Postoperative Risk of Hepatic Decompensation after Orthopedic Surgery in Patients with Cirrhosis

Background and Aims:Previous studies have shown increased hepatic decompensation in patients with cirrhosis undergoing ***,there are little data available in cirrhotics undergoing orthopedic surgery compared to cirrhotics who did not undergo *** aim of this study was to examine the demographics,comorbid conditions,and clinical factors associated with hepatic decompensation within 90 days in cirrhotics who underwent orthopedic ***:this is a retrospective matched cohort *** criteria were cirrhosis diagnosis,age > 18 years,≥ 6 months continuous health plan membership,and a procedure code for orthopedic *** to five cirrhotic controls without orthopedic surgery were matched on age,gender,and cirrhosis diagnosis *** abstraction was performed for demographics,socioeconomics,clinical,and decompensation *** review was performed for *** analysis estimated relative risk of ***:Eight hundred fifty-three orthopedic surgery cases in cirrhotics were matched with 4,263 cirrhotic *** the cases and matched controls,the mean age was 60.5 years,and 52.2% were *** 90 days after surgery,cases had more decompensation compared to matched controls (12.8% vs 4.9%).Using multivariable analysis,orthopedic surgery,a 0.5 g/dL decrease in serum albumin,and a 1-unit increase in Charlson Comorbidity Index were associated with a significantincrease in decompensation within 90 days of ***,chronic obstructive pulmonary disease,and chronic kidney disease were seen with increased frequency in cases *** ***:Cirrhotics who underwent orthopedic surgery had a significant increase in hepatic decompensation within 90 days of surgery compared to matched *** incremental decrease in serum albumin and an incremental increase in the Charlson Comorbidity Index were significantly associated with hepatic decompensation after surgery.

Estimation of food composition of Hodotermes mossambicus （Isoptera： Hodotermitidae） based on observations and stable carbon isotope ratios

the diet of the harvester termite Hodotermes mossambicus was investigated at two sites with distinct dietary components： C4 grasses （δ13 C isotope values, -13.8%0 to -14.0%0） and C3 plants （δ13C isotope values, -25.6%0 to -27.1%0）. By comparing observations of food items carried into the colony by the termites and carbon isotope ratios of whole termites （that determined assimilated carbon）, the relative proportion of the C3 and C4 plant food components of the termite diet was estimated. there was agreement between the observational data and stable carbon isotopic data, with grass representing approximately 93% of the diet ofH. mossambicus at two study sites （urban and rural） on the South African highveld. However, when correcting for mass of food items, that is, C3 and C4, carried by termites, the proportion of grass （C4） in the diet may be underestimated.

Groundwater Recharge and Mixing in Arid and Semiarid Regions:Heihe River Basin,Northwest China

A sound understanding of groundwater recharged from various sources occurring at different time scales is crucial for water management in arid and semi-arid river basins. Groundwater recharge sources and their geochemical evolution are investigated for the Heihe River Basin （HRB） in northwest China on the basis of a comprehensive compilation of geochemical and isotopic data. Geochemical mass- balance modeling indicates that mountain-block recharge accounts for a small fraction （generally less than 5%） of the shallow and deep groundwater sustaining the oasis, whereas infiltration of rivers and irrigation water contribute most of the groundwater recharge. Dedolomitization is the primary process responsible for the changes in groundwater chemical and carbon isotope compositions from the piedmont to the groundwater discharge zone, where the dedolomitization is very likely enhanced by modern agricultural activities affecting the shallow groundwater quality. Analysis of radioactive isotopes suggests that these primary recharge sources occur at two different time scales. Radiocarbon-derived groundwater age profiles indicate a recharge rate of approximately 12 mm/year, which probably occurred during 2000-7000 years B.P., corresponding to the mid-Holocene humid period. the recharge of young groundwater on the tritium-dated time scale is much higher, about 360 mm/year in the oasis region. Infiltration from irrigation canals and irrigation return flow are the primary contributors to the increased young groundwater recharge. this study suggests that groundwater chemistry in the HRB has been influenced by the complex interaction between natural and human-induced geochemical processes and that anthropogenic effects have played a more significant role in terms of both groundwater quantity and quality.

two basic motivations for an upgraded JLab facility are the needs： to determine the essential nature of light-quark confinement and dynamical chiral symmetry breaking （DCSB）; and to understand nucleon structure and spectroscopy in terms of QCD＇s elementary degrees of freedom. During the next ten years a programme of experiment and theory will be conducted that can address these questions. We present a Dyson- Schwinger equation perspective on this effort with numerous illustrations, amongst them： an interpretation of string^breaking; a symmetry-preserving truncation for mesons; the nucleon＇s strangeness σ-term; and the neutron＇s charge distribution.

Single-lens dynamic z-scanning for simultaneous in situ position detection and laser processing focus control

Existing auto-focusing methods in laser processing typically include two independent modules,one for surface detection and another for z-axis *** latter is mostly implemented by mechanical z stage motion,which is up to three orders of magnitude slower than the lateral processing *** alleviate this processing bottleneck,we developed a single-lens approach,using only one high-speed z-scanning optical element,to accomplish both in situ surface detection and focus control quasi-simultaneously in a dual-beam *** probing beam scans the surface along the z-axis continuously,and its reflection is detected by a set of confocal *** on the temporal response of the detected signal,we have developed and experimentally demonstrated a dynamic surface detection method at 140-350 kHz,with a controlled detection range,high repeatability,and minimum linearity error of 1.10%.Sequentially,by synchronizing at a corresponding oscillation phase of the z-scanning lens,the fabrication beam is directed to the probed z position for precise focus ***,our approach provides instantaneous surface tracking by collecting position information and executing focal control both at 140-350 kHz,which significantly accelerates the axial alignment process and offers great potential for enhancing the speed of advanced manufacturing processes in three-dimensional space.

this paper demonstrates the application of a design tool called BiotRIZ. Its developers claim that it can be used to access biological strategies for solving engineering problems. Our aim is to design a roof for hot climates that gets free cooling through radiant coupling with the sky. the insulation in a standard roof stops the sun and convection from warming the thermal mass. But it also restricts the mass＇s longwave view of the cool sky. Different solutions to this conflict are offered by BiotRIZ. the chosen solution is to replace the standard insulation component with an open cell honeycomb. the vertical cells would allow longwave radiation to pass, while arresting convection. the solutions offered by BiotRIZ＇s technological counterpart include no such changes in structure. It is estimated that the thermal mass in the biomimetic roof would remain on average 4.5℃ cooler than in a standard roof over a year in Riyadh, Saudi Arabia.
