Role of cytokines and other factors involved in the Mycobacterium tuberculosis infection

Mycobacterium tuberculosis （Mtb） is a pathogen that is widely distributed geographically and continues to be a major threat to world health. Bacterial virulence factors, nutritional state, host genetic condition and immune response play an important role in the evolution of the infection. The genetically diverse Mtb strains from different lineages have been shown to induce variable immune system response. The modern and ancient lineages strains induce different cytokines patterns. The immunity to Mtb depends on Th1-cell activity [interferon- γ （IFN- γ ）, interleukin-12 （IL-12） and tumor necrosis factor-α （TNF-α）]. IL-1β directly kills Mtb in murine and human macrophages. IL-6 is a requirement in host resistance to Mtb infection. IFN- γ , TNF-α, IL-12 and IL-17 are participants in Mycobacterium-induced granuloma formation. Other regulating proteins as IL-27 and IL-10 can prevent extensive immunopathology. CXCL 8 enhances the capacity of the neutrophil to kill Mtb . CXCL13 and CCL19 have been identified as participants in the formation of granuloma and control the Mtb infection. Treg cells are increased in patients with active tuberculosis （TB） but decrease with anti-TB treatment. The increment of these cells causes down- regulation of adaptive immune response facilitating the persistence of the bacterial infection. Predominance of Th2 phenotype cytokines increases the severity of TB. The evolution of the Mtb infection will depend of the cytokines network and of the infuence of other factors aforementioned.

研究目的对有太阳经络颈部疼痛的病人实施两种不同类型的治疗(一组和二组)。此次试验中的这两种类型的治疗组使用了同样的针灸穴位,但是我在治疗二组中添加了 4个新的穴位:筋会穴的阳经本神GB 13和颧髎SI 18,还有井穴处的太阳经络少泽SI 1和至阴BL67。观察并评估两种治疗组的临床疗效,对比两种疗法并总结两者之间是否有任何的不同疗效,从而为临床治疗所用。研究方法颈部疼痛现在在西班牙已经是一种非常普遍的问题,而且病人们一般都会选择去巴塞罗那的Fractal Studio Acupuncture诊所进行治疗所以我在那里参与了研究。大多数的治疗方法,我让他们去病人的家中。其余的治疗方法我可以在我的诊所做。整个研究都是以易于整合评估与比较治疗效果的为基础设计的。我一共治疗了 60例从门诊部选取的病人,同时将他们随机分为了每组30例病人的2个治疗组(一治疗组和二治疗组)。总的来选取的标准为不论性别同时拥有太阳处颈部疼痛症状病史的20到60岁的病人。除此之外的标准还会选取以下但不仅限于以下的病例:颈部骨折,椎基底动脉供血不足,颈脊髓癌,炎症或其它系统性疾病,和上颈椎韧带不稳[1]的病人。在实验研究中我将年龄、性别、疼痛等级和病程作为主要的变量。在实验研究中,我根据中西医之间的不同之处制定了诊疗的标准。由于太阳筋络的颈部疼痛可能由以上的任意病因造成,所以在这次实验研究中我决定将病因不同的影响排除在外,并专注于在对经络(太阳经络)的治疗研究上。此两种类型的治疗都将使用同样的针灸点但是我在治疗二组中添加了 4个穴位(筋会穴的本神GB 13和颧髎SI18,太阳经络处井穴的少泽SI1和至阴BL 67)。每个病人都将接受10次治疗,一天一次每周5天。整个疗程总共2周。数据的收集开始于病人们的第一次来访时,使用V.A.S.疼痛等级评估让病人从1-10级中评估自己所处的疼痛等级。第一次记录于第一次治疗开始前,最后一次记录于第10次治疗结束后,以此来评估病人们的情况是否有好转。为了可以更加清晰和具体的描述结果,我设计了 4组治愈等级用来描述两治疗组的疗效(A,B,C,D级)其中A代表剩余疼痛等级为0即治愈,B代表疼痛等级降低了 5-9个等级即显效,C代表疼痛等级降低了2-4个等级即疗效有效,D代表疼痛等级降低了 0-1个等级即无效。描述性的数据被整理并用SPSS软件进行了分析,对年龄、病程、前疼痛V.A.S.等级和后V.A.S.疼痛等级使用了 T检验,对年龄使用了卡方分析。最后对两组治疗组的疗效等级的对比使用了秩和检验。研究结果在两组治疗前后在年龄、性别、病程及前疼痛等级上的差别并无统计学意义(P>0.05),所以说两组病例可以互相对比。通过T检验,两组病例在年龄上的差异并无统计学意义(t=0.178,P=0.859)。通过T检验,两组病例在病程上的差异并无统计学意义(t=1.050,P=0.301)。通过卡方分析,两组病例在年龄上的差异并无统计学意义(x2=0.067,P=0.796)。通过T检验,两组病例在前疼痛等级上的差异并无统计学意义(t=1.553,P=0.127)。在疗程结束后两组病例在V.A.S.疼痛等级上的差异并无统计学意义(P>0.05)。通过对比这两组病例,可以发现在疗程后的V.A.S.等级上两组病例并无统计学的意义(t=1.742,P=0.087)。通过秩和检验,两组治疗组的疗效等级上的差异并无统计学意义(Z=0.603,P=0.546)。研究结论两组治疗组同时对太阳处的颈部疼痛有着积极的疗效,在60例病人中达到C级有效(疼痛等级下降2-4个等级)的有39例。达到B级显效(疼痛等级下降了 5-9个等级)的病例数量占第二位,为13例。达到A级治愈(疼痛等级降为0等级)的病例数量与达到D级无效(疼痛等级下降了 0-1个等级)的病例数量一致,为4例.所以,两组病例同时都得到了有效的治疗。根据病例的治愈率来说治疗组一的结果稍好于治疗组二(治疗组一:96.67%的病例情况得到提高;治疗组二:90%的病例情况得到提高)。但根据统计学上的分析来看治疗组一和治疗组二在治疗效果上并不存在统计学上的差异(Z=0.603,P=0.546)。

Mucormycosis: A Review on Environmental Fungal Spores and Seasonal Variation of Human Disease

Mucormycosis is on the rise especially among patients with immunosuppressive conditions. There seems to be more cases seen at the end of summer and towards early autumn. Several studies have attempted to look at the seasonal variations of fungal pathogens in variou indoor and outdoor settings. Only two reports, both from the Middle East, have addressed the relationship of mucormycosis in human disease with climate conditions. In this paper we review, the relationship of indoor and outdoor fungal particulates to the weather conditions and the reported seasonal variation of human cases.

Support Systems Designed for Older Drivers to Achieve Safe and Comfortable Driving

Background: The number of older people is increasing. Many of them expect to maintain a rich social life and to continue driving at an older age. Objective: The present study investigates the mechanisms behind self-regulation and driving cessation in order to suggest development of support systems to prolong older drivers’ safe mobility. Method: Three focus groups were conducted with 19 older active drivers aged 65+ who were divided according to annual mileage driven. Results: A content analysis revealed broad self-regulatory behaviour as already reported in the literature, e.g., avoiding driving at rush hour and at night. The participants also reported difficulty in finding the way to their final destination and an increasing need to plan their travelling. Co-piloting was a behaviour applied by couples to cope with difficulties encountered in traffic. A large part of the discussion was focused on emerging feelings of stress, anxiety and fear when driving in recent years, a feeling induced by external factors e.g., other road users’ behaviour, traffic density or high speed. Apart from health problems, high levels of stress could explain driving cessation, especially for women. An increased feeling of safety and comfort could be achieved by an increased use of support systems specifically designed to respond to older drivers’ needs. Conclusion: Support systems for older drivers should increase comfort and decrease their stress levels. New systems, such as co-pilot function and more developed Global Positioning System (GPS) supporting of the entire travel from door to door, should be developed to respond to the market needs.

Outcomes in randomized controlled trials of exercise interventions in solid organ transplant

AIM To identify the outcome measures that have been used in randomized controlled trials(RCTs) of exercise training in solid organ transplant(SOT) recipients and to link these outcomes to the International Classification of Functioning, Disability and Health(ICF) *** Electronic literature searches of MEDLINE, EMBASE, CINAHL, Cochrane, Scopus, and Web of Science were performed. We sought RCTs that investigated the effect of exercise training in SOT recipients. Reference lists of all eligible publications were searched for other appropriate studies not identified by the electronic search. A complete list of outcome measures used in the RCTs was generated and each of these was linked to an ICF *** Four hundred and thirteen articles were retrieved, of which 35 met our inclusion criteria. The studies included were designed to compare the effects of exercise training programs to usual care or to another exercise training program and reported on recipients of heart(n = 21), kidney(n = 9), lung(n = 3) or liver(n = 2) transplant. Of the 126 outcome measures identified, 62 were used as primary outcome measures. The most commonly occurring primary outcomes were aerobic capacity using the peak VO2(n = 13), quality of life using the shortform-36(n = 8), and muscle strength(n = 7). Theseoutcome measures were linked to 113 ICF categories and the majority of outcomes fall into the body function domain(n = 93). CONCLUSION There is little standardization in outcome measures used in RCTs of exercise interventions in SOT recipients. The ICF framework can be used to select a core set of outcomes that cross all domains of ICF and that would be appropriate to all SOT recipients.

Designing an Effective Method for Automatic Electric Vehicle Charging Stations in a Static Environment

This article outlines an Effective Method for Automatic Electric Vehicle Charging Stations in a Static Environment. It consists of investigated wireless transformer structures with various ferrite forms. WPT technology has rapidly advanced in the last few years. At kilowatt power levels, the transmission distance grows from a few millimeters to several hundred millimeters with a grid to load efficiency greater than 90%. The improvements have made the WPT more appealing for electric vehicle (EV) charging applications in both static and dynamic charging scenarios. Static and dynamic WEVCS, two of the main applications, are described, and current developments with features from research facilities, academic institutions, and businesses are noted. Additionally, forthcoming concepts based WEVCS are analyzed and examined, including “dynamic” wireless charging systems (WCS). A dynamic wireless power transfer (DWPT) system, which can supply electricity to moving EVs, is one of the feasible alternatives. The moving secondary coil is part of the dynamic WPT system, which also comprises of many fixed groundside (primary) coils. An equivalent circuit between the stationary system and the dynamic WPT system that results from the stationary system is demonstrated by theoretical investigations. The dynamic WPT system’s solenoid coils outperform circular coils in terms of flux distribution and misalignment. The WPT-related EV wireless charging technologies were examined in this study. WPT can assist EVs in overcoming their restrictions on cost, range, and charging time.

Aim: Fibulins and ADAMTSs are two families of extracellular matrix proteins implicated in key functional and pathological processes. The fact that the fibulin-1 and ADAMTS-1 proteins interact raises new questions about the roles of these extracellular matrix proteins in modulating tumor progression. Herein, we described the functional implications of the interaction between fibulin-1 and ADAMTS-1 on the behavior of breast cancer cell lines. Methods: Fibulin-1 and ADAMTS-1 were exogenously expressed in MCF-7 and MDA-MB-231 cell lines to assay the effect of their interaction in cellular properties. Results: ADAMTS-1 expression exacerbates tumor effects in terms of proliferation, invasion and mammosphere formation. In contrast, the simultaneous expression of ADAMTS-1 and fibulin-1 impairs these effects. The analysis of the expression of both proteins in human breast cancer tissue arrays provides new insights into the complex roles of fibulin-1 and ADAMTS-1 in this type of tumor. ;Conclusion: Our results suggests that the interaction between ADAMTS-1 and fibulin-1 induces a pronounced anti-tumoral effect.

Energy Audit in the Meat Processing Industry—A Case Study in Hermosillo,Sonora Mexico

The meat processing industry, due to its high energy consumption, needs to be assessed in an energy usage basis. This paper reports the results and implications of an energy audit in a meat processing industry. In addition, this work provides a comprehensive and practical approach to energy saving measures in the assessed company to recognize factors that can determine a possible transition to sustainable patterns of electricity consumption. The paper described the application to energy auditing, developed by AFNOR (2014) for efficient energy management along with ISO 50001 (energy management systems). For a more specific energy auditing the guide described by Dall “O” (2013) was used. In this case, a study of an integrative characterization of the company’s energy consumption is made. The research has been divided in two main sections: the first includes an analysis about the characterization of the energy consumption within a meat processing company in the three sustainable approaches such as economical, societal and mainly environmental implications;second, a proposal for strategic energy management measures focusing on high consumer types of facilities. The results obtained allow the identification of main processes with significant correlations in terms of energy consumption within the company. This data has the potential for energy savings. The data acquisition process prompts the development of practical and accessible energy efficiency measures. In addition, a benchmarking analysis with several tools is performed. Altogether, this work gives guidance on the implementation of energy auditing in industries within its geographical and industrial sector limitations.

We review the case of a 74-year-old patient with ad-vanced colon cancer who suffered recurrent bouts of hypoglycemia. A state of inappropriate, non-suppressed hyperinsulinism in the presence of severe hypoglycemia was diagnosed. We finally discuss the known mecha-nisms behind fasting hypoglycemia in patients with ad-vanced cancer, the diagnosis, and possible treatments of this rare paraneoplastic endocrine complication.
