Evaluation of Pedestrian–Train Fatalities in the State of Maryland: A 5‑Year Retrospective Study

Few studies have been done on the incidences of train-related pedestrian fatalities throughout the United States,with no previous studies reported in the State of Maryland.A retrospective study was conducted at the statewide medical examiner’s office in Maryland to evaluate the characteristics of train-related pedestrian fatalities from 2014 to *** aim of the study was to analyze circumstances of deaths through the medicolegal death investigation and postmortem examination findings,including toxicological study,to help identify epidemiological characteristics of pedestrian–train‑related fatalities.A total of 48 pedestrian-train deaths were *** the 48 cases identified,21 deaths(43.75%)were determined to be accident,20 deaths(41.67%)were suicide,and 7 deaths(14.58%)whose manner of death could not be *** the 21 accidental victims,17 were male and 4 were female(M:F ratio=4.3:1),11(52.38%)were white,6(28.57%)African American,and 4 Hispanic(19.05%),with age ranging from 16 to 58 years(mean age=35).Of the 20 suicide victims,17 were males and 3 females(M:F ratio=5.7:1),16(80%)were white,2(10%)African American,and 2(10%)Hispanic,with age ranging from 22 to 60 years(mean age=40).The majority of accidents occurred during weekday evening rush hours between 4:00 pm and 7:00 pm,while the suicides showed no specific time *** specific peak for seasons of year was found in suicides or *** toxicological studies showed that 52.38%(11/21 cases)of accidental victims were positive for alcohol and 30%(6/20 cases)of suicide victims were positive for *** of death could not be determined in 7 cases because of unclear circumstances of *** death scene investigation and complete postmortem examination,including comprehensive toxicological testing,is very important in all train-related pedestrian *** the manner of death can directly affect the outcome of civil ligation and dispersal of insurance ***

The synthesis of T_8, T_ 10 and T_ 12 silsesquioxane cages from a range of starting materials: phenyl-cis-tetrol, 1,3-divinyltetraethoxydisiloxane and cyclopentyl T resins by using tetra n-butylammonium fluoride(TBAF) as the catalyst is described in this paper. The reaction yields obtained viathe current route are better compared to those viathe literature routes. Some of the cage compounds have been characterized by X-ray crystallography.

The Effect on Physician Interpretation after Ultrasound Video Transmission

Title: The Effect on Physician Interpretation After Ultrasound Video Transmis-sion. Objective: There is a limited amount of qualified sonographers, which is problematic to rural, and prehospital settings that require clinical ultrasound. One solution is the use of telemedicine to transmit ultrasound videos to a distant expert. Our objective was to determine the effect of video image transfer using Skype on physician image interpretation. Methods: Three emergency medicine physicians of varied ultrasound proficiency were given 72 pairs of transmitted and non-transmitted ultrasound video clips in random order. Raters were instructed to determine whether ultrasound clips were positive or negative for pathology. Same interpretations between video pairs were categorized as homodiagnostic while different interpretations were categorized as heterodiagnostic. Raters were also asked to rate the image resolution, image detail, and total image quality using a 10-point Likert scale. For the diagnostic results, rater agreement, accuracy, sensitivity, and specificity were calculated. Results: The raters collectively demonstrated high agreement (92.1%), sensitivity (96.3%), specificity (92.8%), and accuracy (94.1%). Raters 1 (95.8%, κ = 0.912) and 3 (97.2%, κ = 0.942) had near perfect agreement, while rater 2 (83.3%, κ = 0.666) had substantial agreement. Only raters 1 and 3 detected a decrease in image resolution, image detail, and total image quality (p Conclusions: This study demonstrates that transmission via Skype has minimal effect on a physician’s interpretation of an ultrasound video. The implications of these results are that ultrasound video transmission can be used to improve access to specialists without compromising accuracy which will ultimately increase patients’ quality of care.

Water, Sanitation, and Hygiene Factors Associated with Child Illness in Tanzania

Water, sanitation, and hygiene (WASH) are critical to ensuring health and preventing disease in Tanzania where approximately one-third of childhood deaths are related to poor hygiene. This study explored associations between WASH practices and childhood illness. Data came from a cross-sectional survey of 5000 female caregivers living in the Lake Zone region of Tanzania. Measures included self-reported presence of fever, diarrhea, cough and various WASH factors. Multiple logistic regressions were used. Thirty-seven percent of children experienced fever, 26% diarrhea, and 11% cough in the previous two weeks. Unimproved toilets were positively associated with fever (OR 1.25, CI 1.03 - 1.53, p < 0.05) and animal enclosures were negatively associated with diarrhea (OR 0.76, CI 0.61 - 0.96, p < 0.05). Unsafe disposal of a child’s stool was associated with both fever (OR 0.77, CI 0.67 - 0.89, p < 0.05) and diarrhea (OR 1.18, CI 1.0 - 1.38, p < 0.05). Eating soil was associated with both fever (OR 2.02, CI 1.79 - 2.29, p < 0.05) and diarrhea (OR 2.23, CI 1.95 - 2.57, p < 0.05). Eating chicken feces was associated with both fever (OR 2.07, CI 1.66 - 2.58, p < 0.05) and diarrhea (OR 2.38, CI 1.9 - 2.98, p < 0.05). Water shortages were associated with fever (OR 1.21, CI 1.07 - 1.36, p < 0.05) and cough (OR 1.48, CI 1.22 - 1.81, p < 0.05). Policy makers and program designers should consider increasing access to water and sanitation to improve children’s health.

Hydropower production is one of the greatest threats to fluvial ecosystems and freshwater *** that we have entered the Anthropocene,there is an opportunity to reflect on what might constitute a‘sustainable’Anthropocene in the context of hydropower and riverine fish *** elements of existing practices that promote favorable social-ecological outcomes(i.e.,‘bright spots’)is timely given that there are plans to expand hydropower capacity in previously undammed rivers,intensify dam development in some of the world's largest river systems,and re-license existing *** approach this from a pragmatic perspective:for the foreseeable future,hydropower will likely remain an important source of renewable *** offer support for moving toward a more‘sustainable’Anthropocene,we provide syntheses of best practices during the siting,design,construction,operation,and compensation phases of hydropower development to minimize impacts on inland *** each phase,we offer positive examples(or what might be considered‘bright spots’)pertaining to some of the approaches described within our syntheses,acknowledging that these projects may not be viewed as without ecological and(or)societal detriment by all *** findings underscore the importance of protecting critical habitat and free-flowing river reaches through careful site selection and basinscale planning,infrastructure designs that minimize reservoir effects and facilitate safe passage of fish,construction of hydropower plants using best practices that minimize long-term damage,operating guidelines that mimic natural flow conditions,and compensation that is lasting,effective,inclusive,and locally *** from these‘bright spots’may require engagement of diverse stakeholders,professionals,and governments at scales that extend well beyond a given site,river,or even ***,environmental planning that integrates hydropower development into broader discussions

Restricted cross-scale habitat selection by American beavers

Animal habitat selection, among other ecological phenomena, is spatially scale dependent. Habitat selection by American beavers (hereafter, beaver) has been studied at singular spatial scales, but to date no research addresses multi-scale selection. Our objectives were to determine if beaver habitat selection was specialized to semiaquatic habitats and if variables explaining habitat selection are consistent between landscape and fine spatial scales. We built maximum entropy (MaxEnt) models to relate landscape-scale presence-only data to landscape variables, and used generalized linear mixed models to evaluate fine spatial scale habitat selection using global positioning system (GPS) relocation data. Explanatory variables between the landscape and fine spatial scale were compared for consistency. Our findings suggested that beaver habitat selection at coarse (study area) and fine (within home range) scales was congruent, and was influenced by increasing amounts of woody wetland edge density and shrub edge density, and decreasing amounts of open water edge density. Habitat suitability at the landscape scale also increased with decreasing amounts of grass frequency. As territorial, central-place foragers, beavers likely trade-off open water edge density (i.e., smaller non-forested wetlands or lodges closer to banks) for defense and shorter distances to forage and obtain construction material. Woody plants along edges and expanses of open water for predator avoidance may limit beaver fitness and subsequently determine beaver habitat selection.
