Importance of abdominal X-ray to confirm the position of levonorgestrel-releasing intrauterine system:A case report

BACKGROUND Levonorgestrel-releasing intrauterine systems(LNG-IUSs)gradually release levonorgestrel into the uterus and is effective against hypermenorrhoea and dysmenorrhea.Complications associated with the insertion include expulsion,displacement,and uterine perforation.Ultrasonic identification of copper intrauterine devices(IUDs)is possible due to echogenicity from the copper coils.However,the barium sulfate coatings of LNG-IUSs do not always provide hyperechoic images.Both barium sulfate and copper are radiopaque and clearly identifiable on X-ray.Thus,X-ray imaging is required to locate LNG-IUSs.CASE SUMMARY A 46-year-old woman with hypermenorrhoea due to submucosal myomas was treated with LNG-IUS at another hospital.Three LNG-IUS insertions had apparently been followed by spontaneous expulsion,although objective confirmation using imaging was not performed.The patient was referred to our institution for surgery.At the first visit,there appeared to be no device in the uterus,and none was observed on transvaginal ultrasound.However,two LNGIUSs were observed in the pelvis on abdominal plain X-rays prior to surgery.Hysteroscopic myomectomy was performed,and the two LNG-IUSs were found to have perforated the myometrium.The devices were safely removed during surgery,and the submucosal myomas were also removed.The perforated section of the myometrium was minimal+ADs-therefore,a repair operation was not required.CONCLUSION Plain abdominal X-rays facilitate the determination of whether an LNG-IUS is in the uterine cavity.Therefore,it is important to confirm a device’s location,regardless of whether spontaneous expulsion is suspected,prior to inserting another device.

OBJECTIVE:To examine whether specific stimulation of Shenshu(BL23)affects sympathetic nervous activity(SNA)-associated plasma renin concentration(PRC).METHODS:Eight healthy volunteers participated in three pattern conditions in random order:control(Cont),stimulation of Shenshu(BL23),and stimulation of sham point(Sham).All participants were initially in the supine position for>60 min,and then remained in the standing position during the experimental procedure to increase SNA.An electrocardiogram was used to calculate low frequency/high frequency(LF/HF)ratio;blood was collected to analyze PRC.RESULTS:The LF/HF ratio was significantly increased in the standing position when compared with the supine position(P<0.01).There was no difference in LF/HF ratio during or after stimulation of Shenshu(BL23)in the standing position when compared with before the stimulation in the supine position;however,the LF/HF ratio was significantly increased in Cont and Sham conditions(P<0.01).There was no difference in PRC after stimulation of Shenshu(BL23)in the standing position when compared with before the stimulation in the supine position;however,there was a significant increase in PRC in the Cont and Sham conditions(Cont P<0.05,Sham P<0.01).CONCLUSION:Our results demonstrated that specific acupuncture stimulation of Shenshu(BL23)in the standing position decreased SNA-associated PRC,which was not observed during acupuncture stimulation of the sham point.

Flexible Förster resonance energy transfer-assisted optical waveguide based on elastic mixed molecular crystals

Flexible molecular crystal waveguides based on elastic molecular crystals(EMCs)are essential inﬂexible and compact optical materials.An increased loss coefﬁ-cientαdue to self-absorption is often a problem in optical waveguides(OWGs)ofﬂuorescent chemical materials waveguiding photons in active mode.Herein,the development of anthracene-based elastic mixed molecular crystals(EMMCs)is reported for Förster Resonance Energy Transfer(FRET)-assisted OWG.To yield a FRET crystal system based on elastic molecular crystals,1%–5%accep-tor doping forﬂuorescent molecular crystals of 9,10-dibromoanthracene 1 was successful by selecting the same regioisomer having electron-withdrawing group,9,10-diformylanthracene 2,as a dopant.In addition to conversion to the mixed system,there is a difference in the elastic modulus and hardness in EMC C1 and EMMC C2@1.However,the elastic behaviour was also shown in a few percent doping of the acceptor.Theαvalue of this EMMC,composed of 1 including 1%of 2(0.0077 dB/μm),is much lower than that of EMC composed of 1(0.1258 dB/μm)because of reducing self-absorption in the FRET system.An efﬁcient andﬂexible OWG was successfully developed by selecting an appropriate acceptor molecule and its low doping rate for mixed crystal construction.This method is a practical approach in various functional andﬂexible crystal systems.

A Series of Soluble Planar Oligorylenes up to Hexarylene

Preparation of large rylenes often needs the use of solubilizing groups along the rylene backbone,which results in the distortion of the skeleton and thus broadening of the absorption spectrum.In this work,we successfully synthesized 2,5,8,16,19,22-hexakis[2,6-bis(tri-fluoromethyl)phenyl]hexarylene(H)by using an oxidative fusion reaction of perylene trimer in 5%yield.The product is soluble in common organic solvents and could be purified with column chromatography.Single-crystal X-ray diffraction analysis revealed that the hexarylene core is not disturbed by the substituents very much and is almost planar.The longest absorption of H exhibits a sharp peak at 831 nm and an on-set at 875 nm in toluene with a small full-width-at-half-maximum(FWHM)of 660 cm^(-1).The molar extinction coefficient is as large as 131,000 M^(-1)cm^(-1).The substituents could not perturb the electronic properties of the hexarylene because aryl groups are perpendicular to the hexarylene core and the carbon atoms at 2,5,16,19-positions have negligible coeffi-cients on its HOMO and LUMO,estimated by theoretical calculations.As a result,the hexarylene having less absorption at the visible light region was stable enough in solution under the LED room light for more than 1 week.This strategy gives a useful guideline to establish the near-infrared materials that exhibit stability under ambient conditions.

Laparoscopic retroperitoneal lymph node dissection versus open retroperitoneal lymph node dissection for testicular cancer:A comparison of clinical and perioperative outcomes

Objective:This study was performed to evaluate the clinical and perioperative outcomes of laparoscopic retroperitoneal lymph node dissection(L-RPLND)and open retroperitoneal lymph node dissection(O-RPLND)performed by one surgeon at a single center.Methods:We evaluated 30 patients with stage IIA germ cell tumors who underwent retroperitoneal lymph node dissection(15 underwent L-RPLND and 15 underwent O-RPLND)at our institution between April 1,2010 and March 31,2018.The clinical parameters were compared between patients who underwent L-RPLND using the retroperitoneal approach and those who underwent O-RPLND using the transperitoneal approach.There were no significant differences in the background characteristics of the two groups except for the median follow-up duration(46 months for L-RPLND and 71 months for O-RPLND,p=0.02).Results:L-RPLND was associated with a shorter mean operative time(mean 222 min for L-RPLND vs.453 min for O-RPLND,p<0.001).There was significantly less blood loss during surgery in the L-RPLND group compared to the O-RPLND group(mean 165 mL for L-RPLND vs.403 mL for O-RPLND,p<0.001).Parameters related to postoperative recovery were significantly better for the L-RPLND group than for the O-RPLND group.There were no differences in the histopathological characteristics between the two groups.No patients in either group exhibited disease recurrence.Conclusion:Patients who underwent L-RPLND had more rapid recovery,and shorter hospital stay compared to those who underwent O-RPLND;complications were comparable between the two groups.L-RPLND is an efficient procedure with the benefits of minimally invasive surgery.

The dynamic behavior of organelles is essential for plant survival under various environmental conditions. Plant organelles, with various functions,migrate along actin filaments and contact other types of organelles, leading to physical interactions at a specific site called the membrane contact site. Recent studies have revealed the importance of physical interactions in maintaining efficient metabolite flow between organelles.In this review, we first summarize peroxisome function under different environmental conditions and growth stages to understand organelle interactions. We then discuss current knowledge regarding the interactions between peroxisome and other organelles, i.e., the oil bodies, chloroplast, and mitochondria from the perspective of metabolic and physiological regulation, with reference to various organelle interactions and techniques for estimating organelle interactions occurring in plant cells.

GRACE-based estimates of groundwater variations over North America from 2002 to 2017

GRACE-based estimates for groundwater storage(GWS)changes in North America substantially depend upon correction of glacialisostatic adjustment(GIA)effects,which are usually removed with GIA models.In this study,GIA effects are eliminated by employing an independent separation approach with the aid of Global Navigation Satellite System(GNSS)vertical velocity data.Our goal is to provide an independent estimate for monthly GWS changes within North America in 1-degree-grids and their trends over the whole GRACE mission lifetime from April 2002 to June 2017.This estimate is derived from the release-6 version of GRACE monthly level-2 data,GNSS data,land surface models for soil moisture and snow wa-ter equivalent,and satellite altimetric lake leveldata.We find a GWS anomaly in form of an increasing trend in Saskatchewan,which affects the Saskatchewan Province and the states of Montana,North Dakota and Minnesota,and 4 GWS anomalies with declining trends in Nevada,California,Arizona and Texas,respec-tively.The monthly changes of these GWS anomalies,except for the one in Nevada,are validated by well level data.We provide results for average monthly GWS changes and the trends for the 5 anomalies but also in separate form for the 13 affected states or provinces.The increasing trends of the Saskatchewan GWS anomaly and the affected 3 states are related to increasing precipitation and can be elucidated by the decreasing drought intensity level.On the contrary,the declining trends in GWS can be explained by weakening precipitation and are mostly supported by the increasing drought intensity level in the other 4 anomalies and the affected states,which are Nevada,California,Arizona,New Mexico,Texas,Oklahoma,Kansas,and Colorado.Ourestimates of monthly GWS changes and their trendscan serveas alternativeand beneficial input for the sustainable management of groundwater resources in North America.

Dear Editor,Spinal and bulbar muscular atrophy(SBMA)or Kennedy’s disease is a rare X-linked,recessive,lower motor neuron disease caused by a CAG repeat expansion within the first exon of the androgen receptor(AR)gene.Instability of the CAG-triplet repeat impacts AR function;therefore,men with SBMA are thought to be at a very low risk of prostate cancer.We previously reported a case of high-risk prostate cancer with SBMA(repeat length 46).

Crossing the Rivers by Touching the Stones:Alternative Approaches in Technical and Vocational Education and Training From the People’s Republic of China and the Republic of Korea

Purpose:This study aims to answer the following questions:(1)Why have attempts to transplant Western vocational education models failed?(2)Is there anything we can learn from the experiences of Eastern Asian countries when developing their own vocational education models?Design/Approach/Methods:This study reviews the history of transplanting Western skill formation schemes into developing countries,an often-failed die-hard practice supported by both bilateral and multilateral donors.Findings:Our findings suggest that developing countries should design their technical and vocational education and training systems based on their unique cultural,sociological,and economic contexts.It offers two alternative pathways based on the experiences of the People’s Republic of China and the Republic of Korea.Originality/Value:These East Asian examples could broaden the perspectives of policymakers in developing countries aspiring to develop functional skill formation schemes.

Characteristics of In-Hospital Patients with Congenital Heart Disease Requiring Rapid Response System Activations: A Japanese Database Study

Objectives:This study aimed to study the characteristics of in-hospital deterioration in patients with congenital heart disease who required rapid response system activation and identify risk factors associated with 1-month mortality.Methods:We retrospectively analysed data from a Japanese rapid response system registry with 35 participating hospitals.We included consecutive patients with congenital heart disease who required rapid response system activation between January 2014 and March 2018.Logistic regression analyses were performed to examine the associations between 1-month mortality and other patient-specific variables.Results:Among 9,607 patients for whom the rapid response system was activated,only 82(0.9%)had congenital heart disease.Only few patients with congenital heart disease were being treated at the cardiology and cardiovascular surgery departments(12.3%and 9.9%,respectively).Moreover,the incidences of rapid-response events after intensive care unit discharge or surgery were low(6.8% and 12.2%,respectively).The most common reason for rapid response system activation was respiratory dysfunction(desaturation:35.4%,tachypnoea:25.6%,and new dyspnoea:19.5%).Rapid response system interventions and intensive care unit transfers were required for 65.9% and 20.7%of patients,respectively.The mortality rate was 1.2% at the end of the rapid response system intervention and 11.0% after 1 month.Moreover,decreased respiratory rate and decreased heart rate at rapid response system activation were associated with increased 1-month mortality.The adjusted odds ratio was 1.10(95% confidence interval 1.02–1.19)and 1.02(95% confidence interval,1.00–1.04) for respiratory rate and heart rate,respectively.Conclusions:Rapid response systems were rarely activated after cardiac surgery and intensive care unit discharge,which were situations with a high risk of sudden deterioration in patients with congenital heart disease.Therefore,encouraging the use of the rapid response system in these departments will enable interventio
