Primary liver cancer is the fifth most common malignancy in men and the eighth in women worldwide. The liver is also the second most common site for metastatic spread of cancer. To assist in the diagnosis of these liver lesions non-invasive advanced imaging techniques are desirable. Magnetic resonance (MR) is commonly used to identify anatomical lesions, but it is a very versatile technique and also can provide specific information on tumor pathophysiology and metabolism, in particular with the application of MR spectroscopy (MRS). This may include data on the type, grade and stage of tumors, and thus assist in further management of the disease. The purpose of this review is to summarize and discuss the available literature on proton, phosphorus and carbon-13-MRS as performed on primary liver tumors and metastases, with human applications as the main perspective. Upcoming MRSapproaches with potential applications to liver tumors are also included. Since knowledge of some technical background is indispensable to understand the results, a basic introduction of MRS and some technical issues of MRS as applied to tumors and metastases in the liver are described as well. in vivo MR spectroscopy of tumors in a metabolically active organ such as the liver has been demonstrated to provide important information on tumor metabolism, but it also is challenging as compared to applications on some other tissues, in particular in humans, mostly because of its abdominal location where movement may be a disturbing factor.

The aim of this paper was to study reliable automated delineationpossibilities of karst sinkholes using a liDAR-based digital terrain model(DTM) with pixel-based classifications. We applied two approaches toextract sinkholes: (1) general linear modeling (GlM) with morphometricindices derived from DTM;(2) and a local minima-based delineationusing only liDAR DTM as the input layer. The outcome of the localminima was significantly different from the reference ones but found allthe sinkholes without previous knowledge of the area. The GlM-basedoutcome did not differ statistically from the reference. Results showedthat these approaches were efficient in detecting sinkholes based onliDAR derivatives, and can be used for risk assessment and hazardpreparedness in karst areas: GlM had an overall accuracy of 89.5% andlocal minima had an accuracy of 92.3%;both methods identifiedsinkholes but also had commission errors, identifying depressions assinkholes.

Many environmental chemicals and pesticides have been found to alter neuroendocrine communication in exposed biological objects. The environmental loads have primary and secondary effects that can alter the homeostatic regulation potential. Since it is difficult to avoid human exposition, a potentially important area of research to develop in vivo and in vitro experimental models. in this context, the primary aim of this study was to demonstrate the effects of chlorobenzenes on adrenocorticotrophic hormone（ACTH） release. in our experimental study, male Wistar rats were exposed to 0.1, 1.0 and 10 μg/b.w.（body weight） kg of 1,2,4-trichlorobenzene and hexachlorobenzene（Cl B） mix via gastric tube for 30, 60 or 90 days. At the endpoints of the experiment blood samples were taken and animals were decapitated. Primary,monolayer adenohypophysis cell cultures were prepared by enzymatic and mechanical digestion. The ACTH hormone content in serum and supernatant media was measured by immuno-chemiluminescence assay. The Mg2＋-dependent ATPase activity was deter.ined by modified method of Martin and Dotty. Significant differences were detected in the hormone release between the control and treated groups. The hormone release was enhanced characteristically in exposed groups depending upon the dose and duration of exposure. The Mg2＋-ATPase activity enhanced after chronic and subtoxic Cl B exposition. light microscopy revealed that the adenohypophysis seemed to be more abundant. Results indicate that Wistar rats exposed to subtoxic Cl B have direct and indirect effects on hypothalamus–hypophysis–adrenal axis.

in patients with inflammatory bowel diseases (iBD) the prevalence of thrombosis is 6.2%, the average incidence of thromboembolism (TE) is 3.6 times higher compared to normal population. The TE is a common extraintestinal complication of iBD, squarely associated with the iBD activity. The application of antico-agulant and thrombolytic therapy in severe iBD is an unresolved issue. Herein we report the first case in literature of an active iBD patient with an upper limb acute arterial occlusion and successful catheter-directed thrombolysis (CDT). A 46-year-old male patient is reported who had Crohn's disease for 10 years. His right hand suddenly became cold and painful. Angiography proved acute occlusion of the brachial and radial artery. Vascular surgery intervention was not applicable. Endoscopy showed extended, severe inflammation of the colon. Despite the severe endoscopic findings, frequent bloody stools and moderate anaemia, CDT with recombinant tissue plasminogen activator was performed. The control angiography proved improvement, the radial artery pulse appeared. No bleeding complication was observed. This case supports that CDT-after careful estimation of the bleeding risk-can be effective and safe in patients with severe or life-threatening TE and active iBD.

AiM： To characterize the alpha-fetoprotein （AFP） positive and negative hepatocellular carcinoma （HCC） samples. METHODS： Thirty-seven paraffin-embedded human HCC samples were analyzed by immunohistochemistry for the following antigens： AFP,β-catenin, p53, CD44, MSH-2, MlH-1, and HNF-4. The tumors were divided into two groups based on the AFP expression. The immunophenotypic data and important clinical parameters were studied between the two groups. RESUlTS： Twenty-one of the thirty-seven examined HCCs were AFP positive. Seven with nudear p53 staining were AFP positive, while seven tumors with nuclear β-catenin staining were AFP negative. CD44 staining and high histological tumor grade were more frequent among the AFP-positive HCCs. The other immunophenotypical and dinical parameters did not show statistically significant difference in their distribution between the AFP positive and negative samples. CONClUSiON： AFP expression in HCC correlates with unfavorable prognostic factors, while nuclear β-catenin positivity is more common among the AFP-negative liver tumors. This observation supports the microarray data on in vivo human tumors.

Pharmaceutical co-crystals have been explored by many researchers as a strategy to optimize physicochemical properties of solid-state *** improvements of solubility,bioavailability,and the reduced tendency for phase transformation occurrence,are factors that highlight benefits of pharmaceutical co-crystals among other solid *** to the Biopharmaceutical Classification System(BCS),non-ster.idal anti-inflammatory drugs(NSAiDs)are class Ⅱ drugs,which have low aqueous solubility and therefore co-crystallization has the potential to optimize NSAiD product *** this review,we highlight the recent progress made on NSAiDs co-crystals,their co-formers,synthesis,methods and use,while we underline some promising results on in vitro and in vivo co-crystal properties.A celecoxib-tramadol co-crystal reaches phase Ⅲ clinical trials,showing greater analgesic activity than both individual *** aqueous solubility of the co-crystal formed between l-proline and diclofenac is very high in comparison with the pure *** co-crystals with urea and thiourea have an increase of drug release of almost 60%.Co-crystal design brings a new perspective in drug development since the co-former used can also be a biologically active component allowing to combine different anti-inflammatory drugs,which have an incredible spectrum of application due to the analgesic,anti-pyretic and anti-inflammatory properties.

Chronic kidney disease severely deteriorates the outcome of gastrointestinal bleeding: A meta-analysis

AiM To understand the influence of chronic kidney disease(CKD) on mortality, need for transfusion and rebleeding in gastrointestinal(Gi) bleeding *** A systematic search was conducted in three databases for studies on Gi bleeding patients with CKD or endstage renal disease(ESRD) with data on outcomes of mortality, transfusion requirement, rebleeding rate and length of hospitalization(lOH). Calculations were performed with Comprehensive Meta-Analysis software using the random effects model. Heterogeneity was tested by using Cochrane's Q and i2 statistics. Mean difference(MD) and OR(odds ratio) were ***1063 articles(EMBASE: 589; PubM ed: 459; Cochrane: 15) were found in total. 5 retrospective articles and 1 prospective study were available for analysis. These 6 articles contained data on 406035 patients, of whom 51315 had impaired renal function. The analysis showed a higher mortality in the CKD group(OR = 1.786, 95%Ci: 1.689-1.888, P i: 1.386-4.616, P = 0.002), and a rebleeding rate(OR = 2.510, 95%Ci: 1.521-4.144, P in patients with impaired renal function. CKD patients required more unit red blood cell transfusion(MD = 1.863, 95%Ci: 0.812-2.915, P ime in hospital(MD = 13.245, 95%Ci: 6.886-19.623, P increases mortality, need for transfusion, rebleeding rate and lOH among Gi bleeding patients. Prospective patient registries and observational clinical trials are crucially needed.
