Evidence from a familial case suggests maternal inheritance of primary biliary cholangitis

Primary biliary cholangitis(PBC) is an idiopathic autoimmune liver disease characterized by chronic cholestasis and destruction of the intrahepatic bile ducts. Similar to other autoimmune diseases, the pathogenesis of PBC is considered to be a complex etiologic phenomenon involving the interaction of genetic and environmental factors. Although a number of common variants associated with PBC have been reported from genome-wide association studies, a precise genetic mechanism underlying PBC has yet to be identified. Here, we describe a family with four sisters who were diagnosed with PBC. After the diagnosis of the index patient who was in an advanced stage of PBC, one sister presented with acute hepatitis, and two sisters were subsequently diagnosed with PBC. Notably, one half-sister with a different mother exhibited no evidence of PBC following clinical investigation. Our report suggests the possibility of a maternal inheritance of PBC susceptibility. Moreover, judging from the highpenetrance of the disease observed in this family, we inferred that a pathogenic genetic variant might be the cause of PBC development. We describe a family that exhibited diverse clinical presentations of PBC that included asymptomatic stages with mildly increased liver enzyme levels and symptomatic stages with acute hepatitis or advanced liver fibrosis. Additional studies are needed to investigate the role of genetic factors in the pathogenesis of this rare autoimmune disease.

Endoscopic submucosal dissection for early gastric cancer with undifferentiated-type histology:A meta-analysis

AIM: To evaluate the efficacy and safety of endoscopicsubmucosal dissection(ESD) for early gastric cancer(EGC) with undifferentiated-type ***: A systematic literature review was conducted using the core databases. Complete resection,curative resection, en bloc resection, recurrence and adverse event rate were extracted and analyzed. A random effect model was applied. The methodological quality of the enrolled studies was assessed using the Newcastle-Ottawa Scale. Publication bias was evaluated using a funnel plot, the trim and fill method, Egger's test,and a rank correlation ***: Fourteen retrospective studies between2009 and 2014 were identified(972 EGC lesions with undifferentiated-type histology). The total en bloc and complete resection rates were estimated as92.1%(95%CI: 87.4%-95.2%) and 77.5%(95%CI:69.3%-84%), respectively. The total curative resection rate was 61.4%(95%CI: 44.5%-75.9%). The overall recurrence rate was 7.6%(95%CI: 3.4%-16%).Limited to histologically diagnosed expanded-criteria lesions, the en bloc and complete resection rates were91.2% and 85.6%, respectively. The curative resection rate was 79.8%.CONCLUSION: In this analysis, ESD is a technically feasible treatment modality for EGC with undifferentiatedtype histology. Long-term studies are needed to confirm these therapeutic outcomes.

Effects of homogenization temperature on microstructure and mechanical properties of high-speed-extruded Mg–5Bi–3Al alloy

This study investigates the effects of billet homogenization temperature on the dynamic recrystallization behavior during high-speed extrusion and resultant microstructure and tensile properties of the Mg–5Bi–3Al(BA53,wt%)*** billets homogenized at 350 and450℃(350H and 450H billets)are extruded at a high speed of 69 m/*** 350H billet has a relatively smaller grain size and a higher abundance of fine Mg3Bi2particles compared to the 450H *** extrusion of the 350H billet,enhanced dynamic recrystallization occurs as a result of its finer grains and abundance of particles,while the growth of recrystallized grains is suppressed by the grain-boundary pinning effect of ***,the extruded 350H material is characterized by smaller grains,relatively greater number of Mg3Bi2particles,and a higher internal strain energy than the extruded 450H *** tensile strength of the extruded 350H material is higher than that of the extruded 450H material owing to stronger grain-boundary hardening,particle hardening,and strain hardening *** extruded 350H material also exhibits a higher tensile elongation as its smaller grains inhibit the formation of crack-inducing undesirable twins during *** results from this study demonstrate that a decrease in the homogenization temperature from 450 to 350℃leads to improved strength and ductility in the high-speed-extruded BA53 material.

Antibacterial Behaviour of Quaternized Poly(vinyl chloride)-g-Poly(4-vinyl pyridine) Graft Copolymers

Amphiphilic graft copolymers consisting of poly（vinyl chloride）（PVC） main chains and poly（4-vinyl pyridine）（P4VP） side chains were synthesized via atom transfer radical polymerization（ATRP） using direct initiation of chlorine atoms. The successful synthesis of PVC-g-P4 VP graft copolymers was confirmed by Fourier transform infrared spectroscopy（FTIR） and proton nuclear magnetic resonance（1H-NMR）. Transmission electron microscope（TEM） and small angle X-ray scattering（SAXS） analysis showed that PVC-g-P4 VP exhibited microphase-separated, ordered structure with 37.6 nm of domain spacing, which was not observed in neat PVC. For antibacterial applications, the tertiary nitrogen atoms of PVC-gP4 VP was quaternized using 1-bromohexane, as confirmed by FTIR measurements. Bacteria including Escherichia coli（E. coli）, Staphylococcus aureus（S. aureus）, Bacillus cereus（B. cereus）, and Pseudomonas aeruginosa（P. aeruginosa） were completely killed in 24 h on the quaternized PVC-g-P4VP（46% grafting） surface, indicating its excellent antibacterial behavior while it showed to be cytotoxic to mammalian cell.

Study on durability of Pt supported on graphitized carbon under simulated start-up/shut-down conditions for polymer electrolyte membrane fuel cells

The primary issue for the commercialization of proton exchange membrane fuel cell（PEMFC） is the carbon corrosion of support under start-up/shut-down conditions. In this study, we employ the nanostructured graphitized carbon induced by heat-treatment. The degree of graphitization starts to increase between 900 and 1300 ℃ as evidenced by the change of specific surface area, interlayer spacing, and ID/IG value. Pt nanoparticles are deposited on fresh carbon black（Pt/CB） and carbon heat-treated at 1700 ℃（Pt/HCB17） with similar particle size and distribution. Electrochemical characterization demonstrates that the Pt/HCB17 shows higher activity than the Pt/CB due to the inefficient microporous structure of amorphous carbon for the oxygen reduction reaction. An accelerating potential cycle between 1.0 and 1.5 V for the carbon corrosion is applied to examine durability at a single cell under the practical start-up/shutdown conditions. The Pt/HCB17 catalyst shows remarkable durability after 3000 potential cycles. The Pt/HCB17 catalyst exhibits a peak power density gain of 3%, while the Pt/CB catalyst shows 65% loss of the initial peak power density. As well, electrochemical surface area and mass activity of Pt/HCB17 catalyst are even more stable than those of the Pt/CB catalyst. Consequently, the high degree of graphitization is essential for the durability of fuel cells in practical start-up/shut-down conditions due to enhancing the strong interaction of Pt and π-bonds in graphitized carbon.

BACKGROUND A large ganglionic cyst extending from the hip joint to the intrapelvic cavity through the sciatic notch is a rare space-occupying lesion associated with compressive lower-extremity neuropathy.A cyst in the pelvic cavity compressing the intrapelvic-sciatic nerve is easily missed in the diagnostic process because it usually presents as atypical symptoms of an extraperitoneal-intrapelvic *** present a case of a huge ganglionic cyst that was successfully excised laparoscopically and endoscopically by a gynecologist and an orthopedic *** SUMMARY A 52-year-old woman visited our hospital complaining of pain and numbness in her left buttock while *** pain began 3 years ago and worsened,while the numbness in the left lower extremity lasted 1 *** was diagnosed and unsuccessfully treated at several tertiary referral centers many years *** resonance imaging revealed a suspected paralabral cyst(5 cm×5 cm×4.6 cm)in the left hip joint,extending to the pelvic cavity through the greater sciatic *** CA-125 and CA19-9 tumor marker levels were within normal ***,the cyst was compressing the sciatic ***,endoscopic and laparoscopic neural decompression and mass excision were performed simultaneously.A laparoscopic examination revealed a tennis-ball-sized cyst filled with gelatinous liquid,stretching deep into the hip *** excisional biopsy performed in the pelvic cavity and deep gluteal space confirmed the accumulation of ganglionic cysts from the hip joint into the extrapelvic intraperitoneal *** Intra-or extra-sciatic nerve-compressing lesion should be considered in cases of sitting pain radiating down the ipsilateral lower *** large juxta-articular ganglionic cyst was successfully treated simultaneously using laparoscopy and arthroscopy.

Background Takayasu arteritis(TAK)is a disease associated with increased risk of cardiovascular *** aimed to evaluate the incidence,prevalence and risk of stroke in patients with *** Data from 1065 patients were obtained from a national database(2010–2018).The annual incidence and prevalence per 100000 persons were estimated using the registration population in the midst of every year,and the standardised incidence ratio(SIR)of stroke was compared with the general population based on the data from the 2006 national report for cardiovascular and cerebrovascular ***-adjusted incidence rate ratio(IRR)of stroke based on the time interval after diagnosis was also calculated.A time-dependent Cox regression was conducted to investigate predictive factors of *** The overall incidence rate of TAK ranged between 0.2 and 0.3/100000 person-years annually;the prevalence of TAK gradually increased,reaching 3.25/100000 person-years in ***-three(6.9%)patients experienced stroke during follow-up,and the risk of developing stroke was higher than the general population(overall SIR 7.39,95%CI 5.79 to 9.29;men:SIR 5.70,95%CI 2.84 to 10.20;women:SIR 7.06,95%CI 5.41 to 9.05).Most stroke events(90.9%)were cerebral infarction for men,whereas the proportion of cerebral infarction was lower(62.9%)in *** half of stroke events occurred within 6 months after diagnosis,and stroke was more common within 6 months of diagnosis compared with after 3 years in women(IRR 13.46,95%CI 6.86 to 26.40).In Cox regression analysis,age was the sole predictor of stroke(adjusted HR 1.02,95%CI 1.00 to 1.04,p=0.043).Conclusions The annual incidence of TAK was similar to the previous studies from Asia,and the risk of stroke increased in *** patterns of subtype and incidence of stroke were found according to sex,although age was the only predictor.

Gene expression analysis of primary normal human hepatocytes infected with human hepatitis B virus

AIM： To find the relationship between hepatitis B virus （HBV） and hepatocytes during the initial state of infection by cDNA microarray. METHODS： Primary normal human hepatocytes （PNHHs） were isolated and infected with HBV. From the PNHHs, RNA was isolated and inverted into complement DNA （cDNA） with Cy3- or Cy5- labeled dUTP for microarray analysis. The labeled cDNA was hybridized with microarray chip, including 4224 cDNAs. From the image of the microarray, expression profiles were produced and some of them were confirmed by RT-PCR, immunoblot analysis, and NF-κB luciferase reporter assay. RESULTS： From the cDNA microarray, we obtained 98 differentially regulated genes. Of the 98 genes, 53 were up regulated and 45 down regulated. Interestingly, in the up regulated genes, we found the TNF signaling pathway-related genes： LT-α, TRAF2, and NIK. By using RT-PCR, we confirmed the up-regulation of these genes in HepG2, HuhT, and Chang liver cells, which were transfected with pHBV1.2x, a plasmid encoding all HBV messages. Moreover, these three genes participated in HBV- mediated NF-κB activation. CONCLUSION： During the initial state of HBV infection, hepatocytes facilitate the activation of NF-κB through up regulation of LT-α, TRAF2, and NIK.

Decellularized sciatic nerve matrix as a biodegradable conduit for peripheral nerve regeneration

The use of autologous nerve grafts remains the gold standard for treating nerve defects, but current nerve repair techniques are limited by donor tissue availability and morbidity associated with tissue loss. Recently, the use of conduits in nerve injury repair, made possible by tissue engineering, has shown therapeutic potential. We manufactured a biodegradable, collagen-based nerve conduit containing decellularized sciatic nerve matrix and compared this with a silicone conduit for peripheral nerve regeneration using a rat model. The collagen-based conduit contains nerve growth factor, brain-derived neurotrophic factor, and laminin, as demonstrated by enzyme-linked immunosorbent assay. Scanning electron microscopy images showed that the collagen-based conduit had an outer wall to prevent scar tissue infiltration and a porous inner structure to allow axonal growth. Rats that were implanted with the collagen-based conduit to bridge a sciatic nerve defect experienced significantly improved motor and sensory nerve functions and greatly enhanced nerve regeneration compared with rats in the sham control group and the silicone conduit group. Our results suggest that the biodegradable collagen-based nerve conduit is more effective for peripheral nerve regeneration than the silicone conduit.

Evaluation of Some Stem Taper Models for Camellia japonica in Mount Halla, Korea

This study was conducted to evaluate the performance of the four stem taper models on Camellia japonica in Jeju Island, Korea using fit statistics and lack-of-fit statistics. The five statistical criteria that were used in this study were standard error of estimate(SEE), mean bias( E), absolute mean difference(AMD), coefficient of determination(R2), and root mean square error(RMSE). Results showed that the Kozak model 02 stem taper had the best performance in all fit statistics(SEE: 3.4708, E : 0.0040 cm, AMD : 0.9060 cm, R2 : 0.9870, and RMSE : 1.2545). On the other hand, Max and Burkhart stem taper model had the poorest performance in each statistical criterion(SEE: 4.2121, E : 0.2520 cm, AMD : 1.1300 cm, R2 : 0.9805, and RMSE: 1.5317). For the lack-of-fit statistics, the Kozak model 02 also provided the best performance having the best AMD in most of the relative height classes for diameter outside bark prediction and in most of the DBH classes for total volume prediction while Max and Burkhart had the poorest performance. These stem taper equations could help forest managers to better estimate the diameter outside bark at any given height, merchantable stem volumes and total stem volumes of the standing trees of Camellia japonica in the forests of Jeju Island, Korea.
