Aim: to clarify the immuno-active LH (i-LH) and bioactive LH (b-LH) responses and qualitative changes in the cir-culating LH to testosterone undecanoate (tU) injection. Methods: Eight men with Klinefelter's syndrome were re-cruited for the study. they received crossover injections of tU at doses of 500 and 1000 mg. Serum i-LH and b-LHlevels before and at various time intervals after tU injection were measured and the serum i-LH, b-LH, b-LH/i-LH(B/I) and testosterone/sex hormone-binding globulin (t/SHBG) ratio in LH-responders and LH non-responders werecompared. Results: A parallel suppression of serum i-LH and b-LH was consistent with their overall high correlationbetween each other ( r = 0.84, P were decreased by tU injection at both doseswithout dose-response effects. LH-responders had lower baseline serum i-LH and b-LH, and higher E_2 levels and t/SHBG ratio. there was a quantitative change in serum LH as induced by tU without qualitative change within LH-re-sponders os LH-non-responders. Conclusion: A high loading dose (1000 mg) of tU is important for the initial sup-pression of LH. with the lower dose (500 mg), repeated injections will be required to attain such LH suppression forthe purpose of fertility regulation. the lower baseline serum i-LH level may be an intrinsic characteristic of LH-respon-ders.

Aim： to study the relationship between circulating androgens （total testosterone [tt], free testosterone [It] and dihydrotestosterone [DHt]） and high-density lipoprotein cholesterol （HDL-C） in men with and without cardiovascular disease （CVD）. Methods： Cross-sectional analyses included 1 661 baseline samples from the Massachusetts Male Aging Study （MMAS）, a population-based cohort of men ages 40-70 years. Serum hormones were measured by radioimmunoassay and HDL-C was determined following precipitation of the lower density lipoproteins. CVD was determined by self-report. Analyses were performed using multiple linear regression. Results： tt and HDL-C were positively correlated in the entire sample （r = 0.11, P = 0.0001）. After adjusting for confounders, we found this relationship was mostly limited to the 209 men with CVD. Among men with CVD, tt （P = 0.0004）, it （P = 0.0172） and DHt （P = 0.0128） were all positively correlated with HDL-C, whereas in men without CVD only tt correlated with HDL-C （P = 0.0099）. Conclusion： Our results suggest that if androgens contribute to CVD in middle-aged men, the effect is not related to a suppressive effect of endogenous t on HDL-C. （Asian JAndrol 2008 Mar; 10： 193-200）

Measurement of intratesticular sex steroid concentrations in men informs both the development of male hormonal contraceptives and the understanding of male infertility. Given the challenges of using invasive techniques to measure testicular hormone physiology, our group has used a minimally-invasive fine-needle aspiration technique to measure intratesticular hormones in normal healthy men. Herein, we present a post-hoc analysis of the safety and efficacy of testicular fine-needle aspiration （FNA） completed as part of six clinical trials. From 2001 through 2011, a total of 404 procedures were conducted among 163 research volunteers, 85.9% of which were successful in obtaining sufficient fluid for the measurement of intratesticular steroid concentrations. Pain was the most common side effect, with 36.8% of procedures associated with moderate procedural pain and 4.7% with severe procedural pain. Postprocedural pain was uncommon and abated within a few days. Mild local bruising occurred with 14.9% of procedures. two serious adverse events （0.5%） required surgical intervention. the risk of an adverse event was not associated with age, body mass index, testicular size, or the volume of fluid aspirated. testicular FNA to obtain fluid for measurement of intratesticular steroid concentrations frequently causes mild to moderate procedural pain, but serious adverse events occur rarely. testicular FNA has been instrumental for defining human intratesticular hormone physiology and is a minimally-invasive, safe, effective method for obtaining fluid for research on testicular physiology and pathology.

Study on Differences in Spermatogenic Suppression between Azoospermic and Oligozoospermic Responders treated with Levonorgestrel Implant Plus testosterone Undecanoate Injectable in Chinese Men

Objective to investigate possible causes resulting in the differences in the spermatogenesis suppression on individual treated with levonorgestrel (LNG) implants and testosterone undecanoate (tU) injectableMethods totally 21 Chinese male volunteers were given treatment with LNG implants (four rods, 75 mg/rod) and intramuscular injection of tU (500 mg,bimonthly for 3 times). According to the effects of treatment, they were divided into two groups, namely, azoospermia group (group A) and oligozoospermia group (group O). then seminal FSH, LH, t and estradiol (E2) were determined by immunoenzymetric assay, while seminal and serum dihydrotachysterol (DHt) and serum sex hormone binding globulin (SHBG) were by radioimmunoassay, and seminal transferrin (tf) by scatter turbidimetry assay.Results Seminal FSH, LH and serum DHt, SHBG, FtI (t/SHBG ×100) levels were significantly lower in group A than in group O, while higher seminal concentrations ofE2 were observed in azoospermia group.Conclusion the differences in the spermatogenic suppression in Chinese men might be attributed to different rate of peripheral androgen metabolism, variations in serum SHBG levels, 5á-reductase activity and individual aromatase activity during LNG plus tU administration. In addition, seminal sex hormones might be more sensitive indexes to assess the extent of feedback inhibition on hypothalamus-pituitary-testis with exogenous testosterone plus progestogen in the efficacy hormone male contraceptive trials.

目的 :观察国产两根型皮埋剂 Sino-implant及十一酸睾酮 (tU)针剂联合应用对中国男性垂体促性腺激素和睾酮水平的影响。方法 :共征集自愿对象 1 6名 ,前臂埋植 Sino-im-plant,每根含左旋 1 8-甲基炔诺酮 75mg,并于 3周后每月肌注针剂十一酸睾酮 (tU) ,(2 50 mg) ,共 3次。结果 :放置皮埋剂后血清中 FSH、L H水平有所下降。注射第 1针 tU后 ,FSH、L H水平进一步下降 ,于注射后 1周达到谷值 ,之后 FSH、L H水平又有所上升 ;注射第 2、3针 tU后 ,FSH、L H水平变化情况与第 1针时相似。此外 ,皮埋后对象血清睾酮水平也有所下降 ,注射第 1针 tU后 ,则睾酮水平升高 ,并于注射后 1周达峰值 ,之后睾酮水平又有所下降 ;注射第 2、3针 tU后血清睾酮水平变化情况与第 1针时相似。结论 :皮埋剂Sino-implant和 tU配伍应用可对垂体促性腺激素分泌产生协同抑制作用 ,从而使内源性睾酮水平下降 ;但注射 tU可补充睾酮 ,使之维持正常水平。故国产皮埋剂配伍

Aim: to study the effect of the title drugs on the serum FSH, LH andtestosterone (t) levels in Chinese men in a contraceptive trial. Methods:Sixteen male volunteers were administered Chinese-made implant (tworods, each containing 75 mg of levonorgestrel, LNG) plus testosterone un-decanoate (tU) (250 mg, i.m. monthly for3 months). FSH, LH and tlevels were determined with RIA. Results: the serum FSH and LH lev-els declined after the insertion of implants. they declined further after the1st tU injection and one week later, both FSH and LH levels reached thelowest levels, after which, they rose slightly. the profiles of serum LH andFSH levels after the 2nd and 3rd tU injection were similar to that followingthe 1st tU injection. After insertion of the implants, the serum t levels alsodeclined.
