Lacidipine,thiamine pyrophosphate and their combination on the ocular ischemic syndrome induced by bilateral common carotid artery ligation

AIM:To investigate the effect of lacidipine,thiamine pyrophosphate(TPP)and the combination of lacidipine and TPP against oxidative and inflammatory eye damage induced by bilateral common carotid artery ligation in ***:Male albino Wistar rats were categorized as those who underwent sham surgery(SG),right and left common carotid cross-clamping and unclamping procedure(CCU),lacidipine+CCU(LCCU),TPP+CCU(TCCU),and combination of lacidipine and TPP(LTC)+CCU(LTCCU).One hour before anesthesia,the LCCU(n=6)received lacidipine(4 mg/kg,orally)and the TCCU(n=6)received TPP(20 mg/kg,intraperitoneally).The SG(n=6)and CCU(n=6)received the same volume of distilled water from the same *** anesthesia(60 mg/kg ketamine,intraperitoneally),the necks of the rats were opened in the *** was created for 10min by placing clips on the right and left common carotid *** in the SG only underwent subcutaneous *** 10min,the clips were removed and reperfusion was achieved for six ***,the animals were euthanized(120 mg/kg ketamine,intraperitoneally)and the levels of oxidant,antioxidant and proinflammatory cytokines in the eye tissues were *** retinal tissue of the eye was also examined ***:Lacidipine,TPP,and LTC significantly prevent the increase in malondialdehyde,tumor necrosis factoralpha,interleukin-1β(IL-1β),and IL-6 levels,decrease in total glutathione levels,superoxide dismutase and catalase activities and histopathological retinal damage in eye tissue induced by bilateral common carotid artery ligation in *** impact of these drugs on protection is determined to be LTC>lacidipine>***:As a result of the study,it is concluded that LTC may be more effective than lacidipine and TPP alone in treating ocular ischemic syndrome.

The study was sought to enhance the synthesis of thermal stableβ-cyclodextrin glycosyltransferase(β-CGTase)using potato wastewater as a low-cost medium and assess the degree to which it is efficient for industrial production ofβ-cyclodextrin(β-CD)from raw potato *** bacteria producingβ-CGTase was isolated from Saudi Arabia and the promising strain was identified as Bacillus licheniformis using phylogenetic analysis of the 16S rRNA ***-encapsulated cultures exhibited twice-fold ofβ-CGTase production more than free *** electron microscopy(SEM)of polymeric capsules indicated the potential for a longer shelf-life,which promotes the restoration of activity in bacterial cells across semi-continuous fermentation ofβ-CGTase production for 252 *** optimal conditions forβ-CGTase synthesis using potato wastewater medium were at 36 h,pH of 8.0,and 50°C with 0.4%potato starch and 0.6%yeast extract as carbon and nitrogen sources,*** purified enzyme showed a specific activity of 63.90 U/mg with a molecular weight of∼84.6 kDa as determined by SDS-PAGE *** high enzyme activity was observed up to 60°C,and complete stability was achieved at 75°*** levels of activity and stability were shown at pH 8.0,and the pH range from 7.0–10.0,*** enzyme has an appreciable affinity for raw potato starch with a Km of 5.7×10−6 M and a Vmax of 87.71μmoL/mL/min.β-CD production was effective against 25 U/g of raw potato *** outcomes demonstrated its feasibility to develop a fermentation process by integrating the cost-effective production ofβ-CGTase having distinctive properties forβ-CD production with ecofriendly utilization of potato wastewater.

The Relation between Apolipoprotein E4 Genotype and Vascular Dementia

Most studies investigating genetics of dementia have focused on Alzheimer’s disease, but little is known about the genetics of vascular dementia (VD). The aim of this study was to identify the association between Apolipoprotein E4 (Apo E4) genotype and VD in cerebrally infarcted patients. The study was conducted on 100 patients with cerebral infarction: 50 had VD (cases) and 50 didn’t have dementia (controls). Diagnosis of VD was based on Mini-Mental State Examination, Cambridge Cognitive Examination (CAMCOG), the Diagnostic and Statistical Manual of Mental Disorders 4th edition criteria for the diagnosis of VD (DSM-IV), Hachinski Ischemic Score, and computed tomography of the brain (CT brain). Apo E4 allele was assessed through DNA genotyping. The study showed that hypertension (p = 0.027, OR = 4.71), diabetes mellitus (p = 0.003, OR = 6.05) and Apo E4 allele (p = 0.017, OR = 13.39) were the independent risk factors of VD among studied participants. The study concluded that cerebrally infarcted patients with Apo E4 genotype are at high risk of developing VD.

Recent Techniques in Management of Lumbosacral Region Tumours: A Prospective Comparative Study

Background: The lumbosacral plexus consists of the ventral rami from the L2 to S3 nerve roots, with some additions from the L1 and S4 nerve roots as well. The standard treatment for spinal cord compression is urgent surgically decompressive laminectomy over the affected level with resection as much as possible of the exposed tumor to relieve the cord compression. We aim to correlate the impact of preoperative clinical picture, duration of symptoms, and the pathological nature of the lumbo-sacral spine tumors on the outcome of comparative study different modalities of management. Patients and Methods: During the period between 2015 and 2017, thirty cases were studied and surgically managed in Al-Azhar University Hospital, Damietta. Cases were selected for surgery if they were diagnosed as having a symptomatizing lumbosacral lesion. Results: Thirty patients were included in the current study;the commonest lesion in this study was ependymoma eight patients (27%), astrocytoma six patients (20%), and meningioma five patients (17%). Nerve sheath tumor comprised five cases (17%) with the hemangioblastomas three cases (10%), epidermoid one case (3%), and ependymoma of the filum terminale two cases (6%). The mean age of the included patients was 38.96 ± 15.42. The mean duration of lesions was 13.66 ± 10.93. They were 15 male patients (50%), and 15 female patients (50%), with male: female ratio 1:1. The mean duration of symptoms ranged for 1:60 months. The most common presenting manifestations were sensory followed by motor and sphinctric disturbance. Complications occurred in 8 patients (23%), and the most common was postoperative CSF leak. 19 out of 30 cases were surgically totally resected (63%), while 11 cases (37%) were grossly totally resected. Conclusions: Ependymoma and meningioma have more favorable outcome compared to other spinal tumors. Accurate and rapid diagnosis prior to surgery also associated with good outcome.

Intervention Retrofitting and Rehabilitation of Al-Gawhara Palace at the Saladin Citadel, Cairo, Egypt

Al-Gawhara Palace also known as Bijou Palace is located in the south of the Mosque of Muhammad Ali in Saladin Citadel in Cairo (Figure 1 and Figure 2). It was commissioned and constructed by Muhammad Ali Pasha in 1814. Al-Gawhara Palace is one of the most famous historical royal palaces in Egypt and the most important modern architectural heritage that reflects an important historical period of the Egyptian modern history—the period of the rule of the family of “Muhammad Ali Pasha”. The causes of structural deficiency of the palace could be attributed to many reasons and actions, mainly due to the earthquakes and seismic events (in particular the Dahshuor earthquake, in October 1992), degradation of drainage and feeding systems leading to water leakage, lack of awareness of the palace values, deterioration resulting from the use of old government agencies and the conflicts among authorities supervising historical buildings. The present study includes many phases: 1) the significance of the original building from the circumstances and date of construction;2) detailed studies of the architectural features and design of the building as well as construction type, methods and techniques of construction;3) characterization of the construction and building materials;4) stability analysis of the structure under static and the impact of seismic loadings;5) engineering measures for intervention retrofitting of the palace (this pilot study presents the main design studies for intervention retrofitting and the rehabilitation and re-employment of Al-Gawhara Palace, started from the static monitoring, old and modern documenting of the current state of preservation thorough the strengthening project implementation;6) the study also presents the reuse designs to convert the palace to be a national museum, for the purpose of preserving it by proposing the best means to apply the correct principles and criteria for reuse and employment in a manner that preserves its value through the

Management of Post-Craniotomy Persistent CSF Subgaleal Collection in Skull Base Procedures: Local Experience

Background: Post-craniotomy CSF collection is a problem that may cause severe complications as meningitis, wound disruption, prolonged hospitalization, and additional surgeries. Objective: To evaluate our cases with resistant post-operative subgaleal CSF collection, trying to identify causes and optimal management. Methods: Retrospective review of elective skull base cases during the period of January 2104 to January 2019 identification of cases with post-operative CSF subgaleal collection, which either managed conservatively or needed a second surgery. Results: 219 patients, 30 of them suffered subgaleal CSF collection, 22 patients improved with non-operative measures, eight patients needed second surgery with pericranial graft augmentation, and obliteration of subgaleal space resulted in resolution of CSF leak with no morbidities. Conclusion: Meticulous tensionless dural closure, obliteration of subgaleal space, tethering of dural grafts to bone edges are useful techniques in preventing post-operative CSF leak.

Smart Inverter Functionality Testing for Battery Energy Storage Systems

Variable distributed energy resources (DERs) such as photovoltaic (PV) systems and wind power systems require additional power resources to control the balance between supply and demand. Battery energy storage systems (BESSs) are one such possible resource for providing grid stability. It has been proposed that decentralized BESSs could help support microgrids (MGs) with intelligent control when advanced functionalities are implemented with variable DERs. One key challenge is developing and testing smart inverter controls for DERs. This paper presents a standardized method to test the interoperability and functionality of BESSs. First, a survey of grid-support standards prevalent in several countries was conducted. Then, the following four interoperability functions defined in IEC TR 61850-90-7 were tested: the specified active power from storage test (INV4), the var-priority Volt/VAR test (VV) and the specified power factor test (INV3) and frequency-watt control (FW). This study then out-lines the remaining technical issues related to basic BESS smart inverter test protocols.

针对传统建筑物提取方法难以有效描述遥感图像细节特征,导致复杂场景下道路、树木及建筑物之间分割边界不清晰等问题,提出了一种基于多尺度特征融合空洞卷积ResNet(Multiscale-feature Fusion Dilated Convolution ResNet,MFDC-ResNet)模型。首先,为了获取遥感图像建筑物更大范围的特征信息,在深度残差网络中引入空洞卷积增大特征提取的感受野,以捕捉更丰富的多尺度细节特征;其次,为了增强空洞卷积中心点对图像局部区域特征的表达能力,利用3×3卷积核提取遥感图像的中心点区域特征,引入更多的中心点空间先验信息;最后,利用空间金字塔池化模型对不同尺度空洞卷积特征进行融合,获取不同尺度的遥感图像建筑物的上下文信息。在WHU遥感图像数据集上的实验表明,平均交并比mIoU达到0.820,召回率Recall达到0.882。提出算法不仅提高了分割精度,而且有效克服了道路、树木等因素的干扰,得到了较清晰的建筑物边界。

Management of Left Ventricular Aneurysm: A Study from Iraq

Background: The most appropriate surgical approach for post-myocardial infarction left ventricular aneurysm (LVA) is controversial. This study aims to display the results of surgical treatment of LVA in a major Iraqi cardiac surgical center. Methods: The surgical management of LVAs over the period 2001 to 2011 was retrospectively reviewed. The presenting signs and symptoms, results of investigations, operative findings, and outcomes of patients were determined. Results: Twenty-seven true LVAs associated with 4 ventricular septal defects (VSDs) were treated surgically. During the same period, 1136 coronary artery bypass graft (CABG) operations were done, thus LVA represented 2.4%. Males constituted the majority (74.1%). The mean age was 54.6 years old. The typical ECG changes were seen in 42.1%. Apical and antero-apical locations predominated. The majority of patients (84.2%) had subnormal values of ejection fraction (EF). Most patients had multi-vessel coronary artery disease (CAD). The most frequent was the left anterior descending artery (LAD). All patients had CABG except 3. Linear repair and Dor technique were used equally. The commonest postoperative complication was bleeding (38.4%). The overall hospital mortality was 18.5%. Conclusion: Concomitant CABG improves early postoperative course and must be added when significant lesions in coronary arteries particularly the LAD are present.
