Objectives: The aim of the study was to compare the efficacy and safety of GnRH-agonist to the human chorionic gonadotrophin (HCG) trigger in cases of simple ovarian stimulation. Study design: Randomized controlled trial was conducted on 291 women complaining of unexplained infertility visiting Elshatby Maternity University Hospital from February to December 2019. Trial registration unique ID is PACTR202001787868341 (https://***/). Age included from 20 - 43 years. All patients were stimulated by the sequential stimulation protocol using letrozole then FSH injection, when the criteria of ovulation trigger were reached; cases were randomized into two groups using closed envelopes method.

Background: Viral hepatitis in pregnant women can be caused by multiple types of hepatitis viruses and can cause a wide range of symptoms and complications, ranging from asymptomatic infection to maternal and fetal deaths. Objective: The aim of this study was to evaluate the sero-prevalence and the associated factors of viral hepatitis B (HBV), viral hepatitis C (HCV) and viral hepatitis E (HEV) among the Egyptian pregnant females. Methods: This cross-sectional study was conducted in the period from January to July 2017 in two selected antenatal clinics;El-shat by maternity university hospital at Alexandria city and El-Behera health insurance antenatal clinic at Damanhur city on 261 apparently healthy gravid women. Eligible women were evaluated by a questionnaire about socio-demographic data and viral serological tests were done at El-shat by maternity University Hospital Laboratory. Results: The overall HBs-Ag sero-prevalence in the pregnant women included in this study was 7 cases (2.7%) and HCV-RNA PCR was 4 cases (1.5%). Of the total study participants, none of them (0%) were anti-HEV IgM positive and 48 cases (18.4%) were anti-HEV IgG positive. According to our results, neither age of the woman, place of residence, past family history of hepatitis virus infection nor having a pet animal had any strong association to be considered as an associated factor for acquiring HEV infection. Conclusion: Although it seems that there are many improvements in the prevalence of different hepatitis viruses in the country, hepatitis viral infections are still high enough to continue their preventive programs and health education, to protect the mothers and their fetuses from the hazardous effects of these serious infections.

The present work was designed to examine the effect of the presence or absence of cumulus cells on the efficiency of vitrification of immature cattle oocytes. In our experiment, we had two groups: group 1, immature cattle oocytes with cumulus cells and group 2, immature cattle oocytes without cumulus cells. The two groups underwent vitrification using 20% ethylene glycol and 20% DMSO, and then thawed, and in vitro matured in TCM-199 medium and examined after 22 hours for assessment of nuclear maturation. Higher survival rate (p < 0.05) after thawing was observed in group 1 (84.6%) than group 2 (57.8%). After in-vitro maturation, the rate of MII oocytes was significantly higher (p < 0.05) in group 1 (74.4%) than group 2 (47.7%). In conclusion, the cumulus cells are very important in increasing the survivability and developmental rate of vitrified-thawed immature cattle oocytes.

Tumor infiltrating lymphocytes (TILs) are used in evaluating the prognosis and determining treatment of different types of cancer with variable degrees of success. The usage of checkpoint inhibitor immunotherapy as a treatment variety for cancer and Adoptive cell therapy is associated with many complications, severe side effects and high expenses. Recently, in a limited number of metastatic GIT and breast cancers, the identification of T-cell specific against so-called tumor neo-antigens and Adoptive transfer of those lymphocytes resulted in some improvement. In 2020, Detection of a T cell receptor (TCR) in a T cell clone that recognized and killed most human cancer cell lines in vitro via the monomorphic MHC class I-related protein MR1, offers an opportunity for pan-cancer therapy Twenty three years earlier, Moist Heat was used successfully to activate a whole different and new immune response that was able to detect genetic mutation in the affected cancer cells and cured many cases of squamous and basal cell carcinomas. In this commentary review, we aimed to revise the literature for updates of TILs usage in cancer prognosis and treatment.

Retrocaval ureter is a very rare congenital malformation. We report a 10 years’ experience in the diagnosis and treatment of retrocaval ureter, a case series of 3 cases in two different countries of the Middle East. This is a retrospective study that included 3 cases of retrocaval ureters in Egypt and Saudi Arabia. Standard open ureteroureteric anastomosis was performed through a flank incision for each case. Patients’ symptoms were re-evaluated after two to four months. Complete recovery from symptoms occurred, and hydroureter and hydronephrosis regressed in all cases. Early diagnosis and treatment are the keys to prevent hydronephrosis and deterioration of renal functions.

Endometrial Preparation for Frozen Embryo Transfer by Supplemented Natural Cycle versus Hormone Replacement Cycle in Regularly Cycling Women

BACKGROUND: The first successful pregnancy from a frozen-thawed embryo transfer (FTET) was reported in 1983 and after more than three decades, significant changes have been introduced into clinical practice. OBJECTIVE: To compare endometrial preparation for frozen thawed embryo transfer (FTET) in supplemented natural cycle (NC) by hCG (human chorionic gonadotropin) versus HRT (hormonal replacement therapy) by estrogen and progesterone in regularly cycling women. METHODS: A prospective trial was conducted on 40 patients that were divided into two treatment groups. The first group underwent endometrial preparation by supplemented natural cycle by HCG, and the second group underwent endometrial preparation by HRT (estrogen and progesterone). RESULTS: No significant difference was revealed between the two groups regarding ongoing clinical pregnancy rate. CONCLUSION: This study concluded that there is no significant difference in clinical pregnancy rate between the two methods of endometrial preparation for frozen thawed embryo transfer;supplemented natural cycle by hCG or HRT by estrogen and progesterone, in regularly cycling women.

Cervical pregnancy is a rare clinical entity that accounts for less than 1% of all ectopic pregnancies. It results from implantation of the blastocyst in the cervical canal below the level of the internal os. Although non-tubal ectopic pregnancies account for only 5% of ectopics, they contribute to a significant morbidity. The cornerstone in the management of cervical ectopic is early diagnosis by high index of suspension and a qualified sonographer. Management options for cervical ectopic pregnancies range from conservative drug treatment to radical hysterectomy. Over the last few years, the mortality and morbidity rates of ectopic pregnancies have been reduced. This is mainly due to the early recognition of the condition and the wide availability of minimally invasive surgical procedures. We present a case of a 33-year-old woman that was 16 weeks pregnant. She presented initially with recurrent vaginal bleeding followed by minimal lower abdominal pain. Her early US scans were misleading. Several weeks later, a follow up MRI scan suggested cervical ectopic. She was managed surgically with uterine preservation.

Preparation and Properties of PBXs Based on FOX-7 in Controlled Fragmentation Warhead Application

Polymer bonded explosive(PBX)formulations were successfully prepared in the laboratory scale containing 1,1-diamino-2,2-dinitroethene(FOX-7)and hexogen(RDX)as brisant high explosives and different binder types of polyurethane(PU)based on glycidyl azide polymer(GAP) and hydroxyl-terminated polybutadiene(HTPB) as an energetic and inert polymeric binder *** technique was used for the preparation of different PBX formulations based on FOX-7/RDX and PU(GAP/HTPB)with 14% *** sensitivity to different initial impulses and performance characteristics of the explosive and lethal zone of the tested controlled fragmentation warhead by the fragmentation warhead assessment test(arena test)were studied,in which the arena test was carried out with a controlled fragmentation warhead made from Ck45 steel,with dimensions(100 mm length,30 mm outer diameter and 3 mm thickness).Results show that PBXGF4 has lower sensitivity to impact and heat than those of PBXGR4 by 188.4% and 3.2% *** friction sensitivity is the same as that of *** has better performance,in which detonation velocity increases by 2.1% and brisance increases by 0.5% when compared with those of *** was concluded that PBXGF4 which based on FOX-7 bonded with PU/GAP matrix has good characteristics as PBX,specially in the sensitivity to impact and can be applied for replacing PBXs based on RDX in the advanced PBXs for low sensitive fragmentation warheads.

Effects of Plasticizers,Antioxidants and Burning Rate Modifiers on Aging Performance of the HTPB/HMDI Composite Solid Propellant

The effects of plasticizers,antioxidants and burning rate modifiers on the aging performance of the composite solid propellant based on hydroxyl-terminated polybutadiene(HTPB)/hexamethylene diisocyanate(HMDI)were explored by apply-ing an accelerated aging program for 90 day at 70 ℃. The HTPB propellant matrix with the diisooctyl sebacate(DOS)as plasti-cizers and diisooctyl azelate(DOZ), antioxidants as N,N ′-Diphenyl-p-phenylenediamine(AO) and 2,2′-methylenebis(4-methyl-6-tert-butylphenol)(cyanox 2246)and burning rate modifiers as barium ferrite(BF),copper chromites(CC)and fer-ric oxide(FO)were varied. Results show that sample(S1)which based on DOS decreases the stress value and increases the strain value which considered to be an excellent start for aging program. Sample(S3)containing AO presents the higher resis-tance to oxidation showing the better performance that reflects on increasing the shelf life of the composite solid propellant mo-tor. Sample(S5)which based on BF enhances the ballistic performance among over the other tested two samples. The accelerat-ed aging program allowed us to estimate the motor in-service lifetime.
