A combination therapy of ethanol injection and radiofrequency ablation under general anesthesia for the treatment of hepatocellular carcinoma

AIM: to summarize the effects of laparoscopic ethanol injection and radiofrequency ablation (L-EI-RFA), thora- coscopic (t-EI-RFA) and open-surgery assisted EI-RFA (O-EI-RFA) under general anesthesia for the treatment of hepatocellular carcinoma (HCC). MEtHODS: time-lag performance of RFA after ethanol injection (time-lag PEI-RFA) was performed in all cases. the volume of coagulated necrosis and the applied en- ergy for total and per unit volume coagulated necrosis were examined in the groups treated under general (group G) or local anesthesia (group L). RESULtS: the results showed that the total applied energy and the applied energy per unit volume of whole and marginal, coagulated necrosis were significantly larger in group G than those in the group L, resulting in a larger volume of coagulated necrosis in the group G. the rate of local tumor recurrence within one year was extremely low in group G. CONCLUSION: these results suggest that EI-RFA, un- der general anesthesia, may be effective for the treat- ment of HCC because a larger quantity of ethanol and energy could be applied during treatment under pain- free condition for the patients.

＜正＞Dendritic trees influence synaptic integration and neuronal excitability,yet appear to develop in rather arbitrary *** electron microscopy and serial reconstructions,we analyzed the dendritic trees of four morphologically distinct neocortical interneuron subtypes to reveal two underlying organizational principles common to ***,cross-sectional area at any given point within a dendrite is proportional to the summed length of all distal segments spanning beyond ***,the total cross-sectional area is perfectly conserved at bifurcation ***,dendritic cross-sections become more elliptic at proximal locations,resulting in a conservation of the surface to volume ratio throughout the dendritic ***,computer simulations revealed that these conserved morphological features limit distance dependent filtering of somatic EPSPs and facilitate distribution of somatic depolarization into all dendritic *** these features were shared by all interneurons studied,they may represent common organizational principles underlying the otherwise diverse morphology of dendritic trees.

Purpose: the purposes of this study were to estimate accumulated kV X-ray imaging dose throughout dynamic tumor tracking (Dtt) irradiation by Vero 4DRt system and to address an analytical skin dose formula for well-balanced kV X-ray imaging conditions between skin dose and image noise. Method: First, skin dose was measured using kV X-ray tube, chamber, and water-equivalent phantoms. Next, imaging dose for six patients in Dtt treatment was computed using log files. Subsequently, scattered dose ratio was calculated by amount of ionization in front of flat panel detector (FPD) for fields with size of maximum and the chamber for 0 - 200 mm-thickness phantoms and tube voltage of 60, 80, 100, 120 kV, respectively. Furthermore, image noise was computed from FPD images. Results: the skin dose was greater by a factor of 1.4 - 1.6 than those in Synergy XVI system. the image noise in FPD, ?was expressed as N = 0.045×(1/QFPDen)0.479, where QFPDen denotes amount of ionization in front of FPD. then, skin dose, D (N, t, v) was formulated as (0.045/N)(1/0.479)/QFPDen/mAs (t, v) ×D/mAs (v), where QFPDen/mAs (t, v) and D/mAs (v) denote amount of ionization in front of FPD and skin dose per mAs, respectively. Using the formulae, it has been demonstrated that skin dose with 120 kV has become lower than any other tube voltage in this study. Conclusion: Using skin doses for the phantom, the skin dose throughout Dtt irradiation was estimated as 0.50 Gy. Furthermore, skin dose by kV X-ray imaging was described as a function of image noise, phantom thickness, and tube voltage, suggesting image noise may be reduced with higher X-ray tube voltage in this phantom study.

the extract from culture of effective microorganisms (ECEM) is a secondary metabolite produced in symbiotic culture of a mixture of yeasts and lactic bacteria followed by photosynthetic bacteria. Metabolomics studies have demonstrated that ECEM contained several active ingredients associated with anti-inflammation through interactions with nuclear factor-κB. Functional genomics using DNA microarrays revealed that ECEM had distinctive anti-inflammatory and immunostimulatory actions in RAW264.7 cells. ECEM inhibited the LPS-upregulated expression of one group of pro-inflammatory genes (iNos, Il6 and Cox2) but not another group genes (tnf, Ccl7 and Dusp2);it also upregulated the expression of Cd1d1, which participates in antigen presentation on lipids, resulting in activation of natural killer t (NKt) cells related to infection defense and cancer. Bacterial glycosylated lipid and other related complex lipids in ECEM are thought to trigger this upregulation of Cd1d1 expression. these results suggest that ECEM functions in the immune response in both inflammation and antigen presentation and that it contains components important for these functions. these novel effects of ECEM could be invaluable for developing functional drinks or health beverages.

A heavy ion beam probe （HIBP） using a 3-MV tandem accelerator has been installed on large helical device （LHD）. Electrostatic potential in core plasma can be measured under the toroidal magnetic field strength of up to 3 t. By using the HIBP, the transition of potential profiles from electron-root to ion^root is observed in core plasmas during ramp-up of the electron density. Potential fluctuations are also measured electron cyclotron current drive （ECCD）. two kind of characteristic fluctuations are observed. One is a reversed-shear-induced Alfv^n eigenmode （RSAE）, whose frequency varies during the evolution of the rotational transform profile, and the other is with a constant geodeisc acoustic mode （GAM） frequency.

Recent progress in the physics and engineering design study for themodification programme of Jt-60 is presented. In order to achieve a steady state high-βplasmaoperation, which is the dominant issue of this programme, physics design for the MHD control and thestability analysis is investigated. Engineering design and the R & D for the superconducting coils,irradiation shield are performed well towards the mission of programme.

新疆冲乎尔地区的蓝晶石-夕线石型递增变质带由绿泥石-黑云母带、黑云母-石榴石带、石榴石-十字石带、十字石-蓝晶石带和夕线石带组成;递增变质作用峰期温度、压力分别为t=520～640℃,p=(4.5～5.8)×108Pa;十字石-蓝晶石带和夕线石带变质岩中含有丰富的独居石,而且th-U-Pb含量比较高,适合电子探针独居石CHIME法定年,独居石CHIME法确定的变质时代为268±10～261±20Ma,与阿勒泰地区发育的红柱石-夕线石型(低压型)变质带形成时代(262±10～264±22Ma)一致,说明阿尔泰造山带蓝晶石-夕线石型和红柱石-夕线石型递增变质带是同一次大规模的构造热事件的产物,时代为二叠纪中期。
