Significance of functional hepatic resection rate calculated using 3D CT/^(99m)Tc-galactosyl human serum albumin singlephoton emission computed tomography fusion imaging

AIM: To evaluate the usefulness of the functional hepatic resection rate (FHRR) calculated using 3D computed tomography (CT)/99mTc-galactosyl-human serum albumin (GSA) single-photon emission computed tomography (SPECT) fusion imaging for surgical decision ***: We enrolled 57 patients who underwent bi- or trisectionectomy at our institution between October 2013 and March 2015. Of these, 26 patients presented with hepatocellular carcinoma, 12 with hilar cholangiocarcinoma, six with intrahepatic cholangiocarcinoma, four with liver metastasis, and nine with other diseases. All patients preoperatively underwent three-phase dynamic multidetector CT and 99mTc-GSA scintigraphy. We compared the parenchymal hepatic resection rate (PHRR) with the FHRR, which was defined as the resection volume counts per total liver volume counts on 3D CT/99mTc-GSA SPECT fusion ***: In total, 50 patients underwent bisectionectomy and seven underwent trisectionectomy. Biliary reconstruction was performed in 15 patients, including hepatopancreatoduodenectomy in two. FHRR and PHRR were 38.6 ± 19.9 and 44.5 ± 16.0, respectively; FHRR was strongly correlated with PHRR. The regression coefficient for FHRR on PHRR was 1.16 (P 1000 mL, and/or macroscopic vascular invasion was significantly smaller than that for patients without these factors (0.73 ± 0.19 vs 0.82 ± 0.18, P < 0.05). Postoperative hyperbilirubinemia was observed in six patients. Major morbidities (Clavien-Dindo grade ≥ 3) occurred in 17 patients (29.8%). Despite surgical removal of tumors with portal vein tumor thrombus(PVTT) in hepatocellular carcinoma(HCC) patients, early recurrence tends to occur, and overall survival(OS) periods remain extremely short. The role that hepatectomy may play in long-term survival for HCC with PVTT has not been established. The operative mortality of hepatectomy for HCC with PVTT has also not been reviewed. Hence, we reviewed recent literature to assess these parameters. The OS of patients who received hepatectomy in conjunction with multidisciplinary treatment tended to be superior to that of patients who did not. Multidisciplinary treatments included the following: preoperative radiotherapy on PVTT; preoperative transarterial chemoembolization(TACE); subcutaneous administration of interferon-alpha(IFN-α) and intra-arterial infusion of 5-fluorouracil(5-FU) with infusion chemotherapy in the affected hepatic artery; cisplatin, doxorubicin and 5-FU locally administered in the portal vein; and subcutaneous injection of IFN-α, adjuvant chemotherapy(5-FU + Adriamycin) administration via the portal vein with postoperative TACE, percutaneous isolated hepatic perfusion and hepatic artery infusion and/or portal vein chemotherapy. The highest reported rate of operative mortality was 9.3%. In conclusion, hepatectomy for patients affected by HCC with PVTT is safe, has low mortality and might prolong survival in conjunction with multidisciplinary treatment.

Reliable surface treatment has been explored to improve the strength and wear resistance of aluminum alloy parts in automotives. Long duration time as well as long pre-sputtering time are required for plasma nitriding of aluminum or its alloys only with the thickness of a few micrometers. New plasma inner nitriding is proposed to realize the fast-rate nitriding of aluminum alloys. Al-6Cu alloy is employed as a targeting material in order to demonstrate the effectiveness of this plasma nitriding. Mechanism of fast-rate nitriding process is discussed with consideration of the role of Al2Cu precipitates.

Thermoelectric materials, Mg2Si1-xGex （x=0, 0.2, 0.4, 0.6, 0.8, 1）, have been prepared by bulk mechanical alloying （BMA） and hot pressing （HP）. The electrical conductivity, Seebeck coefficient and thermal conductivity were measured from room temperature up to about 700 K. The electrical conductivity of all the samples increases with increasing temperature, while the Seebeck coefficient and thermal conductivity decrease with increasing temperature. Mg2Si and Mg2Si0.8Ge0.2 possess negative type of conductivity, while for other compounds it is positive. At the same time, the effect of hot processing condition on thermoelectric properties was also investigated. The maximum figure of merit of Mg2Si0.6Ge0.4 was obtained with the processing parameter of BMA at 600 cycles and hot pressing at 773 K and i GPa for i h.

Direct Simulation Methods for Scalar-Wave Envelopes in Two-Dimensional Layered Random Media Based on the Small-Angle Scattering Approximation

This study presents stochastic methods to simulate wave envelopes in layeredrandom media. High-frequency Seismograms of small earthquakes are so complexdue to lithospheric inhomogeneity that seismologists often analyze wave envelopesrather than wave traces to quantify the subsurface inhomogeneity. Since thestatistical properties of the inhomogeneity vary regionally, it is important to developand examine direct envelope simulation methods for non-uniform random media. Asa simple example, this study supposes planewave propagation through two-layer randommedia in 2-D composed of weak and strong inhomogeneity zones. The characteristicspatial-scale of the inhomogeneity is supposed to be larger than the wavelength,where small-angle scattering around the forward direction dominates large-angle *** envelope simulation methods based on the small-angle scattering approximationare examined. One method is to solve a differential equation for the twofrequencymutual coherence function with the Markov approximation. The other isto solve the stochastic ray bending process by using the Monte Carlo method basedon the Markov approximation for the mutual coherence function. The resultant waveenvelopes of the two methods showed excellent coincidence both for uniform and fortwo-layer random media. Furthermore, we confirmed the validity of the two methodscomparing with the envelopes made from the finite difference simulations of *** two direct envelope simulation methods presented in this study can be a mathematicalbase for the study of high-frequency wave propagation through randomlyinhomogeneous lithosphere in seismology.
