Controlling the immune response with only clinically approved immunosuppressant drugs is difficult in renal heterotra ns plantation from pigs to nonhuman ***,to the best of our knowledge,no reports exist on the use of fetal pigs as kidney *** study aimed to compare the degree of transplant rejection between neonatal and fetal kidneys,with genetically unmodified pigs as donors and cynomolgus monkeys as *** left kidneys of the recipient monkeys were removed,followed by transplantation of neonatal as well as fetal pig kidneys,which had undergone vascular anastomosis at the same site,into the *** was performed with only US Food and Drug Administration-approved *** fetal kidneys were transplanted into the omentum and paraaortic regions of cynomolgus ***,the engraftment and development of the transplanted tissues were pathologically examined by sampling over time(twice in each experiment).An acute rejection was observed after a few weeks in neonatal renal grafts with vascular ***,fetal pig kidneys were spared from rejection despite the administration of the same immunosuppressive protocol to the monkeys and the recipient blood vessels flowing into the fetal *** immunogenicity of fetal kidneys in pig-monkey renal heterotransplantation was lower than that of neonatal kidneys.

Severe acute hepatitis of unknown etiology is difficult to treat and often progresses to subacute fulminant hepatitis or late-onset hepatic failure. A 45-year-old wellnourished, healthy man had progressive fatigue and his liver function tests showed severe liver dysfunction. The etiology of sever acute cholestatic hepatitis was unknown. The liver function tests normalized gradually, which excluded high persistent total bilirubin after starting on predonine. A liver biopsy showed chronic active hepatitis with mild f ibrosis (A2, F1). Oral Inchinko-to, a Chinese herbal medicine, at 7.5 g daily was prescribed. The treatment was effective with no adverse effects. We present a successfully treated case and discuss hepatoprotective and choleretic effects of Inchinko-to.

Time Series Forecasting with Multiple Deep Learners: Selection from a Bayesian Network

Considering the recent developments in deep learning, it has become increasingly important to verify what methods are valid for the prediction of multivariate time-series data. In this study, we propose a novel method of time-series prediction employing multiple deep learners combined with a Bayesian network where training data is divided into clusters using K-means clustering. We decided how many clusters are the best for K-means with the Bayesian information criteria. Depending on each cluster, the multiple deep learners are trained. We used three types of deep learners: deep neural network (DNN), recurrent neural network (RNN), and long short-term memory (LSTM). A naive Bayes classifier is used to determine which deep learner is in charge of predicting a particular time-series. Our proposed method will be applied to a set of financial time-series data, the Nikkei Average Stock price, to assess the accuracy of the predictions made. Compared with the conventional method of employing a single deep learner to acquire all the data, it is demonstrated by our proposed method that F-value and accuracy are improved.

AIM:To evaluate the efficacy and safety of adjunctive mosapride citrate for bowel preparation before colonoscopy. METHODS:We conducted a randomized,double-blind, placebo-controlled study with mosapride in addition to polyethylene glycol(PEG)-electrolyte *** 250 patients undergoing colonoscopy,124 were randomized to receive 2 L PEG plus 15 mg of mosapride citrate (mosapride group),and 126 received 2 L PEG plus placebo(placebo group).Patients completed a questionnaire reporting the acceptability and tolerability of the bowel preparation *** efficacy of bowel preparation was assessed by colonoscopists using a 5-point scale based on Aronchick's *** primary end point was optimal bowel preparation rates(scores of excellent/good/fair vs poor/inadequate). RESULTS:A total of 249 patients were included in the *** the mosapride group,optimal bowel preparation rates were significantly higher in the left colon compared with the placebo group(78.2%vs 65.6%,P<0.05),but not in the right colon(76.5%vs 66.4%,P=0.08).After excluding patients with severe constipation,there was a significant difference in bowel preparation in both the left and right colon(82.4%vs 66.7%,80.8%vs 67.5%,P<0.05,P<0.01).The incidence of adverse events was similar in both groups. Among the subgroup who had previous colonoscopy experience,a significantly higher number of patients in the mosapride group felt that the current preparation was easier compared with patients in the placebo group(34/72 patients vs 24/74 patients,P<0.05). CONCLUSION:Mosapride citrate may be an effective and safe adjunct to PEG-electrolyte solution that leads to improved quality of bowel preparation,especially in patients without severe constipation.

Multi-Factorial Functional Analysis of the Patients with Reconstruction for Oral Carcinoma

This paper is to evaluate postoperative function which has undergone reconstruction with vascularized or pedicled flaps for the treatment of oral carcinomas. The subjects consisted of 22 cases of 17 male and 5 female patients, and their mean age was 55 years. The sites of reconstruction were the tongue in 12 cases, mandible in 10 cases. The reconstruction was performed using deltopectoral (DP) flaps in 6 cases, pectoralis major musculo cutaneous (PMMC) flaps in 3 cases, DP flap + PMMC flap in one case, forearm (FA) flaps in 8 cases and peroneal (P) flaps with fibula in 4 cases. Postoperative masticatory, speech, swallowing functions and facial appearance were evaluated by Multi-factorial analysis. All functional results seem to be related to the type of resection. By multi-factorial analysis, the type of mandibular resection, flap area, and anterior resection were related to the masticatory or speech function in tongue resection. In mandibular resection, masticatory function was subjected to age;speech function was subjected to type of mandibular resection and plate reconstruction. The facial appearance was subjected to flap area. These results suggested that postoperative function was not related to only one factor, but using the classification of each defect site as one of the factor of multi-factorial analysis was useful.

Automatic Anomaly Detection of Respiratory Motion Based on Singular Spectrum Analysis

The realization of automatic anomaly detection of respiratory motion could be very useful to prevent accidental damage during radiation therapy. In this paper, we proposed an automatic anomaly detection method using singular value decomposition analysis. Before applying this method, the investigator needs a normal respiratory motion data of a patient. From these data, a trajectory matrix representing normal time-series feature is created. Decomposing the matrix, we obtained the feature of normal time series. Then, we applied the same procedure to real-time data and obtained real-time features. Calculating the similarity of those feature matrixes, an anomaly score was obtained. Patient motion was observed by a depth camera. In our simulation, two types of motion e.g. cough and sudden stop of breathing were successfully detected, while gradual change of respiratory cycle frequency was not detected clearly.

Structural Investigation of Cell Wall Mannan Antigen Obtained from Pathogenic Yeast <i>Candida zeylanoides</i>

Antigenicity of pathogenic yeast Candida zeylanoides NBRC 10326 was found to have similar to that of Candida guilliermondii, but it was slightly different, i.e. NBRC 10326 strain cells react strongly with the factor antibodies 1, 4, and 9, and react relatively weakly with factor antibody 5. We have investigated the structural characteristic of cell wall mannan of this strain using a non-degradable method, nuclear magnetic resonance analysis. The results obtained from NMR analysis, the two-dimensional homonuclear Hartmann-Hahn, indicated that C. zeylanoides mannan contains α-1, 3-linked and β-1,2-linked oligomannosyl side chains corresponding to C. guilliermondii specific antigenic factor 9. In addition, it was found that the mannan contains β-1,2-linked oligomannosyl side chains attached to phosphate groups corresponding to anitigenic factor 5, which were not found in C. guilliermondii mannan.

To investigate the chemical structure of cell wall mannan of pathogenic yeast, Candida catenulata IFO 0745 strain, which possess the epitopes of antigenic factors 1, 9, and 34 to genus Candida, we previously performed the two-dimensional nuclear magnetic resonance (NMR) analysis of this mannan, Fr. C, without the need for harsh procedures. In this study, three oligosaccharides, biose, triose, and tetraose, and mannose were isolated from Fr. C by acetolysis. The results of NMR analysis indicate that the chemical structures of these oligosaccharides were identified to Manα1-2Man, Manα1-2Manα1-2Man, and Manα1-3Manα1-2Manα1-2Man. The most of resultant mannose seems to be originated from the α-1,6-linked mannan backbone which is recognized by antiserum to factor 9. The inhibition assay of slide agglutination reaction between Fr. C and antigenic antibodies using three oligosaccharides indicate that the Manα1-2Manα1-2Man and Manα1-3Manα1-2Manα1-2Man possess domains corresponding to immunodominants of antigenic factors 1 and 34, respectively.

Markerless Respiratory Motion Tracking Using Single Depth Camera

The aim of this study is to propose a novel system that has an ability to detect intra-fractional motion during radiotherapy treatment in real-time using three-dimensional surface taken by a depth camera, Microsoft Kinect v1. Our approach introduces three new aspects for three-dimensional surface tracking in radiotherapy treatment. The first aspect is a new algorithm for noise reduction of depth values. Ueda’s algorithm was implemented and enabling a fast least square regression of depth values. The second aspect is an application for detection of patient’s motion at multiple points in thracoabdominal regions. The third aspect is an estimation of three-dimensional surface from multiple depth values. For evaluation of noise reduction by Ueda’s algorithm, two respiratory patterns are measured by the Kinect as well as a laser range meter. The resulting cross correlation coefficients between the laser range meter and the Kinect were 0.982 for abdominal respiration and 0.995 for breath holding. Moreover, the mean cross correlation coefficients between the signals of our system and the signals of Anzai with respect to participant’s respiratory motion were 0.90 for thoracic respiration and 0.93 for abdominal respiration, respectively. These results proved that the performance of the developed system was comparable to existing motion monitoring devices. Reconstruction of three-dimensional surface also enabled us to detect the irregular motion and breathing arrest by comparing the averaged depth with predefined threshold values.
