Uncertainty is an important factor that needs to be considered while analyzing the performance of any engineering system.In order to quantify uncertainty,fuzzy set theory is frequently used by most of researchers,including energy system experts.According to the classical reliability theory,component lifetimes have crisp parameters,but due to uncertainty and inaccuracy in data,it is sometimes very difficult to determine the exact values of these parameters in real-world systems.To overcome this difficulty in the current research,failure and repair rates were taken as triangular fuzzy numbers to determine the fuzzy availability of a system undergoing calendar-based periodic inspection subject to multiple failure modes(FMs).It was assumed that each component in the system had an exponential failure rate and repair rate with fuzzy parameters.System FMs were explicitly taken into account when a functional state of the system was considered.Each FM had a random failure time.On the occurrence of any failure,a random time was selected for the relevant corrective repair work.The proposed research was studied for one of the major sources of green energy,namely a wind turbine system wherein all the derived propositions have been implemented on it.

BACKGROUND Current osteoarthritis(OA)treatments focus on symptom relief without addressing the underlying disease process.In regenerative medicine,current treatments have limitations.In regenerative medicine,more research is needed for intra-articular stromal vascular fraction(SVF)injections in OA,including dosage optimization,long-term efficacy,safety,comparisons with other treatments,and mechanism exploration.AIM To compare the efficacy of intra-articular SVF with corticosteroid(ICS)injections in patients with primary knee OA.METHODS The study included 50 patients with Kellgren-Lawrence grades II and III OA.Patients were randomly assigned(1:1)to receive either a single intra-articular SVF injection(group A)or a single intra-articular ICS(triamcinolone)(group B)injection.Patients were followed up at 1,3,6,12,and 24 months.Visual analog score(VAS)and International Knee Documentation Committee(IKDC)scores were administered before the procedure and at all followups.The safety of SVF in terms of adverse and severe adverse events was recorded.Statistical analysis was performed with SPSS Version 26.0,IBM Corp,Chicago,IL,United States.RESULTS Both groups had similar demographics and baseline clinical characteristics.Follow-up showed minor patient loss,resulting in 23 and 24 in groups A and B respectively.Group A experienced a notable reduction in pain,with VAS scores decreasing from 7.7 to 2.4 over 24 months,compared to a minor reduction from 7.8 to 6.2 in Group B.This difference in pain reduction in group A was statistically significant from the third month onwards.Additionally,Group A showed significant improvements in knee functionality,with IKDC scores rising from 33.4 to 83.10,whereas Group B saw a modest increase from 36.7 to 45.16.The improvement in Group A was statistically significant from 6 months and maintained through 24 months.CONCLUSION Our study demonstrated that intra-articular administration of SVF can lead to reduced pain and improved knee function in patients with primary knee OA.More adequately powe

Persistent gastritis induced by Helicobacter pylori(H.pylori)is the strongest known risk factor for gastric cancer(GC).H.pylori is prevalent in about 50%world's population,while it causes cancer in less than 2%of exposed individuals.1 Our studies found that H.pylori infection can induce oncogenic properties in AGS cells by deregulating multiple factors associated with the cell cycle,apoptosis,and other important events associated with cancer progression.

In the present work,we study the time evolution,significance of the N-S asymmetry excesses presented as a function of the solar cycle and prominent rotational periods(~27 d)separately for the northern and southern hemispheres.We have investigated short-term variations of the hemispheric solar activity(sunspot numbers and sunspot areas)during the time period 2010-2015,which covers the ascending and the maximum phase of solar cycle 24.We have implemented the Lomb-Scargle periodogram and continuous wavelet transform power spectrum techniques to study the time evolution and dominant rotational periods separately for the northern and southern hemispheres,and whole solar disk.Our results showed that the northern hemisphere exhibited longer solar synodic periods than the southern hemisphere,indicating that the northern hemisphere has a lower rotation rate.Moreover,the northern hemisphere was found to be dominant before transferring to the southern hemisphere during mid-2013.Also,the sunspot areas clearly demonstrated a two-peak structure of solar activity in the northern and southern hemispheres respectively during 2012 and 2014.The statistical significance of the southern hemisphere affirmed enhanced excess during the maximum phase of solar cycle 24.

Recharge and vulnerability assessment of groundwater resources in North west India:Insights from isotope-geospatial modelling approach

Recent studies indicate dwindling groundwater quantity and quality of the largest regional aquifer system in North West India,raising concern over freshwater availability to about 182 million population residing in this region.Widespread agricultural activities have resulted severe groundwater pollution in this area,demanding a systematic vulnerability assessment for proactive measures.Conventional vulnerability assessment models encounter drawbacks due to subjectivity,complexity,data-prerequisites,and spatial-temporal constraints.This study incorporates isotopic information into a weighted-overlay framework to overcome the above-mentioned limitations and proposes a novel vulnerability assessment model.The isotope methodology provides crucial insights on groundwater recharge mechanisms(18O and 2H)and dynamics(3H)-often ignored in vulnerability assessment.Isotopic characterisation of precipitation helped in establishing Local Meteoric Water Line(LMWL)as well as inferring contrasting recharge mechanisms operating in different aquifers.Shallow aquifer(depth<60 m)showed significant evaporative signature with evaporation loss accounting up to 18.04%based on Rayleigh distillation equations.Inter-aquifer connections were apparent from Kernel Density Estimate(KDE)and isotope correlations.A weighted overlay isotope-geospatial model was developed combining 18O,3H,aquifer permeability,and water level data.The central and northern parts of study area fall under least(0.29%)and extremely(1.79%)vulnerable zones respectively,while majority of the study area fall under moderate(42.71%)and highly vulnerable zones(55.20%).Model validation was performed using groundwater NO3-concentration,which showed an overall accuracy up to 82%.Monte Carlo Simulation(MCS)was performed for sensitivity analysis and permeability was found to be the most sensitive input parameter,followed by 3H,18O,and water level.Comparing the vulnerability map with Land Use Land Cover(LULC)and population density maps helped in precisely identifying the high-risk s

Recent advancements in science and technology,coupled with the proliferation of data,have also urged laboratory medicine to integrate with the era of artificial intelligence(AI)and machine learning(ML).In the current practices of evidencebased medicine,the laboratory tests analysing disease patterns through the association rule mining(ARM)have emerged as a modern tool for the risk assessment and the disease stratification,with the potential to reduce cardiovascular disease(CVD)mortality.CVDs are the well recognised leading global cause of mortality with the higher fatality rates in the Indian population due to associated factors like hypertension,diabetes,and lifestyle choices.AI-driven algorithms have offered deep insights in this field while addressing various challenges such as healthcare systems grappling with the physician shortages.Personalized medicine,well driven by the big data necessitates the integration of ML techniques and high-quality electronic health records to direct the meaningful outcome.These technological advancements enhance the computational analyses for both research and clinical practice.ARM plays a pivotal role by uncovering meaningful relationships within databases,aiding in patient survival prediction and risk factor identification.AI potential in laboratory medicine is vast and it must be cautiously integrated while considering potential ethical,legal,and privacy concerns.Thus,an AI ethics framework is essential to guide its responsible use.Aligning AI algorithms with existing lab practices,promoting education among healthcare professionals,and fostering careful integration into clinical settings are imperative for harnessing the benefits of this transformative technology.

In Computer-Aided Detection(CAD)brain disease classification is a vital issue.Alzheimer’s Disease(AD)and brain tumors are the primary reasons of death.The studies of these diseases are carried out by Magnetic Resonance Imaging(MRI),Positron Emission Tomography(PET),and Computed Tomography(CT)scans which require expertise to understand the modality.The disease is the most prevalent in the elderly and can be fatal in its later stages.The result can be determined by calculating the mini-mental state exam score,following which the MRI scan of the brain is successful.Apart from that,various classification algorithms,such as machine learning and deep learning,are useful for diagnosing MRI scans.However,they do have some limitations in terms of accuracy.This paper proposes some insightful pre-processing methods that significantly improve the classification performance of these MRI images.Additionally,it reduced the time it took to train the model of various pre-existing learning algorithms.A dataset was obtained from Alzheimer’s Disease Neurological Initiative(ADNI)and converted from a 4D format to a 2D format.Selective clipping,grayscale image conversion,and histogram equalization techniques were used to pre-process the images.After pre-processing,we proposed three learning algorithms for AD classification,that is random forest,XGBoost,and Convolution Neural Networks(CNN).Results are computed on dataset and show that it outperformed with exiting work in terms of accuracy is 97.57%and sensitivity is 97.60%.

In December 2019,a new strain of coronavirus was discovered in China,and the World Health Organization declared it a pandemic in March 2020.The majority of people with coronavirus disease 19(COVID-19)exhibit no or only mild symptoms such as fever,cough,anosmia,and headache.Meanwhile,approximately 15%develop a severe lung infection over the course of 10 d,resulting in respiratory failure,which can lead to multi-organ failure,coagulopathy,and death.Since the beginning of the pandemic,it appears that there has been consideration that preexisting chronic liver disease may predispose to deprived consequences in conjunction with COVID-19.Furthermore,extensive liver damage has been linked to immune dysfunction and coagulopathy,which leads to a more severe COVID-19 outcome.Besides that,people with COVID-19 frequently have abnormal liver function,with more significant elevations in alanine aminotransferase and aspartate aminotransferase in patients with severe COVID-19 compared to those with mild/moderate disease.This review focuses on the pathogenesis of severe acute respiratory syndrome coronavirus-2(SARS-CoV-2)infection in the liver,as well as the use of liver chemistry as a prognostic tool during COVID-19.We also evaluate the findings for viral infection of hepatocytes,and look into the potential mechanisms behind SARS-CoV-2-related liver damage.

Prevalence and risk factors of lymphatic dysfunction in cirrhosis patients with refractory ascites:An often unconsidered mechanism

BACKGROUND The lymphatic system is crucial in maintaining the body fluid homeostasis.A dysfunctional lymphatic system may contribute to the refractoriness of ascites and edema in cirrhosis patients.Therefore,assessment of lymphatic dysfunction in cirrhosis patients with refractory ascites(RA)can be crucial as it would call for using different strategies for fluid mobilization.AIM To assessing the magnitude,spectrum,and clinical associations of lymphatic dysfunction in liver cirrhosis patients with RA.METHODS This observational study included 155 consecutive cirrhosis patients with RA.The presence of clinical signs of lymphedema,such as peau d’orange appearance and positive Stemmer sign,intestinal lymphangiectasia(IL)on duodenal biopsy seen as dilated vessels in the lamina propria with strong D2-40 immunohistochemistry,and chylous ascites were used to diagnose the overt lymphatic dysfunctions.RESULTS A total of 69(44.5%)patients out of 155 had evidence of lymphatic dysfunction.Peripheral lymphedema,found in 52(33.5%)patients,was the most common manifestation,followed by IL in 42(27.0%)patients,and chylous ascites in 2(1.9%)patients.Compared to patients without lymphedema,those with lymphedema had higher mean age,median model for end-stage liver disease scores,mean body mass index,mean ascitic fluid triglyceride levels,and proportion of patients with hypoproteinemia(serum total protein<5 g/dL)and lymphocytopenia(<15%of total leukocyte count).Patients with IL also had a higher prevalence of lymphocytopenia and hypoproteinemia(28.6%vs.9.1%,P=0.004).Seven(13%)patients with lymphedema had lower limb cellulitis compared to none in those without it.On multivariate regression analysis,factors independently associated with lymphatic dysfunction included obesity[odds ratio(OR):4.2,95%confidence intervals(95%CI):1.1–15.2,P=0.027],lymphocytopenia[OR:6.2,95%CI:2.9–13.2,P<0.001],and hypoproteinemia[OR:3.7,95%CI:1.5–8.82,P=0.003].CONCLUSION Lymphatic dysfunction is common in cirrhosis patients with RA.Significant indicators of i
