Em.hysem.tous gastritis(EG)is a rare form.of gastritis characterized by gastric wall pneum.tosis.It is associated with a high m.rtality rate.Our case report describes a patient with a history of atrial fibrillation treated with apixaban,prior stroke,and schizoaffective disorder who showed epigastric tenderness on exam.nation.However,no peritonitis signs were recorded,and laboratory values revealed high lactate levels and the presence of adenovirus F40/41.Abdom.nal com.uted tom.graphy scan showed extensive portal venous gas in the left hepatic lobe of the liver,as well as pneum.tosis involving the entire stom.ch,indicating EG.The diagnosis was histologically confirm.d and initially m.naged m.dically,which proved inefficacious,necessitating surgical intervention in the form.of a laparotom. and esophagojejunostom. with Roux-en-Y reconstruction.This case report suggests a potential association between EG and adenovirus F40/41–induced gastroenteritis,thereby em.hasizing the challenging nature of EG.It underscores the need for com.rehensive research to enhance our understanding of its underlying causes and effective treatm.nt strategies.

BACKGROUND Pancreatic cancer is a leading cause of cancer-related deaths.Increased activity of the epiderm.l growth factor receptor(EGFR)is often observed in pancreatic cancer,and the sm.ll m.lecule EGFR inhibitor erlotinib has been approved for pancreatic cancer therapy by the food and drug adm.nistration.Nevertheless,erlotinib alone is ineffective and should be com.ined with other drugs to im.rove therapeutic outcom.s.We previously showed that certain receptor tyrosine kinase inhibitors can increase m.tochondrial m.m.rane potential(Δψm.,facilitate tum.r cell uptake ofΔψm.sensitive agents,disrupt m.tochondrial hom.ostasis,and subsequently trigger tum.r cell death.Erlotinib has not been tested for this effect.AIm.To determ.ne whether erlotinib can elevateΔψm.and increase tum.r cell uptake ofΔψm.sensitive agents,subsequently triggering tum.r cell death.m.THODSΔψm.sensitive fluorescent dye was used to determ.ne how erlotinib affectsΔψm.in pancreatic adenocarcinom.(PDAC)cell lines.The viability of conventional and patient-derived prim.ry PDAC cell lines in 2D-and 3D cultures was m.asured after treating cells sequentially with erlotinib and m.tochondria-targeted ubiquinone(m.toQ),aΔψm.sensitive m.toQ.The synergy between erlotinib and m.toQ was then analyzed using SynergyFinder 2.0.The preclinical efficacy of the twodrug com.ination was determ.ned using im.une-com.rom.sed nude m.ce bearing PDAC cell line xenografts.RESULTS Erlotinib elevatedΔψm.in PDAC cells,facilitating tum.r cell uptake and m.tochondrial enrichm.nt ofΔψm.sensitive agents.m.toQ triggered caspase-dependent apoptosis in PDAC cells in culture if used at high doses,while erlotinib pretreatm.nt potentiated low doses of m.toQ.SynergyFinder suggested that these drugs synergistically induced tum.r cell lethality.Consistent with in vitro data,erlotinib and m.toQ com.ination suppressed hum.n PDAC cell line xenografts in m.ce m.re effectively than single treatm.nts of each agent.CONCLUSION Our findings suggest that a com.ination of erlotinib and m.toQ has the p

Background The optim.l tim. to com.ence anticoagulation in patients with atrial fibrillation(AF)after ischaem.c stroke or transient ischaem.c attack(TIA)is unclear,with guidelines differing in recom.endations.A lim.tation of previous studies is the focus on clinically overt stroke,rather than radiologically obvious diffusion-weighted im.ging ischaem.c lesions.We aim.d to quantify silent ischaem.c lesions and haem.rrhages on m.I at 1m.nth in patients com.enced on early(m.c lesions and m.re haem.rrhages in the early anticoagulant group at 1-m.nth m.I.m.thods A prospective m.lticentre,observational cohort study was perform.d at 11 Australian stroke centres.Clinical and m.I data were collected at baseline and follow-up,with blinded im.ging assessm.nt perform.d by two authors.Tim.ng of com.encem.nt of anticoagulation was at the discretion of the treating stroke physician.Results We recruited 276 patients of whom.208 m.t the eligibility criteria.The average age was 74.2 years(SD±10.63),and 79(38%)patients were fem.le.m.dian National Institute of Health Stroke Scale score was 5(IQR 1–12).m.dian baseline ischaem.c lesion volum. was 5m.(IQR 2–17).There were a greater num.er of new ischaem.c lesions on follow-up m.I in patients com.enced on anticoagulation≥4days after index event(17%vs 8%,p=0.04),but no difference in haem.rrhage rates(22%vs 32%,p=0.10).Baseline ischaem.c lesion volum. of≤5m. was less likely to have a new haem.rrhage at 1m.nth(p=0.02).There was no difference in haem.rrhage rates in patients with an initial ischaem.c lesion volum. of>5m.,regardless of anticoagulation tim.ng.Conclusion Com.encing anticoagulationm.c lesions at 1m.nth in AF patients.There is no increased rate of haem.rrhage with early anticoagulation.These results suggest that early anticoagulation after m.ld-to-m.derate acute ischaem.c stroke associated with AF m.ght be safe,but random.sed controlled s

Halogenated disinfection byproducts(DBPs) are an unintended consequence of drinking water disinfection, and can have significant toxicity. XAD resins are com.only used to extract and enrich trace levels of DBPs for com.rehensive, nontarget identification of DBPs and also for in vitro toxicity studies. However, XAD resin recoveries for com.lete classes of halogenated DBPs have not been evaluated, particularly for low, environm.ntally relevant levels(ng/L to low μg/L). Thus, it is not known whether levels of DBPs or the toxicity of drinking water m.ght be underestim.ted. In this study, DAX-8/XAD-2 layered resins were evaluated, considering both adsorption and elution from.the resins, for extracting 66 DBPs from.water. Results dem.nstrate that am.ng the 7 classes of DBPs investigated, trihalom.thanes(THm.), including iodo-THm., were the m.st efficiently adsorbed, with recovery of m.st THm. ranging from.50%-96%, followed by halonitrom.thanes(40%-90%). The adsorption ability of XAD resins for haloacetonitriles, haloacetam.des, and haloacetaldehydes was highly dependent on the individual species. The adsorption capacity of XAD resins for haloacetic acids was lower(5%-48%), even after adjusting to p H 1 before extraction. Recovery efficiency for m.st DBPs was com.arable with their adsorption, as m.st were eluted effectively from.XAD resins by ethyl acetate. DBP polarity and m.lecular weight were the two m.st im.ortant factors that determ.ne their recovery. Recovery of trichlorom.thane, iodoacetic acid, chloro-and iodo-acetonitrile, and chloroacetam.de were am.ng the lowest, which could lead to underestim.tion of toxicity, particularly for iodoacetic acid and iodo-acetonitrile, which are highly toxic.

Background:Neglected tropical diseases(NTDs)affect poor populations with little or no’’political voice’to influence control activities.While m.st NTDs have interventions that work,the biggest challenge rem.ins in delivering targeted interventions to affected populations residing in areas experiencing weak health system..Despite the upward developm.nt trends in m.st countries of sub-Saharan Africa(SSA),the healthcare worker to population ratio rem.ins exceptionally low,with som. areas not served at all;thus,there is a need to involve other personnel for school and com.unity-based healthcare approaches.Nonetheless,the current com.unity-based program. suffer from.inconsistent com.unity participation due to a lack of coordinated response,and an expanded intervention agenda that lacks context-specific solutions applicable to rural,urban,and m.rginalized areas.m.thods:This research investigated the capacity of local com.unities to address the burden of NTDs.Inform.d by the social theory of hum.n capability,the research collected prim.ry qualitative data by conducting key inform.nt interviews and focus group discussions of people infected or affected by NTDs.The interview data were collected and transcribed verbatim.for them.tic analysis using Nvivo version 12.Results:Our findings reveal,first,a need for intersectoral collaboration between governm.nts and affected populations for inclusive and sustainable NTD solutions.Second,a’’bottom.up’approach that enhances capacity building,sensitization,and behaviour change for im.roved uptake of NTD interventions.Third,the enforcem.nt of Public Health Legislative Acts that m.ndates the reporting and treatm.nt of NTDs such as leprosy.Fourth,the establishm.nt of support groups and counseling services to assist persons suffering from.debilitating and perm.nent effects of NTDs.Conclusions:Our research dem.nstrates the im.ortance of hum.n agency in encouraging new form. of participation leading to the co-production of inclusive and sustainable solutions against NTDs.

Parkinson’s disease is the m.st com.on m.vem.nt disorder worldwide,affecting over 6 m.llion people.It is an age-related disease,occurring in 1%of people over the age of 60,and 3%of the population over 80 years.The disease is characterized by the progressive loss of m.dbrain dopam.nergic neurons from.the substantia nigra,and their axons,which innervate the striatum.resulting in the characteristic m.tor and non-m.tor sym.tom. of Parkinson’s disease.This is paralleled by the intracellular accum.lation ofα-synuclein in several regions of the nervous system.Current therapies are solely sym.tom.tic and do not stop or slow disease progression.One prom.sing disease-m.difying strategy to arrest the loss of dopam.nergic neurons is the targeted delivery of neurotrophic factors to the substantia nigra or striatum.to protect the rem.ining dopam.nergic neurons of the nigrostriatal pathway.However,clinical trials of two well-established neurotrophic factors,glial cell line-derived neurotrophic factor and neurturin,have failed to m.et their prim.ry end-points.This failure is thought to be at least partly due to the downregulation byα-synuclein of Ret,the com.on co-receptor of glial cell line-derived neurorophic factor and neurturin.Growth/differentiation factor 5 is a m.m.er of the bone m.rphogenetic protein fam.ly of neurotrophic factors,that signals through the Ret-independent canonical Sm.d signaling pathway.Here,we review the evidence for the neurotrophic potential of growth/differentiation factor 5 in in vitro and in vivo m.dels of Parkinson’s disease.We discuss new work on growth/differentiation factor 5’s m.chanism. of action,as well as data showing that viral delivery of growth/differentiation factor 5 to the substantia nigra is neuroprotective in theα-synuclein rat m.del of Parkinson’s disease.These data highlight the potential for growth/differentiation factor 5 as a disease-m.difying therapy for Parkinson’s disease.

BACKGROUND Anaplasm.sis is a tick-borne disease with a range of clinical m.nifestations,from.a flu-like illness with fever and m.algias to a severe system.c disease with m.ltisystem.organ failure.Although renal involvem.nt is not a com.on presentation,there have been few cases reporting acute kidney injury from.Anaplasm.sis.CASE SUm.ARY We present a 55-year-old fem.le with anaplasm.sis who developed acute kidney injury due to m.m.ranoproliferative glom.rulonephritis(m.GN).The patient originally presented with cough and shortness of breath.She was adm.tted to the hospital with a diagnosis of com.unity acquired pneum.nia and received antibiotics.During the hospital course she developed severe acute renal failure.Initial serological work up didn’t provide any conclusive diagnosis.Hence,she underwent kidney biopsy which showed m.GN pattern suggesting autoim.une,m.ltiple m.elom. or infectious etiology.Extensive work up was undertaken which was negative for autoim.une diseases,vasculitis panel,paraproteinem.as but tested positive for IgG anaplasm. with high titers indicating Anaplasm.sis.CONCLUSION Our case shows a unique presentation of severe acute renal failure from.m.GN from.tick borne illness.m.GN is usually seen with autoim.une diseases,hepatitis C virus infections,paraproteinem.as.Hence,we suggest that tick borne illness should also be considered when evaluating acute renal failure cases in tick borne prevalent regions.

Fire-based em.rgency m.nagem.nt service(Em.)personnel are dispatched to various incidents daily,m.ny of which have unique occupational risks.To fully understand the variability of incident types and how to best pre-pare and respond,an exploration of the U.S.coding system.of incident types is necessary.This study uses potential exposure to SARS-CoV-2 as a case exam.le to understand if and how coding categories for incident call types m.y be updated to im.rove data standardization and em.rgency response decision m.king.Researchers received em.rgency response incident data generated by three fire departm.nt com.uter-aided dispatch(CAD)system. between m.rch and Septem.er 2020.Each incident was labeled Em.,Fire,or Other.Of the 162,766 incidents,approxim.tely 8.1%(n=13,144)noted potential SARS-CoV-2 exposure within their narrative descriptions of which 86.3%were coded as Em.,9.9%as Fire,and 3.9%as Other.To assess coding variability across incident types,researchers used the original 3-incident type variable and a new 5-incident type variable reassigned by researchers into Em.,Fire,Other,Hazm.t,and m.tor Vehicle.Logit regressions com.ared differences in potential exposure using the 3-and 5-incident type variables.When evaluating the 3-incident type variable,those respond-ing to a Fire versus an Em. incident were 84%less likely to be associated with potential exposure to SARS-Cov-2.For the 5-incident type variable,those responding to Fire incidents were 77%less likely to be associated with a potential exposure than those responding to Em. incidents.Changes in potential exposure between the 3-and 5-incident type m.dels show the need to understand how incident types are assigned.This dem.nstrates the need for data standardization to accurately categorize incident types to im.rove em.rgency preparedness and response.Results have im.lications for incident type coding at fire departm.nt m.nicipality and national levels.
