Pancreatic stent improves the success rate of needle-knife papillotomy in patients with difficult biliary cannulation

BACKGROUND Needle-knife precut papillotomy(NKP)is typically performed ***,it remains unclear whether pancreatic stent(PS)placement can improve the outcomes of *** To explore whether PS placement improves the success rate of NKP in patients with difficult biliary *** This single-center retrospective study included 190 patients who underwent NKP between January 2017 and December 2021 after failed conventional biliary *** cases with incidental pancreatic duct cannulation during conventional biliary cannulation,the decision for pre-NKP PS placement was made at the endoscopist's *** primary outcome was the difference in the NKP success rate between patients with and without PS placement;the secondary outcome was the adverse event *** Among the 190 participants,82 received pre-NKP PS(PS-NKP group)whereas 108 did not[freehand or freehand NKP(FH-NKP)group].Post-NKP selective biliary cannulation was successful in 167(87.9%)patients,and the PS-NKP had a significantly higher success rate than the FH-NKP group(93.9%vs 83.3%,P=0.027).The overall adverse event rates were 7.3%and 11.1%in the PS-NKP and FH-NKP groups,respectively(P=0.493).A periampullary diverticulum(PAD)and significant intraoperative bleeding during NKP were independently associated with NKP failure;however,a pre-NKP PS was the only predictor of NKP *** the 44 participants with PADs,the PS-NKP group had a non-significantly higher NKP success rate than the FH-NKP group(87.5%and 65%,respectively;P=0.076).CONCLUSION PS significantly improved the success rate of NKP in patients with difficult biliary cannulation.

Type 2 diabetes mellitus(T2DM)is one of the fastest growing public health concerns around the ***-sweetened beverage(SSB)consumption has been proven to be associated with adverse health consequences in the diabetic *** SSB consumption,body weight control,healthy diets,and increased physical activity have been suggested as strategies to improve diabetes prevention and *** literature review provides an overview of:(1)The association between SSB consumption and the risk of T2DM;(2)Types of SSB consumption and T2DM;(3)The effect of obesity and inflammation on the association between SSB consum-ption and risk of T2DM;and(4)SSB consumption in T2DM *** is still work to be done to determine how SSB consumption is related to T2DM,but the current research on identifying the association between SSB consumption and T2DM is promising,with the most promising studies confirming the connection between SSBs,T2DM risk,and diabetes *** studies should explore more effective SSB related diabetes prevention and management interventions.

Influence of Bi addition on dynamic recrystallization and precipitation behaviors during hot extrusion of pure Mg

Low material cost and high extrudability for ensuring price competitiveness with Al alloys,as well as excellent mechanical properties,are essential for expanding the application range of Mg *** is a promising alloying element for developing extruded Mg alloys that satisfy such *** is inexpensive,exhibits a high solubility limit,and forms a thermally stable Mg3Bi2 phase,which improves the commercial viability and enables the high-speed extrusion of Mg–Bi *** this study,the effects of Bi addition on the dynamic recrystallization(DRX)and dynamic precipitation behaviors during hot extrusion of a pure Mg and the resultant microstructure and mechanical properties of the extruded materials were *** addition of 6 wt%and 9 wt%Bi to a pure Mg yielded numerous fine Mg3Bi2 precipitates during the early stage of hot ***,the area fraction of dynamic recrystallized(DRXed)grains decreased because of DRX-behavior suppression by the Zener pinning ***,the DRXed grain size was substantially reduced through the grain-boundary pinning *** size and number of undissolved Mg3Bi2 particles in the homogenized billets increased when the Bi content was increased,which resulted in increased DRX fractions owing to the enhanced levels of particle stimulated *** addition yielded considerable strength improvement of the extruded pure ***,the extruded Mg–Bi binary materials were less ductile than the extruded pure Mg *** lower ductility resulted from the cracking at twins formed in the coarse unDRXed grains of the Mg-6Bi material and the cracking at large undissolved Mg3Bi2 particles in the Mg-9Bi material.

Effects of Sn addition on the microstructure and mechanical properties of extruded Mg-Bi binary alloy

We investigated the effects of Sn addition on the microstructural characteristics and mechanical properties of an extruded Mg-Bi binary alloy by comparing Mg-5 Bi(B5)and Mg-5 Bi-4 Sn(BT54).Both the extruded alloys exhibit a partially recrystallized grain structure with a strong extrusion fiber texture and numerous Mg3Bi2 ***,the addition of Sn significantly decreases the average grain size of the extruded alloy from 123.9 to 75.2μ*** Sn solute atoms inhibit the activity of dislocation slip,which reduces the internal strain energy accumulated in the dynamically recrystallized(DRXed)grains during ***,this reduced strain energy leads to the decrease in the DRXed grain size owing to weakened grain growth during natural *** extruded BT54 alloy exhibits higher tensile strength and ductility than the extruded B5 *** improvement in the strength by the Sn addition is attributed to the combined effects of grain refinement,Sn solute atoms,and increased dislocation *** formation of{10-11}and{10-11}-{10-12}twins during tension is suppressed by the grain refinement,thereby improving the tensile elongation considerably.

Respiratory failure after scoliosis correction surgery in patients with Prader-Willi syndrome: Two case reports

BACKGROUND Sleep-disordered breathing,including hypoventilation and obstructive sleep apnea,is often observed in Prader-Willi syndrome(PWS).Particularly in adolescence,scoliosis causes a progressive restrictive pulmonary pattern,leading to hypoventilation,so timely corrective surgery is ***,the effect is *** addition,since mental retardation of PWS,patient effort-based respiratory tests may be less *** far,no studies have accurately reported on the comparison of respiratory function before and after corrective surgery,and appropriate respiratory function measurement method in *** SUMMARY We present two cases of adolescent PWS with typical characteristics,including obesity,mental retardation,and *** boys,aged 12 and 13,diagnosed with PWS,both underwent scoliosis correction *** and immediately after surgery,arterial blood tests showed no abnormalities and no respiratory symptoms ***,after 6-7 mo,both patients complained of daytime sleepiness,difficulty sleeping at night,dyspnea on exertion,and showed *** and hypoxia were confirmed by polysomnography and transcutaneous CO2 monitoring during sleep and were diagnosed with obstructive sleep apnea and alveolar *** was corrected by nighttime noninvasive ventilation application and normal findings of arterial blood gas were maintained after 6-8 mo *** Even after scoliosis surgery,“periodic”monitoring of respiratory failure with an“objective”test method is needed for timely respiratory support.

Stress analysis model of strip winding system with a sleeve for a coil of thin stainless steel

In a strip winding process,the sleeve is a hollow cylinder that is mounted between a strip coil and a mandrel to maintain uniform coil shape when the strip coil is very thin,but its deformation behavior has not been investigated ***,a finite element（FE）model was presented to calculate the stress distribution in a sleeve and strip coil when 1-3mm-thick stainless steel was wound around the *** FE model was developed by extending aprevious model by adding a sleeve between the mandrel and strip,and by modifying the boundary and interaction *** strip winding process was divided into an initial process and a steady-state *** the initial process,the minimum and maximum pressure required on the belt wrapper to maintain coil shape by self-friction of the strip was calculated by the FE model when the belt wrapper is ejected at the end of the initial *** the initial process,an analytical model of the steady-state process was established to calculate the stress distribution and was compared with the FE model to validate *** suggested analytical model took 11 sto give the same stress distribution that the FE model took 30 dto produce.

Effects of Al addition on microstructure and mechanical properties of extruded Mg-3Bi alloy

Effects of Al addition to a Mg±Bi binary alloy on its microstructural characteristics and tensile properties after extrusion are investigated via extrusion of Mg-3Bi±x Al(x=0,1,and 2 wt%)billets and analysis of the extruded *** Al addition negligibly affects the second-phase particles of the extruded alloy;however,an increase in the Al content causes significant decreases in the average grain size and maximum texture intensity of the extruded material owing to an increase in the area fraction of dynamically recrystallized(DRXed)*** Al addition improves the strength of the extruded alloy;this improvement is attributed to the enhanced grain-boundary hardening and solid-solution hardening effects induced by grain refinement and Al solute atoms,*** the Al content increases from 0 wt%to 1 wt%and 2 wt%,the tensile elongation increases substantially from 2.8%to 9.4%and 16.9%,*** reduction in the number and size of un DRXed grains with increasing Al content suppresses the formation and coalescence of cracks in the un DRXed grains during tension,which results in a significant improvement in the tensile ductility of the extruded *** tensile deformation,large undesirable twins that act as crack initiation sites are locally formed in the un DRXed grains of the Mg-3Bi alloy,whereas relatively smaller twins are uniformly formed in both the DRXed and the un DRXed grains of the Mg-3Bi-2Al ***,the extruded Mg-3Bi-2Al alloy has a substantially higher tensile yield strength±elongation product(2924 MPa%)than the extruded Al-free B3 alloy(381 MPa%).

Characterization of Ga-doped ZnO Nanorods Synthesized via Microemulsion Method

Gallium-doped ZnO （GZO） nanorods were synthesized by microemulsion method with different types of surfactants. The phase and morphology of the above nanorods were investigated by scanning electron microscopy （SEM）, X-ray diffraction （XRD）, high resolution transmission electron microscopy （HR-TEM） and X-ray photoelectron spectroscopy （XPS）. SEM observations show that the ZnO nanorods have diameters around 70-200 nm and lengths up to several micrometers. The room temperature photoluminescence （PL） spectrum of GZO nanorods exhibited a sharp and strong ultraviolet bandgap at 383 nm and a relatively weaker emission associated with the defect level. Resistivity of GZO nanorods synthesized with sodium lauryl sulfate （SLS） and sodium benzene sulfonate （SBS） surfactants showed 2.84 Ω m and 14.2 Ω m, respectively.
