Factors affecting recurrence after surgery for Crohn's disease

Although in Crohn's disease post-operative recurrence is common, the determinants of disease recurrence remain speculative. The aim of this study was to examine factors affecting post-operative recurrence of Crohn's disease. A Medline-based literature review was carried out. The following factors were investigated: age at onset of disease, sex, family history of Crohn's disease,smoking, duration of Crohn's disease before surgery,prophylactic medical treatment (corticosteroids, 5-amino salicylic acid [5-ASA] and immunosuppressants),anatomical site of involvement, indication for surgery (perforating or non-perforating disease), length of resected bowel, anast-omotic technique, presence of granuloma in the specimen, involvement of disease at the resection margin, blood transfusions and postoperative complications. Smoking significantly increases the risk of recurrence (risk is approximately twice as high), especially in women and heavy smokers. Quitting smoking reduces the post-operative recurrence rate. A number of studies have shown a higher risk when the duration of the disease before surgery was short. There were, however, different definitions of 'short' among the studies. Prophylactic cortic-osteroids therapy is not effective in reducing the post-operative recurrence. A number of randomized controlled trials offered evidence of the efficacy of 5-ASA (mesalazine) in reducing post-operative recurrence. Recently, the thera-peutic efficacy of immunosuppressive drugs (azathioprine and 6-mercaptopurine) in the prevention of post-operative recurrence has been investigated and several studies have reported that these drugs might help prevent the recurrence. A combination treatment of entecavir and early-phase corticosteroid in severe exacerbation of chronic hepatitis B

Of patients with severe exacerbation of chronic hepatitis B accompanied by jaundice and coagulopathy,20%-30%have a fatal *** this report,we describe 2 cases of severe exacerbation of chronic hepatitis B with jaundice and coagulopathy who were successfully treated with a combination of entecavir and *** both cases,rapid reductions in serum hepatitis B virus(HBV)-DNA levels were observed,and corticosteroid was stopped after serum HBV-DNA levels became *** treatment was ***,entecavir treatment reduced serum HBV-DNA levels rapidly,although the improvement in liver function was delayed by a few *** this time lag,liver cell injury continued and the disease *** suppressed the excessive host immune response and was useful for stopping progressive deterioration.A combination of entecavir and early-phase corticosteroid may be a useful treatment in severe exacerbation of chronic hepatitis B.

Decomposition of Glycerine by Water Plasmas at Atmospheric Pressure

High concentration of aqueous glycerine was decomposed using a direct current （DC） plasma torch at atmospheric pressure. The torch can generate the plasma with water as the plasma-supporting gas in the absence of any additionM gas supply system and cooling devices. The results indicated that 5 mol% glycerine was completely decomposed by water plasmas at arc powers of 0.55~1.05 kW. The major products in the effluent gas were H2 （68.9%~71.1%）, CO2 （18.9%~23.0%）, and CO （0.2%~0.6%）. However, trace levels of formic acid （HCOOH） and formaldehyde （HCHO） were observed in the liquid effluent. The results indicated that the water plasma waste treatment process is capable of being an alternative green technology for organic waste decomposition.

A high-resolution climatic change since the Late Glacial Age inferred from multi-proxy of sediments in Qinghai Lake

Based on multi-proxy analysis of pollen, carbonate, TOC, TN and δ13C of organic matters, a high-resolution climatic evolution of Qinghai Lake since the Late Glacial Age is recon-structed. The results indicate that the boundary between the Last Glacial Maximum and the Late Glacial Age is at about 18.2 *** BP. The warm and wet period, which began at about 15.4 *** BP, culminated at 7.4 *** BP and came to its end at about 4.5 *** BP. After that, the climate gradually became cold and dry. The multi-proxy analysis indicates that the climate fluc-tuated greatly during the transitional period from the Late glacial Age to the Holocene, and this is in good accordance with that reflected by deep sea cores of North Atlantic, ice cores of Greenland, lake sediments in Europe, loess sequences and Guliya ice core in China. The cli-matic evolutional characteristic of the Qinghai Lake since the Late Glacial Age shows that the driving force of the East-Asia Monsoon correlates with solar radiation on the ten-thousand-year scale.

Four sediment cores were collected from Mikawa Bay, Japan, and excess ^210pb and ^137Cs were measured by gamma spectrometry. Sedimentation rates for the four cores were determined by ^210Pb method. The sedimentation rate range is 0.10-0.70 g/（cm^***）. The bio-mixing depth for each core is less than 7.0 cm, and was determined by the excess 210^pb profiles as well. Therefore, the bioturbation is slight. The 210^pb-derived dates coincided with the results from ^137Cs geochronology. Acceleration in sedimentation rate due to environmental alteration has been found in cores A2.5 and 05AS8, representing two depocenters due to their topography. Evidence of the Tokal Flood in 2000 was found in core 05AS8 according to the profiles of both radioisotopes and trace metals.

The fatigue behavior of four extruded Mg-Y-Zn alloys containing different volume fractions of long-period stacking ordered(LPSO)grains was investigated through a comparative study combining experiments and crystal plasticity finite element *** controlled low-cycle fatigue experiments were conducted at different strain amplitudes and revealed a limited cyclic hardening in Mg_(89)Zn_(4)Y_(7)alloy or softening in Mg_(99.2)Zn_(0.2)Y_(0.6)and Mg_(97)Zn_(1)Y_(2)alloys.A decrease in the fatigue life against the plastic strain with the increase in LPSO phase volume fraction was observed and was related the limited ductility of extruded LPSO ***-strain hysteresis curves were used to calibrate and validate a crystal plasticity model taking into account twinning and *** interaction of the different phases on the distribution of local micro-mechanical fields at the grain scale was then analyzed on synthetic microstructures under strain-controlled *** twinning activity was predicted in coarse unrecrystallized grains and tended to disappear with the increase in the LPSO phase volume ***-like facets observed in LPSO grains were related to high tensile stress,especially at the Mg/LPSO interface,due to the limited number of deformation mechanisms in LPSO crystal to accommodate out-of-basal plane *** increase of the fatigue limit with the increase in LPSO phase volume fraction was finally associated with the decreasing presence of coarse unrecrystallizedα-Mg grains due to a higher dynamic recrystallization activity during the extrusion process.
