Clinical outcomes in patients with native valve infective endocarditis and diabetes mellitus

BACKGROUND There is limited data on the difference in the clinical characteristics and outcomes of patients with severe coronavirus disease 2019(COVID-19)infection in the summer compared to the fall *** To compare the sociodemographic,clinical characteristics,and outcomes among mechanically ventilated patients with severe COVID-19 infection admitted to the intensive care unit(ICU)during the summer and fall surges in the year *** We included patients admitted to the ICU and treated with invasive mechanical ventilation for COVID-19 associated respiratory failure between April 1 and December 31,*** were categorized into summer surge for ICU admissions between June 15,2020,and August 15,2020,and fall surge between October 15,2020,and December 31,*** compared patients'characteristics and outcomes using descriptive and inferential *** A total of 220 patients were admitted to the Grady Memorial Hospital ICU and mechanically ventilated for COVID-19 associated hypoxemic respiratory failure during the period considered(125 during the summer surge and 95 during the fall surge).More women were admitted in the fall compared to summer(41.1%vs 36.8%,difference,4.3%;95%CI:1.2,7.5).Patients admitted in the fall had fewer comorbidities(chronic obstructive pulmonary disease,stroke,diabetes mellitus,obstructive sleep apnea and body mass index≥35 kg/m2).Overall,patients in the fall had a lower ICU mortality rate(27.4%vs 38.4%,difference,-11.0;95%CI:-6.4,-18.2),shorter length of stay on the mechanical ventilator(7 d vs 11 d,difference,4 d;95% CI:2.1,6.6)and shorter ICU length of stay(9 d vs 14 d,difference,5 d;95% CI:2.7,9.4).CONCLUSION Patients admitted with severe COVID-19 infection requiring mechanical ventilation had better outcomes in the fall than *** difference observed is likely attributable to a better understanding of the condition and advances in treatment strategies.

Zn Dissolution-Passivation Behavior with ZnO Formation via In Situ Characterizations

Thermal stability of the Cr-coated zirconium alloy microstructure prepared by pulsed laser deposition

Cr-coated zirconium alloy was prepared by pulsed laser deposition(PLD)for the application of accident-tolerant fuel cladding in light water *** microstructural characteristics of the Cr coating and its evolution with temperature were investigated using grazing incidence X-ray diff raction and in situ heating transmission electron microscopy(TEM).Results show that the microstructure of the laser-deposited Cr coatings consists mainly of fine and non-specific shaped nano-crystals in the inner layer and columnar crystals in the outer *** recrystallization of the Cr-coating layer starts at 300–400℃ to release the high strain introduced by PLD,and the grain coalescence starts at temperatures>400°*** annealing,the(110)-texture gradually intensifi es because of its high reticular density and low close-packed ***,in situ heating TEM observation shows the presence of cavities on the Cr–Zr interface,which may result from the interdiff usion and/or the transformation from amorphous to crystalline.

Stevens–Johnson syndrome and toxic epidermal necrolysis: Updates in pathophysiology and management

Stevens–Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are life-threatening conditions characterized by extensive detachment of the epidermis and mucous membranes. These severe disorders carry a high mortality rate, and their pathogenesis remains largely unclear. Furthermore, optimal therapeutic strategies for SJS/TEN remain a subject of ongoing debate. Early diagnosis of SJS/TEN is challenging, and reliable biomarkers for diagnosis or severity prediction have not been firmly established. Certain drugs, such as carbamazepine and allopurinol, have shown a strong association with specific human leukocyte antigen (HLA) types. Recently, the potential benefits of HLA screening prior to administering these drugs to reduce the incidence of SJS/TEN have been explored. Epidermal cell death in SJS/TEN lesions is caused by extensive apoptosis, primarily through the Fas–Fas ligand (FasL) and perforin/granzyme pathways. Our findings suggest that necroptosis, a form of programmed necrosis, also contributes to epidermal cell death. Annexin A1, released from monocytes, interacts with the formyl peptide receptor 1 to induce necroptosis. Several biomarkers, such as CC chemokine ligand (CCL)-27, interleukin-15, galectin-7, receptor-interacting protein kinases 3 (RIP3), and lipocalin-2, have been identified for diagnostic and prognostic purposes in SJS/TEN. Supportive care is recommended for treating SJS/TEN, but the efficacy of various therapeutic options–including systemic corticosteroids, intravenous immunoglobulin, cyclosporine, and tumor necrosis factor-α antagonists–remains controversial. Recent studies have investigated the potential benefits of tumor necrosis factor-α antagonists. In this review, we discuss recent advances in the understanding and management of SJS/TEN.

Mitigation of Transients in Capacitor Coupled Substations Using Traditional RLC Filter Techniques

This article presents an extensive examination and modeling of Capacitor Coupled Substations (CCS), noting some of their inherent constraints. The underlying implementation of a CCS is to supply electricity directly from high-voltage (HV) transmission lines to low-voltage (LV) consumers through coupling capacitors and is said to be cost-effective as compared to conventional distribution networks. However, the functionality of such substations is susceptible to various transient phenomena, including ferroresonance and overvoltage occurrences. To address these challenges, the study uses simulations to evaluate the effectiveness of conventional resistor-inductor-capacitor (RLC) filter in mitigating hazardous overvoltage resulting from transients. The proposed methodology entails using standard RLC filter to suppress transients and its associated overvoltage risks. Through a series of MATLAB/Simulink simulations, the research emphasizes the practical effectiveness of this technique. The study examines the impact of transients under varied operational scenarios, including no-load switching conditions, temporary short-circuits, and load on/off events. The primary aim of the article is to assess the viability of using an established technology to manage system instabilities upon the energization of a CCS under no-load circumstances or in case of a short-circuit fault occurring on the primary side of the CCS distribution transformer. The findings underscore the effectiveness of conventional RLC filters in suppressing transients induced by the CCS no-load switching.

Enhancement of Visual Attention by Color Revealed Using Electroencephalography

Attention constitutes a fundamental psychological feature guiding our mental effort toward specific objects, concurrent with processes such as memory, reasoning, and imagination. Visual attention, crucial for selecting surrounding information, often decreases in older adults and patients with cerebrovascular disorders. Effective methods to enhance attention are scarce. Here, we investigated whether color information influences visual attention and brain activity during task performance, utilizing EEG. We examined 13 healthy young adults (seven women and six men;mean age: 21.2 ± 0.58 years) using 19-electrode electroencephalograms to assess the impact of color information on visual attention. The Clinical Assessment for Attention cancellation test was conducted under the black, red, and blue color conditions. Wilcoxon’s signed-rank test was used to assess differences in task performance (task time and error) between conditions. Spearman’s rank correlation was utilized to examine the correlation in power levels between task performance and color conditions. Significant variations in total task errors were observed among color conditions. The black condition exhibited the highest error frequency (0.7 ± 0.9 times), followed by the red condition (0.5 ± 0.8 times), with the lowest error frequency occurring in the blue (0.2 ± 0.4 times) condition (black vs. red: P = 0.03;black vs. blue: P = 0.00;red vs. blue: P = 0.032). No time difference was observed. The black condition showed negative delta and high-gamma correlations in the central electrodes. The red condition revealed positive alpha and low-gamma correlations in the frontal and occipital areas. Although no correlations were observed in the blue condition, it enhanced attentional performance. Positive alpha and low-gamma waves might be crucial for spotting attentional errors in key areas. Electroencephalogram Signal Correlations between Default Mode Network and Attentional Functioning

Attentional issues may affect acquiring new information, task performance, and learning. Cortical network activities change during different functional brain states, including the default mode network (DMN) and attention network. We investigated the neural mechanisms underlying attentional functions and correlations between DMN connectivity and attentional function using the Trail-Making Test (TMT)-A and -B. Electroencephalography recordings were performed by placing 19 scalp electrodes per the 10 - 20 system. The mean power level was calculated for each rest and task condition. Non-parametric Spearman’s rank correlation was used to examine the correlation in power levels between the rest and TMT conditions. The most significant correlations during TMT-A were observed in the high gamma wave, followed by theta and beta waves, indicating that most correlations were in the parietal lobe, followed by the frontal, central, and temporal lobes. The most significant correlations during TMT-B were observed in the beta wave, followed by the high and low gamma waves, indicating that most correlations were in the temporal lobe, followed by the parietal, frontal, and central lobes. Frontoparietal beta and gamma waves in the DMN may represent attentional functions.
