Randomized study comparing incidence of radial artery occlusion post-percutaneous coronary intervention between two conventional compression devices using a novel airinflation technique

AIM To compare post-percutaneous coronary intervention(PCI) radial artery occlusion(RAO) incidence between two conventional radial artery compression devices using a novel air-inflation *** One hundred consecutive patients post-PCI were randomized 1:1 to Safeguard or TR band compression devices. Post-radial sheath removal, each compression device was inflated with additional 2 m L of air above index bleeding point during air-filled device application and gradually down-titrated accordingly. RAO was defined as absence of Doppler flow signal performed at 24 h and at 6 wk post-PCI. Patients with missing data were excluded. Statistical significance was defined as P < *** All patients had 6 F radial sheath inserted. No significant differences were observed between Safeguard Radial(n = 42) vs TR band(n = 42) in terms of age(63 ± 11 years vs 67 ± 11 years), clinical presentation(electives, n = 18 vs n = 16; acute coronary syndrome, n = 24 vs n = 26) and total procedural heparin(7778 ± 2704 IU vs 7825 ± 2450 IU). RAO incidence was not significantly different between groups at 24 h(2% vs 0%, P = 0.32) and 6 wk(0%, both).CONCLUSION Safeguard Radial and TR band did not demonstrate significant between-group differences in short-term RAO incidence. Lack of evidence of RAO in all postPCI patients at 6 wk follow-up, regardless of radial compression device indicate advantage of using the novel and pragmatic air-inflation technique. Further work is required to more accurately confirm these findings.

Development of a Framework for Equipment Health Management in the Mining Industries in Zambia

The Zambian mining industry is crucial to the national economy but struggles with inconsistent equipment maintenance practices. This study developed an Equipment Maintenance Management Framework (EMMF) tailored to the industry’s needs. Using surveys, interviews, and on-site visits at eight major mining companies, we identified significant variations in maintenance strategies, CMMS usage, and reliability engineering. The EMMF prioritizes predictive maintenance, efficient CMMS implementation, ongoing training, and robust reliability engineering to shift from reactive to proactive maintenance. We recommend adopting continuous improvement practices and data-driven decision-making based on performance metrics, with a phased EMMF implementation aligning maintenance with strategic business objectives. This framework is poised to enhance operational efficiency, equipment reliability, and safety, fostering sustainable growth in the Zambian mining sector.

Propofol with Varied Functions: A Potential Therapeutic Opportunity for Postoperative Nausea, Vomiting and Pruritus—A Narrative Review

Background: Despite the advances in anesthetics and non-pharmacological techniques, the prevalence of postoperative nausea and vomiting in all patients remains high. It is one of the most common distressing symptoms that cause dissatisfaction among patients after anesthesia and surgery. A sub-hypnotic dose of propofol has been shown to reduce morphine-induced postoperative nausea, vomiting, and pruritus. This review article will provide sufficient knowledge on the role of propofol in minimizing opioid-induced postoperative nausea, vomiting, and pruritus by providing detailed information on propofol antiemetic and antipruritic effects, as well as discussions based on empirically available data. Method: We conducted a narrative review of the literature published between 1990 and 2023 from a range of databases;PubMed, BioMed Central, Biosis Previews, Nature, International Pharmaceutical Abstracts, Springer-Link, and Elsevier. Discussion and Conclusion: The literatures reviewed in this study have demonstrated that propofol may have diverse therapeutic effects including antiemetic and antipruritic. The antiemetic effect of propofol may be an effective therapeutic approach for the prevention of postoperative nausea and vomiting. The literature also demonstrated that the use of propofol for sedation during surgery may as well ameliorates opioids induced postoperative pruritus, which may be beneficial to surgical patients. Also, it was demonstrated that prophylactic use of propofol may be an effective way of preventing nausea and vomiting and pruritus during opioid use.

Defect of CD8+ Memory T Cells Developed in Absence of IL-12 Priming for Secondary Expansion

IL-12 priming plays an important role in stimulation of CD8+ effector T cells and development of CD8+ memory T (Tm) cells. However,the functional alteration of CD8+ Tm cells developed in the absence of IL-12 priming is elusive. In this study,we investigated the capacity of secondary expansion of CD8+ Tm cells developed from transgenic OT I CD8+ T cells. The latter cells were in vitro and in vivo stimulated by ovalbumin (OVA)-pulsed dendritic cells DCOVA and (IL-12-/-)DCOVA derived from wild-type C57BL/6 and IL-12 gene knockout mice,respectively. We demonstrated that IL-12 priming is important not only in CD8+ T cell clonal expansion,but also in generation of CD8+ Tm cells with the capacity of secondary expansion upon antigen re-encounter. However,IL-12 signaling is not involved in CD8+ Tm cell survival and recall responses. Therefore,this study provides useful information for vaccine design and development.

Effect of crystallographic texture and twinning on the corrosion behavior of Mg alloys:A review

Magnesium and its alloys have gained significant popularity due to their light weight and their potential for use as bioresorbable ***,their application is limited in practice due to their relatively poor corrosion *** methods are available for improving the corrosion resistance of Mg alloys for bio-applications such as using different coatings,alloying,and modifying the microstructural parameters such as the grain size and the crystallographic *** review provides a comprehensive summary of the effects of crystallographic texture and twinning,as one of the most important deformation mechanisms of Mg and Mg alloys,on the corrosion *** the crystallographic texture,it is shown that theoretically the basal planes should exhibit a lower corrosion rate but in some cases,such as when there is a galvanic effect or when corrosion films control the overall corrosion behavior,different results may take ***,there are contradictory results concerning the effect of twinning on the corrosion ***,in some cases twinning may provide preferential sites for corrosion due to the higher energies of atoms located in the twin region by comparison with normal atomic positions in the crystalline lattice whereas there are also other examples where experiments show that twins produce more protective films than in the surrounding matrix.

Magnesium alloys are of considerable current interest for use as degradable implants due to their unique properties including biodegrad-ability,biocompatibility,low density and adequate mechanical ***,there is a need to further improve these properties either by alloying or through the use of appropriate *** the different biodegradable Mg alloys now in use,the Mg-Zn series are of special interest and have been the subject of many research *** is primarily because Zn is an essential element for the human body in addition to its positive effects in improving the mechanical strength and lowering the degradation rate of the *** properties of Mg-Zn alloys may be further improved both through the addition of third and fourth alloying elements such as Ca,Ag,Sn or Sr and/or by thermo-mechanical processing where the latter is more environmentally and economically *** practice,procedures based on the application of severe plastic deformation(SPD)are especially suited to produce fine-grained microstructures with improved mechanical,degradation and cell ***-channel angular pressing(ECAP)is a popular SPD technique that has the capability of pro-ducing bulk materials that are sufficiently large for use as typical ***,this review is designed to provide a comprehensive summary of the research that has been undertaken on ECAP-processed biodegradable Mg-Zn alloys.

Endoscopic stenting-Where are we now and where can we go?

Self expanding metal stents (SEMS) play an important role in the management of malignant obstructing lesions in the gastrointestinal tract. Traditionally,they have been used for palliation in malignant gastric outlet and colonic obstruction and esophageal malignancy. The development of the polyflex stent,which is a removable self expanding plastic stent,allows temporary stent insertion for benign esophageal disease and possibly for patients undergoing neoadjuvant chemotherapy prior to esophagectomy. Potential complications of SEMS insertion include perforation,tumour overgrowth or ingrowth,and stent migration. Newer stents are being developed with the aim of increasing technical and clinical success rates,while reducing complication rates. Other areas of development include biodegradable stents for benign disease and radioactive or drug-eluting stents for malignant disease. It is hoped that,in the future,newer stents will improve our management of these diffi cult conditions and,possibly,provide prognostic as well as symptomatic benefi t in the setting of malignant obstruction.

Use of Voltage Limits for Current Limitations in Grid-forming Converters

Renewable generation interfaced through gridforming converters are proposed as a replacement for synchronous generators in power ***,compared to the synchronous generator,the power electronics converter has a strict limit on the current to avoid overcurrent *** gridforming converter acts like a voltage source,directily controlling the *** conflicts with the operation of the conventional current limit control,which is applied to a current *** switch between the voltage control and current control aimed to impose the current limit leads to synchronization *** paper proposes a novel control scheme which can be applied to the grid forming voltage control in order to enforce current *** proposed method has been verified through simulation and hardware tests in both symmetrical and asymmetrical faults to perform current suppression while maintaining synchronization stability in the voltage control mode.
