Predictors of mortality and endoscopic intervention in patients with upper gastrointestinal bleeding in the intensive care unit

Background:The outcomes of patients undergoing esophagogastroduodenoscopy(EGD)in the intensive care unit(ICU)for upper gastrointestinal bleeding(UGIB)are not well *** aims were to determine predictors of 30-day mortality and endoscopic intervention,and assess the utility of existing clinical-prediction tools for UGIB in this ***:Patients hospitalized in an ICU between 2008 and 2015 who underwent EGD were identified using a validated,machine-learning *** regression was used to determine factors associated with 30-day mortality and endoscopic *** under receiver-operating characteristics(AUROC)analysis was used to evaluate established UGIB scoring systems in predicting mortality and endoscopic intervention in patients who presented to the hospital with ***:A total of 606 patients underwent EGD for UGIB while admitted to an *** median age of the cohort was 62 years and 55.9%were *** analysis revealed that predictors associated with 30-day mortality included American Society of Anesthesiologists(ASA)class(odds ratio[OR]4.1,95%confidence interval[CI]2.2-7.9),Charlson score(OR 1.2,95%CI 1.0-1.3),and duration from hospital admission to EGD(OR 1.04,95%CI 1.01-1.07).Rockall,Glasgow-Blatchford,and AIMS65 scores were poorly predictive of endoscopic intervention(AUROC:0.521,0.514,and 0.540,respectively)and in-hospital mortality(AUROC:0.510,0.568,and 0.506,respectively).Conclusions:Predictors associated with 30-day mortality include ASA classification,Charlson score,and duration in the hospital prior to *** risk tools are poorly predictive of clinical outcomes,which highlights the need for a more accurate risk-stratification tool to predict the benefit of intervention within the ICU population.

Structured light, where complex optical fields are tailored in all their degrees of freedom, has become highly topical of late, advanced by a sophisticated toolkit comprising both linear and nonlinear *** undesired structure from light is far less developed, leveraging mostly on inverting the distortion,e.g., with adaptive optics or the inverse transmission matrix of a complex channel, both requiring that the distortion be fully characterized through appropriate measurement. We show that distortions in spatially structured light can be corrected through difference-frequency generation in a nonlinear crystal without any need for the distortion to be known. We demonstrate the versatility of our approach using a wide range of aberrations and structured light modes, including higher-order orbital angular momentum(OAM) beams,showing excellent recovery of the original undistorted field. To highlight the efficacy of this process, we deploy the system in a prepare-and-measure communications link with OAM, showing minimal cross talk even when the transmission channel is highly aberrated, and outline how the approach could be extended to alternative experimental modalities and nonlinear processes. Our demonstration of light-correcting light without the need for measurement opens an approach to measurement-free error correction for classical and quantum structured light, with direct applications in imaging, sensing, and communication.

本文研究了多轴差分吸收光谱技术(MAX-DOAS)的云和气溶胶类型鉴别方法.首先研究了晴朗低气溶胶、晴朗高气溶胶、分散云、连续薄云和连续厚云下,色彩因子、光通量和O4的大气质量因子的变化特征.分析发现随着云和气溶胶光学厚度的增加,色彩因子会逐渐减小.分散云会使色彩因子发生快速变化,连续云会使各高度角的色彩因子趋于一致.另外,厚云会使天顶方向光强剧烈下降,同时O4大气质量因子大幅度增加.根据分析结果,最终形成了MAX-DOAS技术的云和气溶胶类型鉴别方案.利用该鉴别方案,统计分析了2012年6月1日到10月30日的MAX-DOAS观测结果.分散云和连续薄云出现频率最高,达到了66%和14.3%.两种类型下,NO2对流层垂直柱浓度(VCD)的平均值相对晴朗低气溶胶下高出35%和66%.而表征NO2VCD测量值稳定度的标准偏差大约增大了2倍.连续厚云下NO2VCD常出现突然的极大值和极小值.因此实时判别云和气溶胶的种类,将对MAX-DOAS数据的解析和数据质量的保证起到十分重要的作用.

AREVA operates a world-wide unique thermal hydraulic platform to ensure high safety standards in the nuclear industries. This platform is operated as an accredited test and inspection body according to ISO 17025 and 17020 to grant a high and independently confmned quality standard. The accreditation also ensures the independency of the organization and confidentiality to the individual stakeholders, for example research centers, utilities, components suppliers, engineering companies and vendors. Especially for nuclear power plants, it is very relevant to consider that reliability depends on the integrity of its components during its life time-from design through construction, operation and maintenance. For example, a typical NPP （nuclear power plant） has 1,000 to 2,000 large valves and 7,500 to 12,500 small valves, of which about 200 to 400 are designated Safety Class 1. The qualification of these Safety Class 1 components is relevant for reactor new builds but also for installed plants. This paper explains newly established qualification tasks, the corresponding testing infrastructure, and the state of the art of testing technology. By way of example, the paper describes the program and possible sequence of qualifying NPP safety-related components.

Injecting carbon dioxide(CO_(2))into coal seams may unlock substantial carbon sequestration *** the coal acts like a carbon filter,it can preferentially absorb significant amounts of CO_(2).To explore this further,desorption of the adsorbed gas due to pressure drop is investigated in this paper,to achieve an improved understanding of the long-term fate of injected CO_(2) during post-injection *** paper presents a dual porosity model coupling gas flow,adsorption and geomechanics for studying coupled processes and effectiveness of CO_(2) sequestration in coals.A new adsorption?desorption model derived based on thermodynamics is incorporated,particularly,the desorption hysteresis is *** reliability of the proposed adsorption-desorption isotherm is examined via validation *** is indicated that occurrence of desorption hysteresis is attributed to the adsorption-induced pore *** injection ceases,the injected gas continues to propagate further from the injection well,while the pressure in the vicinity of the injection well experiences a significant *** the adsorbed gas near the well also decreases,this decrease is less compared to that in pressure because of desorption *** unceasing spread of CO_(2) and drops of pressure and adsorbed gas depend on the degree of desorption hysteresis and heterogeneity of coals,which should be considered when designing CO_(2) sequestration into coal seams.
