Electrocardiographic Changes in Patients with Type 2 Diabetes Multicenttic Cross Sectional and Descriptive Study in Dakar

Introduction: The resting electrocardiogram is generally called upon in the evaluation of cardiovascular risk in diabetics. Thus we proposed in this work to evaluate electrocardiographic changes in patients with type 2 diabetes. Methods: This was a cross-sectional and descriptive observation that took place from 1 January 2014 to 1 October 2014 in the Internal Medicine/Endocrinology Departments of Pikine National Hospital and Cardiology National Hospital of Grand-Yoff. Results: One hundred (100) patients consisted of 43 male and 57 female diabetics. The sex ration male/female ratio was 0.7. The average age was 58.3 years. Men were twice as likely to be active as women. Rhythm disorders were noted in 20% of patients, 13% of whom were women. Right atrial hypertrophy was found in 11% of patients and left hypertrophy in both men and women (25%). Wolf-Parkinson White syndrome was present in 2% of patients. A complete right bundle block was present in 11% of patients. Primary repolarization disorders were noted in 17% of patients and secondary in 18% of patients, postero-diaphragmatic necrosis in 14% of patients, real posterior necrosis in 2% of patients, extensive anterior necrosis in 15% of patients. The QT interval was lengthened in 21% of patients including 14 women. Left ventricular hypertrophy was present in 33% of patients and right ventricular hypertrophy was found in 10% of patients. The bivariate analysis showed that electrocardiographic abnormalities were more correlated with the association of certain risk factors such as hypertension and dyslipidemia, the feminine gender, but also the poor glycemic balance. On the other hand, physical activity and treatment had a protective effect. Conclusion: The ECG is certainly insufficient for an exhaustive exploration of the heart of the diabetic patient, but still very useful in our conditions of exercise to improve the care of our patients.

文章对“中国老年健康影响因素跟踪调查”近两万名80～105岁高龄老人的数据分析发现，社会经济发展使得2008年高龄老人存活概率和自评日常生活自理能力比1998年有显著改善。医疗条件进步使健康较差高龄老人被“救”存活率提高，使2008年高龄老人客观量测的躯体功能和认知功能比10年前显著下降；这给社会和亿万家庭带来严峻挑战。研究表明，经济社会发展和人类寿命提高带来效益的同时，也会产生一定成本；但它不是以往不少国际文献解释这一现象时所称的“胜利的失败”，所以没有必要为此恐慌，而需要全社会积极应对，努力实现人口健康老龄化。

＜正＞The discovery of fossilised embryos in rocks laid down contemporaneously with the Cambrian explosion has begun to provide unforeseen insights into the role of embryology in the emergence of metazoan phyla, challenging cherished and long-held assumptions, such as the role of life history evolution in early metazoan evolution. Realizing the full potential of these fossils, has however, been limited by traditional analytical techniques. Scanning electron microscopy provides only data on surface features, and while this may be overcome by thin sectioning, it is destructive and provides only a single two-dimensional window onto internal structure that may be difficult to interpret.

Objective:To describe the epidemiological distribution of injury cases in the Philippines from 2011 to ***:A retrospective review of records from the Online National Electronic Injury Surveillance System involving trauma cases was done,and data were collated,tabulated,and statistically ***:A total of 668?179 injury cases were recorded,and 68.99%of these cases involved *** individuals belonged to the group aged 15-29 years(34.01%).The most populated regions in the country,the National Capital Region(14.90%)and RegionⅣA(13.96%),incurred the highest number of recorded *** crash(32.67%)was reported to be the major cause of injury;and open wounds(37.56%)and abrasion(26.23%)were found to be the most common types of ***:The burden of injury remains high in the Philippines due to the lack of resources and inadequate *** findings of the present study on the distribution of injury cases provide useful information for taking appropriate preventive measures,developing effective safety guidelines,allocating limited resources in an appropriate manner,and implementing necessary public health programs.

Background: One target under sustainable Development Goal 3 is to reduce the global maternal mortality ratio to less than 70 per 100,000 births, with no country having a maternal mortality rate of more than twice the global average. So there is a need to identify ways to reduce maternal mortality which is a top priority in development of the management of pregnant women and invention of equipment in procedures (caesarian section) to provide a maternal health and increase life survival. Objective: This study aimed to investigate the prevalence and causes of maternal mortality at Regina Pacis Hospital and Government Health Center Mutengene. Method: This study was a retrospective study, where Data from 2007 to 2017 were collected from the hospital files using a checklist, descriptive and inferential statistics were used to analyse the data. Conclusion: From the finding, maternal mortality is at a pace of increase in the Regina Pacis Hospital and Government Health Centre Mutengene and these health facilitiesmay fail to achieve the target of SDG3 by the end of 2030. Since, the overall prevalence of maternal mortality at both study sites was 240 per 100,000 live births at Regina Pacis Hospital and Government Health Center Mutengene from 2007 to 2017, increase education of women of reproductive age on pregnancy care and training of hospital staff in obstetric management and health records documentation.

Assessment of HRP2-Based RDT in Diagnosing Malaria Cases

Background： HRP2-based Rapid Diagnostic Tests （RDTs） for malaria ensure a rational use of artemisinin-based combination therapy （ACT）. The HRP2 antigen can go through the cerebrospinal fluid （CSF）. Purpose： To assess the HRP2-based RDT in malaria and detect the HRP2 in CSF. Methods： From November 2006 to May 2007, all patients under 15 years showing clinical symptoms of malaria were included. RDT was performed on the CSF and peripheral blood. Results： Out of the 951 patients included, 131 （13.7%） were confirmed through RDT and 130 （13.6%） through thick blood smear. Sensitivity and specificity stood at 96.96% and 99.71% respectively, for uncomplicated cases and at 100% and 99.13% for severe cases. Tests to detect HRP2 in the CSF of 52 cases were negative. Conclusion： Tests to detect HRP2 in the CSF could make it possible to diagnose severe malaria cases when peripheral parasitemia would be below the detection threshold.

Management of Post-Operative Pain after Gyneco-Obstetrical Surgery: Practice of Transversus Abdominis Plane Bloc (Tap Block) Echoguide at the Ignace Deen National Hospital

Objectives: To describe the practice of ultrasound-guided TAP in the management of postoperative pain after gyneco-obstetric surgery. Methods: This was a descriptive prospective study carried out at the Gynecology-Obstetrics department of the Ignace Deen National Hospital over a period of three (03) months from February 01, 2020 to April 31, 2020. Results: In total, we collected 95 patients. These patients had a mean age of 30 ± 9.5 years. The ASA I class was the most represented with 76% of the cases and the cesarean was the most performed intervention. Regarding the assessment of the pain score by the simple verbal scale (SVE) postoperatively at rest, the mean SLE scores at H6 were 0.17 ± 0.38;at H12 of 1.15 ± 0.62;at H24 of 0.84 ± 0.51;at H36 0.45 ± 0.52 and at H48 0.09 ± 0.29. On mobilization, the mean pain scores were 0.77 ± 0.51 at H6, at H12 1.89 ± 0.61;at H24 of 1.53 ± 0.56;at H36 of 1 ± 0.29 and at H48 of 0.82 ± 0.44. The majority of our patients (66.3%) had a mobilization time of less than 24 hours. The mean length of stay was 3.1 ± 1.3 days and most patients (82%) were satisfied with the management of their pain by ultrasound-guided TAP block. Conclusion: Ultrasound-guided TAP is an effective technique for the management of postoperative pain in gyneco-obstetrics surgery. Its integration in a context of multimodal analgesia could improve the management of postoperative pain in gynecological obstetrics.

Prevalence of hepatitis B infection and factors associated in children of Ivorian HBsAg carrier subjects

Aims of the Study: 1) Determine the Prevalence of Hepatitis B virus (HBV) infection in children (contact subjects) of chronic Hepatitis B surface antigen (HBsAg) carrier subjects (index subjects);2) Search for factors associated with HBV infection in these children. Patients and Methods: Retrospective-crosssectional study (January 5th, 2006 through December 31st, 2012). Studied parameters: biological and clinical characteristics of index subjects;Prevalence of HBsAg and Hepatitis B core antibody (HBcAb) in their children. Search for the HBV infection associated factors in the children (univariate analyses through Chi-square or Fisher’s exact test;multivariate analysis through a backward logistic regression). Results: Our 44 subjects’ median age was 43.1 ± 7.49 years and 88.6% of them lived with a spouse. Average number of children per index subjects was 2.3 ± 1.1. Our 92 children’s median age was 9.3 ± 4.55 (ranging from 1 to 15 years), and 43 (44.8%) were vaccinated against HBV. HBV infection prevalence was 24% (23/96 of which, 4 were HBsAg positive and 19 HBcAb positive subjects without HBsAg). Independent factors associated with HBV infection in children of index subjects were HBV DNA for index subjects >2000 IU/ml (OR = 11.5;p = 0.001), existence of HBV in two parents (OR = 7.9;p = 0.03) and absence of HBV vaccination in the children (OR = 30.9;p = 0.003). Conclusion: Immunization coverage for children of index subjects was insufficient, especially before the introduction of HBV vaccine into the enlarged vaccination program. Outside vertical transmission, those children were more exposed to HBV intrafamilial transmission risk when they were not immunized against HBV, when both parents were infected and when HBV viremia in index subjects was higher than 2000 IU/ml.
