommended by the WHO. These results show that Lake Sonfonia is polluted. This could be related to the increase in anthropogenic and demographic activities during the last four years. On the other hand, the analysis of the results showed the absence of pathogenic germs such as salmonella. The Mann-Whitney u statistical test was used to compare the value of the means of each of the variables observed during the two seasons.

Analysis of Maternal Mortality in Obstetrics and Anesthesia Resuscitation in 15 Years at Chu Point“G”about 389 Cases Bamako/Mali

Objective: Analyze the maternal mortality in the two departments of CHu Point “G” in Bamako, because of high maternal mortality rate in our country. Material and Methods: This was an analytical cross-sectional study on maternal deaths from February 19, 2005 to November 19, 2019 for patients admitted in both departments and who died during the pregnancy-puerperal period at CHu Point “G”. All the patients who died outside this pregnancy-puerperal period were not retained. The data were entered and analyzed using SPSS 12.0 software. The statistical test used was that of Chi2, the statistical significance threshold was fixed at 5%. Results: During our study, we recorded 389 maternal deaths out of 16,033 admissions in 15 years and 18,060 live births during the same period making a maternal mortality ratio of 2153.931 and a frequency of 2.426. At the end of our study, we noted that the frequency of maternal deaths was higher in 2014: 12.9% (50/389). The maternal death predominantly affected women aged of 20 - 24 with a frequency of 22.4% (87/389). The multiparity (166/389 making 42.7%), illiteracy (341/389 making 87.7%), the poor evacuation conditions (non-medicalized transport): 263/389 making 67.6%;the evacuation without any evacuation sheet: 259/389 making 66.6%), poor CPN (Prenatal consultation) quality (undone CPN: 191/389 making 49.1%) and the poor monitoring of delivery works (no use of partograph in 343/389 making 88.2%) were the factors favoring maternal deaths. The main causes of maternal deaths were direct in 231/389 making 59.4% with hemorrhage in first line: 21.1% (82/389), infection (61/389 cases making 15.68%), dystocia: 50 cases making 12.85% and high blood pressure and complications (38/389 making 9.76%);indirect in 158/389 cases making 40.6% (Figures 1-3).
